Health Related Quality of Life (HRQoL) among Breast Cancer Patients Receiving Chemotherapy in Hospital Melaka: Single Centre Experience

  • Objective: To investigate the impact of chemotherapy on quality of life (QoL) among breast cancer patients and to evaluate the relationship with age, cancer stage and presence of any comorbidity. Methods: A prospective study was conducted among breast cancer patients receiving chemotherapy in Hospital Melaka from 1st January 2014 to 31st July 2014. The European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) was given to patients to fill in prior chemotherapy (baseline) and after the third cycle of chemotherapy. Socio-demographic and clinical data were collected and analyzed using SPSS version 20. Result: Respondents were 32 female patients [mean age (SD): 49.7(9.93) years]. They reported a significant lower global health status (P < 0.01) and significant higher symptoms of nausea and vomiting (P < 0.01), loss of appetite (P = 0.028) and diarrhea (P = 0.026) after the third cycle of chemotherapy as compared to baseline. Compare to, this study showed significant better emotional functioning (P < 0.01) and social functioning (P < 0.01) than the EORTC QLQ-C30 Reference Values 2008 for breast cancer cases. Under symptom scales higher scores were noted for appetite loss (P = 0.017), nausea and vomiting (P < 0.01). Age, stage and comorbidity had no clear associations with global health status in our patients (P > 0.05). Conclusion: Chemotherapy did reduce the QoL of breast cancer patients. Management of chemotherapy-induced loss of appetite, diarrhea, nausea and vomiting should be improved for a better outcome.

  • A new simple method to estimate the earth quantity which is basic data for making the plan of mountain forest road is tried to develop as one of ways to reduce time cost of plan making. The zero-line surveying method, the concept of 2/3 road width in cut-away part and of same quantity between cut-away and fill-up earth which are the backgrounds to develop this method is introduced and explaned also it's theoritical propriety. Under the precondition which have to apply above concepts for constructing the mountain forest road the exchange table for estimating the earth quantity is made like table 3 and 4 through being calculated by a trigonometric function. Table 3 can be used to estimate the earth quantity on the cut-away road-width according to slope of terrain and different road-width, and table 4 can be applied to calculate the quantity of cut-away earth when to know the road-width of cut-away. When using these tables the performance of field surveying and office work for road plan will be highly increased.

  • Reinforced soil and Expanded Polystyrenes (EPS) mixture (RSEM) is a geomaterial which has many merits, such as light weight, wide strength range, easy for construction, and economic feasibility. It has been widely applied to improve soft ground, solve bridge head jump, fill cavity in pipeline and widen highway. Reutilizing dredged sediment to produce RSEM as earthfill can not only consume a large amount of waste sediment but also significantly reduce the construction cost. Therefore, there is an urgent need understand the basic stress-strain characteristics of reinforced dredged sediment-EPS mixture (RDSEM). A series of cyclic triaxial tests were then carried out on the RDSEM and control clay. The effects of cement content, EPS beads content and confining pressure on the cyclic stress-strain behaviour of RDSEM were analyzed. It is found that the three stages of dynamic stress-strain relationship of ordinary soil, vibration compaction stage, vibration shear stage and vibration failure stage are also applicative for RDSEM. The cyclic stress-strain curves of RDSEM are lower than that of control clay in the vibration compaction stage because of its high moisture content. The slopes of backbone curves of RDSEMs in the vibration shear stage are larger than that of control clay, indicating that the existence of EPS beads provides plastic resistance. With the increase of cement content, the cyclic stress-strain relationship tends to be steeper. Increasing cement content and confining pressure could improve the cyclic strength and cyclic stiffness of RDSEM.

  • Multiple UAV System has been used for various purposes such as reconnaissance, networking and aerial photography. In such systems, it is essential to form and maintain the formation of multiple UAVs. This paper proposes the algorithm that produces an autonomous distributed control for each vehicle for a flexible formation. This command is a repulsive force in the form of the second-order system by the nearest UAV or mission area. The algorithm uses the relative position/speed through sensing and communication for calculating the command without external intervention. The command allows each UAV to follow the reference distance and fill the mission area as densely as possible without overlapping. We determine the reference distance via optimization technique solving the packing problem. The mission area comprises the desired formation outline and can be set flexibly depending on the mission. Numerical simulation is carried out to verify the performance of the proposed algorithm under a complex and flexible environment. The formation is formed in 26.94 seconds and has a packing density of 71.91%.

Analyzing the Spatial Transformation of Johannesburg: Background, Process and Effectiveness (约翰内斯堡空间转型的背景, 进程与效果研究)

  • Due to its speciality, Johannesburg has drawn wide attention from the international academia, which the existing urban theory cannot easily explain. This paper focuses on the spatial pattern of Johannesburg, and finds that it is highly fragmented: first North-South Division due to apartheid and gold mining, secondly, the continuous sprawl due to population densification, and thirdly formation of city-region due to the fill-in spatial policy. The fragmented spatial pattern undermines the social integration and weakens the potential economic sustainability, which make the balanced multi-core spatial pattern inevitable under the influence of the international planning thought. The Johannesburg's spatial transformation comes not only from social integration, but also from the development vison of world-class status of an African city. In reality, since the collapse of apartheid, the municipal government has released series of strategic planning in different executive phase. These different plannings unanimously focus on the world class status although since 2008, the social inclusiveness is more emphasized. Thus, Johannesburg implements some spatial policies, as promoting the corridor of freedom, identifying the key nodes in the city then developing with high density, implementing the in-filling policy, managing the urban growth boundary, effectively preserving the natural system. However, the industrial structure in Johannesburg is already quite high-ended which cannot produce many jobs and also require much investment in advanced infrastructure. Thus, the strategic goals of world-class status of an African city and social inclusiveness which really need widely shared public services to some extent are in tension. After evaluation, we can see that spatial transformation is quite limited. Obviously spatial transformation largely depends on the social-economy. The population is still in flow. In this sense Johannesburg must energetically support the employment-based industry, effectively control the spatial sprawl and carry out institutional innovation which further incentive investment, gradually build an integrated regional governance. In general, how Johannesburg combines globalization and its own condition is still worth thinking in both theory and practice.

Diverse and predominantly sub-adult Epinephelus sp. groupers from small-scale fisheries in South Sulawesi, Indonesia

  • Groupers (Family Epinephelidae) are commonly caught in data-poor small-scale multi-species fisheries for sale on both export and domestic markets. This study presents data on the species composition and size/life-stage structure of Epinephelus spp. groupers caught by small-scale fishers and sold locally in the Indonesian province of South Sulawesi. Data were collected from fishing ports and local markets at 12 sites representing the three seaways around South Sulawesi (Makassar Strait, Flores Sea, Gulf of Bone). Each specimen (n = 3,398) was photographed alongside an object of known length, and total length (TL) was obtained using the Rapid Scaling on Object (RASIO). Of the 23 species identified, four (Epinephelus areolatus, Epinephelus ongus, Epinephelus quoyanus, and Epinephelus fasciatus) collectively comprised 69% of the catch, while the 13 least abundant species contributed less than 5%. The catch was dominated (67%) by the subadult life-stage, with just under 20% in the adult class. Juveniles dominated the catch of Epinephelus fuscoguttatus, a valuable export commodity. Observations of early maturity as well as the sizeable gap between length at first capture (Lc) and length at first maturity (Lm) indicate recruitment overfishing of most species, with the notable exception of Epinephelus rivulatus. The proportion of adult fish was low (≈5%-30%) for the twelve most abundant species (E. areolatus, E. ongus, Epinephelus quoyanus, E. fasciatus, Epinephelus coioides, Epinephelus faveatus, Epinephelus sexfasciatus, Epinephelus maculatus, Epinephelus bleekeri, Epinephelus corallicola, E. fuscoguttatus, Epinephelus polyphekadion). For two moderately abundant species (E. faveatus and E. malabaricus), TL < Lm for all specimens. The limited data available indicate spawning ratio is lower than reported from deep-water fisheries of E. areolatus and E. coioides. The results call for targeted research to fill knowledge gaps regarding the biology and ecology of groupers exploited mainly for domestic markets; highlight the need for species-level data to inform management policies such as minimum legal size regulations; and can contribute towards species-level status assessments.

Health Behavior Level and Related Factors of Survivors After Urological Cancer Surgery in a University Hospital (일개 대학병원에서 비뇨기계 암 생존자의 수술 후 건강행태 수준 및 관련요인)

  • Objective: This study evaluated the health behavior level of urological cancer survivors after surgery. Understand the experience of change and identify the factors that affect it for health. Selection of the best intervention steps and effective intervention adopt a lifestyle. It is intended to contribute to the creation of basic data for development of guidelines. Methods: The study was conducted with patients diagnosed with urological cancer at a hospital in Seoul. Study data were obtained by having 100 patients who agreed to the study self-fill out a questionnaire through interviews, and the 2018 World Cancer Research Fund and American Institute for Cancer Research Score (2018 WCRF/AICR SCORE) was used to estimate the level of health behavior. Results: The study examines health behavior among urolgical cancer survivors based on the 2018 WCRF/AICR SCORE. Higher household income and younger age were associated with better health behavior scores, with those under 60 and earning over 3 million won being more likely to have higher scores. Conclusion: Based on these results, this study requires comprehensive data collection considering the missing variables, suggesting that high household income and young age can positively affect healthy behavior. In addition, we conclude that education for cancer survivors and development of strategies to bridge the health gap for low-income and elderly populations are necessary.

Settlement Instrumentation of Greenhouse Foundation in Reclaimed Land (간척지 온실 기초의 침하량 검토)

  • This study examined the settlement of a 1-2W type greenhouse using a timber pile, which was recently established on Gyehwa-do reclaimed land, in order to obtain base data for the construction of a greenhouse on reclaimed land. The results of this study are as follows. foundation and timber pile increased over time, irrespective of the interior and exterior of the upon investigation of the ground, it was confirmed that there was no soft rock stratum (bedrock), and that a sedimentary stratum existed under the fill deposit, which is estimated to have been reclaimed during the site renovation. It was also found that a weathered zone was located under the fill deposit and sedimentary stratum, and that the soil texture of the entire ground floor consisted of clay mixed with sand, silty clay, and granite gneiss, in that order, regardless of boreholes. In addition, the underground water level was 0.3m below ground, regardless of boreholes. Despite a slight difference, the settlement of the greenhouse or measurement sites (channels). With regard to the pillar inside the greenhouse, except in the case of CH-2, the data at a site located on the side wall of the greenhouse (wind barrier side) indicated vibrations of relatively larger amplitude. Moreover, the settlement showed a significant increase during a certain period, which was subsequently somewhat reversed. Based on these phenomena, it was verified that the settlement range of each site in the interior and exterior of the greenhouse was between 1.0 and 7.5mm at this time, except in the case of CH-1. The results of the regression analysis indicated good correlation, with the coefficient of determination by site ranging between 0.6362 and 0.9340. Furthermore, the coefficient of determination ranged between 0.6046 and 0.8822 on the exterior of the greenhouse, which is lower than inside the greenhouse, but still indicates significant correlation.

Wetting-Induced Collapse in Fill Materials for Concrete Slab Track of High Speed Railway (고속철도 콘크리트궤도 흙쌓기재료의 Wetting Collapse에 관한 연구)

  • Recently, the high speed railway comes into the spotlight as the important and convenient traffic infrastructure. In Korea, Kyung-Bu high speed train service began in bout 400 km section in 2004, and the Ho-Nam high speed railway will be constructed by 2017. The high speed train will run with a design maximum speed of 300-350 km/hr. Since the trains are operated at high speed, the differential settlement of subgrade under the rail is able to cause a fatal disaster. Therefore, the differential settlement of the embankment must be controlled with the greatest care. Furthermore, the characteristics and causes of settlements which occurred under construction and post-construction should be investigated. A considerable number of studies have been conducted on the settlement of the natural ground over the past several decades. But little attention has been given to the compression settlement of the embankment. The long-term settlement of compacted fills embankments is greatly influenced by the post-construction wetting. This is called 'hydro collapse' or 'wetting collapse'. In spite of little study for this wetting collapse problem, it has been recognized that the compressibility of compacted sands, gravels and rockfills exhibit low compressibility at low pressures, but there can be significant compression at high pressures due to grain crushing (Marachi et al. 1969, Nobari and Duncan 1972, Noorany et al. 1994, Houston et al. 1993, Wu 2004). The characteristics of compression of fill materials depend on a number of factors such as soil/rock type, as-compacted moisture, density, stress level and wetting condition. Because of the complexity of these factors, it is not easy to predict quantitatively the amount of compression without extensive tests. Therefore, in this research I carried out the wetting collapse tests, focusing on various soil/rock type, stress levels, wetting condition more closely.

Study UBC Inspection of the Over Result (UBC 검사의 이상결과에 대한 연구)

  • Purpose: As UBC IRMA is being tested, patients out of the reference value are reacting within the value again a few days later the urine collection tested, which causes the reliability of the test to decrease as a result. In this study, we can assume that the physiological changes in the factors would affect the results. The purpose of the study is to find out whether hematuria and proteinuria in the sample as well as the interval time (3 hours or more recommended) have affected the results. As a result, we could discover the changes in factors and increase the reliability of the test. Materials and Methods: 468 people (female: 249, male: 219) who came for the check-up were presented herein for medical examination from 2013.3.15 to 2013.1.2. Some people out of 468 who have reacted onto the reference value were divided into group low titer zone, ow-middle titer zone, and middle-high titer zone and tested for hematuria and proteinuria. During that period, 48 outpatients were asked to fill in a questionnaire regarding the urination interval time. The reagents used were (IDL Biotech AB, Sweden) and UBC IRMA. Results: Of the patients that are formed in the reference value of ($0.1-34.0{\mu}g/L$) turn out to be 52.7 years average age in their low concentration, ($mean{\pm}SD$) of the value of $0.10{\pm}0.02{\mu}g/L$. Among 80 people (50.8%, female: 49.2%), 16 patients (20%) have shown reaction to microscopic hematuria and 10 patients (12.5%) responded to proteinuria. In the average low concentration under 52.5 years of average age, 43 people (53%) have shown reaction to microscopic hematuria and 21 people (26.3%) are proteinuric patients out of 80 patients (male: 50.8%, female: 51.3%). In the middle high concentration of $11.8{\pm}4.82{\mu}g/L$ under the average age 51.7 years, 35 patients (53%) have responded to the microscopic hematuria and proteinuric patients are 26 people (39.3%) out of 66 people (men: 44%, women: 56%). In addition, in the concentration of $51.7{\pm}43.5{\mu}g/L$, some patients who get out of the reference value are observed as the average age of 52.0. 11 patients (78.6%) out of 14 (male: 35.7%, female: 64.3%) react to the microscopic hematuria. There show 6 people (42.8%) who turn out to be as proteinuric patients. As for the interval time, $1.67{\pm}3.71{\mu}g/L$ was the average value among 48 patients (female: 45.8%, male: 54.2%). Conclusion: We cannot see if proteinuria and hematuria directly affect abnormal results of inspection of 8,18 cytokeratin; however, we can find out that they statistically have an influence on highly generating UBC among several mechanisms. Also, although urination interval time was various every 15 minutes, we it does not affect these results.
