A Case of Unilateral Vocal Fold Paralysis Secondary to Sarcoidosis (사르코이드증에 의한 일측 성대 마비 1예)

  • Sarcoidosis is a multisystem granulomatous disease of unknown etiology. Vocal fold paralysis secondary to sarcoidosis is extremely rare but it can develop as a result of compressive lymphadenopathy, granulomatous infiltration, and neural involvement. We report the case of a 56-year-old woman who presented with unilateral vocal fold paralysis and enlarged supraclavicular lymph nodes. Computed tomography of the neck revealed multiple, enlarged, and matted lymph nodes at the cervical level of IV. An ultrasound-guided core needle biopsy of the lymph node was performed, and a histopathological diagnosis of sarcoidosis was made by validating the presence of noncaseating granuloma. After implementation of steroid therapy, the patient exhibited immediate recovery from vocal fold paralysis. Although an extremely rare disease, sarcoidosis should be included in the differential diagnosis of vocal fold paralysis. Accurate diagnosis and prompt steroid treatment may reduce the morbidity of patients with vocal fold paralysis secondary to sarcoidosis.

Effects of Continuous Therapeutic Ultrasound on Subcutaneous Murine Melanoma (초음파치료가 종양세포에 미치는 효과)

  • The use of therapeutic ultrasound(US) in humans with malignant neoplasms has been contraindicated in physical therapy practice. Some studies have shown that results after application of US differ according to tumor type and penetration depth. The purposes of this study were to determine the effects of US on melanoma in mice and to determine treatment dosage. Twenty-four female C57BL/6 mice, age 8 weeks. The right flank of all mice was shaved, and a 0.1 ml suspension of cells was injected subcutaneously into the animals' right flank. In this study, 24 subjects were randomly divided into three groups: experimental group 1(n=8), experimental group 2(n=8), control group(n=8). In the experimental group 1, animals received continuous 3 MHZ US treatment, administered at $2.0W/cm^2$ for five minutes. In experimental group 2, animals received continuous 3 MHz US treatment, administered at $1.0W/cm^2$ for 5 minutes. The control group received the same handling as other experimental groups, including rodent chow, water, US gel application but US head pressure without the power turned on. After 10 days treatment, all mice were killed with a potassium solution. Tumors were excised and weighed on an electrical balance and fixed in a 10% neutral buffered formalin solution. Tumor weights were smaller in experimental group 2(0.3838 g) than in the control group(0.6275 g). Tumor weights of the experimental group 1(0.015 g) were smaller than those of experimental group 2. Continuous therapeutic US decreased the weight of subcutaneous melanoma tumors in mice. The treatment dosage($2.0W/cm^2$) we suggest was more effective than earlier studies on decreasing tumor size with ultrasound.

  • Diagnostic ultrasound has the advantages of being noninvasive without any known deleterious biologic effect, rapid, painless, inexpensive and easily reproducible. The author used a real time sector scanner (Picker, Artis USA:5-7.5 MHz) for examing of lipoma, abscess, hematoma, lymphnode enlargement etc. and evaluated ultrasonograph as a diagnostic aid in head and neck region.

Thyroid Carcinoma Presenting as an Anterior Mediastinal Mass (종격동 종괴로 발현된 잠재성 갑상선암)

  • We have experienced a case of occult papillary thyroid carcinoma presenting as an anterior mediastinal mass in a 40-year-old man. The CT scan revealed a huge mass behind the manubrium of the sternum but the ultrasound examination failed to detect any lesion and developmental defect in the thyroid. Excision of the mediastinal mass and total thyroidectomy were carried out. Histologically, the mediastinal mass turned out to be papillary carcinoma without any portion of the normal thyroid tissue or normal lymph node tissue and the thyroid gland showed a tiny papillary carcinoma with the diameter of 0.3cm. Although a mediastinal mass as the sole presentation of the thyroid carcinoma is very rare, we suggest that a mediastinal mass should be added to the list of possible metastatic thyroid carcinoma.

Establishment of prediction table of parturition day by ultrasonography in Korean Jindo bitches (진도개에서 초음파검사에 의한 분만일 예정표 확립)

  • Serial ultrasonographic examinations were performed on pregnant Korean Jindo bitches. Measurements of inner chorionic cavity diameter and fetal head diameter were made from pregnancy day 15 to parturition. These measurements were converted retrospectively based on the day of parturition (day 0). The data of inner chorionic cavity diameter obtained from day -42 to day -25 and fetal head diameter obtained from day -24 to day -1 were used to prediction of parturition day. Formulas for the prediction of parturition day using the method of least squares were derived. These formulas were then used to predict parturition dates based on single measurements of inner chorionic cavity diameter or fetal head diameter in 17 additional pregnant Korean Jindo bitches. Predicted date of parturition was then compared to actual whelping date. In the prediction of parturition based on inner chorionic cavity diameter, 7 of 10 bitches were coincided prediction date and actual whelping date, and the prediction was accurate to within 1 day in 3 of 10 bitches. The prediction of parturition based on fetal head diameter was accurate to within 1 day in 6 of 7 bitches and within 2 days in 1 of 7 bitches. In conclusion, the ultrasound measurement of inner chorionic cavity diameter and fetal head diameter are practical and accurate tool in the prediction of parturition.

Patterns of Calcification in Thyroid Nodules; Significance and Malignant Potentiality (갑상선결절내 석회화소견과 갑상선암종과의 연관성)

  • Objectives: Calcification of the thyroid nodule has been reported to have a close relationship with differentiated thyroid carcinoma, however there are limited studies on the relationship between the calcified thyroid nodule and thyroid malignancy. The authors studied the clinical significance of calcification within the thyroid nodule. Materials and Methods: There were total of 60 patients who underwent surgery for calcified thyroid nodules which were identified from plain neck X-ray, ultrasound and computed tomography during the period January 1991 to June 1996 at the Department of Otorhinolaryngology-Head and Neck Surgery of Korea University Hospital. Six patients were not included because of recurrence and previous thyroid surgery. Histopathologic and radiologic analysis was done on the remaining 54 patients. Results: Results showed that 25 of 54 cases(46%) to be malignant histopathologically. Of the 25 malignant cases, papillary carcinoma was the most common with 22 cases followed by 2 cases of undifferentiated carcinoma and I case of medullary carcinoma. Statistically high incidence of malignancy was observed when the consistency of calcified thyroid nodule was solid, shape of calcification was irregular and inhomogenous, and adhesion of calcified thyroid nodule to the regional structure was present. Conclusion: The probability that a thyroid nodule is malignant has been reported to be 3­20%. However, about half of the thyroid nodules with calcification found to be malignant on this study, calcification of the thyroid nodule can be used as a guideline for detecting thyroid malignancy.

  • Esophageal diverticulum is very rare disease. Since it usually has no symptoms, it is hard to find and diagnose. Moreover, if the diverticulum is located nearly to thyroid gland, surgeons sometimes misdiagnose as a thyroid nodule. Here, we represent our case which has papillary thyroid cancer and esophageal diverticulum together. In this case, we experienced the surgical procedure and postoperative complications of the esophageal diverticulum. Even though esophageal diverticulum was in preoperative computed tomography and ultrasound, it was neglected before the surgery so that found during the operation. The purpose of this study was to know the preoperative radiological findings of the esophageal diverticulum and to find out what to be aware of during surgery and how to manage the complications after surgery.

Diagnostic Consistency between Sonoelastography and Conventional Sonography of Long Head of the Biceps

  • Background: Sonoelastography (SE) is a new technique that can assess differences in tissue stiffness, the purpose of this study was to evaluate the ability of SE to assess the long head of biceps tendon alteration. Methods: Forty shoulders of 36 consecutively registered patients with clinical symptoms and conventional ultrasonography findings of biceps tendinitis or tendinosis, and 40 asymptomatic shoulders of 20 healthy volunteers were assessed with SE. Transverse and longitudinal images of long head of biceps tendon were obtained using SE. SE images were performed by one orthopedic surgeon and evaluated by two orthopedic surgeons using an experimentally proven color grading system. Results: The transverse images of SE showed a sensitivity of 87.5%, a specificity of 95.0% and a accuracy of 91.3%, the longitudinal images of SE showed a sensitivity of 92.5%, a specificity of 90.0% and a accuracy of 91.3%. Inter-observer reliability of SE was in 'almost perfect agreement' with a weighted kappa coefficient of 0.83. Conclusions: SE is valuable in the detection of the intratendinous and peritendinous alterations of biceps tendon, and has excellent accuracy and excellent correlation with conventional ultrasound findings.

Comparison of Ultrasound Image Quality using Edge Enhancement Mask (경계면 강조 마스크를 이용한 초음파 영상 화질 비교)

  • Ultrasound imaging uses sound waves of frequencies to cause physical actions such as reflection, absorption, refraction, and transmission at the edge between different tissues. Improvement is needed because there is a lot of noise due to the characteristics of the data generated from the ultrasound equipment, and it is difficult to grasp the shape of the tissue to be actually observed because the edge is vague. The edge enhancement method is used as a method to solve the case where the edge surface looks clumped due to a decrease in image quality. In this paper, as a method to strengthen the interface, the quality improvement was confirmed by strengthening the interface, which is the high-frequency part, in each image using an unsharpening mask and high boost. The mask filtering used for each image was evaluated by measuring PSNR and SNR. Abdominal, head, heart, liver, kidney, breast, and fetal images were obtained from Philips epiq5g and affiniti70g and Alpinion E-cube 15 ultrasound equipment. The program used to implement the algorithm was implemented with MATLAB R2022a of MathWorks. The unsharpening and high-boost mask array size was set to 3*3, and the laplacian filter, a spatial filter used to create outline-enhanced images, was applied equally to both masks. ImageJ program was used for quantitative evaluation of image quality. As a result of applying the mask filter to various ultrasound images, the subjective image quality showed that the overall contour lines of the image were clearly visible when unsharpening and high-boost mask were applied to the original image. When comparing the quantitative image quality, the image quality of the image to which the unsharpening mask and the high boost mask were applied was evaluated higher than that of the original image. In the portal vein, head, gallbladder, and kidney images, the SNR, PSNR, RMSE and MAE of the image to which the high-boost mask was applied were measured to be high. Conversely, for images of the heart, breast, and fetus, SNR, PSNR, RMSE and MAE values were measured as images with the unsharpening mask applied. It is thought that using the optimal mask according to the image will help to improve the image quality, and the contour information was provided to improve the image quality.

  • Diagnosis of the long head tendon of biceps is not easy. and an ultrasonography is helpful at the office. We report a case who visited with the right shoulder pain and was diagnosed as a mass around the bicipital groove using the ultrasonography, finally undertook an arthroscopic resection. On the histologic examination, synovial lining cell were not found, the wall was thickened. He diagnosed as ganglion.
