Real-Time 3D Ultrasound Imaging Method Using a Cross Array Based on Synthetic Aperture Focusing: II. Linear Wave Front Transmission Approach (합성구경 기반의 교차어레이를 이용한 실시간 3차원 초음파 영상화 기법 : II. 선형파면 송신 방법)

  • In the accompanying paper, we proposed a real. time volumetric imaging method using a cross array based on receive dynamic focusing and synthetic aperture focusing along lateral and elevational directions, respetively. But synthetic aperture methods using spherical waves are subject to beam spreading with increasing depth due to the wave diffraction phenomenon. Moreover, since the proposed method uses only one element for each transmission, it has a limited transmit power. To overcome these limitations, we propose a new real. time volumetric imaging method using cross arrays based on synthetic aperture technique with linear wave fronts. In the proposed method, linear wave fronts having different angles on the horizontal plane is transmitted successively from all transmit array elements. On receive, by employing the conventional dynamic focusing and synthetic aperture methods along lateral and elevational directions, respectively, ultrasound waves can be focused effectively at all imaging points. Mathematical analysis and computer simulation results show that the proposed method can provide uniform elevational resolution over a large depth of field. Especially, since the new method can construct a volume image with a limited number of transmit receive events using a full transmit aperture, it is suitable for real-time 3D imaging with high transmit power and volume rate.

Safety and Efficacy of Ultrasound-Guided Percutaneous Core Needle Biopsy of Pancreatic and Peripancreatic Lesions Adjacent to Critical Vessels (주요 혈관 근처의 췌장 또는 췌장 주위 병변에 대한 초음파 유도하 경피적 중심 바늘 생검의 안전성과 효율성)

  • Purpose To evaluate the safety and efficacy of ultrasound-guided percutaneous core needle biopsy (USPCB) of pancreatic and peripancreatic lesions adjacent to critical vessels. Materials and Methods Data were collected retrospectively from 162 patients who underwent USPCB of the pancreas (n = 98), the peripancreatic area adjacent to the portal vein, the paraaortic area adjacent to pancreatic uncinate (n = 34), and lesions on the third duodenal portion (n = 30) during a 10-year period. An automated biopsy gun with an 18-gauge needle was used for biopsies under US guidance. The USPCB results were compared with those of the final follow-up imaging performed postoperatively. The diagnostic accuracy and major complication rate of the USPCB were calculated. Multiple factors were evaluated for the prediction of successful biopsies using univariate and multivariate analyses. Results The histopathologic diagnosis from USPCB was correct in 149 (92%) patients. The major complication rate was 3%. Four cases of mesenteric hematomas and one intramural hematoma of the duodenum occurred during the study period. The following factors were significantly associated with successful biopsies: a transmesenteric biopsy route rather than a transgastric or transenteric route; good visualization of targets; and evaluation of the entire US pathway. In addition, the number of biopsies required was less when the biopsy was successful. Conclusion USPCB demonstrated high diagnostic accuracy and a low complication rate for the histopathologic diagnosis of pancreatic and peripancreatic lesions adjacent to critical vessels.

Development of 3D Viewer for Tree Cavity using Pulse Ultrasound (펄스 초음파를 이용한 수목 공동부 3D 구현 프로그램 제작)

  • The pattern of the tree's internal swelling depends on many causes. Since it is difficult to detect these various causes of swelling with a general method, if the state of swelling for a long time cannot be confirmed, serious damage to the trees may occur due to enlargement of the swelling area. In the method of acquiring a tree tomography image, an impulse passing through the tree is generated by tapping the sensor with a rubber mallet, and the moving speed is recorded. In this paper, to measure cracks, cavities, and swelling due to physical damage, we developed a 3D viewer that can know the internal state of a tree using a tree cross-section image acquired from Arbotom to determine the degree of swelling inside the tree. Based on this, we tried to present data that can be referred to when surgical operation of trees is required. In order to acquire a tomographic image of a tree, 6 sensors were attached to the three Yangpala and Maple trees, and a 1 m-long tree was measured using the Arbotom program, and a 3D image was implemented through the 3D Viewer created using MATLAB. In addition to simply acquiring images, the cross-sectional length and volume of the tree were measured. In the actually produced 3D Viewer, the length of the part where the swelling of the maple tree occurred was 33.12 cm, and the swelling of the yangpala tree was measured as 21.41 cm. The volume of the maple tree was measured to be 78.832 ㎤. As a result of comparing the cross-sectional image of the Arbotom and the 3D image, the same result as the real aspect of the tree was obtained, so it can be judged that the reliability of the manufactured software is also secured, and data to be applied to the surgical tree operation through the created Viewer is provided. It is believed that the damage will be minimized.

Comparative Assessment of Fractal Analysis and Histogram in Canine Abdominal Ultrasonographic Images (개 복부초음파영상의 프랙탈 분석과 히스토그램 분석의 비교평가)

  • This study was carried out to show at the fractal analysis complements the practical disadvantage of gray level histogram which is designed to measure the quantitative classification of echo patterns in ultrasonographic image of parenchymal organs such as spleen and kidney and it is a practical method of measurement for quantitative classification. By using ultrasonographs, kidney and spleen of 21 healthy Beagles were fixed under different gain settings to be scanned for echo patterns and results were analyzed with body gray level histogram and fractal analysis. Then it was compared based on the statistical data obtained. Although there was a proportionate increase in histogram along with gain settings, there were consistencies in the fractal dimension. In terms of quantitative analysis in ultrasonographic images, fractal analysis is concluded to complement the practical disadvantage of gray level histogram.

Nineth Rib Syndrome after 10th Rib Resection

  • The $12^{th}$ rib syndrome is a disease that causes pain between the upper abdomen and the lower chest. It is assumed that the impinging on the nerves between the ribs causes pain in the lower chest, upper abdomen, and flank. A 74-year-old female patient visited a pain clinic complaining of pain in her back, and left chest wall at a 7 on the 0-10 Numeric Rating scale (NRS). She had a lateral fixation at T12-L2, 6 years earlier. After the operation, she had multiple osteoporotic compression fractures. When the spine was bent, the patient complained about a sharp pain in the left mid-axillary line and radiating pain toward the abdomen. On physical examination, the $10^{th}$ rib was not felt, and an image of the rib-cage confirmed that the left $10^{th}$ rib was severed. When applying pressure from the legs to the $9^{th}$ rib of the patient, pain was reproduced. Therefore, the patient was diagnosed with $9^{th}$ rib syndrome, and ultrasound-guided $9^{th}$ and $10^{th}$ intercostal nerve blocks were performed around the tips of the severed $10^{th}$ rib. In addition, local anesthetics with triamcinolone were administered into the muscles beneath the $9^{th}$ rib at the point of the greatest tenderness. The patient's pain was reduced to NRS 2 point. In this case, it is suspected that the patient had a partial resection of the left $10^{th}$ rib in the past, and subsequent compression fractures at T8 and T9 led to the deformation of the rib cage, causing the tip of the remaining $10^{th}$ rib to impinge on the $9^{th}$ intercostal nerves, causing pain.

  • The time domain methods of estimating the attenuation coefficient are generally based on the analysis of statisical properties of the reflected echoes form an attenuating medium. Hence, it is often required to have a large number of data samples in order to obtain a statistically stable estimation result. In the attenuation estimation problem, this means that many different speckle patterns are required in the spatial resolution volume of an attenLlation image. In this paper, by using the fact that the speckle pattern Is sensitive to the point spread function of the ultrasound beam, we suggest a method to generate the statiscally uncorrelated or slightly correlated data samples in a given region by rotating a linear transducer and carrying out lateral scans for all rotating angles. This technique is applied to the entropy method for attenuation estimation proposed recently by the authors where the performance is verified by experiments using a tissue equivalent phantom.

The Value of Ultrasonographic Endometrial Measurement in the Prediction of Pregnancy Outcome in In Vitro Fertilization (체외수정시술 주기에서 자궁내막발달과 착상에 관한 연구)

  • The condition of the endometrium is an important factor which may influence the success or failure in IVF-ET. This study was undertaken for evaluation of the value of endometrial growth as an early predictor for the success of IVF. Ultrasonographic endometrial measurement were performed in 43 IVF cycles that conceived, 101 cycles that did not with an IVF-ET There was no significant difference in the endometrial thickness and the serum concentration of estradiol in the pregnant versus nonpregnant group(10.4 vs. 9.9 mm: 2348 vs. 2017 pg/ml no hCG administration day). No correlation was found between the ultrasound image and serum estradiol levels around the time of hCG administration(r=0.54, p=0.13 no Day 2; r=0.45, p=0.14 no Day 1). The duration of gonadotropin treatment, number of follicles, number of oocytes retrieved, and fertilization rate were not statistically different in the two groups, however, there was a significant difference in the number of embryos in the pregnant versus nonpregnant group)p< 0.05). A higher pregnancy rate and ongoing pregnancy rate occured with an endometrial thickness over 11 mm compared with below 7mm(p< 0.05, p< 0.005). however, no significant differences were noted in the implantation rate and abortion rate among the groups that classified according to their endmetrial thickness. The endometrial growth(${\Delta}$) from hCG administration day(DO) to D6 was greater in the women who achieved pregnancy than in the nonpregnant group(p< 0.01). There were no significant differences in serum estradiol levels, implantation rate, pregnancy rate, and abortion rate among the groups that classified according to the pattern of echogenesity of endometrium, however, significantly higher ongoing pregnancy rate was noted in group A, B compared with group C.(p< 0.0001, p< 0.001) These results suggest that there were no ultrasonographically detectable differences in the patterns of endometrial growth and development around the time of hCG administration in patients who conceive versus those that do not in IVF-ET.

Xanthogranulomatous Pancreatitis Mimicking a Pancreatic Cancer on CT and MRI: a Case Report and Literature Review

  • Xanthogranulomatous inflammation is a rare benign condition involving various organs. However, its pancreas involvement is very rare. To the best of our knowledge, only 17 cases have been described in the literature. Interestingly, all reported 17 cases due to various causes underwent surgical resection. Here, we present a case of xanthogranulomatous pancreatitis in a 63-year-old man. He presented with epigastric pain and solid mass mimicking ductal adenocarcinoma in the body and tail of pancreas on magnetic resonance imaging. The patient was diagnosed as xanthogranulomatous pancreatitis via endoscopic ultrasound-guided fine needle aspiration. After that, he was followed up and monitored without any surgical treatment. Here, we show imaging findings and serial image changes of xanthogranulomatous pancreatitis for this case.

Extraction of Lumbar Multifidus Muscle using Ultrasound Imaging (초음파 영상에서 다열근 추출)

  • In this paper, we propose a new method for extracting muscles from lumbar images. The proposed method sets areas without distortions with field expert's assistance as areas of measuring interest and removing noises from initial ultrasonic videos. Then, the method emphasizes the brightness contrast with Ends-in search stretching algorithm and separate thoracic vertebra from subcutaneous fat area using morphological characteristics. 4-directions contour tracing algorithm is applied to extract the bottom of subcutaneous fat area. Extracting thoracic vertebra area requires noise removal and morphological characteristics as well among candidate areas obtained by controlling min-max brightness. The thickness of muscles is then defined as the length between subcutaneous fat area and extracted thoracic vertebra. The experiment which consists of 368 image analysis verifies that the proposed method is more effective in measuring the thickness of muscles than before.

  • Ultrasonic C-scan technique is one of very popular techniques being used for detection of flaws in polymer matrix composite(PMC). However, the application of this technique is very limited for evaluation of defects in PMC fabricated by the automated fiber placement process. The purpose of this study is to develop a novel ultrasonic hybrid system based on nondestructive and non-contact ultrasonic techniques for evaluation of delamination in carbon/epoxy and carbon/PPS composite laminates. It was shown that the newly developed ultrasonic hybrid system based on dual air-coupled pitch-catch technique with ultrasonic scattering reflection concept could provide excellent image with higher resolution of delamination in PMC compared with the conventional pitch-catch method. It is expected that this ultrasonic hybrid technique can be applied for on-line inspection of flaws in PMC during the fabrication process.
