The Analysis of Satisfaction with Clinical Training and the Related Factors in the Korean Medicine Obstetrics and Gynecology (한방부인과학에서 임상실습교육의 만족도와 관련요인 분석)

  • Objectives: The purpose of this study is to analyze csatisfaction with clinica1 training in the department of Korean Medicine Obstetrics and Gynecology and the related factors. Methods: All the students on the graduating class of school of Korean Medicine, Dong-Shin University must be trained for 1 year in the department of Korean Medicine Obstetrics and Gynecology according to clinical training guideline including Objective Structured Clinical Examination (OSCE). After completing clinical training, we distributed questionaries to them about a reflex of the object of study, usefulness, difficulty, satisfaction and preference. And then, we analyzed the related factors including descriptive statistics, frequency analysis and correlation analysis by SPSS 12.0. Results: In general, a reflex of the aim of study (7.31±1.59), satisfaction with contents (7.44±1.58), and difficulty (6.13±1.34) were evaluated restectively. There is no statistically significant difference with sex. There is a statistically significant positive correlation between a reflex of the aim of study and satisfaction with contents. The primary preferences of clinical practice were Clinic Training, Ward Training, PBL (Problem Based Learning), OSCE. In OSCE, a reflex of the aim of study (7.04±1.83) and usefulness (7.02±1.97) were highly evaluated. There is no statistically significant difference with sex. The primary preferences for clinical OSCE practice were breast examination, genital examination, ultrasound scan, and pap smear. Facilities and equipment (6.84±2.01), contents of clinical training (7.29±1.83), the handout (7.20±1.98) and time scheduling (7.29±1.90) were evaluated respectively in general composition of clinical training. Conclusions: Clinical training program in the department of Korean Medicine Obstetrics and Gynecology can be highly estimated from the viewpoint of satisfaction with clinical training and the related factors.

Adrenal Hemorrhage in a Neonate (신생아의 부신 출혈)

  • Neonatal adrenal hemorrhage is frequently associated with birth trauma or perinatal hypoxia. Hemorrhagic necrosis of the adrenal glands is often found at autopsy and many small lesions are usually asymptomatic. A palpable abdominal mass and jaundice are the usual presenting signs. Ultrasound is very useful in the diagnosis of this lesion; however, if the mass has mixed echoic pattern, magnetic resonance imaging (MRl) is helpful for the differential diagnosis from neuroblastoma. We present the case of a female newborn who was found to have a abdominal mass on physical examination. The patient showed anemia and hyperbilirubinemia. An ultrasonogram disclosed a $3.8{\times}3.0$ cm suprarenal mass with mixed echoic pattern. The mass was initially suspected to be neuroblastoma. An abdominal computed tomogram was not able to differentiate the mass. Magnetic resonance imaging revealed markedly increased signal intensity on T1 and T2-weighted sequences. This finding was consistent with adrenal hemorrhage. Serial sonogram demonstrated the mass that resolved completely by 12 weeks of age.

Feline Gastrointestinal Eosinophilic Sclerosing Fibroplasia in a Bengal Cat

  • A 2-year-old neutered male Bengal cat presented with a 6-month history of weight loss and chronic vomiting. An abdominal ultrasound revealed increased thickness of the pylorus and ascending duodenum with concurrent enlargement of the mesenteric lymph nodes. Histologically, branching and anastomosing trabecular pattern of dense collagen was the characteristic feature, and large populations of spindle cells were also observed. These large spindle-shaped cells were positive for smooth muscle actin and vimentin on immunohistochemical examination. Based on these findings, the cat was diagnosed with feline gastrointestinal eosinophilic slcerosing fibroplasia (FGESF), and immunosuppressive therapy was initiated immediately. Unfortunately, the cat's condition deteriorated despite treatment; the cat died 56 days after initiation of therapy. This is the first report of FGESF in South Korea.

Imaging diagnosis of hepatic lipidosis in a cat

  • A 5-year-old intact female Persian cat was referred with anorexia and vomiting. Physical examination revealed icterus in the pinna, oral mucosa, and sclera. Ultrasonography revealed several findings including uniformly hyperechoic hepatic parenchyma compared to adjacent falciform fat, increased attenuation of the ultrasound beam, and poor visualization of intrahepatic vessel borders. Computed tomography revealed hypoattenuation of the hepatic parenchyma with a radiodensity value of -60 Hounsfield units. The adjacent intrahepatic vessels appeared hyperattenuated relative to the hepatic parenchyma as if the vessels were contrast-enhanced. Based on ultrasonography and computed tomography results, the cat was tentatively diagnosed as feline hepatic lipidosis.

  • Adrenal venous malformation is an uncommon disease. We report a case of an incidentally diagnosed adrenal cystic mass during an evaluation for gastritis in a child. A 14-year-old girl admitted to our pediatric department for epigastric pain. After the gastroscopy, she was diagnosed with hemorrhagic gastritis. A 5.5 cm-sized cystic mass was incidentally found adjacent to her left adrenal gland during an ultrasound examination for evaluating her abdominal symptoms. She underwent laparoscopic surgery for the diagnosis and treatment of this cystic mass, which was confirmed to be venous malformation at pathologic diagnosis.

고양이에서 발생한 Perinephric pseudocysts와 선천성 Peritoneo-pericardial Kiaphragmatic Hernia in Cat

  • An 8-year old castrated domestic long-hair cat was presented with a two week history of abdominal distension. Physical examination revealed a non-painful, fluctuant, palpable mass in the right craniodorsal abdomen, and unilaterally muffled heart sounds on the right thorax. Routine clinico-pathological values were unremarkable apart from mild azotemia with a concurrent urine specific gravity of 1.031, which reflect a degree of renal dysfunction. Radiographic and ultrasound examinations of the thorax revealed the cardiac enlargement to be due to the congenital peritoneo-pericardial diaphragmatic hernia with liver occupying the right half of the pericardial sac. There was also a mild gypertrophy of the heart. Radiography and ultrasonography of the abdomen showed the mass to be composed of a large fluid filled cystic structures surrounding the right and left kidneys, and the kidneys themselves were of increased echogenecity. A diagnosis of perinephric pseudocysts was made. The patient responded well to the surgical procedures. Perinephric pseudocysts and peritoneo-pericardial diaphragmatic hernia in the cat are rare, and a case is described and the literature is reviewed in this report.

  • A case of leiomyosarcoma is reported in a 9-year-old female Jin Do. The dog has been suffered from 1 week history of abdominal distension, dyspnea, depression and anorexia. On routine physical examination, radiology and ultrasound, a large occupying mass was noted in the reptroperitoneum. Due to poor prognosis, the uterine mass removed by ovariohystrectomy and submitted to pathology department for diagnosis. Grossly, the mass was firm, tan and contained several areas that were necrotic and hemorrhagic. (omitted)

  • Ectopic thyroid gland is a defined as thyroid tissue located in a site other than its usual pre-tracheal location. Ultrasound examination, CT scan, thyroid scan, and thyroid iodine uptake test are all valuable modalities for a differential diagnosis. Although most of cases are asymptomatic, treatment is indicated when there are symptoms related to progressive growth of mass. This article reports a ectopic thyroid cyst in the anterior neck that was similar to deep neck infection accompanied by acute airway obstruction.

Thyroid Carcinoma Presenting as an Anterior Mediastinal Mass (종격동 종괴로 발현된 잠재성 갑상선암)

  • We have experienced a case of occult papillary thyroid carcinoma presenting as an anterior mediastinal mass in a 40-year-old man. The CT scan revealed a huge mass behind the manubrium of the sternum but the ultrasound examination failed to detect any lesion and developmental defect in the thyroid. Excision of the mediastinal mass and total thyroidectomy were carried out. Histologically, the mediastinal mass turned out to be papillary carcinoma without any portion of the normal thyroid tissue or normal lymph node tissue and the thyroid gland showed a tiny papillary carcinoma with the diameter of 0.3cm. Although a mediastinal mass as the sole presentation of the thyroid carcinoma is very rare, we suggest that a mediastinal mass should be added to the list of possible metastatic thyroid carcinoma.

Hypertrophic pyloric gastropathy with Helicobacter spp. in a dog

  • A 12-year-old, castrated male, mixed dog presented with a history of gradual abdominal distention for a year and anorexia recently, with abdominal radiographs showing a gastric pylorus distention. A solitary, pedunculated, heterogeneous mass arising from the mucosal layer in the pylorus with intact wall layers was identified during ultrasound and computed tomography. The gastric muscular layer was evenly thick. After surgical excision of the mass, histological examination confirmed hypertrophic pyloric gastropathy with polypoid growth and Helicobacter spp. infiltrating the gastric mucosal epithelium. This is the first reported diagnostic imaging case of hypertrophic pyloric gastropathy with Helicobacter spp. in a dog.
