Disseminated Septic Lesions Caused by Staphylococcus epidermidis Mimicking Cysticercosis Detected on Ultrasonography in a Pediatric Patient with Acute Myeloid Leukemia (급성 골수성 백혈병 소아환자에서 초음파 검사에서 낭미충증으로 오인된 표피 포도알균에 의한 파종 감염 병변이 발생한 증례)

  • Staphylococcus epidermidis is a part of the normal skin flora of humans. However, it can cause serious infections in people exposed to foreign bodies or in immunocompromised patients. A 13-year-old boy was hospitalized with fever and myalgia. Painful nodular lesions were detected on the scalp, arms, and legs. Pancytopenia and blasts were present in the peripheral blood. He was diagnosed with acute myeloid leukemia. Magnetic resonance imaging of the whole body showed multiple peripheral rim-enhancing, cyst-like lesions. Ultrasonography showed echogenic nodules inside the cystic lesions in the intramuscular space of the arms and legs. Therefore, cysticercosis was strongly suggested initially. However, an abscess was confirmed on sono-guided biopsy and S. epidermidis was isolated from a microbial culture of the tissue. We report a case of multiple disseminated lesions caused by S. epidermidis in a leukemia patient, initially mistaken for cysticercosis.

  • We analyzed the clinical characteristics and outcome of ileocecal and small bowel intussusceptions (ICI and SBI) in the pediatric patients. From August 2003 to July 2010, 144 children with intussusception were included in this study. We retrospectively reviewed the clinical records and Imaging study findings. A total of 86 children with ICI and 58 children with SBI were diagnosed. Children with SBI were older than ICI ($36.6{\pm}24.6$ months vs. $24.2{\pm}21.6$ months, p=0.002). Typical symptoms such as irritability, abdominal mass, bloody stool were more frequent in ICI than SBI (p<0.05) patients. In the ICI group, intussusceptums were reduced with air reduction (84.5%), surgery (17.4%), and spontaneity (1.2%). All patients in the SBI group were reduced spontaneously. SBI occurred in older age and was reduced spontaneously more frequently than ICI. Conservative management with close observation with follow-up by ultrasonography is recommended for SBI.

  • Snapping triceps syndrome is a rare disease in which medial head of triceps dislocates over the medial epicondyle during elbow flexion. It is difficult to diagnose the snapping triceps syndrome, because that this syndrome is frequently misdiagnosed as other elbow disease such as ulnarnerve dislocation. The dynamic ultrasonographic imaging allows continual visualization of the ulnar nerve and triceps muscle throughout active elbow flexion and extension. We report two patients of snapping triceps syndrome who were diagnosed with the use of dynamic ultrasonography and treated with ulnar nerve anterior transposition and repositioning or resectioning of medial head of triceps.

Ultrasonographic Evaluation of Renal Cortex and Outer Medulla Thickness in Dogs with Chronic Kidney Disease

  • This study was performed to retrospectively pursue any correlation between renal cortex thickness (RCT), outer medulla thickness (OMT) on ultrasonography (US) and chronic kidney disease (CKD) with International Renal Interest Society (IRIS) stage. Medical records and US findings of the dogs diagnosed CKD were reviewed for comparing to those of the clinically healthy dogs from March. 2015 through June. 2016. To evaluate the correlation about normal and CKD patients, RCT and the OMT were measured on US images. RCT and OMT were adjusted by dividing body surface area for standardization (RCTS, OMTS). Also the ratio of RCT/OMT were calculated and these indices were evaluated to investigate any tendency between 5 groups (normal and 4 IRIS stages) using Jonckheere-Terpstra trend test (J-T test). The RCTS showed a declining tendency (p < 0.02) and the OMTS showed an inclining tendency from normal through each IRIS stage (p < 0.01). The RCT/OMT showed also declining tendency (p < 0.01). Although the gold standard for renal function including GFR is lack, it is thought that differentiation between normal and CKD patients could be possible on US measurement of renal cortex and outer medulla thickness, which could be an alternative index for kidney function in diagnostic imaging.

  • A 10-year-old spayed female Yorkshire terrier presented with vomiting, and anorexia. Obstructive choledocholith with concurrent pancreatitis and hepatorenal syndrome was diagnosed based on analyses of the blood profiles, abdominal radiographs, ultrasonography and CT. Choledoliths were removed from the common bile duct, yet the patient died suddenly 10 days after surgery. Recurrent choledolithiasis has not been reported in a cholecystectomized dog and this report focuses on the imaging features of ultrasonography and CT of recurrent choledoliths.

  • Acromio-clavicular (AC) joint cyst have been reported in small series of the orthopedics literature in association with extensive rotator cuff tear, pseudotumor, infection of the shoulder or chondrocalcinosis. Authors experienced one case of AC joint cyst with rotator cuff re-tear after arthroscopic rotator cuff repair, clinicians may need pay attention to AC joint bulging or mass lesion after arthroscopic rotator cuff repair as an important sign to check follow-up imaging study for the cuff integrity. Especially, ultrasonography is recommended for this follow up study, because it is simple to be operated, economic, easily accessible.

Diagnostic Imaging for Lung Lobe Torsion in a Miniature Pinscher (Miniature Pinscher 견에서 발생한 폐염전의 진단영상 1례)

  • A four-year old, female Miniature Pinscher was presented with intermittent coughing. Thoracic radiography and ultrasonography revealed the consolidation of left cranial lung lobe surrounded by pleural effusion and the extraction of left lung lobe was performed through thoracotomy. This case has the clinical significance because lung lobe torsion was diagnosed by non-cardiac ultrasonographic examination, therefore surgical correction was performed without delay and led good outcome. Moreover, the authors believe the present case was the first report on lung lobe torsion in Miniature Pinscher dog.

  • Botryoid Wilms tumor, a very rare variant of Wilms tumor, arises from the pelvocalyceal system, and its occurrence in the fetal or neonatal period has never been reported in the literature. Herein, we report an exceedingly rare and challenging case of botryoid Wilms tumor in a neonate who initially presented with fetal hydronephrosis. Postnatal ultrasonography revealed multiple lobulating hypoechoic masses with varying degrees of intralesional vascularity within the dilated pelvocalyceal system. To our knowedge, this is a case report of botryoid Wilms tumor of the youngest child in English literature.

  • The resource-based relative value scale (RBRVS) compares the value of a medical practice to the consumption of resources, which consist of the work of the physician, practice expenses, and professional liability insurance. At the time of the 2nd revision of RBRVS, the fee for radiological examinations had been reduced due to the high preservation rate. In RBRVS, practice expenses account for most of the compensation of radiological examinations, and physicians' work is relatively undervalued. A new healthcare policy (Moon Jae-In care) consists of the expansion of the National Health Insurance (NHI) coverage, reduction of patient charges for the vulnerable class, and support for catastrophic medical expenses. However, Moon Jae-In care is expected to negatively affect the NHI in Korea financially. The expansion of the insurance coverage for ultrasonography and MRI examinations is a significant part of the Moon Jae-In care, and radiological societies should establish fair compensations for physicians' work within the field of radiology while implementing the Moon Jae-In care.

Clinical Usefulness of Chest Wall Ultrasonography for Detecting Fractures of Costal Cartilage due to Minor Blunt Chest Trauma (경미한 둔상에 의하여 야기되는 늑연골 골절 진단에 있어서 흉벽 초음파 검사의 임상적 유용성)

  • Background: Rib fractures are the most common injuries that are caused by blunt chest trauma. However, fractures of the costal cartilage generally go unnoticed on chest X-rays unless they involve a calcified cartilage. For this reason, the sensitivity of conventional radiography for detecting rib fractures is low, and especially those involving the cartilaginous part of the rib. Thus, we have evaluated the usefulness of ultrasonography for detecting fractures of the costal cartilage that were overlooked on the conventional radiographs of patients who suffered minor blunt chest traumas. Material and Method: A total of 45 patients who suffered minor blunt chest trauma and who had no evidence of rib fractures or other major fractures on conventional radiographs were admitted for ultrasonography between April 2008 and March 2009. There were 24 women and 21 men, and the mean age of the patients was 50.4$\pm$15.91 years (range: 17$\sim$76 years). They were examined for the detection of fractures of the costal cartilage by performing ultrasonography with a 7.5-MHz linear transducer. Result: A total of 30 patients (67%) had fractures of the costal cartilage, whereas 15 patients (33%) had no evidence of chondral rib fractures. The mean number of fracture sites of the fractured costal cartilage was 1.6$\pm$0.81 (range: 1$\sim$4 sites) in 30 patients. Periosteal hematoma was the most common finding associated with fractures of the costal cartilage (n=7, 17%), followed by sternum fracture (n=5, 12%). However, periosteal hematoma was noticed in 1 patient (2%) who was without fracture of the costal cartilage, and sternum fracture was noticed in 1 patient (2%) who was without fractures of the costal cartilage. Conclusion: The results of this study suggest that ultrasonography may be a useful imaging modality for detecting fractures of the costal cartilage that are overlooked on the conventional radiographs of patients who suffer minor blunt chest trauma.
