집단 검진에서 발견된 갑상선 우연암종에 대한 임상적 고찰 (Clinical Investigation of Incidentally Found Thyroid Carcinoma in Mass Screening)

  • Background and Objectives: The introduction of highly sensitive imaging techniques has made it possible to detect many non-palpable nodules, or“incidentaloma”in the thyroid. Discovery of these lesions raises concerns about their malignancy, but the optimal strategy for managing these lesions has not been clearly established. This study was designed to evaluate the usefulness of ultrasonographic exam with new diagnostic criteria and presume the value of mass screening for thyroid cancer. Materials and Methods : Mass screening for thyroid cancer were carried out in conjunction with mass screening for breast cancer. The subjects were 630 women aged 30 years or over. Thyroid glands were examined with 10 MHz transducer ultrasonography by one radiologist. Needle aspiration biopsy were performed when suspicious of malignancy under the new diagnostic criteria. Results and Conclusion : The new ultrasonographic criteria to diagnose thyroid cancer provided useful information and ensured more accurate evaluation. 7 cases of thyroid cancer were discovered and successfully managed. It should be further demonstrated that there is sufficient value of mass screening for thyroid cancer to perform it independently despite early cancer detection.

주관절 결절종에 의한 척골신경 마비 - 2례 보고 - (Ulnar Nerve Palsy Caused by a Ganglion at the Elbow - Two Case Report -)

  • We experienced two cases of ulnar nerve palsy caused by a ganglion that were managed by excision of the ganglion performed concurrently with subcutaneous anterior transposition of the ulnar nerve. Satisfactory results were obtained. The possibility of ulnar nerve compression by ganglion must be considered in patient who complains rapidly progressing ulnar nerve palsy. For those cases, ultrasonography or magnetic resonance imaging seems to be helpful in obtaining preoperative diagnosis.

  • Pseudoaneurysm with arteriovenous fistula is a rare complication of arthroscopy, and can be diagnosed by ultrasonography, computed tomography, magnetic resonance imaging, or angiography. This condition can be treated with open surgical repair or endovascular repair. We report our experience with the open surgical repair of a pseudoaneurysm with an arteriovenous fistula in a young male patient who underwent arthroscopy five months previously.

  • Vomiting in pediatric patient is frequently encountered problem in emergency room or outpatient clinic. In differential diagnosis, age of the patient or accompanying symptoms should be considered in the differential diagnosis. Accurate diagnosis is very important because surgical treatment is necessary in some of the conditions. Imaging diagnosis of conservative modality such as upper gastrointestinal series or colon study is still important radiological examination in the initial differential diagnosis, but recently ultrasonography offers accurate diagnosis in many situations. The cause of vomiting in pediatric are diverse according to the age group :neonatal sepsis, necrotizing enterocolitis, or hypertrophic pyloric stenosis in neonates : gastroesophageal reflux, viral enteritis, or intussusception in infant: midgut volvulus, appendicitis, metabolic disorders, or increased intracranial pressure also an be the cause. knowledge of radiological findings of normal gastrointestinal tract is important to recognize abnormalities. A discussion of radiological findings in variable surgical conditions to present as vomiting in pediatric patients is offered.

Intraneural Venous Malformations of the Median Nerve

  • Venous malformations arising from the peripheral nerve are a rare type of vascular malformation. We present the first case of an intraneural venous malformation of the median nerve to be reported in a child and review the previous two cases of median nerve compression due to a venous malformation that have been reported. These cases presented with painless masses in the volar aspect of the wrist or with symptoms suggestive of carpal tunnel syndrome. Clinical suspicion should lead to the use of Doppler ultrasonography as the first-line diagnostic tool. Magnetic resonance imaging and histopathology can confirm the diagnosis, as phleboliths are pathognomonic of venous malformations. Surgical treatment appears to be the only modality capable of successfully controlling the growth of an intraneural malformation. Sclerotherapy and radiotherapy have never been used to treat this type of malformation.

Uterine leiomyosarcoma in a cat

  • A 16-year-old cat presented with a 2-week history of anorexia and lethargy. Radiography revealed a soft-tissue opacity, heart-shaped mass between the descending colon and urinary bladder. Ultrasonography showed a large uterine body with a heterogeneously hypoechoic, thickened wall and hypoechoic intraluminal fluid. Computed tomography revealed a large, fluid-filled uterine mass with contrast enhancement, without evidence of regional lymph node or pulmonary metastasis. Ovariohysterectomy was performed and leiomyosarcoma was confirmed by histology. No notable abnormalities were observed during the 1-year postoperative follow-up periods. This report describes the diagnostic imaging and treatment of a rare case of feline uterine leiomyosarcoma.

소아에서 충수주위농양으로 오진된 자궁부속기 염전 (Adnexal Torsion Misdiagnosed as Periappendiceal Abscess in a Child)

  • A 4 year old female with acute lower abdominal pain was admitted with the diagnosis of periappendiceal abscess made by ultrasonography. At laparotomy, the appendix was normal, but the left ovary was twisted and necrotic. Appendectomy and left salpingo-oophorectomy were performed. The pathology of the appendix was normal, and the left tube and ovary were ischemic and had hemorrhagic necrosis. Pediatric adnexal torsion may be difficult to diagnosis clinically. Sonography is the preferred imaging study. It usually confirms a pelvic mass but may not establish the diagnosis. The correct diagnosis of adnexal torsion is often made at exploration. The most common erroneous diagnosis is acute appendicitis or periappendiceal abscess. Therapy for adnexal torsion remains controversial. While extirpation has been the standard of treatment in the past, there are current proponents of conservative therapy with adnexal sparing. Early diagnosis may lead to more frequent salvage of affected adnexa.

회귀방법에 의한 초음파 진단기의 측면해상도 개선에 관한 연구 (A recursive scheme for improvement of the lateral resolution in B-scan ultrasonography)

  • The objective of this paper is to present a digital method for improving the lateral resolution of the B-scan images in the medical applications of ultrasound. The method is based upon a mathematical model of the lateral blurring caused by the finite beam width of the transducers. This model provides a simple method of applying a recursive scheme for image restoration with fast computation time. The point spread function (P.S.F.) can be measured by the reflective signals after scanning the small pins located along the depth of interest. From the measured P.S.F., one can compute the coefficient matrices of the inverse discrete-time dynamic state variable equation of the blurring process. Then, a recursive scheme for deblurring is applied to the recorded B-scan to improve the lateral resolution. One major advantage of the present recursive scheme over the transform method is in its applicability for the space-variant imaging, such as in the case of the rotational movement of transducer.

개에서 편측성 난소낭종의 방사선학적 및 초음파학적 진단 2례 (Radiographic and Ultrasonographic Diagnosis of Unilateral Ovarian Cysts)

  • A female, 14-year-old mixed dog weighing 4 kg and a female,8-year-old schnauzer dog weighing 6 kg showing vaginal discharges and vomiting for a few weeks were examined for the preoperative check of anterior cruciate ligament rupture and the recheck after the surgery of nephrectomy. There were no distinct abnormal signs except for the increased CBC at schnauzer dog on blood chemistry analysis. Radiographic and ultrasonographic findings were a large abdominal mass and a cyst-like hypoechogenic or anechogenic cavitary lesion on the ovary region. These findings were consistent with an ovarian cyst. Thereafter highly suspected ovarian cysts by diagnostic imaging were supported by high estrogen concentration on hormonal assay and the results of Pathologic examination as the unilateral ovarian cysts with secondary pyometra.

소음인(少陰人)으로 오진(誤診)한 소양인(少陽人) 결흉(結胸) 치험례(治驗例) (A case of patient diagnosed as and treated with Hyeongbangdojeoksan)

  • One woman, Sixty three years old, suffered from chest pain, abdominal pain, nausea and vomiting for about three months. Although many examinations, brain magnetic resonance imaging(MRI), chest computed tomography(CT), abdominal CT, gastric endoscope, abdominal ultrasonography, and so forth, was performed from every point of view, those were non-significant. At first, I wrongly diagnosed her case as disease of Soumin and gave Soumin Gwakhyangjeongkisan. But she complained of diarrhea and chest discomfort. I re-diagnosed her case as Soyangin Gyeolhyung and treated with Hyeongbangdojeoksan, herbal medicine, consisted of Rhizoma Rehmanniae(生地黃), Lignum Akebiae(木通), Radix Scrophulariae(玄蔘), Semen Trichosanthis(瓜蔞仁), etc. Three weeks have passed from first visit and she did not suffered from pain. I experienced the excellent effect of Hyeongbangdojeoksan to Soyangin Gyeonlhyung.
