Pulmonary Mineralization Secondary to Iatrogenic Hyperadrenocorticism in a Dog (개에서 발생한 의인성 부신피질기능항진증과 속발성 폐 석회화 진단 1례)

  • An 11-year-old, intact male Shih Tzu presented with obesity, thin hair coat, and abdominal distention. The patient had previously received corticosteroid therapy for dermatitis. Thoracic radiographs showed incidental findings of a generally diffuse bronchointerstitial pattern and markedly radiolucent pleural lines between the middle and caudal lung lobes, but clinical signs related to the thoracic radiographic findings were absent. Echogenicity of the hepatic parenchyma was increased and bilateral adrenal glands were not enlarged on ultrasonography. On computed tomography, the central regions of the lung lobes showed ground-glass attenuation, the peripheral regions of the lung lobes were relatively hypoattenuated compared with the central regions, and hyperattenuated nodules were distributed throughout the pulmonary parenchyma. Iatrogenic hyperadrenocorticism and secondary pulmonary mineralization was diagnosed on the basis of diagnostic imaging and adrenocorticotropic hormone stimulation test.

A Correlation Study of Clinical Outcomes by Quantification of Fatty Degeneration of the Subscapularis: Partial vs. Whole Cross-section

  • Background: Fatty degeneration of rotator cuff is a well-known predictor of postoperative outcome. The purpose of this study was to evaluate the clinical features of rotator cuff tears involving subscapularis, and investigate whether fatty degeneration quantified from only the upper subscapularis correlates better with clinical outcomes than quantified from the whole subscapularis. Methods: We retrospectively analyzed 315 consecutive patients who underwent arthroscopic repair for rotator cuff tears involving subscapularis with a minimum follow-up of 1 year. Preoperative and postoperative visual analogue score for pain, range of motion and functional scores were assessed. Integrity of the repaired tendon was assessed at the 1-year follow-up with either magnetic resonance imaging or ultrasonography. Results: The mean Goutallier grade of whole cross-section was significantly lower than that of upper cross-section (1.59 vs. 1.71, p<0.05), but significantly higher than that of lower cross-section (1.59 vs. 1.01, p<0.05). In analysis of 37 re-tears, the occupancy of severe fatty degeneration in upper cross-section was 86.5%, which was significantly higher than that seen in whole cross-section (56.8%, p<0.05). We calculated the cut-off tear size for prediction of re-tears as 19.0 mm for retraction and 11.0 mm for superior-inferior. The cut-off Goutallier grade was 2.5 for both whole and upper cross-sections, but area under the curve was greater in the upper cross-section than the whole (0.911 vs. 0.807). Conclusions: As fatty degeneration of upper subscapularis demonstrated a more distinct spectrum than whole subscapularis, we suggest that measuring fatty degeneration of upper subscapularis can be a more useful method to predict clinical prognosis.

  • The purpose of this study is to analyze the problems of the Current Curriculum of Radiologic Technology Department in Junior College, try to find a future solution of the education of Radiologic Technology and a reform measure, and suggest a new substantial model. So this study refered to sundary records, posed a question by papers, made a reform measure of curriculum on the basis of the results, examined it throughly by discussion with the related professors and the industrials figures, and decided a new model. The characteristics of the reform measure reflected in the new model are as follows; 1. It increased the numbers of credits like 94 or 95 ones, to positively accept a developing medical technique and modern science. 2. It set up various general studies and offered a free selection. 3. It closely related majors and their credits to the national examination of lisence and the task of industrial job site. 4. It kept the balance between the periods of lecture, practice, training and their credits. 5, It reinforced the subjects of fundamental medical science such as Introduction to Medicine, Pathology, Biochemistry, Patient Care, etc. 6. It newly established Clinical Trainings as a regular education course. 7. It newly established Introduction to Computer Science, Ultrasonography and Magenetic Resonance Imaging, to cope with the development of the future medical technique. 8. It newly established Humanism in Medicine as a regular education course. 9. It changed the names of subjects resonably.

The Association Between Masticatory Function Assessment and Masseter Muscle Thickness in the Elderly

  • Purpose: This study investigated the association between the objective indicator of masticatory function assessment and the masseter muscle thickness (MMT) using ultrasound imaging. Methods: A total of 99 subjects (males: 24, females: 75, mean age: 76) were analyzed. The maximum bite force (MBF) was measured with a pressure-sensitive sheet and an image scanner. The mixing ability index (MAI) was calculated by image analysis after asking the subjects to chew a wax specimen. The MMT during rest and clenching were obtained with a diagnostic ultrasound system, and the difference in MMT during rest and MMT during clenching was defined as the difference in masseter muscle thickness (DMMT). Multiple regression analysis was performed to determine the independent variables affecting MBF and MAI. Results: The MBF showed correlation with the number of remaining teeth (β=0.346, p=0.002) and DMMT (β=0.251, p=0.011). The MAI correlated with only the number of remaining teeth (β=0.476, p<0.001). Conclusions: The DMMT reflects the state of masseter muscle contraction, and can be used as a predictor as well as the number of teeth when assessing masticatory function.

Power Doppler ultrasound-guided sialography using the phenomenon of increased blood flow: A technical report

  • Purpose: This report presents a procedure for performing power Doppler ultrasound-guided sialography using the phenomenon of increased blood flow and illustrates its application to practical patient cases. Materials and Methods: The salivary gland was scanned using ultrasound equipment (GE LOGIQ5 Expert® device; GE Medical Systems, Milwaukee, WI, USA) to identify pathological findings related to the patient's chief complaint. To identify the orifice of the main duct, it should be cannulated using a lacrimal dilator. After inserting the catheter into the cannulated main duct, the position of the catheter within the duct was confirmed by ultrasound. A contrast agent was injected until the patient felt fullness, and ultrasound (B-mode) was used to confirm whether the contrast agent filled the main canal and secondary and tertiary ducts. Then, power Doppler ultrasound was performed to determine whether the salivary gland had increased blood flow. Results: In 2 cases in this report, a power Doppler ultrasound scan showed a significant increase in blood flow after contrast medium injection, which was not observed on a preoperative scan. Conclusion: Power Doppler ultrasound was found to be a simple, safe, and effective tool for real-time sialography monitoring.

  • Image fusion is fast catching attention as Wagner pointed out in his 2006 version of the recent progress and development presented at the annual meeting of Society of Nuclear Medicine. Prototypical fusion of bone scan and radiograph was already attempted at in 1961 when Fleming et al. published an article on strontium-85 bone scan. They simply superimposed dot scan on radiograph enabling simultaneous assessment of altered bone metabolism and local bone anatomy. Indeed the parallel reading of images of bone scan and radiography, CT, MRI or ultrasonography has been practiced in nuclear medicine long since. It is fortunate that recent development of computer science and technology along with the availability of refined CT and SPECT machines has permitted us to open a new avenue to digitally produce precise fusion image so that they can readily be read, exchanged and disseminated using internet. Ten years ago fusion was performed using Bresstrahlung SPECT/CT and it is now achievable by PET/CT and SPECT/CT software and SPECT/CT hardware. The merit of image fusion is its feasibility of reliable assessment of morphological and metabolic change. It is now applicable not only to stationary organs such as brain and skeleton but also to moving organs such as the heart, lung and stomach. Recently, we could create useful fusion image of cardiac SPECT and 64-channel CT angiograph. The former provided myocardial metabolic profile and the latter vascular narrowing in two patients with coronary artery stenosis and myocardial ischemia. Arterial stenosis was severe in Case 1 and mild in Case 2.

Repair Integrity and Functional Outcomes after Arthroscopic Repair of Transtendinous Full-thickness Rotator Cuff Tears Minimum Two-year Follow-up

  • Background: To evaluate the clinical outcomes and associated repair integrity in patients treated with arthroscopic repair for a transtendinous rotator cuff tear followed by resection of the remnant rotator cuff tendon. Methods: Between July 2007 and July 2011, we retrospectively reviewed patients who were treated for transtendinous full-thickness tears in the tendinous portion of the rotator cuff by arthroscopic repair. Clinical outcomes were evaluated using the American Shoulder and Elbow Surgeons (ASES) score, the Shoulder Rating Scale of the University of California at Los Angeles (UCLA), the Constant-Murley score, a visual analogue scale (VAS) pain score, and range of motion (ROM). The repair integrity was determined by magnetic resonance imaging or ultrasonography. Results: There were 19 shoulders with transtendinous full-thickness tears in the tendinous portion of the rotator cuff. The ASES, UCLA, Constant-Murley, and VAS pain scores showed significant improvements in function and symptoms (all p<0.001). The active ROM for forward flexion and abduction was also significantly improved (p=0.002 and p<0.001, respectively). The postoperative radiological examination showed cuff integrity without a re-tear in 68.4% of patients. However, the UCLA, ASES, and Constant-Murley scores were not significantly different between healed and re-torn group (p=0.530, p=0.885, and p=0.262, respectively). Conclusions: Although repair of transtendinous rotator cuff tears followed by resection of the remnant rotator cuff tendon in the footprint has a relatively high re-tear rate, no significant difference was observed in the short-term clinical results between the re-tear and healed groups.

  • Infrequent voiding is defiled as two or less micturitions in a day without organic causes. It can 1ead to bladder capacity enlargement and increase in the volume of residual urine and as a consequence it may produce recurrent urinary tract infections(UTI) and or renal damages. We report a case of acute pyelonephritis due to infrequent voiding in a 13 year old girl. The imaging studies revealed floating debris in the bladder on VCUG and dilated ureter on ultrasonography, and parenchymal defects on 99mTc DMSA scan. (J.   • Acute lobar nephronia is a focal of acute bacterial nephritis, affecting one or more of the renal lobules. Nephronia is being diagnosed more frequently in accordance with the advancing imaging techniques of urinary tract. We report a case of acute nephronia in a 4 month old boy who presented persisting urinary tract infection after intial antibiotic treatment. Ultrasonography and computerized tomography showed round wedge-shaped, non-enhancing mass in right kidney. Gallium scan also revealed the lesion in the right kidney. This lesion resolved completely on one month follow up examinations. (J. Korean Soc Pediatr Nephrol 5 : 73- 7, 2001)

Cholelithiasis Complicated with Biliary Sludge and Urolithiasis in a Dog (개의 담낭 슬러지와 요 결석증을 동반한 담석증)

  • A 10-year-old intact female Miniature Schnauzer dog was referred with the primary complaint of persistent anorexia, remittent fever, vomiting and abdominal pain. Hemogram suggested a chronic inflammatory disease. Serum biochemistry showed moderate hepatobiliary cellular damage with severe cholestasis. Abdominal radiography and ultrasonography revealed hepatomegaly, choleliths and sludges in gall bladder and small stones in urinary bladder. Based on diagnostic findings, the case was diagnosed as cholelithiasis complicated with biliary sludge and urolithiais. Using cholecystectomy and cystectomy, choleliths and uroliths were removed from gall bladder and urinary bladder, respectively. The clinical condition was dramatically improved after surgery.
