A Study of Nerve Conduction Velocity of Normal Adults (정상성인의 신경전도속도에 관한 연구)

  • Nerve conduction studies help delineate the extent and distribution of the neural lesion. The nerve conduction was studied on upper(median, ulnar and radial nerves) and lower(personal, posterior tibial and sural nerves) extremities in 83 healthy subjects 23 to 66 years of age. and normal values were established(Table 1). The mean motor terminal latency (TL) were : median. 3.6(${\pm}0.6$)milliseconds ; ulnar. 2.9(${\pm}0.5$) milliseconds ; radial nerve. 2.3(${\pm}0.4$) milliseconds. Mean motor nerve conduction velocity(MNCV) along distal and proximal segments: median. 61.2(${\pm}9.1$) (W-E) and 57.8(${\pm}13.2$) (E-Ax) meters per second ; ulnar. 63.7(${\pm}9.1$) (W-E) and 50.(${\pm}10.0$) meters per second. Mean sensory nerve conduction velocity(SNCV) : median. 34.7(${\pm}6.7$) (F-W), 63.7(${\pm}7.1$) (W-E) and 62.8(${\pm}12.3$) (E-Ax)meters per second ; ulnar. 38.0(${\pm}6.7$)(F-W), 63.4(${\pm}7.5$) (W-E) and 57.0(${\pm}10.1$) (E-Ax)meters per second ; radial, 45.3(${\pm}6.8$) (F-W) and 64.2(${\pm}11.0$) (W-E) meters per second ; sural nerve, 43.4(${\pm}6.1$) meters per second. The amplitudes of action potential and H-reflex were also standardized. Mean H latency was 28.4(${\pm}3.2$) milliseconds. And. the fundamental principles, several factors altering the rate of nerve conduction and clinical application of nerve stimulation techniques were reviewed.

  • The trained examiner can gain considerable information from visual inspections of the elbow joint, Because much of the joint is subcutaneous, any appreciable alteration in the skeletal anatomy often is detectable. Gross soft tissue swelling or muscle atrophy is also early observed. Inspection and palpation of the medial and lateral epicondyles and the tip of the otecranon from an equilateral triangle with the elbow is flexed. Normally, the arc of flexion extension, although variable, ranges from about O to 140 degrees plus or minus 10 degrees. The posterolateral rotatory instability(PLRI) of the elbow is most common pattern of elbow instability. The lateral collateral ligament complex also includes a narrow but stout band of ligamentous tissue blending with the distal and proterior fibers of the capsule to insert distally on the crista supinatoris of the ulna. This is the lateral ulnar collateral ligament(LUCL). A clinical elbow pivot shift test confirms the PLRI. There are also two active apprehension signs.

  • 발음성 삼두근 증후군은 주관절을 굴곡하거나 신전할 때 삼두근 말단부 일부와 척골 신경이 내상과 전방으로 탈구되는 드문 질환이다. 이학적 검사 만으로는 다른 질환으로 오진되는 경우가 많아 정확한 진단이 필수적이며 확진을 위하여는 척골 신경 및 삼두근의 비정상적인 움직임을 관찰할 수 있는 영상 검사가 필요하다. 초음파 검사는 자기 공명 영상 촬영 등 다른 검사에 비해 간편하고 효과적으로 연부 조직의 동적 검사를 시행할 수 있는 장점이 있다. 저자들은 발음성 삼두근 증후군 환자를 동적 초음파 검사를 사용하여 진단하고 척골 신경 전방 전위술 및 삼두근 내두의 이전술을 시행하여 치유하였기에 문헌 고찰과 함께 초음파 검사의 유용성을 보고하고자 한다.

Wrist Motion in Computer Keyboard Typing

  • The purpose of this study is to define how wrist motion is affected by different postures and supporting devices and to discover functional range of wrist motion for keyboard typing. The range of wrist motion (ROM) needed for fourteen experienced typists to type on a computer keyboard was measured by flexible and biaxial electrogoniometers. The most frequent wrist motion during typing was in extended and ulnarly deviated positions in both wrists. Range of wrist motion was similar in both wrists. The average ROM for keyboard typing with the typists' own posture was about 39 in flexion/extension (FEM) and 29 in radial/ulnar deviation (RUD) in both wrists. The range of wrist motion was significantly reduced to 30 in FEM and 27 in RUD with use of either wrist or forearm supporting devices, which suggests that these devices might help to relieve fatigue, discomfort, or pain during and/or after typing. Results of this study will be of interest to clinicians and helpful to those who are professionally or non-professionally involved in typing.

A Case of Acute Cerebral Motor Cortical Infarction Coincidentally Occurred in Segmental Zoster Paresis (분절성 대상포진마비와 동시에 발병한 급성 뇌운동피질경색 1예)

  • Segmental zoster paresis (SZP) is a clinically rare complication of herpes zoster. But it has not been reported that acute cerebral motor cortical infarction coincidentally occurred in SZP. A 86-year-old woman was admitted due to pain, tingling sensation, and weakness of left arm. She had an acute onset of pain and tingling sensation in left arm at first day, shoulder weakness at second day, and multifocal vesicles at fourth day. Deep tendon reflexes of left arm were decreased than right. Electromyography showed an axonal polyneuropathy at superior trunk level of left brachial plexus. Median and ulnar sensory evoked potential tests were normal. Brain MRI showed a high signal in right primary motor cortex on diffusion weighted image. We report a case of acute cerebral motor cortical infarction coincidentally occurred in SZP.

  • Occupational disease among typists such as Carpal Tunnel Syndrome(CTS) and tendonitis has increased along with rapid expansion of office automation. During typing, the posture can be defined as forearm pronation, ulnar abduction, wrist extension and finger flexion. The CTS results from awkward posture between forearm-wrist-hand and the keyboard arrangement. Therefore, ergonomic principles should be emphasized in keyboard design. The objective of the study is to improve keyboard design by analyzing anatomical posture of forearm and hand during typing. An experimental study was performed to investigate relationships between a keyboard and typing performance. Results showed that typing performance is dependent to angles and slopes of a keyboard. Statistical analysis indicated that the suggested ergonomic keyboard improved typing speed significantly(17%).

  • An unusual case is presented as bilateral, complete digital duplication of the hand in a 32-year-old man. Radiographic evaluation showed complete duplication of intact phalanges and metacarpal bones in both hands. Although polydactyly of the hand is reported to occur among approximately 1 in 1000 live births, most of these malformations are rudimentary skin tags. Complete postaxial polydactyly is uncommon; it occurs in approximately 0.014% of all live births. The main goal of the surgical treatment is to establish adequate function and appearance. This case reports the preoperative evaluation and management of bilateral postaxial type III duplication of the fingers.

A Case of Lewis-Sumner Syndrome Improved by Oral Steroid Therapy (경구 스테로이드 치료로 호전된 Lewis-Sumner 증후군 1예)

  • We present a case with stepwise weakness and sensory involvement of both hands for more than 2 months. His nerve conduction study findings revealed prolonged terminal latencies, decreased motor and sensory conduction velocities and conduction blocks of both ulnar nerves, more severely on left side. And there were other abnormalities manifested with mononeuropathy multiplex. Increased cerebrospinal fluid protein was found. We diagnosed him as Lewis-Sumner syndrome and tried high dose oral steroid therapy for 2 months. He showed improvement of motor functioning with persistent conduction block.

Humeral Hemimelia in a Holstein Calf (유우 송아지의 선천성 상완골 형성 부전)

  • Humeral hemimelia is a rare congenital anomaly characterized by deficiency of the humerus with relatively intact scapular. The holstein calf was healthy expecting the deformity of forelimb humerus. The examination of PCR and serum neutralizing test was carried about BVD, akabane virus, Ibaraki virus and Kasba virus in cattle. The results were all negative. Affected calf showed forelimb deformation. Other abnormalitis was not observed in brain, spinal cord, liver, kidney and other visceral organs. Amelia are very rare limb anomalies. We report on a Holstein calf with forelimb deficiencies. This case appears humeral anomalies with ulnar and radius aplasia in calf. The condition in these calves was considered the result of a recurrence of a genetic mutation affecting a putative hemimelia locus.

  • A male, 11-month-old, Shih-tzu dog weighing 5.2kg showing knuckling secondary to severe cranial bowing of left antebrachium with valgus deformity of the carpus was presented to the Veterinary Medicine Teaching Hospital of Seoul National University. Radiographic findings were a premature closure of the distal ulnar physis, a cranial bowing of the radius, subluxation of the elbow joint, and degenerative changes in the carpal joint. The patient was treated by performing an oblique osteotomy of the radius and ulna, followed by application of External Skeletal Fixations. At 18 weeks postoperatively, the radius was healed. An incongruency of the elbow joint and radial angular deformity was corrected.
