성인 심장수술 후 중환자실에 입실한 환자의 계획된 발관 후 비계획적 기관 재삽관 위험요인과 임상결과 (Risk Factors and Clinical Outcomes of Unplanned Reintubation after Planned Extubation in Adult Patients admitted to the Intensive Care Unit after Cardiac Surgery)

  • Purpose : This study aimed to identify risk factors for unplanned reintubation after planned extubation and to analyze the clinical outcomes in patients admitted to the intensive care unit after cardiac surgery. Methods : The study examined patients who underwent intubation and planned extubation admitted to the intensive care unit after cardiac surgery between January 1, 2017, and December 31, 2021. The reintubation group comprised 58 patients underwent unplanned reintubation within 7 days of planned extubation. The maintenance group comprised 116 patients who did not undergo reintubation and were matched with the reintubation group using the rational for matching criteria. Data were collected retrospectively from electronic medical records. We used the independent t-test, Mann-Whitney U test, 𝑥2-test, Fisher's exact test, and logistic regression analysis with SPSS/WIN 27.0. Results : The multivariate logistic regression analysis demonstrated that albumin (odds ratio [OR]=0.38, 95% confidence interval [CI]=0.20-0.72), surgery time (OR=1.54, 95% CI=1.20-1.97), PaO2 before extubation (OR=0.85 per 10 mmHg, 95% CI=0.75-0.97), postoperative arrhythmia (OR=2.82, 95% CI=1.22-6.51), reoperation due to bleeding (OR=4.65, 95% CI=1.27-17.07), and postoperative acute renal failure (OR=2.97, 95% CI=1.09-8.04) were risk factors for unplanned reintubation. The reintubation group had a higher in-hospital mortality rate (𝑥2=33.74, p<.001), longer intensive care unit stay (Z=-7.81, p<.001), and longer hospital stay than the maintenance group (Z=-8.29, p<.001). Conclusion : These results identified risk factors and clinical outcomes of unplanned reintubation after planned extubation after cardiac surgery. These findings should be considered when developing and managing an intervention program to prevent and reduce the incidence of unplanned reintubation.

Optical and Electrical Properties of InAs Sub-Monolayer Quantum Dot Solar Cell

  • 본 연구에서는 분자선 에피택시 (MBE)법으로 성장된 InAs submonolayer quantum dot (SML-QD)을 태양전지에 응용하여 광학 및 전기적 특성을 평가하였다. 본 연구에서 사용된 양자점 태양전지(quantum dot solar cell, QDSC)의 구조는 n+-GaAs 기판 위에 n+-GaAs buffer와 n-GaAs base layer를 차례로 성장 한 후, 활성영역에 InAs/InGaAs SML-QD와 n-GaAs spacer layer를 8주기 형성하였다. 그 위에 p+-GaAs emitter, p+-AlGaAs window layer를 성장하고 ohmic contact을 위하여 p+-GaAs 를 성장하였다. SML-QD 구조의 두께는 0.3 ML 이며, 이때 SML-QD의 적층수를 4 stacks 으로 고정하였다. SML-QD 와의 비교를 위하여 2.0 ML크기의 InAs자발 형성 양자점 태양전지(SK-QDSC)과 GaAs 단일 접합 태양전지 (reference-SC)를 동일한 성장조건에서 제작하였다. PL 측정 결과, 300 K에서 SML-QD의 발광 피크는 SK-QD 보다 고에너지에서 나타나는데(1.349 eV), 이것은 SML-QD가 SK-QD보다 작은 크기를 가지기 때문으로 사료된다. SML-QD는 single peak를 보이는 반면, SK-QD는 dual peaks (1.112 / 1.056 eV)을 확인하였다. SML-QD의 반치폭(full width at half maximum, FWHM)이 SK-QD에 비하여 작은 것으로 보아 SML-QD가 SK-QD보다 양자점 크기 분포의 균일도가 높은 것으로 해석된다. Illumination I-V 측정 결과, SML-QDSC의 개방 전압(VOC) 과 단락전류밀도(JSC)는 SK-QDSC의 값과 비교해 보면, 각각 47 mV와 0.88 mA/cm2만큼 증가하였다. 이는 SK-QD보다 상대적으로 작은 크기를 가진 SML-QD로 인해 VOC가 증가되었으며, SML-QD가 SK-QD 보다 태양광을 흡수할 수 있는 영역이 비교적 적지만, QD내에 존재하는 energy level에서 탈출 할 수 있는 확률이 더 높음으로써 JSC가 증가한 것으로 분석 된다.

${Y_2}{SiO_5}:Ce$ 청색 형광체의 표면 코팅에 따른 음극선 발광특성 (The Cathodoluminance Properties of ${Y_2}{SiO_5}:Ce$ Blue Phosphor with Surface Coatings)

  • $Y_2SiO_5:Ce$ 형광체를 $In_2O_3$, $Al_2O_3$$SiO_2$로 코팅한 후 전계 방출 디스플레이에 요구되는 음극선 발광 특성을 조사하였다. $Al_2O_3$ 코팅으로 $Y_2$SiO(sub)5:Ce 형광체의 발광 효율과 에이징 특성은 감소되었다. 한편 $Al_2$$O_3$코팅으로 형광체의 발광효율은 증가하였으나 발광스펙트럼과 색좌표는 일부 변화하였다. 그러나 $Y_2SiO_5:Ce$ 청색 형과체의 발광 효율은 $SiO_2$코팅으로 크게 증가하였으며, 또한 $SiO_2$ 코팅한 $Y_2SiO_5:Ce$ 형광체의 에이징 특성은 코팅 전에 비하여 크게 향상되었다.

誘導結合플라스마 質量分析法에 의한 石炭의 元素 分析 (Elemental Analysis of Coal by Inductively Coupled Plasma Mass Spectrometry)

  • A simple, fast yet sensitive method is described for the determination of fifty elements in coal by inductively coupled plasma mass spectrometry. The method involves complete dissolution of coal with mixed acid $(HNO_3, HF, HClO_4)$ in hish pressure Teflon bomb and subsequent inductively coupled plasma mass spectrometric(ICP-MS) measurement. The accuracy of the method, being evaluated by the analysis of NIST SRMs (1632a, 1632b) is better than 20% RSD for most elements. The limits of detection defined by two times $\sigma$ (standard deviation of operational blank) are in order of sub-ppm to ppm, which are low enough to quantitate most elements. However, the determiantions for few elements such as V, As, Se are severely interfered by molecular ions such that their accurate determiantions are not possible. Analytical results for twentyon coals from eight countries including six ones world major coal producing, Korea, Japan, China, I.C.S., U.S.A., Canada, Australia, and South Africa are presented. While the results for major elements agree well with the existing ones, those for a few trace elements disagree considerably. The existing values are considerably higher. It is not possible to judge which are more accurate. However, the exisisting values are more likely to be errorous inasmuch as they are obtained without using high purity reagents and clean laboratory techniques.

The Comparisons of GRBAS Perceptual Judgments according to Levels of Utterances

  • The present study was performed to investigate adequate levels of utterances which can give essential as well as useful information about the patients' voice, by examining the degrees of correlation between the levels of utterances (vowels, words, and phrase paragraph reading) and the entire utterance including all of the levels. For this purpose, a total of 10 individual utterance samples (5 vowels, 3 words, 1 phrase, 1 paragraph reading) were collected from each of the 30 subjects with voice disorder patients, and four experienced voice therapists evaluated them using GRBAS. The results showed that four therapists highly agreed upon on 'G' parameter. The coefficient of the correlation between each level of utterance and entire utterance tended to be above 0.70. Judgements of the vowel /$\varepsilon$/ as well as /o/ highly correlated with the judgement of the entire utterance. Regardless of severity, the judgement of the entire utterance highly correlated with the judgements of the vowel /u/ and the paragraph reading. These results suggest that experienced voice therapists can precisely evaluate patients' voice quality with only one sustained vowel in the clinic field, as is done with the entire utterance evaluation.

팔당호 식물플랑크톤의 제한영양염과 성장률의 경시적 변화 (Temporal Changes of Limiting Nutrients and Phytoplankton Growth Rate in Lake Paldang)

  • 팔당호 식물플랑크톤의 제한영양염과 생리학적 성장 특성을 평가하기 위하여 2002년 3월부터 10일까지 팔당호의 수질환경 조사와 함께 실내 배양실험을 실시하였다. 총인의 농도와 Chl. a의 상관성 분석결과, 팔당호의 호수생산성은 인의 영향을 많이 받으며 TN/TP ratio와 식물플랑크톤의 성장잠재력 실험을 통해서도 제한영양염은 인으로 평가되었다. 특히, 봄철이 다른 계절에 비해 상대적으로 용존무기인이 낮은 농도로 존재하기 때문에 높은 인 제한을 받는 것으로 나타났다. 그러나 인의 제한정도는 계절적인 변이를 나타내었으며, 시기에 따라 질소와 규소에 의한 제한가능성도 나타났다. 팔당호 식물플랑크톤 군집의 최대성장률(${\mu}_{max}$)은 0.8${\sim}$l.1$day^1$의 범위로 나타났으며, 반포화농도($K_u$)는 0.1${\sim}$O.8${\mu}M$로 5월에 $0.8{\mu}M$로 가장 높고 9월에 $0.1{\mu}M$로 가장 낮은 것으로 조사되었다. 이는 영양염 흡수에 영향을 미치는 인의 cell quota와 관련이 있는 것으로 나타났으며, 5월에 $0.13{\mu}gP/{\mu}gChl.$ 3로 낮은 값을 나타냈다. 식물플랑크톤의 성장특성 분석을 통해 팔당호에서는 봄에 비해 여름과 가을철에 발생한 식물플랑크톤의 성장이 경쟁적으로 빠르게 발달할 수 있는 잠재성이 있는 것으로 판단된다. 결과적으로, 팔당호 식물플랑크톤 성장은 주로 인에 의하여 제한되며 여름 몬순 이후 수체의 안정기동안 높은 성장을 할 수 있는 생리적 특성을 가지고 있는 것으로 추정된다.

살충제 buprofezin의 가수분해 반응 메카니즘 (Kinetics and mechanism of hydrolysis of insecticidal buprofezin)

  • [ $45^{\circ}C$ ]의 15%(v/v) dioxane 수용액중에서 살충성 buprofezin(IUPAC : tert-butylimino-3-iso-propyl-5-phenylperhydro-1,3,5-thiadiazin-4-one)의 가수분해 반응속도상수와 pka상수(5.60)를 측정하고 pH-효과, 용매효과(m=0.34, n=2.45 및 $1{\gg}m$), 열역학적 활성화 파라미터(pH 4.0, ${\Delta}H^{\neq}$= 11.12 $kcal{\cdot}mol^{-1}$, ${\Delta}S^{\neq}$=-5.0e.u. 및 $E_{act.}$=11.76Kcal), 반응 속도식등의 반응 속도론적 및 생성물분석(1-isopropyl-3-phenyl urea) 등의 비속도론적 실험결과를 얻었다. 이들 자료의 검토로부터 pH 8.0이하의 산성용액에서는 특정($k_{H3O+}$)및 일반 산-촉매반응에 의한 $A-S_{E}2$형 및 A-2(또는 $A_{AC}2$)형 반응, 그리고 pH 9.0이상의 알카리성 용액에서는 일반염기 촉매반응($k_{H2O}$)에 의한 친핵성 첨가-제거 ($Ad_{N}-E$) 반응이 사면체($sp^{3}$) 중간체를 경유하는 궤도-조절 반응으로 진행되는 일련의 가수분해 반응메카니즘을 제안하였다. 또한, Buprofezin은 산성(pH8.0이하)용액보다 염기성(pH8.0이상) 용액중($k=10^{-8}sec.^{-1}$)에서 더욱 안정하였으며 $45^{\circ}C$의 중성(pH 7.0) 수용액 중에서 반감기($t=\frac{1}{2}$)는 약 3개월이었다.

과학(科學)과 과학교육(科學敎育)에 대한 중등과학교사(中等科學敎師)의 태도(態度) 조사연구(調査硏究) (A Survey on Attitudes Toward Science and Science Teaching Among the Secondary School Science Teachers)

  • The student's attitude toward science is generally influenced by their teachers. Therefore, teachers' positive attitudes toward science and science teaching play an important role to change the student's attitude toward science. The purpose of this survey is to investigate the status on attitudes toward science and science teaching among the secondary school science teachers in Korea. The attitudes were surveyed by the questionnaire which was developed by Sung-Jae Pak. The instrument is designed to use Likert type scale and is composed of two kinds of scale: one is the attitudes toward science scale (the AT scale) and the other is the attitudes toward science teaching scale (the AT Kale), which contains 24 questions respectively. Each of them has 6 sub-level areas. The six areas of attitude toward science are as follow: The goals and values, process and method of science, knowledge and the view of nature, social and cultural aspects, scientist and a career in science, the preferences and willingness. The six areas of attitude toward science teaching are as follow: The goals and values, process and method of science teaching, the content and structure, social and cultural aspects, science educator and a career in science teaching, satisfaction and willingness. From 152 teachers' responses, the tendency of total as well as each area and the contrast of their backgrounds at the level of 5% significance were analyzed by SPSS computer program. Some results and conclusion of the study are as follow: 1. The overall attitude of the measured secondary school science teachers shows a positive trend tendency. Also total positive attitude toward science teaching are slightly higher than that of science, which support the fact that the teachers are not scientists but they are directly involved in teaching of science. 2. The attitudes toward science are moderately correlated to the attitudes toward science teaching (r=0.52). 3. The areas of knowledge, nature-view and tile area of social aspects of science show a very negative tendency. Also the two areas are not correlated to some other areal at the level at 5% significance. 4. Female science teachers exhibit just a little more positive attitudes than those of male teachers in science teaching. 5. The science teachers who wanted to have a profession of scholar or educator exhibit a little more positive attitudes than others in science and total attitudes (AS+AT). 6. The more the science teachers have "intellectual delights" the more their attitudes toward science and science teaching are positive. 7. At the level of 5% significance, there are differences which college they graduated from, but there are no differences in multiple comparison at 10% level. 8. The differences in their background dose not appear in such as age; teaching career; academic career; deny the superstition; their onlook for the scientist or educator when they were freshmen; major the basic science; opinions about the U.F.O. and the origin of life. 9. The responses of certain individual statement are quite different from the overall tendencies, which strongly suggest the de1ailed analysis and deeper study. For the continuing study, it is recommendable to revile the measuring tools with the theoretical study for the better validity and reliability, and investigate the status of the attitudes toward science and science teaching among the science teachers with sufficient samples.
