Despite the increasing number of osteochondral lesions of the talus, there are a lack of definite evidence-based treatment protocols. Several types of treatments are available, each having their advantages and disadvantages. First-line therapy consists of well-conducted conservative treatment. Surgical treatment is the second choice. Treatments are chosen based on the size of the lesion, location, chronicity, and the condition of the neighboring cartilage. This article reviews the current updates in the treatment of osteochondral lesions of the talus to help clinicians use the available treatment strategies more efficiently.
Many articles on operative treatment strategies for osteochondral lesions of the talus (OLT) have been published. On the other hand, there are only a few reports on the conservative treatment of OLT. Understanding the natural history of OLT is essential to establishing the correct treatment. Conservative treatment might lead to comparable clinical outcomes compared to the current options of the operative treatment in isolated OLT lesions. OLT lesions can be divided into two (or possibly three) types. Hence, the prognosis and outcome of certain operative treatments should be evaluated further. The prognosis of OLT combined with instability or malalignment appears poorer than that of isolated OLT lesions. Therefore, concomitant pathologies should be evaluated together when treating patients with OLT.
Thirty-two cases of tetralogy of Fallot corrected totally using extracorporeal circulation in this department are presented during the period from April 1983 to Feb. 1986. Types of right ventricular outflow tract obstruction were a case of pulmonic valvular stenosis, 3 cases of infundibular stenosis, and 28 cases of combined type. There were associated anomaly such as 3 cases of pulmonary arterial hypoplasia, 7 cases of atrial septal defect, a case of left superior vena cava, and 2 cases of right side aortic arch. Transannular patch for right ventricular outflow tract reconstruction was necessary in 12 cases. Operative death was 6 cases and late death was a case, but other remaining cases followed up over 2 months carried out normal life.
Eleven patients with esophageal diverticulum were operated on between August 1982 and August 1992 at the Department of Thoracic and Cardiovascular Surgery of Chonnam National University Hospital have been studied. 9 patients were male and 2 were famale and the age distribution was between 20 and 55 years. Various subjective symptoms were noticed preoperatively. Diagnosis was confirmed by esophagography. The types of esophageal diverticulum were traction type in 6 cases and pulsion type in 5 cases. There were nine cases of midesophageal diverticula, 1 cases of upper-thoracic esophageal diverticulum and 1 cases of epiphrenic diverticulum. Diverticulectomy alone in 10 cases and diverticulopexy was performed in 1 case. Fistulectomy in 5 cases, lobectomy in 1 case, segmentectomy in 1 case, and Eloesser`s procedure in 1 case were performed with associated procedures. There were no death or morbidity and all patients have achieved marked improvement of symptoms except three patients who had a concomitant mucoepidermoid tumor, had a complicated postoperative pyothorax, and had a postoperative recurrence of midesophageal diverticulum. Recurrence of symptoms were not noticed during follow up except 1 recurrence of diverticulum confirmed by esophagogram.
Loupes-assisted microsurgery often needs an operational head lamp for a better condition of light. This may cause serious neck fatigue due to the weight the operational head lamp, the limitation of the operator's head movement from the hind cable, and some cost for purchasing and management of it. On the contrary, a mountain head lamp is very light, full of freedom in movement, applicable to all types of loupes, and relatively very cheap. In conclusion, a mountain head lame can be a cost-effective light source for Loupes-assisted microsurgery.
Dissecting aortic aneurysm is a disease which is characterized by hemorrhagic intramural seperation of aortic wall and extension for varlng distances proximally, distally, or both from the site of the intimal tear. Most aortas show some type of medial degeneration most commonly described as cystic medial necrosis. DeBackey classified this disease according to involved aorta and site of intimal tear to 3 basic types, such as type I, II and III. Type III is defined that dissecting process arrises in the descending thoracic aorta just distal to origin of the left subclavian artery and extends distally for a varing distance. We expirienced a case of dissecting aneurysm, type III of DeBackey's classification which dissecting process is limited to the descending thoracic aorta in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. This patient was 40 year old woman and she had suffered from intermittent sharp back pain for 3 years .before admission. Excision of the aneurysm and Dacron graft were placed successfully under the left atrio-femoral bypass with artificial pump. The hospital course was uneventful.
Angiokeratomas represent vascular lesions and are histologically characterized by superficial dermal vascular ectasia with overlying epidermal hyperkeratosis. Angiokeratoma circumscriptum is one of five types in the group of the angiokeratomas. The lesion is present from birth but may appear in childhood or adulthood. Angiokeratoma circumscriptum is typically hyperkeratotic papules or nodules situated unilaterally on the foot or distal aspect of the leg, but can occur elsewhere on the body. Isolated tongue involvement is very rare. We report a 12-year-old boy with angiokeratoma circumscriptum of the posterior dorsal tongue with a review of the literatures.

Basically stem cells have characteristics of multi-potency, differentiation into multiple tissue types, and self-renew through proliferation. Recent advances in stem cell biology can make identifying the stem-cell like cells in various mammalian tissues. Stem cells in various tissues can restore damaged tissue. Stem cells from the adult nervous system proliferate to form clonal floating colonies called spheres in vitro, and recent studies have demonstrated sphere formation by cells in the tympanic membrane, vestibular system, spiral ganglion, and partly in the organ of Corti. The presence of stem cells in the ear raises the possibilities for the regeneration of the tympanic membrane & inner ear hair cells & neurons. But the gradual loss of stem cells postnatally in the organ of Corti may correlate with the loss of regenerative capacity and limited hearing restoration. Future strategies using endogenous stem cells in the ear can be the another treatment modality for the patients with intractable inner ear diseases.
Flap thinning is a procedure for making a thick flap thinner. This procedure does more than simply fill in the defected area, and it is better for reconstructing the area both functionally and aesthetically. However, because flap thinning is a rather blind procedure, it may have harmful effects on flap vascularity. Therefore, the vasculature of a flap must be understood before performing flap thinning. This paper analyzes the basic anatomy underlying flap thinning based on the previous anatomic study that categorized flaps into 6 types by their vascular structures. This paper also reviews specific studies of frequently practiced flap procedures (deep inferior epigastric artery perforator flap, thoracodorsal artery perforator flap, and anterolateral thigh flap) and presents important precautions for flap thinning procedures. Finally, this paper briefly examines the axiality of the subdermal plexus, which needs to be taken into account when performing flap thinning.
Mediastinal tumors have long fascinated the thoracic surgeon of their variety and unpredictability of iagnosis prior to exploration. We report the analysis of the 34 cases of mediastinal tumors, experenced in the dept.of the Thoracic and Cardiovascular Surgery of the National Medical Center.Between the 1993.9-1992.12. The age distribution was relatively even and the mean age was 35 years old.The thymomas were 16 cases [ 47% ].the lipoma were 4 cases [12%].the germ cell tumors were 7 cases [20%].the neurogenic tumors were 3 cases [8%].Histologically analysised .The malignant tumors were 16 cases [17.6%] in classified by hisotlogical types.the tumor size, location, and the clinical manifestation are presented. The successful removal was done in 28 cases [ 100%] among 28 cases of benign mediastinal tumors. Among the 6 cases of malignancy mediastinal tumors, the surgical intervention had done in 5 cases [ 83%] and inoperatable cases were 1 case [16.6%].There was no operative death.
