Background: Stroke is a neurological disorder characterized by an impaired static balance. A change in poor posture after stroke may worsen static balance. The balance control through an upright posture may include kinesiology taping of the middle back. Objectives: To investigated the effect of kinesiology taping of middle back on static balance in patients with stroke. Design: A randomized controlled trial. Methods: A total of 10 patients with stroke were divided into two groups. The experimental and control groups received kinesiology taping and placebo taping of the middle back, respectively. After 24 h, static balance (i.e., sway area and path length) was measured in closed eyes condition. Results: The experimental group (kinesiology taping group) showed a significant decrease in sway area and path length after the intervention. In addition, kinesiology taping group showed a significant decrease in sway area and path length compared to the control group. Conclusion: Kinesiology taping of the middle back can improve static balance in stroke patients.
Objectives : It is known that cigarette smoking is associated with increased risk of both thrombotic and hemorrhagic stroke. However, in Korea, especially in the academic world of Korean Medicine, there is a lack of study about the relation between cigarette smoking and stroke. We carried out a case-control study to clarify the relation between cigarette smoking and stroke. Method : We interviewed 441 stroke patients (236 men, 205 women) as a case group and 432 non-stroke patients (208 men, 224 women) as a control group. We investigated the smoking pattern of all patients and stroke-subtype of the case group. Smoking pattern is classed into two (Class I), four (Class II) and five groups (Class III). Class I consists of current non-smokers and current smokers. Class II consists of non-smokers, former smokers, current light smokers and current heavy smokers. Class III consists of never smoked, secondhand smokers, former smokers, Current light smokers and current heavy smokers. Stroke-subtype consists of cerebral infarction and cerebral hemorrhage. Results : The percentage of current smokers of case group is higher significantly than that of control group. The number of cigarettes smoked per day is associated positively with the risk of stroke. According to our study, generally cigarette smoking is related with ischemic stroke. The percentage of secondhand smokers of the case group is lower than that of the control group. According to our study, characteristically secondhand smoking is associated with hemorrhagic stroke. Conclusions : From the above results we found that cigarette smoking is an important risk factor of stroke - especially cerebral infarction - and the number of cigarettes smoked per day is associated positively with the risk of stroke. Characteristically according to our study, secondhand smoking is associated with hemorrhagic stroke. Overall we conclude that cigarette smoking may be an important preventable factor for stroke.
Objectives : Coldness of hands and feet has been regarded to be associated with blood circulation. This study was performed to assess the correlation between erythrocyte deformability and coldness of hands and feet. Methods : 145 patients who had taken an erythrocyte deformability test were selected for this study among hospitalized patients within 4 weeks after their cerebral infarction at the Internal Medical Department at Kyunghee Oriental Medical Center from August 2008 to August 2009. We divided the selected patients into two groups, with and without of coldness of hands and feet groups. The two groups were compared on risk factors for atherosclerosis and examined for variables including erythrocyte deformability. Results : 1. On demographic variables of the patients, only body mass index was significantly lower in the coldness of hands and feet group than the control group. There was no significant difference of the rate of hypertension, diabetes mellitus, hyperlipidemia, ischemic heart disease, smoking, drinking and carotid artery stenosis between the two groups. 2. According to blood test, the coldness of hands and feet group showed significantly lower erythrocyte deformability index than the control group. 3. Body mass index and erythrocyte deformability index showed a close relationship with coldness of hands and feet in multivariate analysis. Conclusions : The correlation between the erythrocyte deformability and coldness of hands and feet in cerebral infarction patients were decreased erythrocyte deformability participated in the mechanism of coldness of hands and feet.
Two stroke low speed diesel engines are mainly used for marine propulsion or power plant prime mover. These have many merits such as higher thermal efficiency, mobility and durability. However various annoying vibrations sometimes occur in ships or at the plant itself. Of these vibrations, torsional vibration is very important and it should be carefully investigated during the initial design stage for engine's safe operation. In this paper authors suggest a new estimation method of for shaft's can be calculated equivalently from accumulated fatigue cycles number due to torsional vibration. The 6S70MC-C($25,320ps{\times}91rpm$) engine for ship propulsion was selected as a case study, and the accumulated fatigue cycles numbers for shafting life time converted from the measured angular velocity and torsional vibration stress was calculated. This new method can be realized and confirmed in test model ship with two stroke low speed diesel engine.

This study aims to investigate the impact of the High Pressure Selective Catalytic Reduction system (SCR-HP) on a large marine two-stroke engine performance parameters by employing thermodynamic modelling. A coupled model of the zero-dimensional type is extended to incorporate the modelling of the SCR-HP components and the Control Bypass Valve (CBV) block. This model is employed to simulate several scenarios representing the engine operation at both healthy and degraded conditions considering the compressor fouling and the SCR reactor clogging. The derived results are analysed to quantify the impact of the SCR-HP on the investigated engine performance. The SCR system pressure drop and the cylinder bypass valve flow cause an increase of the engine Specific Fuel Oil Consumption (SFOC) in the range 0.3-2.77 g/kWh. The thermal inertia of the SCR-HP is mainly attributed to the SCR reactor, which causes a delayed turbocharger response. These effects are more pronounced at low engine loads. This study supports the better understanding of the operating characteristics of marine two-stroke diesel engines equipped with the SCR-HP and quantification of the impact of the components degradation on the engine performance.
A conventional tuned mass damper (TMD) provides a passive control option to suppress the structures' wind- or earthquake-induced vibrations. However, excessive displacements of the TMD raise concerns in the practical implementation. Therefore, this study proposes a novel TMD designed for and deployed on a high-rise sightseeing tower. The device consists of an integrated two-way slide rail mount and an eddy current damper (ECD) with a stroke control mechanism. This stroke control mechanism allows the damping coefficient to automatically increase when the stroke reaches a predetermined value, preventing excessive damper displacements during large earthquakes. The corresponding two-stage damping parameters are designed with a variable-thickness copper plate to enable the TMD stroke within a specified range. Thus, this study discusses the detailed design schemes of the device components in TMD. The designed two-stage damping parameters are also numerically verified, and the structural responses with/without the TMD are compared. As seen in the results, the proposed TMD yields effective control authority to limit the acceleration response within a comfort level. In addition, this TMD resolves the spatial availability for the damper movement in high-rise buildings by the controllable damping mechanism.
Purpose: The aim of this study was to evaluate the clinical characteristics of classic heat stroke in Korea and to identify factors of prognosis for heat stroke by comparing a survival group with a non-survival group. Methods: We retrospectively analyzed 27 patients with heat stroke who visited the Emergency Department of Kyungpook National University Hospital from March 2001 to February 2005. First, we divided the patients into two groups, the classic heat stroke group and the exertional heat stroke group, and compared them. Second, we compared the survival group with the non-survival group. Age, sex, cause, place where patients were found, underlying diseases, cooling time, performance of endotracheal intubation, initial Glasgow Coma Scale, initial vital sign, and laboratory findings were reviewed. Results: Five of 27 patients in heat stroke died. The classic heat stroke group had 20 patients. They were old and had more patients in the bathroom than the exertional heat stroke group had. The non-survival group showed lower blood pressure, lower initial GCS score, and higher respiratory rate than the survival group. In laboratory findings, the non-survival group also showed lower$HCO_3-$ level, lower albumin level, lower glucose level, more prolonged PT, and higher CK-MB level than the survival group. Delay in recognition of heat stroke and cooling were poor prognostic factors in heat stroke. Conclusion: The classic heat stroke group had patients who were old and found in the bathroom. Early recognition and treatment of heat stroke is important to reduce mortality. Cooling time, initial GCS score, mean arterial pressure, resipratory rate, $HCO_3-$, PT, CK-MB, and albumin seem to be meaningful when forming a prognosis for heat stroke patients.
In this study , a simple method has been developed to detect the required spark voltage by using the primary spark voltage instead of the secondary spark voltage. Through engine motoring experiments, this method testified to be quite satisfactory. Though the required spark voltage is affected by many in-cylinder conditions, temperature is one of the most important factors. The temperature increases significantly by combustion and the required spark voltage also changes by the temperature during the expansion stroke. On the basis of this fact, misfire can be monitored by comparing the required spark voltage between compression stroke and expansion stroke. So, in this study, two step ignition method is introduced to monitor combustion at expansion stroke. The test result shows that this method can be used to detect complex misfire pattern.
Purpose: The purpose of this study was to investigate the effects of aquatic exercise on rehabilitation in home stroke patients. Method: A quasi-experimental pretest and posttest design was used to examine the change of balance, ADL, IADL, pain, depression and self-efficacy atthe completion of 6-weeks aquatic exercise program. The subjects consisted of 25 adults who had a stroke. Thirteen experimental and twelve control subjects completed pre and post test measures. Outcome variables were balance, ADL, IADL, pain, depression and self-efficacy. Chi-Square test(Fisher's exact method) was used to examine the equality of the subjects and Wilcoxon Rank Sum Test used to examine the group difference by using SAS. Results: There was no significant difference in balance and depression between the two groups. However, there was a significant difference in ADL, IADL, pain and self-efficacy between the two groups. Conclusion: Aquatic exercise can improve ADL, IADL, self-efficacy and reduce pain. Therefore, further research is recommended by increasing the length of aquatic exercise and number of subjects, so that its effects can be more generalizable.
A hiccup is an involuntary, spasmodic contraction of the diaphragm accompanied by a sudden closure of the glottis, which is reported commonly in patients with brain stem disease such as ischemic stroke, dolichoectatic basilar artery, tumor, encephalitis, and multiple sclerosis. 1) Intractable hiccup is an uncommon, chronic and incapacitating disturbance defined as a hiccup bout lasting more than 48hours or recurring despite various treatments and affecting male subjects more than female. 2) Constipation and hiccup are common symptoms in stroke patients and purgation therapy has been often used. We discovered two patients who had a hiccup symptom after purgation therapy(diarrhea) and so reported course and result of treatment.
