Head-to-Head Comparison between Xpert MTB/RIF Assay and Real-Time Polymerase Chain Reaction Assay Using Bronchial Washing Specimens for Tuberculosis Diagnosis

  • Background: With the introduction of Xpert MTB/RIF assay (Xpert), its incorporation into tuberculosis (TB) diagnostic algorithm has become an important issue. The aim of this study was to evaluate the performance of the Xpert assay in comparison with a commercial polymerase chain reaction (PCR) assay. Methods: Medical records of patients having results of both Xpert and AdvanSure TB/NTM real-time PCR (AdvanSure) assays using the same bronchial washing specimens were retrospectively reviewed. Results: Of the 1,297 patients included in this study, 205 (15.8%) were diagnosed with pulmonary TB. Using mycobacterial culture as the reference method, sensitivity of the Xpert assay using smear-positive specimens was 97.5%, which was comparable to that of the AdvanSure assay (96.3%, p=0.193). However, the sensitivity of the Xpert assay using smear-negative specimens was 70.6%, which was significantly higher than that of the AdvanSure assay (52.9%, p=0.018). Usng phenotypic drug susceptibility testing as the reference method, sensitivity and specificity for detecting rifampicin resistance were 100% and 99.1%, respectively. Moreover, a median turnaround time of the Xpert assay was 1 day, which was significantly shorter than 3 days of the AdvanSure assay (p<0.001). Conclusion: In comparison with the AdvanSure assay, the Xpert assay had a higher sensitivity using smear-negative specimens, a shorter turnaround time, and could reliably predict rifampin resistance. Therefore, the Xpert assay might be preferentially recommended over TB-PCR in Korean TB diagnostic algorithm.

  • Transportation provides an infrastructure vital to economic growth, and it is also an integral part of production. As a port is regarded as the interface between the maritime transport and domestic transport sectors, it certainly play a key role in any economic development. Ship's delay caused by port congestion has recently has recently attracted attended with the analysis of overall operation in port. In order to analyse complicated port operation which contains large number of variable factors, queueing theory is needed to be adopted, which is applicable to a large scale transportation system in chiding ship's delay in Inchon port in relation to ship's delay problem. The overall findings are as follows ; 1. The stucture of queueing model in this port can be represented as a complex of multi-channel single-phase 2. Ship's arrival and service pattern were Poisson Input Erlangian Service. 3. The suitable formula to calculate the mean delay in this port, namely, $W_q={\frac{{\rho}}{{\lambda}(1-{\rho})}}{\frac{e{\small{N}}({\rho}{\cdot}N)}{D_{N-1}({\rho}{\cdot}N)}}$ Where, ${\lambda}$ : mean arrival rate ${\mu}$ : mean servicing rate N : number of servicing channel ${\rho}$ : utilization rate (l/Nm) $e{\small{N}}$ : the Poisson function $D_{(n-1)}$ : a function of the cumulative Poisson function 4. The utility rate is 95.0 percents in general piers, 75.39 percents in container piers, and watiting time 28.43 hours in general piers, 13.67 hours in container piers, and the length of queue is 6.17 ships in general piers, 0.93 ships in container piers, and the ship turnaround time is 107.03 hours in general piers, 51.93 hours in container piers.

  • We, on behalf of the Korean Gemini Office (KGO), introduce three proposal routes besides a standard semester program available for the Korean Gemini users: Fast Turnaround (FT), Director's Discretionary Time (DDT), and Poor Weather (PW). By presenting useful statistics and some examples implemented through these routes, we aim to provide the KAS members insights how well these observing routes have been utilized by the Gemini partners. Finally, we provide several useful suggestions to the KAS community for preparing these programs.

Characteristics of Occupational Exposure to Benzene during Turnaround in the Petrochemical Industries

  • Objectives: The level of benzene exposure in the petrochemical industry during regular operation has been well established, but not in turnaround (TA), where high exposure may occur. In this study, the characteristics of occupational exposure to benzene during TA in the petrochemical companies were investigated in order to determine the best management strategies and improve the working environment. This was accomplished by evaluating the exposure level for the workers working in environments where benzene was being produced or used as an ingredient during the unit process. Methods: From 2003 to 2008, a total of 705 workers in three petrochemical companies in Korea were studied. Long- and short-term (< 1 hr) samples were taken during TAs. TA was classified into three stages: shut-down, maintenance and start-up. All works were classified into 12 occupation categories. Results: The long-term geometric mean (GM) benzene exposure level was 0.025 (5.82) ppm (0.005-42.120 ppm) and the short-term exposure concentration during TA was 0.020 (17.42) ppm (0.005-61.855 ppm). The proportions of TA samples exceeding the time-weighted average, occupational exposure level (TWA-OEL in Korea, 1 ppm) and the short-term exposure limit (STEL-OEL, 5 ppm) were 4.1% (20 samples of 488) and 6.0% (13 samples of 217), respectively. The results for the benzene exposure levels and the rates of exceeding the OEL were both statistically significant (p < 0.05). Among the 12 job categories of petrochemical workers, mechanical engineers, plumbers, welders, fieldman and scaffolding workers exhibited long-term samples that exceeded the OEL of benzene, and the rate of exceeding the OEL was statistically significant for the first two occupations (p < 0.05). Conclusion: These findings suggest that the periodic work environment must be assessed during non-routine works such as TA.

  • This paper proposes an improved scheduling model and algorithm which threads in the fixed-priority policy have faster response time and turnaround time than existing ones in SRT system. Sub run queue is added into the system and is used for the quick execution and aging of threads in the fixed-priority policy. Using simulation two method are compared. The results shows that the new scheduling model gets threads in the fixed-priority policy to run faster than existing ones.

  • This paper aims to compare four operating priority rules of RMGC (Rail Mounted Gantry Crane) used in semi-automated container terminal. The four priority rules employed in this paper are FCFS (First-come-first-served), LCFS (Last-come-first-served), TOS (Turn-over-served) and NFS (Nearest-first-served). And to compare the four operating priority rules, this paper analyzed productivity of RMGC and CC (Container Crane), waiting time of YT (Yard Truck) and RT (Road Truck) in container yard, and turnaround time of RT in container terminal of each priority rule by using stevedoring simulation. As a result, NFS was evaluated as the best rule to improve the overall productivity of container terminal in terms of all criteria.

Gemini Observatory - a new opportunity for the Korean community

  • Through KASI, Korean astronomers will have access to the Gemini Observatory starting in 2015. Gemini operates two optical/infrared 8m-class telescopes: one on the northern hemisphere in Hawaii, one on the southern hemisphere in Chile. Both are equipped with state of the art instruments and adaptive optics systems that will be presented. Gemini also offers the opportunity for instrument development and welcomes visiting instruments. In terms of operations, Gemini offers to its partners three ways of applying for time: through semester-basis calls for standard programs, through yearly calls for Large and Long Programs, and through monthly calls for fast turnaround programs. Gemini operates in queue mode (observing for the principal investigators and allowing flexibility in the time domain), as well as in the classical visitor mode (where principal investigators come to the telescope). I will explain the requirements and advantages of all these modes as well as a full update on the Gemini Observatory.

  • Given a schedule of train to be routed by a Railway Stock, Railway Stock Routing Problem determine a sequence of train while satisfying turnaround time and maintenance restrictions. The objective is to minimize the Railway Stock during a week and each day simultaneously. And we prove that Railway Stock Routing Problem with maintenance restrictions is NP-hard. In this paper, we present two stage approaches that solve the Railway Stock Routing Problem in a reasonable time. In first stage we relax maintenance restrictions and formulate as a Min-cost-flow problem. Then, in the second stage, we attempt to satisfy maintenance restrictions using ours heuristic algorithm. We show the computational result of applying to an actual train schedule data.

  • In ubiquitous environment the multi-agent system is effective for providing intelligent services. In this paper we introduce a new scheduling service model for improving the communication performance in the multi-agent system. It is achieved by assigning a priority to the message according to the weight of formalized ACL message and the number of messages exchanged between the agents. An experiment reveals that the proposed approach allows smaller processing time per message and turnaround time than the scheduling with the existing JADE platform. Moreover, the proposed scheme allows a scalable messaging system.
