Health-Related Quality of Life of Patients with Intermediate Hepatocellular Carcinoma after Liver Resection or Transcatheter Arterial Chemoembolization

  • Background: The aim of our present study was to compare quality of life (QoL) between intermediate-stage (BCLC-B) HCC patients who had undergone either liver resection or transcatheter arterial chemoembolization (TACE). Materials and Methods: A total of 102 intermediate-stage HCC patients participated in our study, including 58 who had undergone liver resection and 44 who had undergone TACE. Baseline demographic characteristics, tumor characteristics, and long-term outcomes, such as tumor recurrence, were compared and analyzed. QoL was assessed using the Short Form (SF)-36 health survey questionnaire with the mental and physical component scales (SF-36 MCS and PCS). This questionnaire was filled out at HCC diagnosis and 1, 3, 6, 12, 24 months after surgery. Results: For the preoperative QoL evaluation, the 8 domains related to QoL were comparable between the two groups. The PCS and MCS scores were significantly decreased in both the TACE and resection groups at1 month after surgery, and this decrease was greater in the resection group. These scores were significantly lower in the resection group compared with the TACE group (P<0.05). However, these differences disappeared at 3 and 6 months following surgery. One year after surgery, the resection group showed much higher PCS scores than the TACE patients (P=0.018), and at 2 years after surgery, the PCS and MCS scores for the resection group were significantly higher than those for the TACE group (P<0.05). Eleven patients (19.0%) in the resection group and 17 (38.6%) in the TACE group suffered HCC recurrence (P<0.05). Univariate and multivariate analyses indicated that tumor recurrence (HR=1.211, 95%CI: 1.086-1.415, P=0.012) was a significant risk factor for poorpostoperative QoL in the HCC patients.Conclusions: Due to its effects on reducing HCC recurrence and improving long-term QoL, liver resection should be the first choice for the treatment of patients with intermediate-stage HCC.

Gastric calcifying fibrous tumor of the stomach presenting as a subepithelial tumor

  • We describe a case of a 48-year-old Korean woman who had a subepithelial mass incidentally discovered by endoscopic examination. Endoscopic mucosal resection revealed a well-circumscribed whitish solid mass within the submucosal space. Microscopically, the tumor was comprised of sparse spindle cells in the dense collagenous stroma with several calcifications and lymphoid aggregates. Immunohistochemical analysis showed that the tumor cells are negative for c-kit, smooth muscle actin, desmin, S-100 and CD34. Based on these findings, the tumor was diagnosed with calcifying fibrous tumor.

기능성 부갑상선암 (Functioning Parathyroid Carcinoma - A Case Report -)

  • The parathyroid carcinoma is a rare cause of primary hyperparathyroidism. There was approximately 170 reports in the would literature. and only two documented cases in Korean literature. It is still difficult to distinguish. histologically, benign from malignant parathyroid tumors. However, if diagnotic criteria are that strict, it can be suscessfully cured by initial operation. The initial operation should be an en bloc resection of the tumor, avoiding rupture of the tumor capsule and spillage of tumor cells. This report details the management of a 51 year old female with functioning parathyroid carcinoma who underwent an en bloc resection of the tumor at the time of initial operation.

원발성 종격동 정상피종: 1례 보고 (Primary Mediastinal Seminoma: A Case Report)

  • A case of mediastinal seminoma is presented. A male driver, 27 years old Korean, has been suffered from substernal pain, cough and moderate swallowing difficulty since 5 months prior to this admission. At the time of onset, he visited at a local clinic to find some mass in his anterior mediastinum on chest P-A and lateral X-ray check. Recently, intermittent hiccups with much aggravated dysphagia forced him to visit our hospital, and admitted for radical resection under the impression of anterior superior mediastinal tumor of thymus origin. Median sternotomy was done and total resection of the tumor of 8.0X11.0X3.5cm was done without any specific complication and biopsy of the tumor revealed as primary mediastinal seminoma of the mediastinum. Supplementary prophylactic irradiation therapy was done with a tumor dose of 4,000 rad in 4 weeks after operation. Postoperative hospital course was uneventful and patient was joyful with his occupation for 3 months after discharge.

  • Carotid body tumors are uncommon tumors of the head and neck Surgery is the primary treatment for the tumor. Large carotid body tumors frequently encircle the common, internal, and exernal carotid arteries, and extensive bleeding often complicates the resection, increasing the risk of carotid artery rupture and damange to major cranial nerves. Grafting should be used in high-risk patients. We have experienced a case of carotid body tumor which encircle the common, internal and external carotid arteries, treated with ligation of external carotid artery and grafting using Gortex between common carotid artery and internal carotid artery.

성상세포종의 방사선치료성적 (Radiotherapy Results of Brain Astrocytomas)

  • A retrospective analysis of survival data of i2 cases with brain astrocytomas was presented. All patients received post·operative radiotherapy in the period of $1973\~1983$ at YUMC, Yonsei Cancer Center. There were 24 patients with Grade II, 12 patients with Grade III and 16 patients with Grade IV astrocytomas. Survival rates o ere analyzed according to histologic grade of malignancy, age, tumor location, radiation dose and extent of surgical tumor resection. 5year actuarial survival for patients with Grade II astrocytomas was $32.9\%$ and Grade III was $42.9\%$. The 1 year and 2 year survival rate of Grade astrocytomas were $46.7\%$ and $0\%$. Histologic grade of tumor was important prognostic factor in brain astrocytomas. Age and extent of surgical resection were significant prognostic (actors in all grades of astrocytomas and tumor location and radiation dose were significant in Grade f astrocytomas.

A Case of Complete Resection of a Solid Pseudopapillary Tumor with Hepatic Metastasis

  • Solid pseudopapillary tumor (SPT) of the pancreas is a rare neoplasm with low malignant potential, which has a good prognosis with more than 95% survival at 5 years. Only approximately 10% to 15% cases of SPTs are malignant. This report presents a case of a 38-year-old woman who developed malignant SPT of the pancreas with synchronous multiple hepatic metastases. She underwent a successfully complete surgical resection for multiple hepatic metastatic tumors in addition to primary tumor.

Resection for Pancreatic Cancer Lung Metastases

  • Background: Pancreatic cancer is a highly aggressive solid tumor. Patients with metastases from pancreatic cancer have poor survival rates. Here, we report the outcomes of 6 patients for whom resection of lung metastases was performed after a pancreatectomy to treat pancreatic cancer. Methods: We retrospectively reviewed the perioperative clinical data of patients with lung metastases resulting from primary pancreatic cancer who were treated with lung resection between 2008 and 2015. We report 6 cases where lung resection was performed to treat lung metastases after a pancreatectomy. Results: The number of lung metastases was 1 in 5 cases and 2 in 1 case. The surgical procedures performed to treat the lung metastases included 4 wedge resections and 2 lobectomies. The cell type of the primary tumor and metastases was tubular adenocarcinoma in 5 cases and intraductal papillary-mucinous carcinoma in 1 case. All 6 patients survived with a mean follow-up period of 65.6 months, although the disease recurred in 2 patients. Conclusion: Resection of lung metastases resulting from primary pancreatic cancer may lengthen survival, provided the patient can tolerate surgery.

원발성 흉벽 골종양24례 보고 (Primary Bony Thorax Tumor Report of 24 cases)

  • Most clinicians have taken a lot of interest in tumors arising from the bony thorax because not only of their rarity and predictable diagnosis which could be reflected as a unique radiologic shadow but also variable surgical modes for maintenance of chest wall stability encountered after en-bloc resection. By the retrospective review, we have analyzed 24 cases of primary bony thorax tumors which were experienced and surgically treated at the St. Mary`s hospital of Catholic Medical College from Jan. 1969 to Sept. 1984. The results are as follows: 1. Age incidence was evenly distributed through all decades and the male-female ratio is 15:9. 2. 16 cases out of 24 were benign tumors and the commonest one of which was fibrous dysplasia. 3. Remaining 8 cases were malignant tumors and among which osteogenic sarcoma was the commonest one. 4. The majority of tumors [22/24] were developed from the rib and the remains were from the sternum. 5. Common manifestation were palpable mass or swelling and localized tenderness. 6. Various kinds of operative procedure were underwent: single resection of rib including tumor,14 cases; multiple resection of ribs with chemotherapy or myoplasty, 2 cases; en-bloc resection of the chest wall and reconstructive procedure, 5 cases; partial resection of sternum, 1 case; bone biopsy and chemotherapy, 2 cases.

  • Tracheal neurilemoma, an extremely rare benign tracheal tumor that there has been only one case reported in 1996 throughout the nation, is a slowly progressing disease that obliterates the upper airway, delays diagnosis for its symptom similarity to asthma, and makes intubation for operation difficult. Bronchoscopic is therefore needed for diagnosis. There are two options for the treatment methods, a bronchoscopic resection or open surgical resection; however if intubation is difficult, then the bronchoscopic resection is used first to keep the airway open for the surgical resection. In this case, the severe tracheal stenosis impeding intubation made the surgical resection of the primary tracheal neurilemoma with extratracheal mass impossible; therefore, bronchoscopic laser resection was applied first to optain the airway passage for endotracheal intubation, followed by a successful open surgical resection.
