In Vitro imaging of MRI and Ultrasound for Colorectal Carcinoma (직결장암 조직의 자기공명영상과 초음파 소견에 대한 비교 연구)

  • Purpose : To evaluate and compare the accuracy of magnetic resonance imaging (MRI) and ultrasound (US) for detection and estimation of invasion depth of colorectal carcinoma (CRC) by correlation with histopathologic findings in vitro, and to find out the best MR pulse sequence for accurate delineation of tumor from surrounding normal tissue. Materials and Methods: Resected specimens of CRC from 45 patients were examined about tumor detectability and invasion depth of US using high frequency (5-17 MHz) linear transducer in a tube filled with normal saline and MRI in a 8-channel quadrate head coil. The institutional review board approved this study and informed consent was waived. MRI with seven pulse sequences of in- and out-of-phases gradient echo T1 weighted images, fast spin echo T2 weighted image and its fat suppression image, fast imaging employing steady-state acquisition (FIESTA) and its fat suppression image, and diffusion weighted image (DWI) were performed. In each case, both imaging findings of MRI and US were evaluated independently for detection and estimation of invasion depth of tumor by consensus of two radiologists and were compared about diagnostic accuracy according to the histopathologic findings as reference standard. Seven MR pulse sequences were evaluated on the point of accurate delineation of tumor from surrounding normal tissue in each specimen. Results: In specimens of CRC, both imaging modalities of MRI (91.1%) and US (86.7%) showed relatively high diagnostic accuracy to detect tumor and evaluate invasion depth of tumor. In early CRC, diagnostic accuracy of US was 87.5% and that of MRI was 75.0%. There was no statistically significant difference between two imaging modalities (p > 0.05). The best pulse sequence among seven MR sequences for accurate delineation of tumor from surrounding normal tissue in each specimen of CRC was fast spin echo T2 weighted image. Conclusion: MRI and US show relatively high diagnostic accuracy to detect tumor and evaluate invasion depth of resected specimen of CRC. The most excellent pulse sequence of MRI for accurate delineation of tumor from surrounding normal tissue in CRC is fast spin echo T2 weighted image.

Risk Factors Affecting Lymph Node Metastasis and Recurrence in Early Gastric Cancer (조기위암에서 림프절 전이 및 재발에 영향을 주는 위험인자 분석)

  • Purpose: The prognosis of operated early gastric cancer is quite excellent and the 5-year survival rate shows to be over $90\%$. The less extensive treatment has been considered to be attractive. However, lymph node metastasis remains a main risk factor for recurrence of early gastric cancer. The author performed this study in order to determine which clinicopathologic factors of early gastric cancer influence lymph node metastasis and recurrence. Materials and Methods: A retrospective study was conducted on 222 patients with early gastric cancer who had been treated by gastrectomy combined with D2 or more extended lymph node dissection between January 1991 and December 1997 at the Department of Surgery, Kyunghee University Hospital. Results: Lymph node metastasis was observed in 26 patients ($11.7\%$), and the depth of tumor invasion and tumor size among clinicopathologic factors affected lymph node metastasis. The 5-year recurrence rate was $4.4\%$, and it was revealed that lymph node metastasis and depth of tumor invasion had a greater effect on recurrence than other clinicopathologic factors. Conclusion: The high risk factors of early gastric cancer in recurrence were submucosal tumor invasion, tumor size more than 2 cm, and lymph node metastasis. Patients of early gastric cancer with such high risk factors should undergo radical gastric resection than limited surgery. (J Korean Gastric Cancer Assoc 2001;1:119-123)

  • Purpose : To evaluate and compare the diagnostic accuracy of MRI and ultrasound(US) for estimation of invasion depth of gastric carcinoma by correlation with histopathologic findings in vitro and to find out the best MR pulse sequence for detection and accurate delineation of tumor. Materials and Methods : Resected specimen of total or subtotal gastrectomy from 53 patients with gastric carcinoma were done of imaging studies of MRI and US. And US was examined by using high frequency linear transducer for tumor invasion depth by a radiologist. In each case, both imaging findings of MRI and US were evaluated independently for tumor detection and invasion depth by consensus of two radiologists and were compared the diagnostic accuracy between two imaging modalities according to the histopathologic findings. MR imaging with five MR pulse sequences, spin echo T1 and in- and out-of phase gradient echo T1 weighted images, FSE and SSFSE T2 weighted images, were performed. Five MR pulse sequences were evaluated and compared on the point of detection and accurate distinction of tumor from surrounding normal tissue. Results : In EGC, diagnostic accuracy of US(77%) was superior than that of MRI(59%) but no statistically significant difference was noted between two imaging modalities(p=0.096). In AGC, both imaging modalities of MRI and US showed relatively high diagnostic accuracy as 97% and 84% respectively. Diagnostic accuracy of MRI was statistically better than that of US at the significant level(p<0.001). The best MR pulse sequence among five in each specimen was FSE T2WI(75.5%, 40/53) in both EGC and AGC. In AGC, FSE T2WI showed excellent imaging quality by showing very high ratio (93.5%, 29/31) of accurate delineation of tumor. Conclusion : MRI and US show relatively high diagnostic accuracy in the evaluation of tumor invasion depth of resected specimen in AGC. The most excellent pulse sequence of MRI for the evaluation of tumor invasion depth is FSE T2WI on the point of detection and accurate delineation of tumor in both EGC and AGC.

  • High energy electron beams were to concentrically dose inside a tumor and more energy is a shape decreased of dose. Therefore, it is useful to radiation therapy of a tumor. Also high energy electron beams ionized into collision with a atom in structure material of tissue and it has big changes to dose distribution by multiple scattering. The study had to establish characteristic of electron beams from interaction of electron beams and materials. Experiment method was to measure dependence of electron beam central axis for depth dose curve, field flatness and symmetry and field size dependence. The results were able to evaluate data for a datum pint of electron beam. Also radiotherapy has to be considered for not only energy pencil of lines but characteristic, electron guide and isodose curves distribution.

Results of Surgical Treatment for Primary Gastric Adenocarcinoma - Single Institute Experience for 14 Years - (위선암에서 외과적 치료 결과 - 단일병원의 14년간 경험 -)

  • Purpose: The aim of this study was to evaluate the clinicopathologic features, treatment outcomes, and prognostic factors of gastric cancer based on 14 years' experience in a single medical center, and to compare treatment outcomes with a previous study. Materials and Methods: We retrospectively studied 2,327 patients who were operated on for gastric cancer between 1993 and 2006 at Korea University Hospital. Results: The resection rate was 92.8% and curative resection was achieved for 1,960 (90.8%) patients. The 5-year survival rate was 70.0% for all patients undergoing resection and 79.2% for patients undergoing curative resection. The 5-year survival rate was 1.5% for unresected cases. Age, tumor size, location of the tumor, gross tumor type, depth of tumor invasion, lymph node involvement, distant metastasis, tumor stage, combined resection, complications, histology, and type of operation each had prognostic significance on univariate analysis. On multivariate analysis, lymph node involvement, depth of invasion, venous invasion, and age were independent prognostic factors. Conclusion: The 5-year survival rate for patients who underwent curative resection was 79.2%. Depth of invasion, lymph node involvement, venous invasion, and age were independent prognostic factors. The fact that tumor stage is the most important prognostic factor after curative resection, increases the importance of early detection.

  • The wedge filter has two movements, fixed and dynamic. In this study, the depth dose distribution was analyzed to determine the stability of the dose distribution and dose volume histograms obtained by evaluating the usability of the critical normal tissue dose around the tumor dose. The depth dose was analyzed from the dose distribution from a Linac (6 MV and 10 MV irradiation field of energy $20{\times}20cm^2$, wedge filter with a SSD of 100 cm and $15^{\circ}$, $30^{\circ}$, $45^{\circ}$ Y1-in (Left -7 cm), Y2-out(Right +7 cm). To analyze the fluctuations of the depth dose, a fixed wedge and dynamic wedge toe portion was examined according to the energy and angle because the size of the fluctuations was included in the error bound and did not show significant differences. The neck, breast, and pelvic dosimetry in tumor tissue are measured more commonly with a dynamic wedge than a fixed wedge presumably due to the error range. On the other hand, dosimetry of the surrounding normal tissue is more common using a fixed wedge than with a dynamic wedge.

Significance of HPV Infection and Genic Mutation of APC and K-ras in Patients with Rectal Cancer

  • Background: Significance of HPV infection and genic mutation of APC and K-ras in rectal cancer has been investigated but not clarified. The objective of our study was to investigate these parameters in patients with rectal cancer to analyze correlations with biological behaviour, to determine relationships among the three, and also to demonstrate survival prognosis effects. Methods: From December 2007 to September 2008, 75 rectal cancer cases confirmed by histopathology in the Tumor Hospital of Xinjiang Medical University were enrolled. The control group consisted of normal rectal mucous membrane taken simultaneously, a least 10 cm distant from the carcinoma fringe. HPV DNA, the MCR of APC and exon-1 of K-ras were detected by PCR and PCR-SSCP. All results were analyzed in relation to clinical pathological material, using chi-square and correlation analysis via SPSS.13 and Fisher's Exact Probability via STATA. 9.0. All 75 patients were followed up for survival analysis using Kaplan-Meier and Log-rank tests. Results: 55 out of 75 cases demonstrated gene HPV L1 while it was notdetected in normal rectal mucosa tissue. HPV infection was correlated with age and lymphatic metastasis (P<0.05) but not other characteristics, such as ethnicity, tumor size, histological type, tumor type, Duke's stage and infiltration depth. Some 43 cases exhibited APC genic mutation (57.3%) and 34 K-ras genic mutation (45.3%). APC genic mutation was correlated with gender(P<0.05), but not age, histological type, infiltration depth, lymphatic metastasis and Duke's stage. In 55 cases of rectal cancer with HPV infection, there were 31 cases with genic mutation of APC (56.4%) and 24 with genic mutation of K-ras (43.6%). For the 20 cases of rectal cancer with non-HPV infection, the figures were 12 cases (60%) and 10 (50.0%), respectively, with no significant relation. Survival analysis showed no statistical significance for K-ras genic mutation, APC genic mutation or HPV infection (P>0.05). However, the survival time of the patients with HPV infection was a little shorter than in cases without HPV infection. Conclusions: Our results suggest that HPV infection might be an important factor to bring about malignant phenotype of rectal cancer and influence prognosis. Genic mutation of APC and K-ras might be common early molecular events of rectal cancer, but without prognostic effects on medium-term or early stage patients with rectal cancer.

Correlations of Oral Tongue Cancer Invasion with Matrix Metalloproteinases(MMPs) and Vascular Endothelial Growth Factor(VEGF) Expression (Matrix Metalloproteinases(MMPs) 및 Vascular Endothelial Growth Factor(VEGF)의 발현을 통한 구강 설암의 침윤 기전 연구)

  • Purpose: In oral tongue cancer, the degree of tumor invasion has a significant effect on the prognosis. We hypothesized that the destruction of extracelluar matrix and neovascularization are related to tumor infiltration mechanism. By studying the the tissues of early stage oral tongue cancer patients, we are intend to clarify the invasion related factors in oral tongue cancer. Material and Methods: To demonstrate the invasion process in early T-stage oral tongue cancer, the expressions of extracellular matrix destruction related molecules(MMP2, MMP9) and neovascularization related molecule(VEGF) were observed by immunohistochemical study. Also, immunohistochemical staining of CD31 was done for quantification of neovascularization. With the experiment showed above, we analyzed relationship between expression of each substances and tumor invasion depth, tumor free survival rates and cervical lymph node metastasis rate in early T-stage oral tongue cancer. Results: The expression rates of MMP2, MMP9, VEGF in 38 early oral cancer patients were 52.6%, 78.9% 52.6%, respectively. Significant correlation was found between the VEGF expression and microvessel density showed by CD31 immunohistochemical staining(p<0.001). VEGF expressions were significantly related with tumor invasion depth(p=0.002). The tumor free survival rate of those patients with VEGF-positive tumors was significantly poorer than in those with VEGF-negative tumors(p=0.019). Conclusion: This results indicate that VEGF is a useful marker for predicting the tumor invasion in patients with early tongue cancer and could be used as a beneficial factors in defining operative field and prognosis.

Reproducibility Evaluation of Shear Wave Elastography According to the Depth of the Simulated Lesion in Breast Ultrasonography (유방초음파 검사에서 모조 병소의 깊이에 따른 전단파 탄성초음파의 재현성 평가)

  • Elastography utilizes the fact that the tissue of a malignant tumor is harder than that of a benign tumor and increases the specificity of diagnosis according to the elastic modulus of the tumor, helping to reduce unnecessary biopsies. However, the reliability of elastography can be influenced by the equipment used and the examiner's skills. In this study, the researchers analyzed the reproducibility of elastography by evaluating phantom images when measuring the elasticity values repeatedly. Phantoms were created using silicone and gelatin with different levels of stiffness, and they were inserted at varying depths from the surface. The elasticity values were measured using shear wave elastography. The study aimed to determine whether the reproducibility of elasticity values remains consistent depending on the stiffness and depth of the lesions. The experimental results showed that there was no statistically significant correlation between the elasticity values obtained through shear wave elastography and the depth or stiffness of the lesions. However, in the lesions with the lowest stiffness, the elasticity values were statistically significant (p<0.001) and showed a high correlation with the depth of the lesions. Although there were variations in the measured elasticity values based on the differences in lesion stiffness and depth, these differences did not significantly impact the diagnosis. Therefore, shear wave elastography remains a reliable diagnostic method, and it is suggested that it can be helpful in the diagnosis of breast lesions.

Automatic Volumetric Brain Tumor Segmentation using Convolutional Neural Networks

  • Convolutional Neural Networks (CNNs) have recently been gaining popularity in the medical image analysis field because of their image segmentation capabilities. In this paper, we present a CNN that performs automated brain tumor segmentations of sparsely annotated 3D Magnetic Resonance Imaging (MRI) scans. Our CNN is based on 3D U-net architecture, and it includes separate Dilated and Depth-wise Convolutions. It is fully-trained on the BraTS 2018 data set, and it produces more accurate results even when compared to the winners of the BraTS 2017 competition despite having a significantly smaller amount of parameters.
