Prenatally Diagnosed Uncommon Mosaic Autosomal Trisomy

  • 산전에서 성염색체와 13번, 18번, 21번 염색체를 제외한상염색체의 모자이시즘은 발생빈도가 낮고 증례보고가 적어서 예후 예측이 어렵다. 저자들은 삼염색체성 5번, 16번, 20번의 산전진단 4례를 보고하고자 한다. 모자�� 삼염색체성 20번 2례 중 증례 1은 양수 염색체 검사에서 36.6%의 모자이시즘을 보였으나 재검한 양수 검사에서는 보다 높은 빈도 (62.1%)를 보였다. 증례2에서는 양수 염색체 검사에서 모자이시즘 삼염색체성 20번이 5.25% 였으나, 재검 양수천자결과는 정상 핵형을 보였다. 증례 3은 30개의 양수세포에서 삼염색체성 16번의 모자이시즘이 13.6% 관찰되었다. 임신 종결 후, 총60개의 태아 혈액 세포에서 모자이시즘 없는 정상 핵형이 관찰되었으나 태아의 피부 섬유아세포에서 얻은 40개의 중기상 세포에서는 22.5%의 삼염색체성 16번 모자이시즘을 보였다. 부검결과 심실중격결손(ventricular septal defect)이 관찰되었다. 증례 4는 76개의 중기상 세포에서 10.5%의 삼염색체성 5번 모자이시즘을 보였으나 태아의 초음파검사에서는 정상소견을 보였다. Level III 모자이시즘은진성 모자이시즘으로 간주되지만 발생빈도가 낮은 상염색체의 삼염색체성 모자이시즘은 태아의 예후를 예견하기 어려우므로 산전 진단시 여러 조직의 재검 및 태아 초음파 소견과 함께 다양한 임상적 접근 방법으로 그 해석에 신중을 기해야 할 것으로 사료된다.

First trimester screening for trisomy 18 by a combination of nuchal translucency thickness and epigenetic marker level

  • Purpose: The aim of this study was to assess the diagnostic efficacy of noninvasive prenatal screening for trisomy 18 by assessing the levels of unmethylated-maspin (U-maspin) and fetal nuchal translucency (NT) thickness during the first trimester of pregnancy. Materials and Methods: A nested case-control study was conducted using maternal plasma samples collected from 65 pregnant women carrying 11 fetuses with trisomy 18 and 54 normal fetuses. We compared the U-maspin levels, NT thicknesses, or a combination of both in the first trimester between the case and control groups. Results: U-maspin levels and NT thickness were significantly elevated in the first trimester in pregnant women carrying fetuses with trisomy 18 when compared to those carrying normal fetuses (27.2 vs. 6.6 copies/mL, P<0.001 for U-maspin; 5.9 vs. 2.0 mm, P<0.001 for NT). The sensitivities of the U-maspin levels and NT thickness in prenatal screening for fetal trisomy 18 were 90.9% and 90.9%, respectively, with a specificity of 98.1%. The combined U-maspin levels and NT thickness had a sensitivity of 100% in prenatal screening for fetal trisomy 18, with a specificity of 98.1%. Conclusion: A combination of U-maspin levels and NT thickness is highly efficacious for noninvasive prenatal screening of fetal trisomy 18 in the first trimester of pregnancy.

자연 유산 수태산물의 세포유전학적 분석 (A Cytogenetic Analysis of Abortus with Spontaneous Abortion)

  • 목 적: 자연 유산의 가장 흔한 원인은 수태산물의 염색체 이상으로 알려져 왔다. 따라서 환자들의 상담 및 치료에 유용한 정보를 얻기 위해 염색체 이상의 빈도와 유형을 조사하였다. 대상 및 방법: 2006년1월부터2007년 12월까지 수탁된 75례의 자연유산 수태산물에서 세포유전학적 분석을 수행하였다. 결 과: 수태산물의 염색체 이상 빈도는 32.0% (24/75례)였다. 염색체 이상들 중 삼염색체는 62.5% (15/24례)였고, 대부분의 삼염색체로는 21번 염색체의 삼염색체가 26.6% (4/15례), 다음으로 흔한 이상으로는 22번 삼염색체가 3례, 20번 삼염색체가 2례였다. 비정상 핵형의 모성 나이의 평균은 $34.3{\pm}3.3$세였다. 결 론: 유산 수태산물의 세포유전학적 분석은 자연 유산 환자들을 위한 진단과 유전상담에 중요하다고 할 수 있다.

자연 유산 수태산물의 세포유전학적 분석 (A Cytogenetic Analysis of Abortus with Spontaneous Abortion)

  • Chromosomal abnormalities of abortus have also been used to investigate the most common etiology of spontaneous abortion, but the frequency and the types of spontaneous abortions have also demonstrated considerable variation among in different countries and races. A cytogenetic analysis of 75 abortuses was performed at the GenDix, Inc. from January, 2006 to December, 2007. The frequency of chromosome abnormalities in abortus was 32.0% (24/75 cases). Among the chromosomal abnormalities, trisomy was 62.5% (15/24 cases) and the most frequent trisomy was trisomy 21 with 26.6% (4/15 cases). The average maternal age of normal and abnormal karyotypes was $34.1{\pm}3.3$ and $34.3{\pm}3.3$. Cytogenetic analysis of abortus is important for diagnosis and genetic counseling for parents with spontaneous abortion.

Observed frequency of fetal trisomy between 16 and 24 gestational weeks in pregnant women older than 34 years at delivery

  • Purpose: Increased maternal age is a major risk factor for chromosomal abnormalities. The maternal age-specific risk of fetal trisomy was theoretically calculated. We investigated the actual frequency of fetal trisomy between 16 and 24 gestational weeks in pregnant women over the age of 34 at delivery. Materials and Methods: We retrospectively, over a four-year period, reviewed the medical records of women with singleton pregnancies that started their antenatal care before the 10th week of pregnancy. Pregnant women aged 34 to 45 years at the time of delivery were enrolled and divided into groups of one-year intervals. We investigated the frequency of Down syndrome and all trisomies as a function of the maternal age and compared with the theoretical maternal-age-specific risk. Results: Of the 5,858 pregnant women enrolled in the study, the rate of trisomy 21 was 0.29% (17 cases). The observed frequencies of trisomy 21 in women with maternal ages of 35 years and 40 years were 1:1,116 and 1:141, respectively. The rate of all trisomies was 0.39% (23 cases). The observed frequencies of all trisomies in women with maternal ages of 35 years and 40 years were 1:372 and 1:56, respectively. Conclusion: The frequencies of Down syndrome and all trisomies were proportional to the maternal age. However, the observed frequencies of Down syndrome and all trisomies between the 16 and 24 gestational weeks were lower than the theoretical rates.

  • A 27-year-old pregnant woman who had one son with mental and growh retardation and dysmorphic features, was referred for genetic counselling. Cytogenetic investigations revealed 4/22 translocation in the mother(46, XX, t(4;22)(p14;P11)), partial trisomy 4p in son(46, XY, -22, +der(22), t(4;22)(p14;p11)mat). The father had normal karyotype. Amniocentesis and chorionic villi sampling were performed in 3 successive pregnancies. The karyotypes of fetus in 3rd, 4th pregnancies by amniocentesis were 46, XX, t(4;22)(p14;p11) and 46, XX, t(4;22) (p14;p11), and the karyotype of fetus in 5th pregnancy by chorionic villi sampling was found to be 46, XX, -22, +der(22) t(4;22)(p14;p11)mat. We report 3 succesive prenatally diagnosed familial partial trisomy 4p and 4/22 translocation of maternal origin with review of literature.

Rapid detection of aneuploidy using FISH in uncultured amniocytes for prenatal diagnosis : 8-year experience

  • 목 적:산전진단에 있어 빠른 진단을 위해 그 유용성이 널리 알려져 있는 FISH 방법을 미배양 세포에 적용할 때, 그 민감도를 높이기 위해 본 연구소의 경험과 기준을 소개하고자 한다. 방 법:1999년 5월부터 2006년 6월까지 본연구소에서 다운증후군 고위험군, 에드워드 증후군 고위험군, 고령산모, 초음파 이상소견 등의 적응증을 주소로 시행한 7,893례의 양수검체를 대상으로 빠른 진단을 위해 8,613례의 미배양 양수세포에 FISH 검사를 시행하였다. 분석은 함춘유전연구소의 기준에 따랐으며, 기존의 세포유전학적 결과와 최종 비교하였다. 결 과:8613례의 FISH 검사 결과, 30개 이상의 세포관찰이 가능하고, 정상인 경우 정상세포의 비율이 75%, 비정상의 경우 비정상 세포의 비율이 70%에 해당하는 8,502례의 결과를 얻었으며, 세포유전학적 결과와도 일치하였다. 결 론:산전진단 시 빠른 진단을 위한 FISH검사는 매우 유용하며, 정확한 분석을 위해 그 기준을 마련하는 것은 매우 중요하다 하겠다. 그러나 비용과 인력이 많이 소요되는 한계점을 가지고 있다.

Noninvasive prenatal test for fetal chromosomal aneuploidies by massively parallel sequencing of cell-free fetal DNA in maternal plasma: The first clinical experience in Korea

  • Purpose: Noninvasive prenatal test (NIPT) by massively parallel sequencing (MPS) of cell-free fetal DNA in maternal plasma marks a significant advancement in prenatal screening, minimizing the need for invasive testing of fetal chromosomal aneuploidies. Here, we report the initial clinical performance of NIPT in Korean pregnant women. Materials and Methods: MPS-based NIPT was performed on 910 cases; 5 mL blood samples were collected and sequenced in the Shenzhen BGI Genomic Laboratory to identify aneuploidies. The risk of fetal aneuploidy was determined by L-score and t-score, and classified as high or low. The NIPT results were validated by karyotyping for the high-risk cases and neonatal follow-up for low-risk cases. Results: NIPT was mainly requested for two clinical indications: abnormal biochemical serum-screening result (54.3%) and advanced maternal age (31.4%). Among 494 cases with abnormal biochemical serum-screening results, NIPT detected only 9 (1.8%) high-risk cases. Sixteen cases (1.8%) of 910 had a high risk for aneuploidy: 8 for trisomy 21, 2 for trisomy 18, 1 for trisomy 13, and 5 for sex chromosome abnormalities. Amniocentesis was performed for 7 of these cases (43.8%). In the karyotyping and neonatal data, no false positive or negative results were observed in our study. Conclusion: MPS-based NIPT detects fetal chromosomal aneuploidies with high accuracy. Introduction of NIPT as into clinical settings could prevent about 98% of unnecessary invasive diagnostic procedures.

에드워드 증후군 환아의 증례보고 (EDWARDS SYNDROME : REPORT OF A CASE)

  • 에드워드 증후군은 18번 세염색체(trisomy)성 질환으로 신체의 모든 기관에 다수의 영향을 주며 정신 지체, 발육 지연, 호흡 곤란, 선천성 심장 질환 등의 전신 질환과 손가락의 굴곡변형과 족부후방돌출(rocker-bottom feet)의 소견을 보인다. 산모가 에드워드 증후군 환아를 임신했을 경우 양수과다, 작은 태반, 단일 제대 동맥의 소견을 보인다. 에드워드 증후군을 가진 환아는 생존율이 매우 낮다. 절반이 자궁 내에서 사망하며, 출생아의 50%는 생존율이 2개월이고, $5{\sim}10%$는 생존율이 1년 정도이다. 에드워드 증후군을 가진 환아가 충치 치료를 주소로 내원하였다. 환아의 전신 질환과 심장 수술 병력, 저체중, 기도확보 유지가 어려운 점을 고려하여 전신마취 하에 치과 치료를 시행하였다. 저자는 에드워드 증후군 환아의 치과 치료 후 다소의 지견을 얻었기에 보고하는 바이다.

Comparative genomic hybridization analysis of fetal chromosomal aberrations

  • Comparative genomic hybridization (CGH) can now be applied to detect the origin of extra or missing chromosomal material in cases with common unbalanced aberrations and in prenatal investigations. This method has been used in 13 cases of fetal samples for this study; 3 for amniocytes, 2 for cord blood and 8 for abortus tissues. These samples were previously subjected to GTG-banding. Our study showed aneuploidy in 8 cases, and partial monosomy, partial trisomy or marker chromosome in the remaining 5. The CGH disclosed further small genetic imbalances in 4 of all 13 cases: a prenatal sample showing del(20)(q13) by GTG confirmed a loss of the segment 20p13-pter by CGH; a marker chromosome manifested normal CGH profile; chromosome der(?)(?;15) found in an abortus sample by GTG turned out to be a loss of 15pter-q14 (partial monosomy) and a gain of 10pter-q22 (partial trisomy); the der(15) shown by GTG represented partial trisomy of 3q24-qter. These findings show that CGH is very useful and efficient for cytogenetic investigations of clinical cases.
