A polymeric film composed of Carbopol, Poloxamer and hydroxypropyl methylcellulose was prepared to develop a buccal patch and the effects of composition of the film on adhesion time, swelling ratio, and dissolution of the film were studied. The effects of plasticizers or penetration enhancers on the release of triamcinolone acetonide (TAA) were also studied. The hydrogen bonding between Carbopol and Poloxamer played important role in reducing swelling ratio and dissolution rate of polymer film and increasing adhesion time. The swelling ratio of the composite film was significantly reduced and the adhesion time was increased when compared with Carbopol film. As the ratio of Poloxamer to hydroxypropyl methylcellulose increased from 0/66 to 33/33, the release rate of TAA decreased. However, no further significant decrease of release rate was observed beyond the ratio of 33/33. The release rate of TAA in the polymeric film containing polyethylene glycol 400, a plasticizer, showed the highest release rate followed by triethyl citrate, and castor oil. The release rate of TAA from the polymeric film containing permeation enhancers was slower than that from the control without enhancers. Therefore, these observations indicated that a preparation of a buccal patch is feasible with the polymeric film composed of Cabopol, Poloxamer and hydropropyl methylcellulose.
Herpes zoster most commonly occurs in elderly patients, and usually affects sensory neurons. Therefore, its characteristic symptoms are segmental pain, itching, and sensory changes in the affected areas. A 71-yr-old woman experienced painful herpetic rash on the right cervical 2-4 dermatomes for 16 days. Two days after the onset of the rash, she was diagnosed with herpes zoster, and prescribed 250 mg famciclovir three times a day for 7 days, pregabalin 150 mg twice a day, and tramadol 150 mg once a day for 14 days, by a dermatologist. Despite medication, her pain was rated at an intensity of 6/10 on the numeric rating scale. In addition, she complained of severe itching sensation on the affected dermatomes. Superficial cervical plexus block (SCPB) was performed at the right C4 level with 15 ml 0.5% lidocaine plus triamcinolone 30 mg. Five days after the procedure, pain and itching completely disappeared. SCPB may be an effective option for the treatment of acute pain and itching arising from herpes zoster, and for the prevention of postherpetic neuralgia.
Pachydermodactyly is a rare distinct form of fibromatosis characterized by acquired, asymptomatic dense fibrosis and fibroblastic proliferation around one or more proximal interphalangeal joints. Intralesional triamcinolone injection may be helpful in improving the clinical appearance. A 20-year-old unemployed female patient gave a four years history of progressive swelling of the fingers affecting predominantly the proximal interphalangeal joints of index fingers of left hand and index, middle fingers of right hand. There was no pain and tenderness. Physical examination, complete blood count, antinuclear antibody, rheumatoid factor were unremarkable and radiograph of both hands showed soft tissue swelling only without bony abnormality. We excised the redundant soft tissue from the both sides of proximal interphalangeal joint with longitudinal elliptical fashion. After operation, the fingers of the patient showed marked improvement cosmetically. There were no motor or sensory changes. Pachydermodactyly was first reported in 1996 in Korea. Histologic features include an increased dermal accumulation of collagen and occasionally an increased number of fibroblast. We report for one patient with pachydermodactyly treated with longitudinal elliptical excision and obtained an improvement of clinical appearance.
Caudal epiduroscopy has been introduced as an alternative technique for direct injection of epidural steroid and lysis of adhesion. Futher, it gives a better understanding of the role of epidural adhesion in recurrence of sciatica and low back disorder after surgery. We experienced a clinical application of flexible fiberoptic epiduroscope inserted through the sacral canal. A 37-year-old woman was suffering from right lumbar radiculopathy after an operation for a herniated disc. A series of volumetric caudal steroid injections and physical therapy had little effect on her symptoms. Patient was thought to be a good candidate for epiduroscopy. Flexible flberoptic epiduroscopy was as follows: 60 ml of normal saline irrigation and epidurogram, 40 mg of triamcinolone in 10m1 of normal saline was directed around right L5 nerve root. The following morning, patient reported reduced pain in her leg.
Traumatic lacerations are common in the orofacial region as a result of accidents. Due to the frequent movement of the skin around the mandible, scars in that area are more likely to widen or become hypertrophic. Treatment of facial laceration was performed on three patients and followed by regular check-up. It was discovered that the scars have become hypertrophic, so steroid injections were used. The results were satisfactory with the decrease in sizes and hardnes of the scar. Facial scarring is a particularly distressing phenomenon and has always been a challenge to treat as the scars are more likely to widen or become hyphertrophied due to the frequent movement of the muscle in the facial area. We confirmed that the positive effect of steroid on hypertrophic scars. So, we suggest that proper treatment and periodic follow-up, adjuvant treatments especially steroid injection is necessary in patients with lacerations.
An 18-year-old warmblood gelding was presented to Jeju National University Equine Hospital with chronic bilateral forelimb lameness. Navicular syndrome was suspected based on clinical findings, the hoof test, palmar digital nerve block, and radiographic results. Computed tomography (CT) was performed under general anesthesia. Bone cysts, enlarged vascular channels, sclerosis, and enthesophytes were identified in the navicular bone on CT images. Mineralization in the deep digital flexor tendon was also observed. CT can be a useful diagnostic tool for identifying lesions of the navicular bone and adjacent structures in horses. The horse was treated with an intra-bursal injection of triamcinolone and gentamicin. Lameness started to improve two days later and the horse was sound after two months of the injection. CT enabled us not only to diagnosis of navicular syndrome but also to determine the degree and extent of the lesions.
This study was undertaken to investigate the direct effect of three different kinds of steroid to Trichophyton(T.) mentagrophytes growth in vitro. A loopful of T. mentagrophytes was inoculated into Sabouraud's media and Sabouraud's media containing various concentrations of dexamethasone (0.04%-0.00004%) prednisolone (0.25%-0.00025%), triamcinolone acetonide (0.2%-0.0002%), Sabouraud's media containing imidazole nitrate (econasole nitrate) 10 micro gm. per cc. of media, and Sabouraud's media containing econazole nitrate $10{\mu}g$ per cc. of media of various dexamethasone concentrations (0.04%-0.00004%). After inoculation they were left for two weeks at room temparature and the colony diameter were compared between Sabouraud's media and Sabouraud's media containing various concentrations, of three different steroids, Sabouraud's media containing econazole nitrate and Sabouraud's media containing econazole nitrate of various dexamethasone concentrations. For complementary study, hanging slide culture were done between Sabouraud's media and Sabouraud's media containing various dexamethasone concentrations to compare the degree of branching of hyphae and sporulation serially. The study results were as follows: 1. There is no significant difference in growth rate of T. mentcgrophytes between Sabouraud's media and Sabouraud's media containing various concentrations of three different steroids. 2. There is no significant difference in growth rate of T. metagrophytes between Sabouraud's media containing econazole nitrate 10 micro gm per cc. of media and Sabouraud's media containing econazole nitrate 10 micro gm per cc. of media of various dexamethasone concentrations. 3. In hanging slide culture, there is no significant difference in branching of hyphae and sporulation between Sabouraud's media and Sabouraud's media containing various dexamethasone concentrations.
In methods of finding causes for cleft palate, many cases have been studied by investigators using teratogenic agents. Among them, a synthetic agent known as triamcinolone acetonide (TA) was widely used. When this drug was injected into mice during palatogenesis, it induced lowered body weight and a deformed mandible. But many cases have been studied on growth changes, only of the developmental stages of the palate. Therefore, the objective of this study was to evaluate craniofacial growth in experimentally induced cleft palate mice after finishing palatogenesis namely just before birth. Normal, alcohol treated, and TA treated DDY mice were obtained at 18-days of gestation and heads were prepared for serial sectioning in the sagittal plane. The midsagittal sections were photographically enlarged (${\times}40$) and measurements made to asses the amount of growth. The obtained results were as follows. 1. The incidence of cleft palate was 41.2% when TA was injected. 2. The body weight of the cleft palate group was lower than the control group. 3. In the cleft palate group, mandibular length (H-M) was lighter than the control group. 4. In the cleft palate group, degree of staining was not distinct compared to the control group by the double staining method. 5. In the cleft palate group, anteroposterior posture of the tongue tip to facial plane (C-M) was more posterior than the control group. 6. The cause of posterior posture of the tongue tip to facial plane (C-M) in the cleft palate group, was not short and retracted tongue but the mandibular length was increased. 7. The anteroposterior relationship of hyoid cartilage to cranial base was the same in all groups.
Background: The purpose of this study is to administer conservative treatment in 30 patients diagnosed with idiopathic frozen shoulder, following the suggested frozen shoulder rehabilitation program and to assess the clinical outcome using a prospective study. Methods: Thirty patients diagnosed with idiopathic frozen shoulder, treated with steroid hormone injection on the articular joint with an intra-articular steroid (triamcinolone 40 mg+lidocaine 4 ml) injection and started on stepwise shoulder extension exercise were chosen. The subjects were divided into two groups of 15 people each with one group undergoing rehabilitation with continuous passive motion (CPM) and the other group without it. Follow-ups were done before rehabilitation and at 4-week intervals with the 24th being the final week. At every follow-up, passive range of motion (ROM) was measured and surveys on pain and clinical score were administered. Results: In the last follow-up, both groups showed statistically significant improvements in all evaluation criteria. However, no statistical difference in all values of the ROM and Constant score evaluation criteria was observed between the groups. Only in the last follow-up, group 1 had a visual analog scale (VAS) score of $2.4{\pm}2.1$ points, which was lower, with statistical significance, than the VAS score of group 2, which was $4.4{\pm}3.1$ points (p<0.001). Conclusions: Study using CPM in treatment of frozen shoulder has been inadequate, meaning that there is still room for improvement and need for more study on setting a more specific protocol and guidelines for this procedure.

Recently, the incidence of pulmonary cryptococcosis is gradually increasing in rheumatoid arthritis (RA) patients. Pulmonary rheumatoid nodules (PRN) are rare manifestations of RA. Eighteen months ago, a 65-year old woman was admitted to hospital due to multiple nodules ( $2.5{\times}2.1{\times}2cm$) with cavitations in the right lower lobe. She was diagnosed with RA three year ago. She had been taking methotrexate, leflunomide, and triamcinolone. A video-assisted thoracoscopic surgery biopsy was performed and PRN was diagnosed. However, a newly growing huge opacity with cavitation was detected in the same site. Pulmonary cryptococcal infection was diagnosed through a transthoracic computed tomograpy guided needle biopsy. Cryptococcus antigen was detected in serum but not in cerebrospinal fluid. The patient was treated with oral fluconazole which resulted clinical improvement and regression of the nodule on a series of radiography. Herein, we report the case of pulmonary cryptococcosis occurring in the same location as that of the PRN.
