  • This study has purpose to perform leaving the hospital nursing plan during hospital treatment of patient with the knee ligament Injury on the basis of the concerns between the time of leaving the hospital after 2 weeks in hospital and the concerns through practical life after leaving the hospital systematically. This study is also tried to make exercise plan to prevent atrophy of Quardriceps muscle of the thigh caused by the limitation of activities. The subjects of study are 12 people who were in the hospital in affiliation to a university in Seoul, were operated on the knee ligament, and were discharged from the hospital within 2 weeks. Data collection time was for about 4 months from December, 94 to March, 95. Research method was performed 2 times by unorganized open questions. The collection of first data is through direct interview about the concerns before leaving the hospital, and secondary data is through direct interview about the concerns by practical life and experiences after leaving the hospital. The data was categorized by classifying several common elements among similar contents according to data by the situational analysis of Giorgi. The result of this study is as follows : 1) The concerns before leaving the hospital are like that the concerns of recovery possibility, the limitation of activities, the grim realities of life, the lack of confidence about self-nursing, the eyes of the surroundings, the burden to the others, and so on. 2) The concerns after leaving the hospital are like that the demands of the inconvenience by the limitation of self-nursing, the support about self-nursing by the difficulties of performing treatment order, the support of physical activities limitation by physical tiredness and the limitation of social activities, the support of information by the necessities of education in using crutches before leaving the hospital and continuing informational needs, psychological and mental support of normal physical recovery by the worries of physical recovery and physical change, and social support by thankfulness for his family and relatives. I suggest to make more systematic leaving the hospital nursing plan and exercise education plan for nursing plan after leaving the hospital of the patients with the knee ligament injury on the basis of this content.

  • Objective : Depression is a significant and growing problem among college students. Depression contributes to numerous academic, social, and health problems, including suicide. This study surveyed depression awareness and the need for establishing a depression support program through Kyunghee University. Methods : We used the Kyunghee University Mail delivery system for college students to conduct this survey on depression and depression. Results : All of the students who responded to the survey, 41.86% reported that they had experienced depression. Among students experiencing depression, 27.27% experienced suicidal ideation, and 56.56% said they wanted to receive mental health counseling or treatment. All the respondents, 47.04% of respondents said they would to go to the external medical center, not school's internal counseling center of school, for such treatment. All the respondents, 73.75% said the student depression/suicide problem was serious. In addition, 71.34% of respondents required a depression support program. Conclusion : The university's depression support program needs to improve its accessibility by developing content focused on the early detection of, and improved awareness of, depression.

Influencing Women's Actions on Cervical Cancer Screening and Treatment in Karawang District, Indonesia

  • Introduction: The impact of cervical cancer prevention programs depends on persuading women to go for screening and, if needed, treatment. As part of an evaluation of a pilot project in Indonesia, qualitative research was conducted to explore the factors that influence women's decisions regarding screening and treatment and to generate practical recommendations to increase service coverage and reduce loss to follow up. Methods: Research was conducted at 7 of the 17 public health centers in Karawang District that implemented the pilot project. Interviews and focus group discussions were held with 20 women, 20 husbands, 10 doctors, 18 midwives, 3 district health officials, and 16 advocacy team members. Results: Free services and mobile outreach events encouraged women to go for screening, along with promotional efforts by community health workers, advocacy teams, and the mass media. Knowledge and perceptions were the most important barriers to screening: women were not aware of cervical cancer risks, did not know the disease was treatable, and were fatalistic. Factors facilitating treatment were social support from husbands, relatives, and friends and the encouragement and role modeling of health workers. Barriers to prompt treatment included limited access to services and the requirement for husband's consent for cryotherapy. Conclusion: As cervical cancer prevention services are scaled up throughout Indonesia, the findings suggest three strategies to expand screening coverage and ensure prompt treatment: strengthening community mobilization and advocacy activities, modifying the service delivery model to encourage a single visit approach to screening and treatment, and working to gain men's support.

Legal Grounds for Withholding or Withdrawal of Life-Sustaining Treatment (연명의료의 중단 - 대법원 2009.5.21. 선고 2009다17417 판결과 관련하여 -)

  • Is it lawful to withhold or withdraw life-sustaining treatment applied to a patient in a terminal condition or permanent unconscious condition? In Korea, there are no such laws or regulations which control affairs related to the withholding or withdrawal life-support treatment and active euthanasia as the Natural Death Act or the Death with Dignity Act in the U. S. A. And in addition there has had no precedent of Supreme Court. Recently Supreme Court has pronounced a historical judgment on a terminal care case. The court allowed the withdrawal of life-sustaining treatment from a patient in a permanent unconscious state. Fundamentally the court judged that the continuation of that medical treatment would infringe dignity and value of a patient as a human being. And the court required some legal grounds to consider such withdrawal or withholding of medical care lawful. The legal grounds are as follow. First, the patient is in a incurable and irreversible condition and already entered a stage of death. Second, the patient executed a directive, in advance, directing the withholding or withdrawal of life-support treatment in a incurable and irreversible condition or in a terminal condition. Otherwise, at least, the patient's will would be presumed through his/her character, view of value, philosophy, religious faith and career etc. I regard if a patient is in a incurable and irreversible condition or in a terminal condition, the medical contract between a patient and a doctor would be terminated because of the actual impossibility of achievement of it's purpose. So I think the discontinuation of life-sustaining care would be legally allowed without depending on the patient's own will.

Treatment of Drug Susceptible Pulmonary Tuberculosis

  • Tuberculosis (TB) remains a major global health problem, and the incidence of TB cases has not significantly decreased over the past decade in Korea. The standard short course regimen is highly effective against TB, but requires multiple TB-specific drugs and a long treatment duration. Recent studies using late-generation fluoroquinolones and/or high-dose rifapentine-containing regimens to shorten the duration of TB treatment showed negative results. Extending the treatment duration may be considered in patients with cavitation on the initial chest radiograph and positivity in sputum culture at 2 months of treatment for preventing TB relapse. Current evidence does not support the use of fixed-dose combinations compared to separate drugs for the purpose of improving treatment outcomes. All patients receiving TB treatment should be monitored regularly for response to therapy, facilitation of treatment completion, and management of adverse drug reactions. Mild adverse effects can be managed with symptomatic therapy and changing the timing of the drug administration, but severe adverse effects require a discontinuation of the offending drugs.

  • Objectives : This study was performed to report two cases of improved hemifacial spasm by herbal remedy and moxa-treatment. Methods : This study calendered the improvements of symptoms of two patients with hemifacial spasm who visited the author's clinic. Ukgansangamibang(抑肝散加味方) and moxa-treatment on chung-wan(CV12) were prescribed to the patients concerned due to their antipathy against acupuncture treatment. Results : Significant improvements were observered from both patients not only for hemifacial spasm but neuropsychosis-related symptoms Conclusion : The study shows remarkable results of the efficacy of herb remedy and moxa-treatment, for the treatment of hemifacial spasm , however the cases investigated in this study were only two(2). Therefore, more studies should be followed to support the efficacy of Oriental Medicine for the treatment of hemifacial spasm.

  • Blunt bronchial injuries rarely occur in children. This can be a life threatening condition and respiratory management is important for successful treatment. We present here a pediatric patient who had traumatic bronchial transection with difficult airway management. Surgical treatment was carried out under ventilatory support using extracorporeal membrane oxygenation (ECMO) in the emergency room. During the application of ECMO, systemic heparization was unnecessary and. there were no thrormbotic complications. In conclusion, ventilatory SUpport using ECMO is useful for treating selected patients with blunt trauma regardless of using heparin.

  • Organ transplantation is currently the most fundamental treatment for organ failure, but there is a shortage of organ supply compared to those in need. Regenerative medicine has recently developed a decellularization technique that overcomes the limitations of conventional organ transplantation and attempts to reconstruct damaged tissues or organs to their normal state. Several decellularization methods have been suggested. In this experiment, the decellularization methods were used to find effective decellularization methods for humanlike porcine placenta. The optimal conditions for decellular support are low DNA content and high glycos amino glycans (GAGs) and collagen content. In order to satisfy this condition, SDS and Triton X-100 and SDS + Triton X-100 were used as the detergent used for decellularization in this experiment. The contents were compared according to the decellularization time (0, 12, 24, 48 and 72 hours), and the concentrations of SDS (0.2, 0.5, 0.7 and 1.0%) were mixed in 1.0% Triton X-100 to analyze the contents. When decellularized using SDS and Triton X-100, respectively, it was confirmed that the contents of DNA and GAGs were opposite to each other. And decellularization treatment for 24 hours at 0.5% SDS was able to obtain an effective decellular support. If decellularization studies of various detergents can be obtained an effective decellular support, and furthermore, cell culture experiments can confirm the effect on the cells.

  • A Pd-Cu-Ni alloyed hydrogen membrane has fabricated on porous nickel support formed by nickel powder. Porous nickel support made by sintering shows a strong resistance to hydrogen embrittlement and thermal fatigue. Plasma surface modification treatment is introduced as pre-treatment process instead of conventional HCl wet activation. Nickel was electroplated to a thickness of $2{\mu}m$ in order in to fill micropores at the nickel support surface. Palladium and copper were deposited at thicknesses of $4{\mu}m$ and $0.5{\mu}m$, respectively, on the nickel coated support by DC sputtering process. Subsequently, copper reflow at $700^{\circ}C$ was performed for an hour in $H_2$ ambient. And, as a result PdCu-Ni composite membrane has a pinhole-free and extremely dense microstructure, having a good adhesion to the porous nickel support and infinite hydrogen selectivity in $H_2/N_2$ mixtures.

Predictors of Sick Role Behavior in Patients Receiving Radiotherapy for Cancer (방사선치료를 받는 암환자의 환자역할 행위 예측인자에 관한 연구)

  • Cancer is still a threat to human beings. The incidence and mortality rate of cancer have been gradually increasing as the life span has been lengthened. Radiotherapy is one of the most commonly used treatments for cancer. This study explored the influence of social support and stress on sick role behavior of patients receiving radiotherapy for cancer. The subjects for this study were 60 patients undergoing radiotherapy for cancer, selected from the radiotherapy treatment unit of the out patient departments of two major medical centers in Jeonju. Data were collected from February 1 to 28, 1990 by a Likert Scale Questionnaire and an interview schedule designed by the inverstigator. Data analysis included percentages, mean and standard deviation, t or F-test, Pearson Correlation Coefficient and stepwise multiple regression. Results included the following : 1. Support came primarily from sons and daughters (90.1%) ; the type of support was primarily emotional support from friends(60.0%) ; informational support came from health personnel(81.7%) ; and material support was sons and daughters(40.0%) ; satisfaction with support was highest for the spouse(4.02$\pm$.52). 2. Among the patient's demographic status was occupation the was the only socioeconomic characteristic influencing sick role behavior (F=2.91 , p=.029). 3. Directly perveived support was positively correlated with sick role behavior (r=.2374, p=.034). 4. Stepwise multiple regression was used to determine the predictors of sick role behavior. Directly perceived support was the most significant predictor accounting for the hightest contribution to sick role behavior(5.6%). Directly perceived support. socioeconomic status, perceived stress and indirectly perceived support variables together, accounted for only 6.8% of sick role behavior.
