Treatment of severe sepsis and septic shock associated with urogenital tract infection (요로감염과 관련된 중증 패혈증 및 패혈성 쇼크의 치료)

  • Urinary tract infections are among the most common infectious diseases and are the major causes of mortality and morbidity. These diseases result in many severe hospitalizations each year. Severe sepsis and septic shock are common and life-threatening medical conditions, and large cases are associated with urinary tract infection. The medical term "severe sepsis" is defined as sepsis complicated by hypotension, organ dysfunction, and tissue hypoperfusion, whereas "septic shock" is defined as sepsis complicated either by hypotension that is refractory to fluid resuscitation or by hyperlacteremia. A recent multicenter-study in Korea reported that the rate of in-hospital mortality associated with severe sepsis and septic shock was > 34%. Among the causative diseases, urogenital tract infection showed a high correlation. Moreover, it is very important that clinicians detect severe sepsis and septic shock early and treat them properly. The principles of initial treatment include provision of sufficient hemodynamic resuscitation and early administration of appropriate antibiotic therapy to mitigate uncontrolled infection. Initial resuscitation includes the use of vasopressors and intravenous fluids, and it is a key to achieve the target of initial resuscitation. Supportive care in the intensive care unit, such as glucose control, stress ulcer prophylaxis, blood transfusion, deep vein thrombosis prophylaxis, and renal replacement therapy, is also significant. We have summarized the key components in the treatment of severe sepsis and septic shock in patients with urinary tract infection. Urologists should be aware that appropriate early treatment is necessary to prevent fatal outcomes in these patients.

  • The North Korean medical laws are consisted of 'People's Health Act' and 'Medical Act' in the peak of the North Korean constitutional law. Before the legislation of 'People's Health Act', a number of medical laws and regulations existed. But, at present, there is no information about its amendment and effectiveness. 'People's Health Act' legislated in 1980 declared fundamental principles and policies of the North Korean health care system. 'Medical Act' legislated in 1997 is the basic law among the North Korean medical laws. It presented the goals and fundamental principles of the North Korean health care, and then regulated the basics about 'Tests and Diagnosis', 'Medical Treatment', and 'Medical Appraisal'. 'Medical Act' of North Korea was established later than South Korea, and its provisions is smaller in number. And there are lots of abstract and declaratory provisions compare with South Korean 'Medical Act'. Especially there is no provision about the kind and requirements of medical personnel and medical institutions, so it is hard to grasp the North Korean health care system at once. Regarding the medical treatment, there are many similar contents between the North and South Korean 'Medical Act'. But, the provisions, such as regarding mixing the new medicine and the korean traditional medicine, encouraging natural therapies in medical treatment, and informing the patient's protector of bad diagnostic result if there is concern to have a bad influence on patient, are different from the South Korean 'Medical Act'.

Accordingly, this study is examine in the radiotherapy technique which is developed that importance to therapeutic principles and methods. Materials and Methods: We had investigated record for radiotherapy technique which is developed and we had acquired information widely at clinical experience data. Results: The cancer treatment which uses the radiation of today is repeating a dazzling development. Past the treatment which is two-dimensional it does not correspond in therapeutic objective but currently 3 dimension three-dimensional moulding treatment or centurial control radiation treatment this the fourth dimension therapeutic technique which is in parallel to be introduced the complication solution in compliance with a normal organization protection and the radiation which are a difficult problem of during that time radiation treatment possibly did. Conclusion: 3-dimensional and 4-dimensional modern radiation treatment which is developen is that corresponding in objective of radiation treatment and the evaluation that again it does thick, judges in about the cancer treatment which uses radiation with the fact that the protection of normal organization is almost become accomplished.

  • It is different from other countries, they limit the possibility of long-distance travel of infectious waste by principles around treatment and recommend to use various techniques. Medical waste generated from the health medical institutions requires more thorough management because it causes potential danger to anybody exposed and also greatly harmful to natural ecosystems. The amendment bill of Wastes Control Act points out growing risk while consigning to remote waste treatment facilities because of the low processing cost and propose area-division as a solution. This study based on literature reviews, comparison of foreign countries and Korean acts. It requires area-division strategies as a suitable alternative considering the domestic situation.

Inhibition of Excitotoxic Neuronal Cell Death By Total Extracts From Oriental Medicines Used For Stroke Treatment (뇌졸중 치료 생약 추출물의 흥분성 신경독성 억제효과)

  • The methanol extracts were prepared from 46 oriental medicines currently used for stroke treatment, and the effects were assessed on the excitotoxic neuronal cell death induced by L-glutamate(Glu) in primary cultured rat cortical neurons. The extracts from Angelicae gigantis Radix, Manitis Squama, Acori graminei Rhizoma, Uncariae Ramulus et Uncus, Alpiniae Fructus, Paeoniae Radix, and Cnidii Rhizoma inhibited the Glu-induced neurotoxicity with the IC$_50$ values of 95.2, 218.6, 263.3, 295.1, 297.9, 310.1, and 446.7 $\mu$g/ m$\ell$, respectively. The extracts from Arisaematis Rhizoma, Loranthi Ramulus, Anemarrhenae Rhizoma, Carthami Flos, Clematidis Radix, Bambusae Concretio Silicea, and Angelicae koreanae Radix also exhibited significant inhibition of the toxicity. In contrast, the extracts from Aconiti Tuber Araliae cordatae Radix, Curcumae Rhizoma, Leonuri Herba, Polygalae Radix, Salviae Radix, and Siegesbeckiae Herba increased the Glu-induced toxicity at the concentrations of 500 and 1000 $\mu$g/m$\ell$. Rest of the extracts evaluated in the present study showed minor or negligible inhibition. liken together the oriental medicines including Angelicae gigantis Radix, Muitis Squama, Acori graminei Rhizoma, Uncariae Ramulus et Uncus, and Alpiniae Fructus appear to exert pharmacological effects through the inhibition of excitotoxic neuronal cell death. Further studies are in progress to characterize active principles in these extracts.

  • Today FTA extends over the world and Korea as a main member of international trade is no exception. In the past Korea, as the developing countries, has made endlessly effort to induce foreign investment from foreign enterprise and/or government to be a truly OECD countries today and made it. Korea's trade economy was reached 1 trillion dollars in 2012. Now we have to find a new way to produce, process, procure goods from foreign investment and also need to protect our profit and/or rights within foreign judicial territory. There are two method to protect foreign enterprise or government. First they rely on general principles in WTO or Bilateral Investment Treaty that the principle of equality, national treatment, and most-favored-nation treatment, you can create a predictable environment to protect foreign enterprise and/or government. Second they need to incorporate contractual clauses in their agreement such as stabilization clause, force majeure, arbitration, governing law or sovereign immunity. Of course there are many things left behind to consider I hope it will be helpful to those who prepare foreign investment contract.

  • There is an increasing appreciation of the vital role that biomechanics play in the performance of oral implant. The aim of this article is to provide some basic principles that will allow a clinician to formulate a biomechanically valid treatment plan. However, at this point in the history of oral implantology, the clinician should realize that we do not know enough to provide absolute biomechanical rules that will guarantee success of all implants in all situations. To examine the biomechanical questions, one must begin with an analysis of the distribution of biting forcess to implants. Related topics, such as stress transfer to surrounding tissues and interrelationships between bone biology and mechanical loading are major subjects, deserving a separate discussion. Once rigid fixation, angulation, crestal bone level, contour, and gingival health are achieved, stress beyond physiologic limits is the primary cause of initial bone loss around implants. The restoring dentist has specific responsibilities to reduce overload to the bone-implant interface. These include proper diagnosis, leading to a treatment plan designed with adequate retention and form, and progressive loading to improve the amount and density of bone and further reduce the risk of stress beyond physiologic limits. The major remaining factor is the development of occlusal concept in harmony with the rest of the stomagnetic system.

Comparison between Moxibustion and the Intense Pulsed Light (IPL) and the Clinical Application of Photo-moxa (구법(灸法)과 intense pulsed light(IPL)의 특성 비교 및 광선구(光線灸)의 한방임상(韓方臨床) 활용(活用))

  • Objectives : The purpose of this study is to compare the principles between moxibustion and the 'Intense pulsed light (IPL)' and to investigate the clinical application of the photo-moxa for the Korean medicine fields. Methods : The characteristics of moxibustion, IPL, parabolic reflector moxa and the dermatology surgical use of moxibustion were reviewed in the literature. Results : IPL is one type of light treatments that is employed by radiating the short-pulse wave, which is transformed from the light of high intensity. There has been used parabolic reflector in Asia for more than 2,000 years, and this hand-held device used for moxibustion in traditional medicine. Moxibustion is one of the treatment tools in Korean medicine using the heat energy. The fluence or energy density of IPL in clinical circumstances is similar to parabolic reflector moxa of moxibustion. IPL and parabolic reflector moxa shared same treatment principle employing the heat energy. Therefore IPL could apply for many indications in Korean medicine as a moxibustion tool and acupuncture. Conclusions: IPL may be plausible to be a fascinate method in phototherapy of Korean medicine.

The Combined Anti-apoptotic Effect from Tamiflu and Pinoresinol of Forsythia fructus Extract Against Influenza Virus Infection (연교 추출물 Pinoresinol와 Tamiflu의 병용효과로부터 Influenza Virus 감염에 의한 세포사멸 억제효과)

  • The fruit body of Forsythiae Fructus (Oleaceae), a common Korean medical herb, is widely used in the treatment of cold and inflammation. In order to elucidate the action mechanism and the active principles from the plant against anti-influenza virus, the influenza virus hemagglutinin (HA) and neuraminidase (NA) gene RT-PCR and Viral Screening & Identification (VSI) assay were conducted, and the activity against viral replication was also investigated. Consequently, one active constituent, namely pinoresinol showed the in vitro antiviral principle using a cytopathic effect (CPE) reduction method, indicating pinoresinol possessed anti-influenza viral activity. Furthermore, combination of pinoresinol and Tamiflu exhibited higher activities than Tamiflu alone against influenza virus (H3N2) infection. The results suggested that combination of pinoresinol with Tamiflu could be a better candidate for an ant-H3N2 viral agent in the treatment of the influenza.

Surgical Treatment of Empyema with Bronchopleural Fistula (기관지 흉막루의 외과적 치료)

  • Empyema with bronchopleural fistula is an uncommon, but serious problem. Early diagnosis and adequate drainage of the empyema cavity are well established principles for the initial management of this condition and will enable patient to recover from the toxic effects of loculated pus. 37 patients of empyema with bronchopleural fistula were treated at the department of the chonbuk National University Hospital between 1981 and 1988. The age group of fifty and sixty decades occupied 64.8%. Bacteriologic cultures of the pus were postive in 56.8%. The most common organism of the culture was staphylococci(42.9%). And the others were Pseudomonas(19%). Klebsiella(14.3%), and E. doli. No growth of pathologic organism was reported 43.2%. 24 patients of empyema with bronchopleural fistula were nonoperative causes : There were 10 pulmonary tuberculosis, 3 abscess, 9 ascending infection, one bronchiectasis and one tumor, respectively. The remaining 13 were occurred as postoperative complications ; pneumonectomy in 6, lobectomy in 4, decortication in 2, and lobectomy with segmentectomy in 1. When used as the initial mode of drainage, closed thoracostomy was performed to almost all of the patients, but 4 patients were died during this treatment. Main operations were performed except 4 died patients ; open thoracostomy in 21, open thoracostomy with myoplasty in 3, decortication in 5, decortication with resection in 3, and completion pneumonectomy in 1. Open thoracostomy was performed in 21 patients which results were favorable except one death. Permanent open thoracostomy is an old but still useful minor operation in patients with empyema with bronchopleural fistula. The overall mortality rate was 15% (6 patients) and the causes of the death were respiratory insufficiency or sepsis, or both.
