Functional Anatomy and Biomechanics of the Patellofemoral Joint (슬개대퇴 관절의 기능적 해부학 및 생체역학)

  • Even though, anterior knee pains are most frequently encountered in knee clinics, many physicians use a 'recipe'-type of approach to their treatment. But many predisposing factors have been included internal rotation of the femur, valgus knee alignment, external rotation of the extensor mechanism, patellar alta, a flat trochlear groove, patella with poor congruence and ligatmentous laxity. Many scientific principles on which these commonly used treatment are based, can lead to refinements and improvement in treatment. We reviewed and summarized the recent functional anatomical and biomechanical data that are most relevant to the contemporary treatment of patellofemoral joint disorders.

  • Good health and longevity is the goal of huamn beings. Recently, 'Dietary treatment' has become influential as one of the means for it in western medicine. Whereas in oriental medicine, 'Dietary treatment' was not recognized as therapeutic method but care of health. in this paper, the viewpoints of 'Dietary treatment' in oriental and western medicine was compared and searched for new possibilities in oriental medicine. And the results were as follows. 1. In oriental and western medicine, food was obviously recognized as a source of nourishment, and moreover oriental medicine took even a human soul into consideration. 2. Western medicine made much of nourishment and was analytical and therapeutic-centered. on the other hand, oriental medicine took a serious view of prevention and care of health. 3. Oriental medicine considered that intake of food was a adoption of Gi(氣) and then it helped a circulation of Gi and beneficial for the production of Jeong(精). 4. The principles of diet in oriental medicine was reasonable combination of food, balance of Oh-Mi(五味), temperance of food and intake by physical constitution.

Cancer Treatment Using Multiphoton Photodynamic Therapy

  • Photodynamic therapy (PDT), a newly established treatment for solid tumors, involves the systemic administration of a tumor localizing photosensitizer that is only activated when exposed to light of appropriate wavelength. Photoactivation of photosensitizer in the presence of oxygen results in the formation of highly cytotoxic molecular species, which precipitates necrosis. PDT has now become a promising means for the treatment of cancer due to its specificity, relatively minimal side effects, and inexpensive. However, the application of PDT has been restricted to the treatment of superficial lesions or the use of interstitial light delivery. A single photon generally activates the photochemical reaction in traditional PDT. However the use of multi photon excitation, where two or more photons simultaneously excite a photosensitizer, allows for the use of wavelengths twice as long. Such wavelengths exhibit better transmittance through tissue and thereby deeper penetration is achieved. This paper will review theoretical principles of multi photon excitation, challenges associated with multi photon PDT and update the current and future role of multi photon PDT in cancer.

Pharmacopuncture for Cervicogenic Dizziness

  • Objectives: Pharmacopuncture is a treatment that medicinal fluid including herbal extract is injected in body under qi/flavor theory and meridian theories. There are a number of studies investigating the efficacy of pharmacopuncture for cervicogenic dizziness but its usage differs in each study. This study aimed to review previous studies of pharmacopuncture treatment for cervicogenic dizziness to navigate the direction of improvement. Methods: Literature review was conducted on studies aimed at pharmacopuncture for cervicogenic dizziness. The randomized controlled studies which assessed the efficacy of pharmacopuncture on cervicogenic dizziness were selected. The studies were searched in Pubmed, RISS, OASIS and CNKI. After selecting eligible studies, the authors read the articles and summarized the points those are necessary in pharmacopuncture treatment for cervicogenic dizziness. Results: Twenty seven studies and 2,709 participants were included. The diverse solutions were used and the most popular were Salvia miltiorrhiza, Angelica gigas, a compound in Gastrodia elata, Panax notoginseng saponins. Some studies investigated the efficacy of conventional medical compounds (lidocaine, vitamin B, dexamethasone) injected in acupoints. Adopting acupoints and combining with body acupuncture have shown the tendency to enhance the cure rate. Pharmacopuncture could improve the efficacy of conventional treatment for cervicogenic dizziness. Conclusion: As the solution and injection method varies, principles of pharmacopuncture for dizziness should be investigated.

Antiepileptic Drugs in Children : Current Concept

  • An epileptic seizure is defined as the transient occurrence of signs and/or symptoms due to abnormally excessive or synchronous neuronal activity in the brain. The type of seizure is defined by the mode of onset and termination, clinical manifestation, and by the abnormal enhanced synchrony. If seizures recur, that state is defined as epilepsy. Antiepileptic drugs (AEDs) are the mainstay of treatment. Knowledge about initiating and maintaining adequate AEDs is beneficial for the clinician who treats children with epilepsy. This article will delineate the general principles for selecting, introducing, and discontinuing AEDs and outline guidelines for monitoring adverse effects. In general, AED therapy following a first unprovoked seizure in children is not recommended. However, treatment should be considered after a second seizure. In children and adolescents, if they are seizure-free for at least 2 years, attempts to withdraw medication/s should be made, taking into account the risks vs. benefits for the individual patient. The decision on when and what AED to use should be tailored according to the patient. For optimal treatment, the selection of adequate AEDs can be achieved by considering the precise definition of the patient's seizure and epilepsy syndrome. Continuous monitoring of both therapeutic and adverse effects is critical for successful treatment with AEDs.

Surgical Treatment of Post-pneumonectomy Empyema Thoracis (전폐절제 수술후 발생한 농흉치험)

  • Post-pneumonectomy empyema thoracis is an uncommon, but very serious problem. Early diagnosis & adequate drainage followed by thoracoplasty and or myoplasty are very important principles for the management of the empyema thoracis & will enable patient to recover from the toxic effects. During the period of January, 1985 to December, 1990, 13 patients with post-pneumonectomy empyema thoracis were treated in the department of Thoracic and Cardiovascular Surgery, Yonsei University College of Medicine. There were 10 males % 3 females ranging from 31 years to 79 years of age. The occurrence ratio of left to right side was 8: 5. The underlying pathologic lesions of empyema thoracis were pulmonary tuberculosis[7], lung ca. [2] pneumothorax[2], lung abscess[1] pneumonia[1]. We treatment procedure for post-pneumonectomy empyema thoracis were open window thoracostomy in 10 cases, Clagett procedures in 2 cases, one thoracoplasty, and two cases of Clagett procedures followed by open window thoracostomy in one cases.

  • As the various kinds of load who generate higher harmonics increases according as the power electronics technology develops, harmonic treatment standard is proposed by IEEE and IEC. Because establishment of harmonic reduction device is required for existent installment to satisfy harmonic treatment standard, the problem of investment charge and installation space increasing may be occurred. In order to solve these problems, a novel transformer connection method using principles of PCT and ZED is suggested.

  • Objectives : The Hwabyung Research Center of The Korean Society Of Oriental Neuropsychiatry has attempted to develop the 'Clinical Guidelines for Hwabyung'. Methods : The Hwabyung Research Center constructed a committee of experts and advisory group. Relevant literature was collected and evaluated in order to discover effective oriental psychotherapy as well as the management of Hwabyung. Results : We found some evidences that proved the effectiveness of oriental psychotherapy in the treatment of Hwabyung. The principles of management were also reviewed. Conclusions : Psychotherapy and management for Hwabyung were studies. We hope that the 'Clinical Guidelines for the Treatment of Hwabyung' is helpful for both oriental medical doctors and patients.

  • State Administration of Traditional Chinese Medicine of the People's Republic of China lead Research on Chao medicine's Constitutional Treatment project by which could involve the following principles. First, constitution can be identified, second, constitution correlates to ones susceptibility to diseases, third, constitution can be harmonized, fourth, prescription made according to ones constitution and patterns. These ideas is originated by Lee Je-ma's theory of Four Constitution Type in "DongEuiSooSeBoWon"(Longetivity and Life Preservation in Eastern Medicine). This is much similar to currents trends of personalized medicine in western medical sciences. Therefore further developing Lee's theory would have significant value on various fields of medicine.

  • Successful treatment of patients with multiple complex facial bone fractures is dependent on the precise clinical and diagnostic image, well-established systematic principles of fracture repair. The oral and maxillofacial surgeon should appreciate the postoperative complications and minimize or manage adequately. Most of complications can be treated secondarily, but we should appreciate the fatal complications which are impossible to treat.
