RECOGNITION OF MALOCCLUSION AND ORTHODONTIC TREATMENT NEED OF 7~18 YEAR-OLD KOREAN ADOLESCENT (7~18세 청소년의 부정교합에 대한 인식과 교정치료 수요에 관한 연구)

  • The purpose of this study was to evaluate self perception of occlusal status and recognition for orthodontic treatment as well as the prevalence of orthodontic treatment need objectively and subjectively. 3979 (male 2107, female 1872) school students of age 7~18 were analyzed by questionnaire and clinical examination and the results were as follows, 1. Perception of occlusal status on one's own was more generous than that of orthodontist's and it was affected by the factors such as age, patterns of malocclusion. 2. Recogniuon of subjective need for orthodontic treatment was more generous than that of objective need for the treatment, and factors such as age and sex of patient, parents' age and rate of education, income, occupation, classification of malocclusion and regional discrepancy were influenced. 3. Negative aspect of orthodontic treatment was influenced by the environmental factors. 4. A survey of want for orthodontic treatment showed one's alteration in recognition of the occlusal status with age and esthetic component was much more emphasized than functional component. 5. The multivariate discriminanat analysis for orthodontic treatment group showed that mother's rate of education, classification of malocclusion, demographic characteristics were critical in the determination of treatment group. 6. Test of inter-examiner reliability showed moderate coincidence.

Effect of dental fear on dental use behavior and treatment need degree (치과공포가 치과이용행태 및 치료요구도에 미치는 영향)

  • This study was conducted in order to know whether DFS questionnaire which can measure dental fear is suitable for dental fear measuring method and to know effect of dental fear on dental use behavior and treatment need degree. This survey was conducted from May 1, 2007 to May 31, 2007. A total of 281 surveys eligible among all the collected surveys were used in the analysis. Collected data were analyzed by using SPSS 12.0. The obtained results were as follows: 1. Based on the reliability analysis, we concluded that the questionnaire of dental fear was reliable, and the Cronbach' s alpha coefficients of the all subscales were more than 0.6. 2. Among the dental fear of 3 factors, treatment fear cause stimulation factor was higher than the others. Most of all, following two situations(when preparation was made and when people hear dental instrument operating sound) are the highest in the treatment fear. And women are higher than men in every item. 3. In the Dental treatment need degree caused by dental fear degree, high fear group was $4.76{\pm}1.954(M{\pm}SD)$ which was higher than that of low fear group.(low fear group was $4.07{\pm}2.184(M{\pm}SD)$. In conclusion, dental fear and dental treatment need degree are related each other.

An Epidemiological Study on the Dental Treatment Needs of Adolescents in Yangsan (양산시 거주 청소년의 치과치료 필요도에 대한 조사연구)

  • To evaluate the oral health status and to assess the resultant dental treatment needs in adolescents, 2,062 adolescents aged 14-17 years attending middle and high schools in Yangsan were surveyed by clinical examination and questionnaires. The obtained results were as follows. In the dental caries examination based on WHO criteria, the treatment needs of 66.7% of the subjects were determined. Assessment of dental erosion by the VEDE system indicated the treatment needs in 27.8% of the subjects, while MIH examination based on EAPD criteria indicated the treatment needs in 14.7%. Assessment of malocclusion using the occlusal index showed good occlusion in 67.8%, no need for treatment in 19.7%, slight need for treatment in 6.8%, definite need for treatment in 4.5%, and worst occlusion in 1.1% of the subjects, showing 12.5% of average treatment need. The prevalence and treatment need for periodontal disease was found to be 71.2% by CPITN assessments. The assessment of temporomandibular disorders by Helkimo's anamnestic index showed no symptoms in 67.1%, mild symptoms in 13.4%, and severe symptoms in 19.5% of the examinees. The overall treatment need of TMD was 20.6%. Periodontal diseases were ranked the highest in treatment need, followed by dental caries, dental erosion, MIH, TMD, and malocclusion in order. The results of this study indicated the overall status of adolescents'oral health and dental treatment needs. These might hopefully provide fundamental data and contribute to establishing promotional projects for adolescent oral health in Korea.

The effect of need of oral health management to oral health impact profile among elderly over 65 years (65세 이상 노인의 구강건강관리요구도가 구강건강영향지수(OHIP-14)에 미치는 영향)

  • Objectives : The aim of the study was to identify the need of oral health education and prevention? for over 65 years elderly. Methods : his study was to identify the need of oral health management and oral health impact profile among elderly over 65 years. 200 elderly participated in the study, lived in KungBuk and KungNam areas, visited Senior welfare center, from 1st September to 30st December 2008. Results : 1. This majority of respondents are female(74.0%), 75-79 years(29.5%), none education(42.5%), living alone(45.5%), income from children(46.0%), and health insurance(65.5%). 2. In the need of oral health management category, the need of dental treatment are professional toothbrushing, gum treatment, treatment for dental caries, treatment for xerostomia. In the need of prevention and education, the majority participants are 'required'. In oral health impact profile category, the majority participants are 'feel no difficulty during speaking(59.0%)', and 'feel no difficulty during tasting(47.0%)'. In the category, the positive answers are more than negative answers. 3. According to general characteristic with the need of oral health management, famle, obviously income, high level of life are significantly different in the need of prevention and education category. Obviously income is significantly different in the need of dental treatment category. According to general characteristic with the oral health impact profile, getting older, high education are significantly different in disadvantage category. In the case of no spouse, anxiety, physical difficulty, mental difficulty and disadvantage are high score in oral health impact profile. In the case of living alone, pain, anxiety, and disadvantage are high score in oral health impact profile. In the case of no income, limitation of function, pain, anxiety, mental difficulty and disadvantage are high score in oral health impact profile. In the case of no health insurance, anxiety, physical difficulty mental difficulty and lack of sociality are high score in oral health impact profile. 4. The oral health impact profile are positive correlation with the need of dental treatment and the need of prevention education. The effect of oral health impact profile are significantly different with spouse, average of income, the need of prevention education. Conclusions : In Conclusion, the need of prevention education and dental treatment for individual oral health promotion are related with general life condition and life level. Also these are influence of quality of life relate with oral health. These findings are require of development of oral health services program and system from bottom to top.

Distribution of Periodontal Status and Treatment Need between Smoker and Nonsmoker (흡연자와 비흡연자의 치주상태 및 치주치료요구도 - 일부 대학생 흡연자 중심 -)

  • Objectives : The purpose of this study was to investigate distribution of periodontal status and treatment need between smokers and nonsmokers. Methods : Interview and measurement was completed by 82 smoker students and 59 nonsmoker students in Taegu Health College from April 1 to 31, 2001. The community periodontal index of treatment needs(CPITN) was analyzed by percentage. Results : In distribution of CPITN by age, almost all subjects had periodontal diseases except for aged 20~24 years nonsmokers(4%). Smoking 5 cigarettes per a day, up to 10, and above 10 increased 5.3%. 7.1%. and 9.5% in 4 score of CPITN, respectively. In years of smoking, smokers who had above 5 years(13.5%) were higher than below those(6.7%), 20% for smoker who take tooth brushing once per a day had higher than 8.3%(or three in depth of pocket above 6mm. In treatment need, all subjects should take education(or oral health except for aged 20~24 years nonsmokers(4%), 96% of smokers and nonsmokers were need scaling. Conclusion : This study indicated that treatment needs for periodontal diseases and scaling in smoker were higher than nonsmoker those.

How to establish dental treatment plans for the patients with severe disabilities (치과적 중증 장애인 환자를 위한 치료 계획 수립)

  • This article discusses how to conduct treatment planning and decision making in special needs dentistry. Special needs patients often lack cooperative ability during dental treatment and have a deteriorated oral health status. To overcome the limitation in communication with special needs patients and solve their complicated dental problems, dentists need to have extensive preoperative information about the patients and their caregivers. Treatment procedures should be organized in a patient-centered and cost-effective manner. Additionally, clinical outcomes need to be predicted taking into consideration of the patients' condition. The clinical experience of committed dentists is another factor that enhances the benefits of extensive treatment in special needs patients with many limitations. The insightful treatment decision-making of dentists will contribute to improving the oral health of special needs patients despite the various obstacles.

Amiodarone Versus Propafenone to Treat Atrial Fibrillation after Coronary Artery Bypass Grafting: A Randomized Double Blind Controlled Trial

  • Background: Atrial fibrillation (AF) is one of the most common complications after cardiac surgery. Several therapeutic and preventive strategies have been introduced for postoperative AF, but the treatment and prophylaxis of AF remain controversial. We aimed to compare the efficacy of intravenous amiodarone and oral propafenone in the treatment of AF after coronary artery bypass grafting (CABG). Methods: This was a randomized controlled trial performed in two hospitals in Shiraz, Iran from 2009 to 2012. We included all patients who underwent elective CABG and developed AF postoperatively. The patients were randomly assigned to receive propafenone or amiodarone. The duration of AF, the success rate of the treatment, the need for cardioversion, the frequency of repeated AF, and the need for repeating the treatment were compared. Results: The duration of the first (p=0.361), second (p=0.832), and third (p=0.298) episodes of AF, the need for cardioversion (p=0.998), and the need to repeat the first and second doses of drugs (p=0.557, 0.699) were comparable between the study groups. Repeated AF was observed in 17 patients (30.9%) in the propafenone group and 23 patients (34.3%) in the amiodarone group (p=0.704). Conclusion: Oral propafenone and intravenous amiodarone are equally effective in the treatment and conversion of recent-onset AF after CABG.

A study on illegal dental treatment (치과 무면허 불법 시술에 대한 연구)

  • Objectives : This study intended to figure out illegal treatment by unlicensed person in dental area and factors, which were considered as user's position on the basis of the local community investigation of health conditions in chungbuk provinces in 2008. Methods : This study used chi-square test and complex sample design of multi-variate logistic regression analysis to question 12,443 peoples who have experience on illegal dental treatment. Results : Multi-variate logistic regression analysis results showed that factors having related to illegal dental treatment are sex, age, education standards, subjective awareness of oral health condition, drinking experience of lifetime, unmet need of dental treatment, chewing difficulty, use of dentures, experience of scaling and the use of interdental care instrument. Conclusions : As a result of this study, in order to root out illegal dental treatment, we need to expand the breadth of health insurance coverage so that it can reduce the burden of dental expenses. Moreover, we need to set out health service of public oral health to inform the importances of maintaining good oral health and the problems of illegal dental treatment by unlicensed person.

Associations between normative and self-perceived orthodontic treatment needs in young-adult dental patients (치과에 내원한 젊은 성인의 교정치료의 필요성과 자기 인식간의 관계)

  • Objective: Our aim was to determine the correlation between dental treatment need as assessed by the components of the index of orthodontic treatment need (IOTN) and a self-evaluation questionnaire in a population selected from the dental clinics of Shiraz, Iran. Methods: The dental treatment needs of 240 participants (120 men and 120 women) were measured on the basis of the IOTN dental health component (IOTN-DHC), IOTN aesthetic component (IOTN-AC), IOTN AC determined by the examiner (IOTN-ACE), and a self-evaluation questionnaire (SEQ). The correlations between the treatment needs assessed by all the methods as well as the correlations of the treatment needs with age and gender were assessed using a Spearman's correlation coefficient (${\alpha}$ = 0.01). Results: All methods were significantly correlated ($p{\leq}0.001$). The associations of IOTN-ACE with IOTN-DHC and SEQ were moderately significant (rho = 0.49 and 0.41, respectively). The associations of IOTN-AC with IOTN-DHC and SEQ were weakly significant (rho = 0.24 and 0.35, respectively). The IOTN-DHC was moderately associated with SEQ (rho = 0.53). Gender and age were not associated with treatment needs estimated by any of the methods used ($p$ > 0.06). Conclusions: The correlations between the self-evaluation and the IOTN components in young adults were close to those reported in children, and IOTN-ACE was more reliable than IOTN-AC.

Risk Factors Associated with the Need for Operative Treatment of Intussusception in Children (소아에서 발생한 장중첩증에서 수술적 치료의 필요와 관련된 위험인자)

  • The aim of this study was to identify the risk factor related to the need for operative treatment and avoid unnecessary non-operative management for intussusception in children. We retrospectively reviewed medical records of patient treated for intussusception at our institution between January 2006 and January 2013. Clinical features such as gender, age, seasonal variation, symptoms and signs, treatment results were analyzed. Univariate and multivariate analyses including a chi-square test for categorical variables and logistic regression analysis were performed. During the study period, 356 patients were treated for intussusception. 328 (92.1%) was treated successfully by the non-operative pneumoreduction, and 28 (7.9%) required operative management. On univariate analysis, risk factors which were related to the need for operative treatment were age, vomiting, bloody stool, lethargy, and symptoms duration. A logistic regression analysis in order to assess for independent predictors of operative treatment was performed. Age (<6 vs ${\geq}12$ months) (OR 4.713, 95% CI 1.198~18.539, p=0.027) and symptoms duration longer than 48 hours (OR 4.534, 95% CI 1.846~11.137, p=0.001) were significantly associated with a requirement for operative treatment. We conclude that younger age and a longer duration of symptoms (${\geq}48$ hours) are the independent risk factor related to the need for operative treatment for intussusception. Early surgical intervention or transfer to a hospital with pediatric surgical capabilities should be considered for patients with these findings.
