Clinical Audit in Radiation Oncology: Results from One Academic Centre in Delhi, India

  • The objective was to analyze the radiotherapy (RT) practice at the cancer centre of a tertiary academic medical institution in Delhi. This audit from an Indian public institution covered patient care processes related to cancer diagnosis, integration of RT with other anti-cancer modalities, waiting time, overall treatment time, and compliance with RT. Over a period of one year, all consecutively registered patients in radiotherapy were analyzed for the audit cycle. Analysis of 1,030 patients showed median age of 49.6 years, with presentation as stage I and II in 14.2%, stage III and IV in 71.2% and unknown stage in 14.6%. A total of 974 (95%) were advised for RT appointment; 669 (68.6%) for curative intent and 31.4% for palliation. Mean times for diagnostic workup and from registration at cancer centre to radiotherapy referral were 33 and 31 days respectively. Median waiting time to start of RT course was 41 days. Overall RT compliance was 75% and overall duration for a curative RT course ranged from 50 days to 61 days. Non-completion and interruption of RT course were observed in 12% and 13% respectively. Radiotherapy machine burden in a public cancer hospital in India increases the waiting time and 25% of advised patients do not comply with the prescribed treatment. Infrastructure, machine and manpower constraints lead to more patients being treated on cobalt (74%) and by two-dimensional (78%) techniques.

Survey on Satisfaction and Symptom Improvement of Korean Medicine Treatment in 122 Cases by Traffic Accident (교통사고 환자 122례에 대한 한방치료 만족도 및 호전도 조사)

  • Objectives: This study conducted a survey research to characterize traffic accident(TA) patients seeking Korean medicine treatment and to analyze the symptom improvement and satisfaction scores. Methods: A survey was conducted in 122 outpatients, who visited OO University Korean Hospital due to TA-associated symptoms from November, 2017 to May, 2018. The questionnaire included information on patient demographic characteristics, accident circumstance details, pain levels, reason for treatment selection, treatment methods, treatment purpose, symptom improvement and satisfaction. All statistical analyses were performed using Windows SPSS version 20.0 Results: The characteristics of traffic accident patients using Korean medical institution were usually accidents that occurred while driving slowly, and minor injuries like a sprain. The most important consideration in choosing Korean medical institution was its past experience. The biggest reason for switching treatment from Western medicine to Korean medicine was for diverse treatments. Satisfaction with Korean medicine was measured between very satisfaction and satisfaction. Patients showed the highest satisfaction with acupuncture, followed by pharmacopuncture and chuna manual therapy. Most treatments were measured between very satisfaction and satisfaction. After treatment, 95.90 percent of the patients said they would recommend Korean treatment. Conclusions: Although this study has limitations as research in survey format, we intended to analyze determining factors for the use of Korean medicine treatment through satisfaction, symptom improvement, and Numeric rating scale (NRS) change.

Patterns of care and treatment outcomes for primary thyroid lymphoma: a single institution study

  • Purpose: The aim of this study was to analyze the patterns of care and treatment outcomes in patients with primary thyroid lymphoma (PTL) in a single institution. Materials and Methods: Medical records of 29 patients with PTL treated between April 1994 and February 2012 were retrospectively reviewed. Diagnosis was confirmed by biopsy (n = 17) or thyroidectomy (n = 12). Treatment modality and outcome were analyzed according to lymphoma grade. Results: The median follow-up was 43.2 months (range, 3.8 to 220.8 months). The median age at diagnosis was 57 years (range, 21 to 83 years) and 24 (82.8%) patients were female. Twenty-five (86.2%) patients had PTL with stage IEA and IIEA. There were 8 (27.6%) patients with mucosa-associated lymphoid tissue (MALT) lymphoma and the remaining patients had high-grade lymphoma. Patients were treated with surgery (n = 2), chemotherapy (n = 7), radiotherapy (n = 3) alone, or a combination of these methods (n = 17). Treatment modalities evolved over time and a combination of modalities was preferred, especially for the treatment of high-grade lymphoma in recent years. There was no death or relapse among MALT lymphoma patients. Among high-grade lymphoma patients, 5-year overall survival (OS) and 5-year progression-free survival (PFS) were 75.6% and 73.9%, respectively. Complete remission after initial treatment was the only significant prognostic factor for OS (p = 0.037) and PFS (p = 0.003). Conclusion: Patients with PTL showed a favorable outcome, especially with MALT lymphoma. Radiotherapy alone for MALT lymphoma and chemotherapy followed by radiotherapy for high-grade lymphoma can be effective treatment options for PTL.

Clinical Application of Botulinum Toxin to Contact Granuloma and Vocal Nodule (보툴리눔 독소를 이용한 성대육아종과 성대결절 치료)

  • In the field of otolaryngology-head and neck surgery, botulinum toxins are widely used for the treatment of spasmodic dysphonia and vocal tremors. Recently, the applications of botulinum toxin have gradually expanded with time, to include vocal fold granuloma, mutational falsetto, bilateral vocal cord paralysis, and chemical reduction for arytenoid dislocation as an adjunctive modality. According to a nation-wide multicenter study conducted by the Korean Society of Laryngology, Phoniatrics and Logopedics for treatment modality of contact granuloma, among the various treatment modalities, botulinum toxin injection showed the highest response rate and lowest recurrence rate in both primary and refractory cases. Therefore, botulinum toxin could be reserved as a second-line treatment for contact granuloma in which the first treatment was not effective, but also could be used as a first-line treatment depending on the patient's and institution's situation. For recalcitrant nodules, injection of botulinum toxin into the bilateral thyroarytenoid muscle will reduce glottal contact force and result in a forceful chemical voice rest. In special situations, botulinum toxin injection could be one of the alternative treatment options for recalcitrant vocal nodules.

Dental implant cost estimation using the Activity-Based Costing approach (활동기준원가(Activity Based Cost)를 적용한 치과 임플란트 원가산정)

  • Purpose: There is a growing concern for the cost management of medical institutions. The purpose of this study was to estimate Activity-Based Costing (ABC) for dental implant cost. ABC refers to allocating resources or cost based on the activities of services. Materials and methods: A dental institution located in the metropolitan area was selected in this study. The tax accounting data of the institution were utilized to confirm total cost, and the institution was asked to make out clinical activities to figure out what activities were carried out. The direct cost and indirect cost for dental implant were separately estimated, and cost driver was analyzed to estimate the indirect cost accurately. Results: The rates of the direct and indirect cost respectively stood at 35.8 and 49.5 percent. The cost for a dental implant was found to be approximately 1,579 won, and the cost of prosthetic surgery and treatment that included implant surgery accounted for the largest portion of the cost, which was 470 thousand won (30%). And the weight of training and education on dentistry was relatively higher than that of the other kinds of treatment. Conclusion: In order to ensure accurate and scientific costing for dental implant, not only direct medical procedure but every pre- and post-procedure activity should fully be taken into account. Pre-activities, post-activities, education and training are included in the indirect cost, but all these activities are mandatory and associated with the quality of treatment and the satisfaction level of patients.

A STUDY ON MICROLEAKAGE OF COMPOSITE RESIN AFTER SURFACE TREATMENT (표면 처리방법에 따른 복합레진의 미세누출에 관한 실험적 연구)

  • Adhesion of composite resin to tooth structure has been of tremendous signgicance in clinical dentistry. Due to the lack of adhesion between composite restorative resins and enamel and dentin, microleakage occurs at the tooth/restoration interface. This may lead to discoloration, secondary caries, marginal breakdown, postoperative sensitivity, and even pulpal pathology. According to extensive use of composite resin, every effort on improving bonding strength and reducing microleakage between a tooth and composite resin has been continued. This study was conducted to determine the difference in microleakage in enamel and dentin treated with air-abrasion, acid etching and combination when restored with composite resin. Class V cavities were prepared on 30 premolars. The specimens were divided into following groups. group 1:air-abrasion+Scotchbond Multi-purpose group 4 :air-abrasion+All-Bond 2 group 2:acid etching+Scotchbond Multi-purpose group 5 :acid etching+All-Bond 2 group 3:combination+Scotchbond Multi-purpose group 6 :combination+All-Bond 2 #combination:air-abrasion + acid etching The specimens were filled with Z-100 after application of Scotchbond Multi-purpose and All-Bond 2. Thermocycling was conducted by alternately dipping the specimens in $5^{\circ}C$ and $55^{\circ}C$ water for 30 seconds 500 times. 1% methylene blue was applied and the specimens were left for 24 hours at $37^{\circ}C$. After washing out the dye, the tooth was sectioned buccolingually along the axis. The sectioned surface was observed with stereoscope for dye penetration. The author has measured the microleakage in teeth prepared with air-abrasion, acid ethching and combination to study the difference in microleakage following different methods of tooth surface treatment and has come to following results. 1. In comparing microleakage between groups, group 1 and 4 showed statistically significant difference from group 2, 3, 5 and 6(p<0.05). There was no significant difference among group 2, 3, 5, 6(p>0.05) nor between group 1 and 4(p>0.05). 2. In comparing microleakage among tooth surface treatment methods, Air-abrasion group showed significantly more microleakage than acid etching group and combination(airabrasion + acid etching) group(p<0.05). Combination(acid etching+air-abrasion)group tended to show lesser microleakage than acid etching group, but this was not statistically significant(p>0.05). 3. In comparing microleakage between bonding agents, there was no statistically significant difference between Scotch bond Multi-purpose and All-Bond 2(p>0.05).

  • Rice hull was used as a additive in order to find the effect for incresing of mushroom growth and yield in Chungnam Provincial techinical institution. 1 Treatment of 80% rice hull in small Neutaribeosut mycelial grow duration is shorter about 11 days and yield increased about 7% than conventional culture. 2. In case of Chongpung Neutaribeosut bottle culture, mycelial growth duration is shorter about two to three days in additive of 30 to 80% rice hull compared to conventional but yield similar to conventional. 3. Treatment of 30% rice hull in Agrocybe aegerita bottle culture, mycelial growth and yield increased 6days and 6% than convrntional, respectively 4 Treatment additived of 30% to 40% rice hull in Ganoderma lucidum bottle culture, similar to 454ays demand in mycelial grow duration and 38g yield/bottle in conventioal culture methods.

Water quality forecasting on upstream of chungju lake by flow duration (충주호 상류지역의 유황별 장래수질예측)

  • In order to define about concern with discharge and water-quality, it is calculated drought flow, low flow, normal flow and wet flow in Chungju watershed from flow duration analysis. Water quality modeling study is performed for forecasting at upstream of Chungju lake. It is devided method of modeling into before and after the equipment of environmental treatment institution. And it is estimated the change of water quality. Before the equipment of environmental treatment, BOD concentration is increased from 23000 to 2006 years at all site and decrease on 2012 years. The rate of increasing BOD concentration is showed height between 2000 years and 2003 years most of all site. And after the equipment of environmental treatment, it is showed first grade of BOD water quality in most of sample site beside Jucheon river. The result of water quality modeling using drought flow showed that a lot of pollution occurred. And water quality using wet flow is good, so much discharge make more improve water quality than little discharge.
