Correlation between Patient Satisfaction and Rehabilitation Motivation on Musculoskeletal and Neurological Patients in a Physical Therapy Environment (물리치료 환경에 대한 근육뼈대계 및 신경계 환자의 환자만족도와 재활동기의 상관성)

  • Purpose : This study was conducted to investigate correlation between patient satisfaction and rehabilitation motivation in a physical therapy environment. Methods : This study conducted a survey on musculoskeletal and neurological patients receiving rehabilitation treatment at a hospital. The subjects of the study were patients who were currently receiving rehabilitation treatment, either hospitalized or outpatient. 234 people were collected. A questionnaire was consisted of a total of 55 questions, including 27 questions about motivation for rehabilitation, 14 questions about physical therapy service environment, and 14 questions about patient satisfaction and intention to revisit. The detailed items in rehabilitation motivation consisted of 8 questions about task-oriented motivation, 7 questions about change-oriented motivation, 4 questions about obligatory motivation, 4 questions about external motivation, and 4 questions about intrinsic motivation, and in the physical therapy service environment, 4 questions about facility service and therapist service. , 6 questions, 4 questions about services used, 3 questions about friendliness, 4 questions about professionalism, 3 questions about treatment satisfaction, and 2 questions each about repeat visit and recommendation. Results : Facility service (r=.21) was highly correlated for task-oriented motivation, therapist service (r=.22) for change-oriented motivation, therapist service (r=.31) for mandatory motivation, therapist service (r=.19) for external motivation, and facility service (r=.56) for internal motivation. Revisit for task-oriented motivation (r=.47) is kind to change-oriented motives (r=-.13) was highly correlated with kindness (r=.19) for mandatory motives, recommendation (r=.14) for external motives, and expertise (r=.52) for internal motives. There was a high correlation between professionalism (r=.61) for facility services and kindness (r=.53) for therapist services, and revisit (r=.40) for service use. Conclusion : According to the results of this study, it was found that there was a correlation between patient satisfaction and rehabilitation motivation in a physical therapy environment.

  • Sewage treatment facilities are growing rapidly in quantitatively and qualitatively. However, privatization such as up-size and professionalism, etc. are insufficient, compared to global companies' cases. Infrastructure Asset management has not been applied widely despite of sewage treatment facilities are one of the most critical social infrastructure. Maintenance, repair, and rehabilitation (R&R) practices of sewage treatment facilities have been studied and infrastructures asset management techniques have been applied. Cost model which makes it possible to calculate managerial costs such as major cost, etc. needs to be developed only with project basic information when proceeding new sewage treatment facility construction and management and overseas sewage project. Effects to converse major repair cost into capital, application of sewage treatment facility's asset management, expected effects, etc. were analyzed and suggested an alternative on the improvements of legal system.

A Study on the Spatial Composition of Rehabilitation Center for the Disabled with the Change of Welfare Paradigm (장애인복지패러다임의 변화에 따른 장애인복지관의 공간구성에 관한 연구)

  • In this study, some methods for the organization of space in the welfare center for the disabled are suggested corresponding to the change of welfare paradigm for the disabled, by means of investigating and analyzing the existing organization of space, division of the areas and relevant programs. The results of the study are as follows. 1) The method of the spatial organization in the welfare center for the disabled is divided largely into an area of the welfare complex center used as a facility of uses and an area of the controlling center that manages and makes use of domiciliary welfare. 2) The division of counselling and management is the center for the aforementioned two areas. This division occupying the minimum space in the welfare center has to be expanded. Besides, the space for the volunteers for 'domiciliary welfare' is necessary. This can be administered synthetically along with the room for volunteers in the division of social rehabilitation. 3) It is necessary for the division of medical rehabilitation to have a common waiting area due to its close connection with other treatment rooms. It should be recommended to have it in the water treatment room. It is required to have a parents' waiting area in the treatment room for child rehabilitation. 4) In case of the division of educational rehabilitation, the daytime care center should be closely related to the division of medical rehabilitation. Meanwhile, the classrooms after school have a limited use so that they can be shared with the room for female rehabilitation in the division of social rehabilitation. 5) It is in reality for the division of social rehabilitation to have an expanded area of domiciliary welfare. The room for volunteers for domiciliary welfare should be arranged, too.

Factors related to the treatment duration of infants with congenital muscular torticollis

  • Objective: Congenital muscular torticollis (CMT) is a disease with abnormal head and neck posture in infants. It affects the child's movement and development and can lead to complications. Therefore, this study aims to find out what factors influence the rehabilitation treatment duration of infants with CMT. Design: Cross-sectional study. Methods: The subjects were 63 infants under 90 days of age who were diagnosed with CMT. Age, thickness of the sternocleidomastoid muscle (SCM) on the affected and non-affected side, head tilt angle, and head rotation angle of the affected head in infants were collected. The ratio of muscle thickness was calculated from the thickness of the affected SCM and the thickness of the non-affected SCM (A/N ratio). All subjects underwent conservative physical therapy twice a week for 30 minutes, and the end of the treatment was when the angle of head tilt was normal or less than 5 degrees, and the treatment duration was calculated. Results: Age, thickness of affected SCM, and head tilt were significantly correlated with treatment duration (p<0.05). The thickness of the non-affected SCM, A/N ratio, and head rotation angle did not show any correlation with treatment duration. The factors affecting the treatment duration were head tilt and age, showing 21% explanatory power of adjusted R2. Conclusions: The main factors affecting the treatment duration of infants with CMT are head tilt and age. Therefore, more attention should be directed to the infant's head tilt and age for effective physical therapy of infants with CMT.

  • The purpose of this study is to introduce prosthetic discipline and ambulation training after hemipelvectomy due to osteosarcoma. Over the past years, when the malignant bone tumors occurs in the extremities amputation is not enough to prevent a part recurrence and distal transformation resulting in fatal prognosis. On the other hand, these procedures could bring about a difficulty in rehabilitation in curing patient who have had hemipelvectomy. However the recent development of chemotherapy and diagnostic facility have permitted the orthopedic surgeons to many try amputations for the treatment of the malignant bone tumors. Unfortunately, there has not been many researches on hemipelvectomy. Since there is no studies found on hemipelvectomy either. Therefore, we introduce successful procedures for rehabilitation through the ambulation training for patients who have had amputation. One of our patients, who is an eighteen years old male, has had hemipelvectomy on the eighteenth of June in 1997 after his anticancer treatment over 12 times. He has had physical therapy of prosthetic ambulation training at the department of rehabilitation medicine Yonsei University Medical Center from the fifteenth of October to December '2nd in 1997.

Multifaceted Risk Factors for Mental Health Problems of Adolescents in Correctional Facilities: Toward an Integrated Implication for Rehabilitation and Reintegration into Society

  • This study explores a model that examines how multidimensional risk factors explain mental health problems of young offenders. One hundred and ninety six students aged 13 to 15 in correctional facilities were assessed for mental health symptoms that examined the effects of multifaceted risk factors on mental health conditions. Consistent with the hypothesis of this study, secure unit students appear to have experienced various forms of risk factors and those factors have influenced mental health conditions. Results show that correctional facilities should reframe therapeutic and preventive approaches to disadvantaged students and develop integrated services and programs for rehabilitation and reintegration into society. It is recommended that those involved in treatment plans in correctional facilities consider that different treatment plans are necessary for each young offender.

Study of the Geriatric Health Care Facility in Japan (일본의 노인보건시설에 대한 연구)

  • The objective of the present study is to review of the system, type of care and utility of the Geriatric Health Care Facility(GHCF) in Japan. Geriatric Health Care Facilities in Japan were started with subsidies from the Ministry of Health and Welfare in 1987 to encourage return of the elderly from hospitals to their homes rather than other destinations such as nursing homes or hospitals. Concerning to the type of care, there is the difference between GHCF and other geriatric care facilities(i.e., geriatric hospital and nursing home). GHCF provides both medical and nursing care. The following services are available for the GHCF's user's. As institutional care services, rehabilitation training, ADL exercise, nursing care and management of medicine, are available. For the out-patients, supplying meals, taking a bath, and rehabilitation services, are also available. The medical treatment fee at the facilities is about US $ 1,500 per month. Expenses for meals, daily necessities, shall be borne by the recipient, Those expenses are about US $ 360 per month. In anticipation of the coming of the aging society, the Goverment must be formulated consecutively several vital policies of measures, especially GHCF, for the elderly in the future few years.

Case Study of Investment Adequacy Analysis After Implementing Master Plan on Sewerage Rehabilitation (하수도정비기본계획 시행 후의 투자적정성 분석에 관한 사례 연구)

  • The objective of this study is to analyze the investment adequacy of the projects implemented according to the master plan on sewerage rehabilitation at Seoul. The planned and actually implemented ratio of invested money on sewage treatment plants (STPs) to sewers were compared in two temporal periods. Though the planned ratio of investment on STPs to sewers was 50:50 (in 2009-2020), the actual implemented ratio in 2009-2013 was 34:66. Until 2020, the greater investment ratio on STPs to sewers should be made considering the necessity of coping with stricter legal compliance on advanced treatment, stormwater treatment and so on. The priority of the planned and partially implemented projects among four STPs and at each STP was evaluated. Considering only the performance indicator of reduced load of BOD, T-N, T-P per the capacity of each STP facility, the performance among four STPs was shown as Jung-Rang>Tan-Cheon>Seo-Nam>Nan-Ji. The reverse order of the performance results in the past may be considered for future investment priority, but the efficiency of operation implemented at each STP, deteriorated status of each STP, investment in the past and so forth should also be considered. As for the priority of projects conducted within each STP, projects related to legal compliance (such as advanced tertiary treatment, stormwater treatment, etc.) have highest priority. Odor-related project and inhabitant-friendly facility related projects (such as building park on STPs, etc.) has lower priority than water quality related projects but interactivity with end-users of sewerage should also be important.

A Study on the Regulation for Type and Design Guideline of Mental Health Service (정신보건서비스 유형 및 시설기준 수립을 위한 관련 법제도 분석 연구)

  • Purpose : The information of mental health facilities in Korean law is so unclear that people hardly enable to understand what sort of proper mental health service is prepared for them. Futhermore, there is not enough regulation and standard to classify each type of facilities in the law. Therefore the purpose of this study is to provide data on the basis for classification and facility standards by analysing Korean law and policies. Methods : This study was conducted by a research on law and regulation of mental health facility. Results : The result of this study can be summarized into three points. The first one, current law and policy do not reflect a change in community mental health services paradigm. The second one, the classified facility should be designed to fit the community mental health services. The third one, overall, it requires more detailed guidelines to enhance the quality of mental health services. Besides, the treatment as well as the function of the prevention and rehabilitation are necessary criteria that can also be enhanced. Implications : This study looked at the classification and facility standard of mental health facility by the change in community mental health services paradigm. Forward according to these changes, there is a need for specific guidelines for mental health facility.

  • The patients with a central nervous impediment and a sequela caused by a Stroke are continuously increasing, and the burden of family and society for Stroke patients are also increasing. Since Stroke patients are required to receive a long time medical treatment and care. It adds a economical burden as well as a mental and physical burden of patient's family. In addition, it is a primary factor which deepens the disparity of medical resources in social aspect. But the social concern for these patients are very insufficient and are entirely left to the patient's family The mediation of Skilled Nursing Facility for Stroke patients enable to receive more special and proper medical treatment and recuperation service in the middle of time when they return to home from hospital. It also contributes to the settlement of the charge of patient's family and the serious disparity of medical resources. Therefore, this study will summon the social understanding of the necessity of Skilled Nursing Facilities through the above debate.
