A Case of Nasal Surgery for a Positive Airway Pressure-Intolerant OSAS Patient Due to Nasal Obstruction (코막힘으로 인해 양압기에 적응하지 못한 폐쇄성수면무호흡증 환자에서 시행된 코수술 1례)

  • Positive airway pressure (PAP) is currently recommended as a primary treatment for obstructive sleep apnea syndrome (OSAS) and positively affects various subjective and objective parameters related to OSAS, such as the apnea-hypopnea index, excessive daytime sleepiness, and blood pressure. However, PAP also exhibits various adverse effects, including skin breakdown, pressure intolerance, claustrophobia, unintentional mask removal, mouth leaks, and dryness. Especially, unintentional mask removal due to nasal obstruction may result in poor PAP compliance. A 47-year-old male patient with severe OSAS who had low PAP compliance due to nasal obstruction underwent nasal surgery. After the surgery, nasal obstruction was corrected and the patient experienced improved PAP compliance (from 30.4% to 86.7%). This case demonstrates that nasal surgery may be useful for improving PAP compliance in OSAS patients with nasal obstruction.

Effect of Foot Care Education for Patients with Rheumatoid Arthritis Involving the Feet (족부에 침범한 류마티스 관절염 환자에게 적용한 발 관리 교육의 효과)

  • Purpose: The purpose of this study was to evaluate effects of foot care education on foot pain, foot care compliance, and disability of daily living for people with rheumatoid arthritis involving the feet. Methods: This study was conducted as a randomized controlled trial. The participants were 49 patients, including 24 in the experimental group and 25 in the control group. The experimental treatment was the provision of foot care education leaflets and 20-minute 1:1 explanations of the content of the leaflet. After 2 weeks, foot care compliance was confirmed by phone call. The experimental effects were assessed by measuring changes in foot pain, foot care compliance, and disability of daily living using a pre test before the foot care education and a post test, 1 month after the foot care education. Results: Foot pain and disability of daily living in the experimental group decreased more than that of the control group. Foot care compliance in experimental group increased more than that of the control group. Conclusion: Results indicate that this foot care education program is an effective intervention to decrease foot pain and the disability of daily living, and increase the foot care compliance in patients with rheumatoid arthritis involving the feet.

Community-based Hypertension Control Program in Hwachon : Problems and Possibilities (보건소조직을 통한 고혈압관리사업)

  • Hypertension has already become a serious health problem in many countries. Treatment is effective, however, and the detection and long-term management of those at risk pose sustained challenges. Community programs can be an important strategy for primary prevention of hypertention and for monitoring the progress and promoting compliance of hypertensive patients. Hwachon Health Demonstrain Project has designed community hypertention control program, especially emphasizing role of public health sector, and operate that model from 1990 at Hwachon county, Kwangwon province. This paper appraise the effectiveness of curent activities. Many paper appraise the effectiveness of curent activities. Many hypertensive persons who have not recognized his disease were identified and began hypertension treatment. However about two thirds of patients have not complied continuously with medical advice. Consequently, the project team redirects its efforts. Less emphasis is being placed screening and identification of patients, while more emphasis is being placed on increasing compliance with treatment recommendations. Some approches to improve hypertension control program are also discussed in the context of the field experience and literature on this suvjest.

Factors Influencing the quality of life of lung transplant patients (폐이식 환자 삶의 질 영향 요인)

  • Purpose : This study aimed to identify the factors affecting the quality of life of lung transplant patients. Method : The participants were patients who received lung transplants at a general hospital in City Y. Data were collected through structured questionnaires from June 2023 to August 2023. General characteristics, lung transplantation-related characteristics, quality of life, functional disability, treatment compliance, social support, anxiety, and depression-were measured. For data analysis, hierarchical multiple regression was performed using SPSS/WIN 29.0. Results : In the final model, quality of life was associated with income, self-help group attendance, functional disability, social support, treatment compliance, and anxiety and depression. Together, they explained 61.2% of the total variance in the results. Conclusion : Our findings suggest that improving lung transplant patients' quality of life requires active intervention in stress management to perform treatment instructions well, a social support system that can help them financially, and encouraging and participating in social activities as patients.

A Study on Impact Assessment for Application of Strengthened Compliance Concentration of Effluent Limit from PSTWs in the Jinwee-stream Watershed (유역하수도에서 강화된 방류수 수질 준수농도 적용을 위한 진위천수계 수질영향 평가)

  • The different compliance concentration of effluent limit is applied to effluent discharged from public sewage treatment works(PSTWs) in each watershed on the basis of water quality thereof in accordance with the enforced Watershed Sewer System Maintenance Plan in 2013. It is necessary to set the compliance concentration of effluent limit for PSTWs in watershed sewer system, in order to achieve water quality criteria for regional watersheds or target water quality under TMDL program. Watershed Environmental Agencies establish the Watershed Sewer System Maintenance Plan and set the compliance concentration of effluent limit for PSTWs under the plan. The agencies have a plan to apply strengthened effluent BOD concentration limits for PSTWs in I to IV area grade, respectively. Effluent BOD concentration limits will be strengthened from 5~10 mg/L to 3 mg/L in II~III area grade, from 10 mg/L to 5 mg/L in IV area grade. Uniform application of effluent BOD concentration limits to PSTWs in the watershed sewer system need to be complemented considering type of sewage treatment technology employed and watershed characteristics. Therefore, this study presents assessment methodology which analyze for the compliance concentration of effluent limit to affect water quality of discharge watershed using simulation model for the Jinwee-stream watershed.

Effect of NOD on BOD Test for the Effluents of Biological Treatment Plant (하수처리장 유출수의 NOD를 고려한 BOD 측정에 관한 연구)

  • The biochemical oxygen demand (BOD) test is widely used to determine the pollution strength of water, to evaluate the performance of wastewater treatment plants and to judge compliance with discharge permits. However, nitrification is a cause of significant errors in measuring BOD, particularly when a large population of nitrifying organisms is existing in water such as effluents from biological treatment plants. In order to investigate the amount of nitrogenous oxygen demand (NOD), BOD with and without inhibitor was measured as samples in the biological treatment plants. About 81% of effluent BOD from the biological treatment plant used in this experiment was comprised of NOD. In the case of influents, the NOD accounted for about 9% of BOD. The inhibited 5-day BOD (Carbonaceous BOD) test must be considered in evaluating the performance of wastewater treatment plant and judging compliance with discharge permit limitations.

A Basic Study on Color Preference of Medicine in Patient for Improving Compliance (복약순응도 증진을 위한 환자 대상 약물 색채선호도에 관한 기초연구)

  • Everyone has own color preference and the color preference can affect human psychology in various good or bad ways. Especially patients with chronic diseases often have depressed mood to take pills for their disease treatment. Therefore we evaluate the possibility of their color preference to pharmacotherapy in good way which increases the compliance of pharmacotherapy. We performed this study by a Questionnaire survey for total 150 agreed patients at K University hospital from Aug 7 to Sep 30, 2014. Questionnaire survey was performed to choose color of providing color kit (11 different color range kit) depend on each 16 questions which asked the patient characteristics, health condition, general color preference, color preference for medicine and expectation for drug compliance. In results, most preferred color was blue (20%), whereas disliked color was black (30%) in general life. However, most preferred color for pill was white (32%), for syrup was also white (31%) or orange (28%), and for vitamin was yellow (47%). When we asked the possibility of increasing compliance, if the current taking pill color will be changed for your preference color like a candy bar, 50.4% of respondents marked 4 or 5 of like-scale expressing positive opinion on it. In conclusion, compliance is very important to succeed the treating disease and may apply the psychologic application such as each patient color preference for drug compliance in the future.

A Study on Hip Arthroplasty Patient Compliance of Medical Regimen (인공고관절 치환술 환자의 치료지시 이행에 관한 연구)

  • Purpose: The purpose of this study is to examine how well patients who had hip arthroplasty comply with medical regimens given to them after the operation. Method: The subjects of the study were patients who had arthroplasty at P Hospital between April 1, 2001 and August 30, 2002. 20 patients of the subjects experienced complications after the operation and the other 20 did not. Data from a survey using the qustionnaire were statistically analyzed in terms of real number, percentage point, mean and standard deviation by using $X^2$-test, t-test and ANOVA. Result: 1) the surveyed patients were significantly different in the compliance of medical regimen among them according to their education background as one of the subjects general characteristics. 2) It was found that the group of complication was higher in the compliance of medical regimen than that of non-complication. The two groups showed statistically significant difference with each other in the degree of compliance with therapeutic instructions than the experimental group in terms of the maintenance of abduction after the operation, training instructions on step-by-step basis, urination cotrol on bed, accurate use of crutch, compliance with medication, balance among medical treatment, training, leisure, rest and nutrition, instructions by physicians, nurses and physical therapists, use of low armchairs and toilet bowels and no bending of the body forward, and use of a non-operated leg in case of go upstairs or downstairs. Conclusion: It seems necessary to develop systematic and sessional education programs for improving the compliance of medical regimen, ultimately reducing complications following hip arthroplasty.

The Compliance and Effect of CPAP in Obstructive Sleep Apnea Syndrome (폐쇄성 수면 무호흡 환자에서 지속적 상기도 양압술의 순응도와 효과)

  • Objectives: Continuous positive airway pressure (CPAP) is effective in the treatment of obstructive sleep apnea syndrome (OSAS), but the major limitation of CPAP may be poor compliance. The aims of the study were to investigate the compliance and side effects of CPAP, and to evaluate the efficacy of CPAP in patients with OSAS. Methods: This study enrolled 106 patients with OSAS who took the CPAP treatment. The severity of daytime sleepiness was measured using Epworth Sleepiness Scale (ESS), and sleep quality and depressive symptoms were assessed by Pittsburgh Sleep Quality Index (PSQI) and Beck Depression Inventory (BDI), respectively. Results: During 29 months of the study period, 41.5% of patients were using CPAP and 38.7% of patients stopped using it. Compared to non-compliant patients, compliant patients had a higher PSQI score and obstructive apnea index. Among non-compliant patients, 51.2% of them stopped using CPAP within 1months. 85.7% of non-compliant patients were discomforted by the CPAP, but much more nasopharyngeal symptoms were reported in the compliant group. ESS (p<0.01), PSQI (p<0.01) and BMI (p<0.01) were reduced significantly after CPAP treatment but not BDI (p=0.86). Conclusions: We concluded that CPAP can reduce the daytime sleepiness, nocturnal sleep disturbance, and body mass index. To increase the compliance of CPAP, we suggest that some education and support are needed at the early stage of the CPAP treatment.

  • The use of auxiliary labels in many advanced countries has shown that patients and caregivers understanding, safety and compliance with proper administration of medication is significantly increased. Although in Korea we give required information orally and written on the packaging when we dispense medication, many international studies have demonstrated a marked increase in full compliance when auxiliary labels were used. The pharmacist must insure that the patients understand how to take correctly in order to get the maximum effect of treatment. (omitted)
