본 연구는 뇌졸중 환자에게 지역사회 보행 훈련과 트레드밀 보행 훈련이 보행 능력과 뇌졸중 영향 척도에 어떠한 영향을 미치는지 알아보기 위하여 실시하였다. 본 연구에는 총 22명의 뇌졸중 환자가 참여하였으며, 대상자들은 무작위 추출법으로 지역사회 보행 훈련군 11명과 트레드밀 보행 훈련군 11명으로 각각 배정되었다. 모든 대상자들은 전통적 물리치료를 6주 동안 주 5회, 하루 30분씩 시행하였으며, 지역사회 보행 훈련과 트레드밀 보행 훈련을 하루에 30분씩 각각 추가적으로 실시하였다. 연구 결과 지역사회 보행 훈련군이 트레드밀 보행 훈련군에 비해 보행 속도와 분속수, 뇌졸중 영향 척도에서 유의한 차이를 보였다(p<.05). 이것은 지역사회 보행 훈련이 트레드밀 보행 훈련에 비해 뇌졸중 환자의 보행 능력과 뇌졸중 영향 척도에 유익한 영향을 준 것이라고 할 수 있다. 따라서 지역사회 보행 훈련은 뇌졸중 환자에게 실행 가능하며 적합한 훈련이 될 수 있을 것이라고 생각된다.
Purpose: This is designed to study the effect of weight-support walking training through motor learning on motor functions of children with cerebral palsy, in particular their activity of daily living and balance. Methods: Thirteen children with spastic cerebral palsy, at gross motor function classification system (GMFCS) levels III~IV, underwent treadmill walking training. It used principles of weight support, 4 times a week for 7 weeks, 10 minutes at a time, before and after neurodevelopmental physical therapy. Everyday functions were measured using Functional Independence Measure for Children (Wee-FIM). The ability to keep their balance was measured using electronic measuring equipment from good balance system and the assessment was made before and after the experiment. Results: There were significant differences (p<0.05) between pre and post experiment levels of functional independence in everyday life, in self-care activities, mobility, locomotion and social cognition. With regard to changes in standing balance, there were significant differences before and after the experiment (p<0.05) in GMFCS level III. There was a reduction in the agitation velocity in the x- and y-axes which measures the left-to-right shaking; in GMFCS level IV, velocity moment was reduced. Conclusion: Walking training using a treadmill can help improve the everyday activity and balance in children with spastic cerebral palsy. It can also be served as a useful purpose as a method of intervention in pediatric care.
The purpose of this study was to the Pre-Post maximal exercise for analyze the Physiological Variable(Lactate, LDH, Glucose) in the runners. The subjects were 12 volunteers 6 sprinters and 6 distance runners who agreed to intention of this study. Subjects performed until possible all-out trials. Blood samples were taken from a median cubital vein Pre-Post exercise and every rest 5min during the all-out trial. The results obtained were summarized as follow ; 1) Lactate of Pre-Post exercise in treadmill test showed no significant difference between sprinters and distance runners, but showed significant in sprinters and distance respectively 2) LDH of Pre-Post exercise in treadmill test showed no significant difference between sprinters and distance runners, and that showed no significant in sprinters and distance respectively 3) Glucose of Pre-Post exercise in treadmill test showed no significant difference between sprinters and distance runners, and showed no significant in distance, but showed significant in sprinters therefor, since the runners has an intermittent characteristic which includes a frequent momentary change, it needs the fitness training that being consisted of various training, in the training method, it needs the training process which is from the whole fitness to specific fitness to improve general physical ability.
본 연구에서는 흰쥐의 좌골신경을 손상시킨 후 트레드밀 운동을 적용하여 신경돌기 성장과 좌골신경의 축삭 재생 및 신경성장 인자 발현 그리고 신경기능지수의 변화를 연구했다. 본 연구결과 좌골손상 후 트레드밀 운동을 실시한 그룹이 비운동군에 비해 축삭재생이 촉진되었고, 원위부의 좌골신경에서도 NGF, BDNF단백질 발현이 상당히 증가된 것으로 나타났다. 또한 좌골신경지수를 검사한 결과에서도 운동을 실시한 흰쥐가 비운동 흰쥐에 비해 기능적 회복이 상당히 빠른 것으로 나타났다. 이러한 결과는 좌골손상 후 운동의 실시가 좌골신경의 축삭재생 촉진과 신경영양인자의 발현증가를 통해 기능적 회복에 도움이 될 수 있음을 보여주는 것이다.
Objective: This study focused on subacute stroke patients who were asked to kick a ball while walking on a treadmill. The aim of the study was to determine the effect of a body weight-supported treadmill training (BWSTT) combined with ball-kicking on muscle strength, balance, and gait. Design: Single blind, randomized controlled trial. Methods: Twenty stroke patients who volunteered to participate in this study were randomly assigned to either the BWSTT combined with ball-kicking (BWSTT-BK; 10 participants) group or the BWSTT group (10 participants). Participants in the BWSTTBK group performed treadmill walking combined with simultaneous ball-kicking for 30 minutes daily for 5 weeks. Participants from the BWSTT group performed only treadmill walking. The muscular strength, balance, and gait ability were measured before and after the 5-week training. To assess for muscular strength, a digital muscle tester was used to measure hip flexor, knee extensor, and dorsiflexor strength. To assess for balance, the Berg Balance Scale (BBS) and Timed Up and Go Test (TUG) was used. To assess for gait, the 10 meter walk test (10MWT) and Functional Gait Assessment (FGA) was used. Results: The BWSTT-BK group showed significantly improved muscular strength, balance, and gait according to BBS, TUG, 10MWT, FGA, and digital muscle testing scores compared to the BWSTT group (p<0.05). In addition, within-group comparison showed significant improvement in all variables (p<0.05). Conclusions: These findings suggest that BWSTT-BK results in more favourable outcomes for stroke patients. Therefore, BWSTT-BK may be useful for the recovery of gait ability of stroke patients.
Purpose : The goal of this study was to identify the effects of treadmill walking training (TW) and ergometer bicycle training (EB) on gait and balance in stroke patients. Methods : The subjects consisted of 42 stroke patients. They were randomly divided to two groups: TW(n=20) and EB(n=22). Each group trained along with the conventional physical therapy, three times a week for six weeks. The ability of gait was assessed by the 10m walk test and Timed Up and Go test(TUG). The ability of balance was assessed by Berg Balance Scale(BBS) and Balance Performance Monitor(BPM). Results : There was no significant difference between the 10m walking test and TUG groups, but there was a statistical difference between before and after the training for all groups. The result of BBS that assessed balance showed a significant reduction between before and after the training for all groups, but there was no difference between the two groups. While the two groups showed no difference in the BPM assessment, only the EB showed a significant improvement of before and after the training in each group. Conclusion : The outcomes suggest that stroke patients can improve their gait and balance performance through the TW and EB trainings. Although dynamic mean balance showed significance from EB, no significant difference was found between two groups. Even though it cannot be determined through this study which training group is more effective among the above mentioned two, it could be suggested that each training is effective to gait ability and the ability of balance of stroke patients.
Purpose : The purpose of this study was to identify the effect of body weight support treadmill training (BWSTT) and parallel bar gait training(P-bar) on gait and balance ability of subacute stroke patients. The subjects were consisted of 27 patients with subacute stroke, and they were randomly devided into two groups which were BWSTT group and P-bar group. Method : The timed up and go(TUG), 10m gait speed were used to measure gait speed, Bergs balance scale(BBS) was used to measure dynamic balance ability, and balance performance monitor(BPM) was used to measure sway area, sway path, max velocity. Result : 1. The TUG and 10m gait speed of BWSTT group and P-bar group were significantly decreased (p<.05). The TUG and 10m gait speed were different significantly between BWSTT group and P-bar group(p<.05). 2. The BBS and sway area of BWSTT group and P-bar group were significantly decreased (p<.05). The BBS and sway area were not different significantly between BWSTT group and P-bar group(p>.05). 3. The sway path and max velocity of BWSTT group and P-bar group were significantly decreased (p<.05). The sway path and max velocity were not different significantly between BWSTI group and P-bar group(p>.05). Conclusion : The outcomes suggest that patient with subacute stroke can improve their gait and balance through body weight support treadmill training.
The purpose of this study was to determine the effect of treadmill training on gait, balance, and trunk control in a patient with hemiparesis. A female subject who had suffered a left hemiparesis 12 months previously was selected for this study. A single subject ABA design was used. Eight data-collection sessions were conducted during each of three phases (baseline-intervention-withdrawal). During baseline and withdrawal phases, the treatment based on Bobath approach was performed for the subject, and during the intervention phase, treadmill walking training was added. Assessment tools were made using the 10 m walk test, Rivermead Visual Gait Assessment(RVGA), Berg Balance Scale(BBS), and a seated Lateral Reach Test(LRT). During the intervention phase, the time measured in 10 m walk test and the scores of RVGA and BBS were significantly improved, and the number of steps in 10 m walk test and LRT showed a small improvement. During withdrawal phase, the time measured in 10 m walk test and the scores of RVGA and BBS were shown the carry-over effect. This findings indicate that treadmill training has significant effect to gait function and balance in a patient with chronic hemiparesis.
To evaluate training effect, the step exercise was loaded to three mem for nine weeks. Step score, cardiopulmonary functions and blood constituents were measured before, during and after the test exercise (50 cm-step exercise and treadmill running), and were compared with the pre-tranining values. The results were as follows: 1) By the training, Harvard step score increased remarkably, expecially in the early stage of training. 2) The post-training values of maximal oxygen uptake increased very significantly and it seemed to be due to increases of stroke volume and tissue oxygen extraction. 3) After the training, the degree of increase in expired volume was small during the treadmill exercise. 4) By the training, increasing rate of respiratory quotient lessened during the exercise and it was considered to be caused by the decreases of carbohydrate consumption and anaerobic metabolism. 5) The blood cholesterol concentrations were harldy changed with this degree of training. 6) The blood lactate level decreased during the recovery periods and the values of the recovery 0 and 5 minutes decreased remarkably, in comparison with the pre-trained values. The above results suggest that the 9 week-training of the step exercise brings about the enhancement of circulatory functions and tissue oxygen utilization, and changes of food-stuffs used during the exercise.
