생물테러리즘 대응을 위한 기술적 측면의 발전방향 (Direction of Development of Reaction to Bio-terrorism)

  • 본 연구는 생물테러리즘의 발생시 이에 대응하는 개념과 선진국들의 생물테러리즘의 대응태세를 알아본 후 한국의 대응태세를 알아보는 연구이다. 생물테러리즘의 발생에 대응하는 태세는 사건의 발생 흐름에 따라 탐지단계, 보호단계, 진단단계, 제독 및 해독단계로 구분해 볼 수 있다. 탐지단계는 한국은 접촉식으로 개발하고 있으나, 선진국에서는 원거리에서 탐지할 수 있는 장비를 개발하고 있다. 한국도 원거리에서 탐지할 수 있는 체계를 개발해야 할 것이다. 접촉식 장비도 민삼하게 작동할 수 있는 장비로 발전 되어야 한다. 보호장비중 한국의 방독면은 세계적으로 그 품질이 우수하나, 기타 피복류 등에 대해서는 첨단 과학기술을 적용할 수 있도록 개선되어야 한다. 진단 장비도 초동조치팀이 현장에 출동해서 즉시 판단할 수 있도록 하는 장비로 개발되어야 한다. 제독을 위한 물자는 인체에 무해하고, 장비에도 무리를 주지 않는 새로운 물질로 개선되는 추세로서 한국에도 이러한 물자가 확보되어야 한다. 해독을 위한 기술은 백신 및 항생제를 개발하는 것으로서 전 세계적으로 노력을 공유하여 개발할 필요가 있으며, 이를 위해 한국의 의료기술을 더욱 발전시킬 필요가 있다. 이외에도 대응매뉴얼, 훈련 모델, 공보 노력, 트라우마 증후군 대비 등의 노력을 기울여야 할 것이다.

외상 후 성장 척도 확장판(The Posttraumatic Growth Inventory-Expanded: PTGI-X)의 한국판 타당화 연구 (Validation of the Korean Version of the Posttraumatic Growth Inventory-Expanded)

  • 본 연구는 외상 후 성장의 정도를 측정하는 외상 후 성장 척도의 확장판(The Posttraumatic Growth Inventory-X: PTGI-X)을 국내에 타당화하고자 하였다. PTGI-X 척도는 기존 PTGI의 '영적·종교적 관심의 증가 요인'에 대한 보완의 필요성에 따라 실존적 깊이 증가를 측정하는 문항이 추가된 척도이다. 이를 위해 외상 경험이 있는 성인 625명을 대상으로 K-PTGI-X의 요인구조를 확인하고, 타당도와 신뢰도를 검증하였다. 먼저, 탐색적 요인분석을 통해 외상을 경험한 국내 성인을 대상으로 적절한 PTGI-X의 요인구조를 확인하였고, 4요인 구조가 가장 적절한 것으로 나타났다. 다음으로, 확인적 요인분석을 실시한 결과 4요인 모형에 문항 간 상관을 추가한 모형이 모형적합도가 좋은 것으로 나타났다. 내적 신뢰도, 분산추출지수, 개념 신뢰도를 검토하여 문항들이 요인을 구성하는 데에 문제가 없으며 신뢰로운 도구임을 확인하였다. 타당도를 확인하기 위해 관련성이 높은 것으로 알려진 핵심신념붕괴, 의도적 반추와의 유의한 상관관계를 확인하여 동시 타당도를 검증하였고, 침습적 반추와 통계적으로 유의하지 않은 관계를 확인하여 변별 타당도를 검증하였다. 마지막으로 K-PTGI-X의 이해를 더하기 위해 인구통계학적 변인에 따른 차이분석을 실시하였다. 연구결과를 바탕으로 본 연구의 논의와 시사점을 제시하였다.

Sustained release of alginate hydrogel containing antimicrobial peptide Chol-37(F34-R) in vitro and its effect on wound healing in murine model of Pseudomonas aeruginosa infection

  • Background: Antibiotic resistance is a significant public health concern around the globe. Antimicrobial peptides exhibit broad-spectrum and efficient antibacterial activity with an added advantage of low drug resistance. The higher water content and 3D network structure of the hydrogels are beneficial for maintaining antimicrobial peptide activity and help to prevent degradation. The antimicrobial peptide released from hydrogels also hasten the local wound healing by promoting epithelial tissue regeneration and granulation tissue formation. Objective: This study aimed at developing sodium alginate based hydrogel loaded with a novel antimicrobial peptide Chol-37(F34-R) and to investigate the characteristics in vitro and in vivo as an alternative antibacterial wound dressing to treat infectious wounds. Methods: Hydrogels were developed and optimized by varying the concentrations of crosslinkers and subjected to various characterization tests like cross-sectional morphology, swelling index, percent water contents, water retention ratio, drug release and antibacterial activity in vitro, and Pseudomonas aeruginosa infected wound mice model in vivo. Results: The results indicated that the hydrogel C proved superior in terms of cross-sectional morphology having uniformly sized interconnected pores, a good swelling index, with the capacity to retain a higher quantity of water. Furthermore, the optimized hydrogel has been found to exert a significant antimicrobial activity against bacteria and was also found to prevent bacterial infiltration into the wound site due to forming an impermeable barrier between the wound bed and external environment. The optimized hydrogel was found to significantly hasten skin regeneration in animal models when compared to other treatments in addition to strong inhibitory effect on the release of pro-inflammatory cytokines (interleukin-1β and tumor necrosis factor-α). Conclusions: Our results suggest that sodium alginate -based hydrogels loaded with Chol-37(F34-R) hold the potential to be used as an alternative to conventional antibiotics in treating infectious skin wounds.

빈맥을 이용한 심부전 모델에서 회복궤도 (Recovery Trajectory in Tachycardia Induced Heart Failure Model)

  • 배경: 빈맥을 이용하여 심부전을 만드는 방법은 확장성 심근증 모델 중에서 가장 좋은 방법으로 심장에 외과적 손상 혹은 약물의 독성을 최소화할 수 있고 사람의 심부전에 가장 가까우며 조작하기 쉬운 장점이 있다. 새로운 술식의 효과를 검증하기 위하여 심부전이 진행중인 모델에서 동물실험을 시행하는 것은 결과를 얻기 전에 실패할 가능성이 높다. 심부전의 진행을 중지시킨 회복궤도의 변화를 비교하는 방법은 심부전 악화에 의한 사망률을 줄일 수 있는 새로운 방법이지만 빈맥 조작 기술에 따라 저자마다 상당한 차이가 있어 이에대한 자료의 정립이 필요한 실정이다. 대상 및 방법: 21마리의 개(체중 25-35kg)를 대상으로 1)정상의 심장, 2) 심부전 심장, 3) 회복기 4주 4) 회복기 8주 등 4가지로 나누었다. 전신마취하에 우심실첨부에 박동기 전극을 삽입하여 빈맥은 처음 170회/분부터 매주 20회씩 프로그래머를 이용하여 증가시켰다. 4주 후 심부전이 발생하면 8주간의 회복기 동안 회복 궤도를 추적하였다. 심장의 크기와 혈역학적 변화를 관찰하고자 초음파는 2주마다, Swan-Ganz 도자와 열희석법은 4주마다 검사를 실시하여 이완기 말기 좌심실 체적, 수축기말기 좌심실 체적, 심박출율, 중심정맥압, 폐동맥압, 폐동맥 쐐기압, 우심실압, 일회박출량 등을 측정하였으며, 정상과 심부전 심장 상태에서 혈중 카테콜라민을 측정하였다. 그 외 심전도 및 대퇴동맥 도자를 넣어 맥박수, 혈압을 측정하였다. 정상심장, 심부전 심장, 회복기 4주 및 8주에서 측정한 값은 평균$\pm$표준편차로 표시하였다. 결과: 4마리(20%)가 심부전에 의한 합병증으로 사망하였다. 이완기 말기 좌심실 체적은 측정시기에 따라 40.8$\pm$7.4, 82.1$\pm$21.1, 59.9$\pm$7.7, 46.5$\pm$6.5ml로 수축기말기 좌심실 체적과 비슷한 변화양상을 보였으며 심박출율은 50.6$\pm$4.1, 17.5$\pm$5.8, 36.3$\pm$7.3, 41.5$\pm$2.4%였다. 혈압과 맥박은 의의 있는 양상을 보이지 않았으며 중심정맥압, 우심실압, 폐동맥압, 폐동맥 쐐기압 등은 심부전 시에 의의 있는 증가를 보이다가 회복기에는 감소하는 양상을 볼 수 있다. 일회박출량은 21.5$\pm$8.2, 12.3$\pm$3.5, 17.9$\pm$4.6, 15.5$\pm$3.4ml으로 회복기에 심부전 상태로부터 회복하는 경향을 볼 수 있었다. 혈중 카테콜라민은 정상 133.3$\pm$60.0pg/dL에서 심부전 시에는 479.4$\pm$327.3pg/dL로 증가를 보였다(p=0.008). 결론: 빈맥을 이용한 심부전 모델은 외과적손상이 적고, 병의 정도를 임의로 조절할 수 할 수 있는 간편한 방법이다. 회복기에는 심기능 및 심장비대가 회복하는 경향을 보이므로 향후 새로운 술식의 평가를 위하여 회복 궤도를 이용하는 경우 실험 동물의 심부전 악화에 의한 사망율을 줄일 수 있는 새로운 방법이다.

A Study on Relationship between Physical Elements and Tennis/Golf Elbow

  • Objective: The purpose of this research was to assess the agreement between job physical risk factor analysis by ergonomists using ergonomic methods and physical examinations made by occupational physicians on the presence of musculoskeletal disorders of the upper extremities. Background: Ergonomics is the systematic application of principles concerned with the design of devices and working conditions for enhancing human capabilities and optimizing working and living conditions. Proper ergonomic design is necessary to prevent injuries and physical and emotional stress. The major types of ergonomic injuries and incidents are cumulative trauma disorders (CTDs), acute strains, sprains, and system failures. Minimization of use of excessive force and awkward postures can help to prevent such injuries Method: Initial data were collected as part of a larger study by the University of Utah Ergonomics and Safety program field data collection teams and medical data collection teams from the Rocky Mountain Center for Occupational and Environmental Health (RMCOEH). Subjects included 173 male and female workers, 83 at Beehive Clothing (a clothing plant), 74 at Autoliv (a plant making air bags for vehicles), and 16 at Deseret Meat (a meat-processing plant). Posture and effort levels were analyzed using a software program developed at the University of Utah (Utah Ergonomic Analysis Tool). The Ergonomic Epicondylitis Model (EEM) was developed to assess the risk of epicondylitis from observable job physical factors. The model considers five job risk factors: (1) intensity of exertion, (2) forearm rotation, (3) wrist posture, (4) elbow compression, and (5) speed of work. Qualitative ratings of these physical factors were determined during video analysis. Personal variables were also investigated to study their relationship with epicondylitis. Logistic regression models were used to determine the association between risk factors and symptoms of epicondyle pain. Results: Results of this study indicate that gender, smoking status, and BMI do have an effect on the risk of epicondylitis but there is not a statistically significant relationship between EEM and epicondylitis. Conclusion: This research studied the relationship between an Ergonomic Epicondylitis Model (EEM) and the occurrence of epicondylitis. The model was not predictive for epicondylitis. However, it is clear that epicondylitis was associated with some individual risk factors such as smoking status, gender, and BMI. Based on the results, future research may discover risk factors that seem to increase the risk of epicondylitis. Application: Although this research used a combination of questionnaire, ergonomic job analysis, and medical job analysis to specifically verify risk factors related to epicondylitis, there are limitations. This research did not have a very large sample size because only 173 subjects were available for this study. Also, it was conducted in only 3 facilities, a plant making air bags for vehicles, a meat-processing plant, and a clothing plant in Utah. If working conditions in other kinds of facilities are considered, results may improve. Therefore, future research should perform analysis with additional subjects in different kinds of facilities. Repetition and duration of a task were not considered as risk factors in this research. These two factors could be associated with epicondylitis so it could be important to include these factors in future research. Psychosocial data and workplace conditions (e.g., low temperature) were also noted during data collection, and could be used to further study the prevalence of epicondylitis. Univariate analysis methods could be used for each variable of EEM. This research was performed using multivariate analysis. Therefore, it was difficult to recognize the different effect of each variable. Basically, the difference between univariate and multivariate analysis is that univariate analysis deals with one predictor variable at a time, whereas multivariate analysis deals with multiple predictor variables combined in a predetermined manner. The univariate analysis could show how each variable is associated with epicondyle pain. This may allow more appropriate weighting factors to be determined and therefore improve the performance of the EEM.

서울시 중년남성에서 육체적 활동량이 총 사망률에 미치는 영향에 관한 코호트 연구 (A Cohort Study of Physical Activity and All Cause Mortality in Middle-aged Men in Seoul)

  • Although previous studies revealed the association of physical activity with mortality rate, it is unclear whether there is a linear trend between physical activity and mortality rate. In this study, the association of physical activity with the risk of all-cause mortality was analysed using Cox's proportional hazard model for a cohort of 14,204 healthy Korean men aged 40-59 years followed up for 4 years(Jan. 1993-Dec. 1996). Physical activity and other life style were surveyed by a postal questionnaire in December 1992. Total of 14,204 subjects were grouped into quartiles by physical activity. Using death certificate data, 123 deaths were identified. The second most active quartile had a lowest mortality .ate with relative risk of 0.44(95% C.I. : 0.23-0.84) compared with most sedentary quartile, showing a J-shape pattern of physical activity-mortality curve. By examining the difference in proportion of cause of the death between most active quartile and the other quartiles, there was no significant difference of proportional mortality from cardiovascular deaths, cerebrovascular deaths or deaths from trauma. The covariates were stratified into two group between which the trend of RR was compared to test the effect modification. There was no remarkable effect modification by alcohol intake, smoking, body mass index, calorie consumption, percent fat consumption. In conclusion, moderate activity was found to have more protective effect on all-cause mortality than vigorous activity and that the J-shape pattern of physical activity-mortality curve was not due to the difference of mortality pattern or effect modification by alcohol intake, smoking, body mass index, calorie consumption and percent fat consumption.

파라쿼트를 투여한 백서의 급성 폐 손상 모델에서 메틸렌블루 투여가 Inducible Nitric Oxide Synthase 유전자 발현에 미치는 효과에 관한 연구 (The Effect of Methylene Blue on Inducible Nitric-oxide Synthase in a Rat Model of Acute Lung Injury Induced by Paraquat)

  • Purpose: This study was designed to determine if methylene blue inhibited the lipid peroxidation, the production of NO, and the gene expression of iNOS in acute lung injury induced by paraquat and if the inhibitory effect was dose dependent. Methods: Female Sprague-Dawley rats were divided into four groups: the control group, the group treated with paraquat only, the group treated with paraquat and a low dose of methylene blue (2 mg/kg), and the group treated with paraquat and a high dose of methylene blue (20 mg/kg). Methylene blue was administered via the jugular vein 1 h after paraquat administration, and animals were sacrificed 6 and 24 h after paraquat administration. Malondialdehyde (MDA) as lipid peroxidation, reduced glutathione (GSH) as an antioxidant defense, the plasma NO concentration, and the expression of iNOS mRNA in the lung tissue were measured Results: Lung MDA contents decreased, with no significant difference between the methylene-blue groups and the paraquat-only group. Lung GSH contents were significantly elevated at 24 h in the methylene-blue groups compared with the paraquat-only group. Plasma NO concentrations were significantly reduced at 6 and 24 h in the methylene-blue groups compared with the paraquat-only group. There was also a significant decrease in the plasma NO concentration at 6 h in the high-dose methylene-blue group compared with the low-dose methylene-blue group. The expression of iNOS mRNA in the lung tissue was slightly decreased in the methylene-blue groups. It was also markedly increased at 24 h in the paraquat-only group compared with the methylene-blue groups. The gene expression was relatively decreased in the high-dose methylene-blue group compared with the low-dose methylene-blue group. Conclusion: This study suggests that methylene blue has an inhibitory effect on the plasma NO concentration and the expression of iNOS mRNA in lung injury induced by paraquat. No inhibitory effect of methylene blue on lipid peroxidation or dose-dependent inhibitory effects were clearly shown.

  • Disposable blade is widely used for palatal and oral mucosal incision in oral and maxillofadal surgery nowadays, But its design and durability need for improvement, Especially, there are so many hard tissues intraoral area, such as bone and tooth, therefor the sharpness of the surgical blade was easily destroyed, The purpose of this study was to make basic data for developing new design of surgical blade using in oral and maxillofacial area including for the patients who have cleft lip and palate deformities, Some questionnaires about the usefulness of currently used surgical blades were sent to 150 dentists, the 54 of them made a reply, Secondly, The used-once blade and fresh new blade were examined under the scanning electron microscope with the 4000-times magnification, Lastly, the tissue reaction following the surgical incision with a fresh-new and a used blade on rat buccal cheek mucosa and hard palate was evaluated with light microscope with hematoxilin-eosin staining, The time interval from the surgical trauma to taking a sample were 1 day, 3 days, 7 days, and 14 days, At each time schedule, 2 Sprague-Dawley rats were sacrificed, Many dentists were agreed to need for changing the design of the surgical blades and also demand to improve the durability of the blades, They were also eager to adopt the new design of blade if it was available, The blade used in surgical extraction procedure was heavily damaged in its sharpe edge of number 15 blade, The histological differences were not prominent, but the delayed healing was detected in buccal mucosal defects especially in the surgical group with used blade, There are slight different changes in hard palatal defects between a used and a new blade group, In this study, we could find that there are imperative demanding on improvement of surgical blade design and durability for oral and maxillofadal area, The blade currently using in surgical extraction was easily damaged, The animal model of this study was not perfect for the purpose of this study.

Periodontal and endodontic pathology delays extraction socket healing in a canine model

  • Purpose: The aim of the present exploratory study was to evaluate extraction socket healing at sites with a history of periodontal and endodontic pathology. Methods: The mandibular 4th premolar teeth in 5 adult beagle dogs served as experimental units. Periodontal and endodontic lesions were induced in 1 premolar site in each animal using wire ligatures and pulpal exposure over 3 months (diseased sites). The contralateral premolar sites served as healthy controls. The mandibular 4th premolar teeth were then extracted with minimal trauma, followed by careful wound debridement. The animals were sacrificed at days 1, 7, 30, 60, and 90 post-extraction for analysis, and the healing patterns at the healthy and diseased extraction sites were compared using radiography, scanning electron microscopy, histology, and histometry. Results: During the first 7 days of healing, a significant presence of inflammatory granulation tissue was noted at the diseased sites (day 1), along with a slightly accelerated rate of fibrin clot resolution on day 7. On day 30, the diseased extraction sites showed a greater percentage of persistent fibrous connective tissue, and an absence of bone marrow formation. In contrast, healthy sites showed initial signs of bone marrow formation on day 30, and subsequently a significantly greater proportion of mature bone marrow formation on both days 60 and 90. Radiographs exhibited sclerotic changes adjoining apical endodontic lesions, with scanning electron microscopy showing collapsed Volkmann canals protruding from these regions in the diseased sites. Furthermore, periodontal ligament fibers exhibited a parallel orientation to the alveolar walls of the diseased sites, in contrast to a perpendicular arrangement in the healthy sites. Conclusions: Within the limitations of this study, it appears that a history of periodontal and endodontic pathology may critically affect bone formation and maturation, leading to delayed and compromised extraction socket healing.

가토대퇴정맥에서 미세혈관문합시 Argatroban의 국소세척 및 전신투여가 혈전형성에 미치는 영향 (EFFECTS OF LOCAL IRRIGATION AND/OR INTRAVENOUS ADMINISTRATION OF ARGATROBAN ON THROMBOSIS IN MICROVASCULAR ANASTOMOSES OF FEMORAL VEIN OF RABBIT)

  • Purpose: For the reconstruction of maxillofacial defect created by trauma, infection, or tumor etc, the role of microvascular anastomosis or vessel graft has been increased. Many methods has been tried to increase the success rate of microvascular anastomosis. Various anticoagulants and thrombolytic agents have been used to reduce the failure rate of microvascular anastomosis and avoid re-operation. Many drugs, however, have been used in the limited cases because most of these drugs may cause complications, such as allergy, fever or systemic bleeding. This study was performed to evaluate the influence of the Argatroban on patency and thrombosis in microvascular anastomosis when it is used for local irrigation or general administration. Materials & methods: Eight mature rabbits, weighing 2kg, were used. After exposing both femoral veins, the artificial thrombotic model was made by crushing injury using a smooth needle holder, and the transverse incision were made on femoral vein. The animals were divided into 4 groups according to Argatroban administration methods; control group (n=4), topical irrigation of lumen with saline solution; experimental group 1 (n=4), topical irrigation of lumen with Argatroban saline solution; experimental group 2 (n=4), topical irrigation of lumen with heparin followed by intravenous injection of Argatroban; experimental group 3 (n=4), topical irrigation of lumen with Argatroban followed by IV of Argatroban. Microvascular anastomosis was done with 10-0 Ethilon. The patency was evaluated by empty-and-refill test 30 minutes and 3 days after microanastomosis. The thrombus formation was examined 3 days after microanastomosis by surgical microscope. The histologic findings were also examined. Results: 1. Thirty minutes after microvascular anastomosis, the patency of all experimental groups was better than that of control group, but there was no significant difference among groups. 2. Three days after microvascular anastomosis, the patency of all experimental groups was more improved than that of control group (p<0.05). There was no significant difference among experimental groups. 3. Three days after microvascular anastomosis, the amount of thrombus in all experimental groups was less than that of control group (p<0.05). There was no significant difference among experimental groups. 4. Histologically, a lot of luminal thrombus was observed around sutured area in control group. Few luminal thrombus was observed in all experimental groups. The necrotic changes were observed on the sutured vein wall in all specimens. Conclusion: These results indicate that topical irrigation and/or intravenous administration of Argatroban is effective in improving patency and preventing thrombus formation after microvascular anastomosis.
