An Atypical Case of Middle East Respiratory Syndrome in a Returning Traveler to Korea from Kuwait, 2018

  • We report a case of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in a 61-year-old businessman returning from Kuwait. The patient arrived there on August 16, 2018, developed watery diarrhea on August 28 (day 0), and came back to Korea on September 7 (day 10) as his condition worsened. Upon arrival, he complained of diarrhea and weakness, but denied any respiratory symptoms, and he directly went to visit an emergency room. Chest radiography revealed interstitial infiltrates in the lungs, and he was immediately transferred to an isolation unit. Quantitative real-time PCR analysis of sputum samples taken on day 11 returned positive for MERS-CoV. No secondary MERS-CoV infection was identified among people who had close contact with him. This case underscores the importance of a high index of suspicion of MERS-CoV infection in any febrile patients who present after a trip to the Middle East.

  • Purpose: This study was done to investigate knowledge of and compliance with the multidrug-resistant organism (MDRO) infection control guidelines among student nurses on clinical practicum, Methods: survey questionnaire on MORO infection control was administered to a convenience sample of 259 nursing students from 3 different nursing schools Results: The mean knowledge score was 28.01/39 (71.82%). The percentages of correct answers for basic concepts, route of transmission, hand washing/ protective devices and environment management, were 55.40, 81.14, 84.94 and 69.17 respectively. The mean compliance score was 3.83/5. The compliance scores for education, communication, contact precaution, environment management, and hand washing were 3.06, 3.33, 3.86, 4.50, 3.92 and 4.29 respectively. 96.9% of subjects knew that they should wash hands after touching MORO patient while only 22.8% of subjects knew how to collect samples for VRE surveillance culture, The highest compliant item was hand washing after touching MORO patient. The Lo-west compliant item was referring to infection control manual. Conclusion: Comprehensive MDRO infection control education programs for nursing students should be developed to decrease MORO infection.

Seed Transmission of Tomato yellow leaf curl virus in White Soybean (Glycine max)

  • Tomato yellow leaf curl virus (TYLCV) infection of the common bean (Phaseolus vulgaris) has been reported, but soybean (Glycine max) has not previously been identified as a TYLCV host. Five cultivars of white soybean were agro-inoculated using an infectious TYLCV clone. At 30 days post-inoculation, they showed infection rates of 25% to 100%. Typical TYLCV symptoms were not observed in any inoculated plants. To examine whether TYLCV was transmitted in soybean seeds, DNA was isolated from bundles of five randomly selected seeds from TYLCV-inoculated soybean plants and amplified with a TYLCV-specific primer set. With the exception of one bundle, all bundles of seeds were verified to be TYLCV-infected. Virus dissemination was also confirmed in three of the 14 bunches. Viral replication was also identified in seeds and seedlings. This is the first report demonstrating that soybean is a TYLCV host, and that TYLCV is a seed-transmissible virus in white soybean.

Occupationally Acquired Plasmodium knowlesi Malaria in Brunei Darussalam

  • Simian malaria is a zoonotic disease caused by Plasmodium knowlesi infection. The common natural reservoir of the parasite is the macaque monkey and the vector is the Anopheles mosquito. Human cases of P. knowlesi infection has been reported in all South East Asian countries in the last decade, and it is currently the most common type of malaria seen in Malaysia and Brunei. Between 2007-2017, 73 cases of P. knowlesi infection were notified and confirmed to the Ministry of Health in Brunei. Of these, 15 cases (21%) were documented as work-related, and 28 other cases (38%) were classified as probably related to work (due to incomplete history). The occupations of those with probable and confirmed work related infections were border patrol officers, Armed Forces and security personnel, Department of Forestry officers, boatmen and researchers. The remaining cases classified as most likely not related to work were possibly acquired via peri-domestic transmission. The risk of this zoonotic infection extends to tourists and overseas visitors who have to travel to the jungle in the course of their work. It can be minimised with the recommended use of prophylaxis for those going on duty into the jungles, application of mosquito/insect repellants, and use of repellant impregnated uniforms and bed nets in jungle camp sites.

Use of ALLGIO Probe Assays for Detection of HBV Resistance to Adefovir in Patients with Chronic Hepatitis B, Kerman, Iran

  • Hepatitis B virus (HBV) infection is contagious with transmissiobn vertically or horizontally by blood products and body secretions. Over 50% of Iranian carriers contracted the infection prenatally, making this the most likely route of transmission of HBV in Iran. To evaluate the resistance to adefovir (ADV) therapy in patients with chronic hepatitis B infection, a study was conducted on 70 patients (63 males and 7 females), who had received in first line lamivudine and second line adefovir. All were tested for the presence of hepatitis B surface antigen (HBsAg), hepatitis B envelope antigen (HBeAg), serum alanine amino transferase (ALT) level and HBV DNA load before and after treatment with ADV. In all samples, resistance to lamivudine and ADV was tested with real time PCR. Among seventy patients with chronic hepatitis B infection, 18 (25.7%) were resistant to LAM and 8 (11.4%) were resistant to ADV. Only one patient was negative for the presence of HBS-Ag (5.6%) and two were negative for HBe-Ag (11.1%). In this study we used a new method (ALLGIO probe assay) that has high sensitivity in detection of adefovir resistance mutants, which we recommend to other researchers. Mutant strains of the YMDD motif of HBV polymerase can be found in some patients under treatment with lamivudine and ADV. ADV has been demonstrated to be efficient in patients with lamivudine resistant HBV.

가금인플루엔자 바이러스의 인체 감염 가능성 (The Possibility of Avian Influenza Virus Infection in Human)

  • Avian influenza(AI) is an epizootic disease of variable severity caused by type A influenza viruses of the orthomyxovirus group. Chickens were the most frequently affected avian species with AI viruses. There were many outbreaks of fowl plague, now known as highly pathogenic AI(HP AI), throughout the world since Perroncito described the fowl plague in 1978 in Italy. In recent years HPAI viruses of different serotypes such as H5, H7 and H9 has been isolated from humans on several occasions either related with outbreak of HPAI in birds or not. In 1997, one of the most noteworthy events in AI history was the human mortality with H5N1 HPAI virus infection in Hong Kong. Six persons of total 18 persons with clinical signs of influenza were died. Recently the human cases with mortality related with HP AI outbreaks in poultry industry has been increased such as outbreaks of HP AI throughout Asia countries including Korea, Japan, China, Vietnam, Thailand and others in 2003. Although these outbreaks revealed the capable of spreading from birds to human, the capability for transmission between people was not clear. Therefore, this report will review the possibility of HP AI infection in human associated with HPAI outbreak in poultry industry.

Epidemiological Studies of Avian Paramyxovirus Type 4 and 6 in Commercial Chicken Flocks in Korea

  • Avian paramyxovirus (APMV) type 4 and 6 were isolated during an avian influenza (AI) surveillance program of wild birds. This study also conducted experimental infection of wild-bird-origin APMV type 4 and 6 in specific pathogen free (SPF) chickens to study pathogenicity and transmission within domestic flocks. In addition, serological prevalence data of APMV type 4 and 6 in domestic fowls was conducted with chicken sera collected from 2007 to 2009 in order to understand infection status. The results of the animal experiment showed that APMV type 4 and 6 had the ability to infect chickens with sero-conversion and to transmit the virus from infected birds to contacted birds, but showed low pathogenicity. Serological tests revealed that APMV type 4 was widespread in the poultry industry, especially in layer flocks, but the positive rate for APMV type 6 was very low. This study concluded that wild bird-origin APMV type 4 and 6 could infect the chickens by inter-species transmission and the seroprevalence of APMV type 4 was quite high in Korean poultry. However, since almost all the chicken flocks had a high level of antibody titer against APMV type 1, there was possibility of cross reaction between APMV type 1 and 4, which made the interpretations more complicated. In order to understand infection status in the natural environment, additional study is necessary regarding the seroprevalence of APMV type 4 and 6 in the wild bird population.

돼지 생식기호흡기증후군바이러스의 농장단위 방역대책 수립을 위한 혈청학적 및 바이러스학적 감염유형 분석법 적용 및 비교 (Comparison of Serological and Virological Analysis for Infection Patterns of Porcine Reproductive and Respiratory Syndrome Virus to Establish a Farm Level Control Strategy)

  • Porcine reproductive and respiratory syndrome virus (PRRSV) has plagued pig populations worldwide causing severe economical impacts. In order to establish effective strategies for prevention of PRRS, infection patterns on the herd level are primarily evaluated. In the present study, therefore, serological and virological analyses were conducted in 20 pig farms suffering from PRRS. Seroprevalence levels in each farm were grouped into 3 patterns: SN (Stable sow groups/Not infected piglet groups, SI (Stable sow groups and Infected piglet groups), and UI (Unstable sow groups and Infected piglet groups). The rates of each serological pattern were 15% (n=3), 10% (n=2), and 75% (n=15), respectively. In addition, the pattern analysis was extended to virological monitoring on the same farms that further included suckling pig groups. As a result, the infection pattern was classified into 4 categories: SNI (Stable sow groups/Not infected suckler groups/Infected piglet groups), SII (Stable sow groups/Infected suckler groups/Infected piglet groups), UNI (Unstable sow groups/Not infected suckler groups/Infected piglet groups), and UII (Unstable sow groups/Infected suckler groups/Infected piglet groups). The rates of each viroprevalence were estimated at 50% (n=10), 30% (n=6), 10% (n=2), and 10% (n=2), respectively. PRRSV viroprevalence results of suckling pig groups revealed that 8 farms were considered virus positive. In 2 farms among these farms, PRRSV appeared to be transmitted vertically to suckling piglets from their sows. In contrast, piglet-to-piglet horizontal transmission of PRRSV seemed to occur in sucking herds of the remaining farms. Thus, this virological analysis on suckling piglets will provide useful information to understand PRRSV transmission routes during the suckling period and to improve a PRRS control programs. Our seroprevalence and viroprevalence data found that infection patterns between sow and piglet groups are not always coincident in the same farm. Remarkably, 15 farms belonging to the UI seroprevalence pattern showed four distinct viroprevalence patterns (SNI; 7, SII; 4, UNI; 2 and UII; 2). Among these farms, 11 farms with unstable seroprevalence sow groups were further identified as the stable viroprevalence pattern. These results indicated that despite the absence of typical seroconversion, PRRSV infection was detected in several farms, implying the limitation of serological analysis. Taken together, our data strongly suggests that both seroprevalence and viroprevalence should be determined in parallel so that a PRRS control strategies can be efficiently developed on a farm level.

국내 마우스에서 분리된 Cryptoporidium parvum의 송아지로의 감염시험 (Experimental Cryptosporidium parvum infection in a Korean native calf isolated from a Korean mouse)

  • 국내 마우스에서 분리하여 계대중인 Cryptosporidium parvum 원충이 송아지에 감염되는지 알아보기 위하여 25일령의 한우 송아지에 1×106개의 오오시스트를 감염시킨 결과 점종 후 4일 째부터 분변 내에 오오시스트를 배출하였다. 오오시스트의 배설은 8일간 지속되었고, 그 수는 인충 점종 후 6일째에 가장 많았다(분변 gram당 4.9×105개 오오시스트). 감염된 송아지의 분변 내로 오오시스트가 배출되는 동안 분변의 성상은 정상이었다. 이상의 결과로 볼 때 국내 마우스에서 분리된 C. parvum은 송아지에게도 교차감염이 이루어져 숙주특이성이 없음을 알 수 있었다.

한우 송아지의 소바이러스성 설사바이러스 지속감염률 조사 (Prevalence for persistent infection with bovine viral diarrhea virus in Korean native calves)

  • Bovine viral diarrhea (BVD) is very important disease in cattle industry with a worldwide distribution. Detection and elimination of persistently infected calves with bovine viral diarrhea virus (BVDV) was valuable strategy for BVD eradication because those calves were main source for transmission. We surveyed persistent infection with BVDV by reverse transcription polymerase chain reaction (RT-PCR) and immunohistochemistry (IHC) using whole blood and skin. Five hundred thirty nine Korean native calves were tested. Four calves (0.7%) were positive for BVDV antigen for both tests. Those calves remained positive for follow-up by RT-PCR and IHC. Therefore they were identified as persistently infected with BVDV. We confirmed that immunohistochemistry using skin biopsy samples was very useful tool to detect persistently infected calves with BVDV. As far as we know, this would be first report on persistent infection with BVDV in Korea.
