Low Back Pain due to Lumbosacral Transitional Vertebra -A case report- (요천부 이행성 척추증에 의한 요통의 치험 -증례 보고-)

  • The presence of an enlarged transverse process on one or both sides of the last lumbar vertebra is a common congenital anomaly of the lumbar vertebra. It is thought to be a genetic or developmental anatomical variant. The first reported assimilation of the fifth lumbar vertebra into the sacrum associated with low back pain was in 1917 by Bertolotti. However, clinical significance of lumbosacral transitional vertebra has not been fully considered due to lack of scientific investigations dealing with it. We experienced a case of symptomatic lumbosacral transitional vertebra during management of low back pain. Low back pain was relieved after infiltration of local anesthetics and steroid into the false joint of lumbosacral transitional vertebra. This result may possibly indicate a significant correlation between low back pain and lumbosacral transitional vertebra.

The Study of Transitional Justice in El Salvador (발전전략으로서의 과거청산 - 엘살바도르 이행기 정의의 특수성 사례 분석 -)

  • El Salvador had suffered some 75,000 casualties, mostly civilian, from violent civil conflict(1980-92). In 1992, after negotiations, the government and FMLN signed a historic comprehensive peace accord which brought an end to the war and instituted wide-reaching political and social reforms. Many scholars call it as Transitional Justice in El Salvador. Transitional Justice in El salvador has had two identifiable stages. In the First stage, institutional reforms, such as DDR(disarmament, demobilization, reintegration), and a truth commission were implemented. The second phase corresponds to the period subsequent to the truth commission report, with the failure to implement the commission's recommendations, including those related to reparations and justice. This essay explain how was transitional justice in El salvador different from the other cases, and what was the purpose of extraordinary transitional justice in El Salvador. In detail, the first section of this essay examines the history of the civil war and peace process in El Salvador, and then explores the relationships between cold war and transitional justice in El Salvador. Finally, this essay suggests that truth commission's mandate which investigate 'serious acts of violence that have occurred since 1980' was very important role to accomplish peace and transitional justice in El Salvador.

Flow Characteristics of Transitional Boundary Layers on a Flat Plate Under the Influence of Freestream Turbulent Intensity (자유유동 난류강도 변화에 따른 평판위 천이 경계층의 유동특성에 관한 실험적 연구)

  • Flow characteristics in transitional boundary layers on a flat plate were experimentally investigated under three different freestream conditions i. e. uniform flow with 0.1 % and 3.7% freestream turbulent intensity and cylinder-wake with 3.7% maximum turbulent intensity. Instantaneous streamwise velocities in laminar, transitional and turbulent boundary layers were measured by I-type hot-wire probe. For estimation of wall shear stresses on the flat plate, measured mean velocities near the wall were applied to the principle of Computational Preston Tube Method (CPM). Distributions of skin friction coefficients were reasonably predicted in all developed boundary layers. Intermittency profiles, which were estimated using Conditional Sampling Technique in transitional boundary layers, were also consistent with previously published data. It was predicted that the incoming turbulent intensity had more influence on transition onset point and transition process than freestream turbulent intensity existed just over the transition region. It was also confirmed that non-turbulent and turbulent profiles in transitional boundary layers could not be simply treated as Blasius and fully turbulent profiles.

Development and Effects of the Collaborative Transitional Care Program for Continuity of Care in Patients Transferred to General Wards from ICUs (중환자실 환자의 간호지속성 유지를 위한 중환자실-병동간 연계형 전환간호 프로그램 개발 및 효과)

  • Purpose: This study was conducted to develop and evaluate the collaborate transitional care program for improving continuity of care in patients transferred to general wards from ICUs. Methods: 18 years and older who were hospitalized in adult intensive care units at A university affiliated medical center was recruited for the study. The experimental group for patients transferred from an ICU consisted of 33 patients and family caregivers; 34 patients and family caregivers for the control group. This study was utilized a quasi-experimental research design. The collaborative transitional care program was administered in transfer process. Data were collected two times by interviews, medical records, and telephone using questionnaires. Results: There were statistically significant differences between the two groups on relocation stress (p<.001), perceived health status (p<.001), satisfaction of caring (p=.011), physical domain (p=.022) and mental domain (p<.001) of the QOL. There were significant differences between the families of the two groups on burden (p<.001) and satisfaction of caring (p<.001). Conclusion: The collaborative transitional care programs administered in transfer process to general wards from an ICU have positive effects on patients and families' intrinsic and extrinsic factors. This program will be able to be utilized in clinical fields to improve continuity of care for patients and families between ICUs and general wards.

A Case of Bertolloti Syndrome -A case report- (Bertolotti 증후군 치험 1예 -증례 보고-)

  • Bertolloti syndrome is the association between low back pain (LBP) and the presence of a lumbosacral transitional vertebrae. A transitional vertebrae is composed of one or both transverse process connecting with the sacrum or ilium and the presence of a vestigial intervertebral disc space caudal to the transitional vertebrae. We experienced a case of low back pain with intermittent radiating pain in patient with complete lumbosacral sacralization. Long term relief of low back pain was maintained with intra and/or periarticular infiltration of local anesthetics and steroid. The relief of LBP suggests a correlation between LBP and lumbosacral sacralization.

Model Development for Machining Process Sequencing and Machine Tool Selection (가공 순서 결정과 기계 선택을 위한 모형 개발)

  • Traditionally, machining process sequence was influenced and constrained by the design information obtained from CAD data base, i.e., class of operations, geometric shape, tooling, geometric tolerance, etc. However, even though all the constraints from design information are considered, there may exist more than one way to feasibly machine parts. This research is focused on the integrated problem of operations sequencing and machine tools selection in the presence of the product mix and their production volumes. With the transitional costs among machining operations, the operation sequencing problem can be formulated as a well-known Traveling Salesman Problem (TSP). The transitional cost between two operations is expressed as the sum of total machining time of the parts on a machine for the first operation and transportation time of the parts from the first machine to a machine for the second operation. Therefore, the operation sequencing problem formulated as TSP cannot be solved without transitional costs for all operation pairs. When solved separately or serially, their mutual optima cannot be guaranteed. Machining operations sequencing and machine tool selection problems are two core problems in process planning for discretely machined parts. In this paper, the interrelated two problems are integrated and analyzed, zero-one integer programming model for the integrated problem is formulated, and the solution methods are developed using a Tabu Search technique.

  • The purpose of this study is to clarify the transitional process of spatial configuration of 'Topdong Yangdalmal'; One of the existing 22 old settlements in cheongju city. The results are as follows. 1) Side roads are derived from Inner road until 1970, so large-scale lots are subdivided small-scale lots. The houses are mostly in 1970s and 1980s. The existing old houses are 11, their orientation are East South, and the entrance gives on side road. The orientation, court yard, and entrance of the houses nearby side roads keep ongoing, in spite of alteration and extension. Especially, Entrance is changed by the new arterial roads and subdivided lots, but, the houses nearby side roads are continued as in the past. 2) The newly built houses in 1970's are 20, and it's arrangements are equal to existing old one. The orientation of houses after 1980's is the same in 1970's. But, the entrance is changed by subdivided lots, and the size of the court yard are reduced or disappeared. In other hand, the houses as the same size of the existing houses, and Storyincrease continue court yard as in the past. Therefore, the houses which are nearby inner road, and increase story, are continued, in spite of the newly built one.

  • This study explores the process of transitional career choice for North Korean defectors who are successfully living their lives after transition to a different system labor market. To achieve this study purpose, what is the process of transitional career choice for North Korean defectors due to the system transition? At present, he (Ed- who is he?) has a subjective sense of success in his professional life and conducted in depth interviews with three men and eight women as objects that can explain the process of experience in rich and detailed ways. To this end, the author interviewed three men and eight women who are satisfied with their current job and analyzed them with the grounded theory method proposed by Strauss & Corbin (1998). As a result, the paradigm model was derived from the central phenomenon of 'conversion of perspective', and the core category was 'conversion of perspective and challenge new career'. The transitional career choice process was derived into four stages according to the flow of time and interaction as 'reality recognition stage', 'active change recognition stage', 'support and coping strategy stage', and 'growth stage' and positive reflections from transitional learning and potential factors of planned chance skills were found.

PREDICTION OF THERMAL STRATIFICATION IN A U-BENT PIPE: A URANS VALIDATION

  • In the present study, CFD is employed to investigate phenomena occurring during a process of thermal stratification in U-bent pipes at transitional Reynolds number. URANS evaluation had been chosen for its low computational costs during transient analysis and for the evaluation of modeling performance in these conditions. Application of CFD at transitional Reynolds number and buoyancy driven flows indeed contains deeper uncertainties in relation to the range of applicability for hydrodynamic and thermal models. The methodology applied in the work points out, through validations with the basic problems constituting the complex stratified phenomenon, the applicability of the current turbulence modeling. Accurate predictions have been found in relation to transitional Reynolds number in bent pipes and region of stability induced by the gravitational field. On the other hand the defects introduced in the unstable region of the U bent pipe, are discussed in relation to the adopted modeling.

The Impact of Financial Development on Economic Growth: Empirical Evidence from Transitional Economies

  • This article examines the role of financial development in economic growth in a number of transitional economies where the financial systems were newly established or reformed only in the early 1990s to facilitate their transition from centrally planned economies to market-based ones. Based on a dataset collected from 29 transitional economies and 5 Asian developing economies covering the period 1990-2020, an empirical endogenous growth model is specified and estimated using the generalized method of moments (GMM). Three measures of financial development are used to investigate the relative role of the banking system and stock exchange market in the process of transition and growth. The results show that the three measures of financial development are crucial determinants of economic growth in transitional economies but the link seems to be in an inverted U-shape. This suggests the existence of thresholds for different channels of the financial sector to expand to positively influence growth. When becoming too large relative to the size of the economy, the financial system would have become a factor not conducive to growth. The growth convergence hypothesis is also confirmed and the impacts of other growth determinants are overall consistent with the extant literature.
