Transferred newborn from oriental hospital postpartum care center; symptoms and high risk factor (한방병원 산후조리센터에서 전원된 신생아에 대한 연구)

  • Objective : To find risk factor and symptoms for transferred newborn from postpartum care center Method : We performed a follow-up study using medical record. Subjects comprised 188 neonates(include 4 twins) and 184 women in childbed who were hospitalized in oriental postpartum care center from January, 1, 2002 to December, 31, 2002. Result: Transferred newborn was total 24, N.V.D. 17, C.S. 7, male and female were 12 separately. The symptom of transfer was fever(16), diarrhea(4), jaundice(3), convulsion(1) Transferred number and rate was male 12(10.7%), female 12(15.8%). In delivery method, total normal vaginal delivery(N.V.D.) was 136 and transferred 17(12.5%), total cesarean section(C.S.) was 52 and transferred 7(13.5%). Transferred rank was N.V.D. female(17.9%). C.S. male(15.6%), C.S. female(10%), N.V.D.(8.6%). Of the 188 neonates. immature infants were 2 and transferred 1(50%), term infants were 184, transferred 23(12.5%), post-term infants were 2 and transferred none(0%). In age of women in childbed, transferred ratio was 15.4%(over 35), 15.2%(25-29), 10.5%(30-34). In fever,. C.S. infants were more transferred than N.V.D. and male infant had a tendency to transfer in later period of hospitalization and female in early period. It seems that transfer was related to abortion. In diarrhea, all was N.V.D. and their diagnosis was rotavirus. The average age of women in childbed was yonger(28.5). In jaundice, all was 1st, and average birth weight was lighter(3153g). The average age of women in childbed was older(30). In convulsion, birth weight was lighter(3153g), and age of women in childbed was older(30). Conclusion : There's any relation with sex and delivery method, birth weight and mother's age in transfer. But we cannot find any statistically significance. Further prospective studies are needed to find risk factors in transfer.

Correlations between the Status of the Umbilical Cord and Neonatal Health Status

  • Purpose: This study aimed to identify correlations between the status of the umbilical cord and neonatal health status. Methods: In total, 172 newborns were enrolled who were admitted to the newborn nursery with a gestational age of 35 weeks or older and a body weight of 2 kg or above. Data were collected on the basic personal information of the newborns, the diameter and soft tissue status of the umbilical cord, and neonatal health status after birth. Analyses were performed using t-test, analysis of variance, χ2 test, and Fisher exact test. Results: Umbilical cord diameter exhibited a statistically significant difference by sex (t=2.71, p=.007). A thin umbilical cord diameter was associated with a 1-minute Apgar score less than 8 points (t=2.47, p=.015) and with being transferred to the intensive care unit (t=2.45, p=.015). Poor soft tissue status of the umbilical cord was associated with a 1-minute Apgar score of less than 8 points (χ2=16.68, p<.001) and with oxygen being supplied (χ2=4.81, p=.028). Conclusion: Assessing the umbilical cord diameter and status in newborns is an important tool for evaluating neonatal health status after birth, and this point also underscores the importance of professionals' careful observations in the newborn nursery.

Establishment of Human-Mouse Chimeric Animal by Injecting Human Embryonic Stem Cells into Mouse Blastocoele Cavity

  • Chimeric animals are referred to as an organism composed of tissues derived from more than one species. In order to examine if a pluripotency of embryonic stem cells can cross the limitation of a species, we tried to establish human-mouse chimeric animals. Human embryonic stem cells were genetically modified to express eGFP using eukaryonic expression vector pcDNA 3.1 (In Vitrogene) for an easy identification. After selection with neomycin, approximately 15 cells were implanted into mouse blastocoele cavity. Ten chimeric blastocysts were transferred to one of the uterine horn of 2.5 days pesudopregnent ICR female. Out of 272 blastocysts transferred to pseudopregnant recipients 20 live newborn were obtained after 20 days. When newborn were obtained, pups were quickly removed immersed into 4% PFA. By histological examination using fluorescent microscope, green fluorescence was observed from the liver, heart, and spleen in newborn mice. Three weeks after born, presence of eGFP sequence within mouse genome (tail and kidney) was reconfirmed by PCR. eGFP sequence was amplified from the progenies of the animal suggesting a genetic transmission of the transgene. These chimeric mice having human cells at the beginning of development, are expected to recognize human cells as “self”, therefore, human cells or tissues will be able to escape the immunological surveillance of the host if grafted into the animal. These animals will serve as a good model system for studying the graft rejection in tissue transplantation and the potential of the cells to work well in many human disease.

A Case of Hemolytic Disease of a Newborn by an Anti-$Di^a$ Antibody Treated with Intravenous Immunoglobulin (정맥용 면역글로불린 투여로 호전된 항-$Di^a$ 항체에 의한 신생아 용혈성 질환 1예)

  • Hemolytic disease in a newborn that causes early jaundice is common. It is often due to the Rh (D) and ABO incompatibility, but rarely due to unexpected antibodies. Among these unexpected antibodies, the anti-$Di^a$Dia antibody rarely occurs. The anti-$Di^a$ antibody was observed in the serum and red-cell eluate of an infant, and in the serum of his mother. The frequency of the appearance of the $Di^a$ antigen in the Korean population is estimated to be 6.4-14.5%. This paper reports a case of hemolytic disease in a newborn associated with the anti-$Di^a$ antibody. A full-term male infant was transferred to the authors' hospital due to hyperbilirubinemia the day after his birth. The laboratory data indicated a hemoglobin value of 11.6 g/dL, a reticulocyte count of 10.6%, a total bilirubin count of 14.4 mg/dL, a direct bilirubin count of 0.6 mg/dL, and a positive result in the direct Coombs' test. Due to the identification of an irregular antibody from the maternal serum, an anti-$Di^a$ antibody was detected, which was also found in the eluate made from the infant's blood. The infant had been treated with phototherapy and intravenous immunoglobulin since the second day after his birth and was discharged due to an improved condition without exchange transfusion. Therefore, in cases of iso-immune hemolytic disease in a newborn within 24 hours from birth who had a negative result in an antibody screening test, the conduct of an anti-$Di^a$ antibody identification test is recommended due to the suspicion of an anti-$Di^a$ antigen, followed by early administration of intravenous immunoglobulin.

  • The etiologic agents of haemorragic fever with ranal syndrom (HFRS) in Korea are Hantaan and Seoul virus in the genus Hantavirus, family Bunyaviridae. In order to elucidate the role of maternal immunity to Hantavirus infection in rats, the protective effect of the maternal antibody were studies by using rats experimentally infected with Seoul virus strain HR80-39. Antibody titers of sera and viral antigen against Seoul virus were investigated by indirect immunofluorscence antibody technique (IFA). The dam sera had IFA antibody titers ranging from 1:128 to 1:1,024 after parturition. In fetuses, IFA antibody titers ranged from 1: 16 to 1:64 just after birth, increased to peak titers ranged from 1:256 to 1:1,024 in the 2nd week after birth. Challenged newborn rats had IFA antibody titers ranging from 1:64 to 1:1,024 after inoculation. No viral antigen was detected in lungs or other organs of the newborn rats. The maternal antibody to Seoul virus was transferred prenatally through placenta and postnatally via colostrum from immune dams to their offspring. These results demonstrated that maternal antibody to Seoul virus was quite effective in protecting newborn rats against same virus infection.

ENDOCRINE CHANGES AND CIRCULATING INSULIN-LIKE GROWTH FACTORS IN NEWBORN CALVES FED COLOSTRUM, MILK OR MILK REPLACER

  • To examine whether colostral growth factors are transferred to the general circulation, concentrations of plasma cortisol, insulin, prolactin, growth hormone, insulin-like growth factors(IGFs) -I and -II, IGF-binding proteins(IGFBPs) and total protein were measured in newborn calves fed colostrums, milk of milk replacer before and after feeding at 12 h intervals during the first two days after birth. Plasma protein concentrations increased with time after than in milk- or milk replacer-fed calves. The mean protein concentration was greater in colostrum-fed than in milk- or milk replacer-fed calves. Plasma cortisol levels transiently declined after each feeding regardless of the type of diet, while insulin levels tended to increase. Mean concentrations of these hormones did not differ between dietary groups, nor did they change with time after birth. Plasma concentrations of prolactin and growth hormone did not differ between dietary groups and also did not change with time after birth or after feeding. Concentrations of IGF-I and IGF-II transiently increased at the second feeding period, but these, as well as plasma IGFBP profiles, were not different between groups or before and after feeding. Results did not indicate significant transfer of colostral growth factors across the newborn ruminant small intestine.

  • Congenital diaphragmatic hernia [CDH] is a surgical emergency in the newborn infant because it causes severe cardiorespiratory distress. Congenital diaphragmatic eventration [CDE] may also produce severe cardiorespiratory distress in the newborn infant. CDH is an anatomically simple defect that can be easily repaired by reduction of the displaced viscera from the pleural cavity and closure of the diaphragmatic defect. But these infants mortality has not been reduced and still remains very high. The barrier to survival is pulmonary parenchymal and vascular hypoplasia as well as the complex syndrome of persistent fetal circulation. Between May, 1985 and Oct, 1987, 4 neonates with CDH and 1 neonate with CDE were seen in respiratory distress within 12 hrs of birth at St. Francisco general hospital. Each had severe acidosis and hypoxia. And was transferred from a local clinic. They were surgically repaired within 24 hrs of birth. Three neonates lived and two died. Two of the three neonates with CDH operated in the first 6 hrs died. The remaining two [one with CDH, the other with CDE] operated between 6hrs and 24 hrs lived. One case of mortality was combined with bilateral pulmonary hypoplasia and contralateral pneumothorax. The other one case of mortality was combined with complex syndrome of persistent fetal circulation after honeymoon period.

A Case of Hypernatremic Dehydration in an Exclusively Breast-Fed Newborn Infant (모유수유 신생아에게 발생한 고나트륨혈성 탈수증 1례)

  • Sporadic reports of hypernatremic dehydration in breastfed newborn infants have appeared in medical literature for at least 3 decades. We report the first case of hypernatremic dehydration resulting from inadequate breast-feeding in Korea. A 14-day old baby, born to a mentally retarded mother, was transferred to our hospital with a body weight loss of 460 g since birth(17%) and a serum sodium(Na) level of 179 mEq/L, after initial hydration at another hospital. On admission, a cardiac murmur was heard and an enlarged liver was palpated. Cardiac ultrasonogram revealed ventricular septal defect and ostium secundum atrial septal defect. During hydration, a seizure-like attack developed. Serum Na decreased to 135 mEq/L on the 5th day of admission. Brain ultrasonography and brain magnetic resonance image revealed no remarkable abnormalities. Electroencephalography was normal. She suffered from prerenal azotemia, hyperglycemia and disseminated intravascular coagulation at admission but was treated successfully. Heart failure was also controlled with dobutamine, diuretics and digoxin.

Effect of Superovulation and Synchronization on Ovarian Response, Pregnancy Rate and Number of Newborn in Rabbit (다배란처리와 발정동기화가 난소반응, 수태율 및 산자수에 미치는 영향)

  • This study was carried out to investigate effects of superovulation and time of embryo recovery on ovarian response, recovery rate and developmental stage of embryo in donor and effects of methods of synchronization, number of corpus luteum (CL), stage of embryo and time of embryo transfer on ovarian response, conception rate and number of newborn in recipients which were transferred on 2.5, 3.5 and 4.5 days after synchronization. The results obtained are as follows; 1. The ovulation point of superovulated donor on 2.5, 3.5 and 4.5 days after copulation was 23.3, 35.3 and 23.3, respectively. The number of embryos recovered from the donors on 2.5, 3.5 and 4.5 days after copulation was 23.3, 25.8 and 19.8, respectively. The ovulation point and number of embryos recovered on 3.5 days were greater than those of 2.5 and 4.5 days. Among 232 embryos recovered on 3.5 days after copulation, 84 were blastocyst and 62 were hatching blastocyst. 2. The number of CL in recipients on 2.5, 3.5 and 4.5 days after synchronization was 3.2, 2.9 and 3.8 and showed no difference among the days. 3. When the number of CL was 0, 2-3, 4-6 and more than 7 the pregnancy raet of recipients was 0, 37.5, 66.7 and 75%, respectively. The pregnancy rate of recipients increased as the number of CL increased. 4. The pregnancy rate of transferred morula, blastocyst and hatching blastocyst was 32.0, 37.2 and 24.7%, respectively. The blastocyst nhowed highest pregnancy rate. 5. When the recipients were synchronized by HCG, the number of CL, unruptured follicle, hemorrhage, pregnancy rate and number of young were 5.5, 6.4, 3.3, 72.7% and 3.3, whereas that of GnRH were 2.3, 4.4, 2.8, 25.0% and 1.2, respectively. Recipients synchronized by HCG showed better results than GnRH. 6. When the embryos were collected on 2.5 days after copulation and transferred to the synchromized recipients, the pregnancy rate and the number of young born was 62.58% and 3.1, respectively. Those of 3.5 and 4.5 days after copulation was 57.1% and 1.3, and 37.5% and 1.6. The 2.5 days showed higher pregnancy rate and number of young born than 3.5 and 4.5 days.

  • This study was carried out to determine effectively the sex of rabbit embryos using H-Y antiserum. H-Y antiserum was obtained from inbred SD strain female rat which was immunized by injection of testis cell of inbred SD strain male rate into its spleen. The titer of antiserum was identified by sperm cytotoxicity test and culture of rabbit embryos with antiserum. The developed or undeveloped embryos were separated by exposure the embryos to the antiserum with H-Y antibody. Developed embryo were transferred to the recipients and sex of offspring were examined. 1. In the sperm cytotoxicity test, the rate of dead sperm showed no difference between two antisera from spleen and testis cell as antigens. It is confirmed that H-Y antibody in antiserum was absorbed by H-Y antigen in male rat spleen cells. 2. When rabbit morulae were exposed to antiserum and complement, the rate of embryos developed or arrested was 51 and 49% respectively and the rate was closely same as natural sex ratio of 50:50%. 3. When rabbit morulae were cultured for 12h in the medium containing antiserum produced by antigen of testis cell, the rate of embryos developed or arrested was 48 and 52% respectively and the rate was closely same as natural sex ratio of 50:50%. 4. Eighty rabbit embryos which were not affected by H-Y antiserum were transferred to four recipients. Two recipients were pregnant and born 13 pups among which 2 (14%) were male and 11 (86%) were female. In conclusion, existence of H-Y antibody in the serum from female rat immunized by injecting testis cell from newborn male rat to the spleen of the female rat was confirmed. When rabbitmorulae were exposed to H-Y antiserum and complement, about a half of embryos were developed to blastocysts. When the rabbit embryos not affected by H-Y antiserum were transferred, the rate of female offspring was 86%. Therefore, it was identified that most of embryos which were not affected by H-Y antiserum were female.
