지연성 근육통에 대한 경피신경전기자극의 주파수별 효과 비교 (The Comparative Study on the Frequency of Transcutaneous Electrical Nerve Stimulation for Delayed-Onset Muscle Soreness)

  • Objectives : The aim of this study was to investigate difference of the effects of transcutaneous electrical nerve stimulation(TENS) with different frequencies in participants having delayed onset muscle soreness(DOMS). Methods : We recruited 36 healthy participants, but 3 of them were dropped out. They were randomly divided into 3groups : 3 Hz TENS(n=11), 100 Hz TENS(n=11) and sham TENS(n=11). DOMS of the both triceps surae muscle induced by repetitive concentric, ecentric exercise. The result measurements were pain perception(visual analogue scale, VAS), mechanical pain threshold(MPT) by pressure algometer, electrical contraction and fatigue by surface electromyography. The measurements were on first visit, before and after treatment except first. This study was prospective, randomized, controlled, single-blinded trial. Results : In 100 Hz TENS group, VAS was significantly decreased during whole session compared with 3 Hz and control group, and after each treatment, too. In 3 Hz TENS group, VAS was significantly decreased during whole session compared with control group, and after 2nd, 3rd treatment, too. In 100 Hz TENS group, MPT increased the most among 3 groups during whole session and after 1st treatment, but there were no statistical significances. Conclusions : Both 3 Hz and 100 Hz TENS improved delayed onset muscle soreness, but 100 Hz TENS group is more effective than 3 Hz TENS group.

경피신경전기자극이 전립선 침생검 조직검사 시 통증과 혈장 베타 엔돌핀 농도에 미치는 효과 (The Effect of Transcutaneous Electrical Nerve Stimulation on Acute Pain and Beta-Endorphins of Needle Biopsy)

  • Purpose: The purpose of this study was to examine the effect on plasma beta endorphin concentration level and the influences on pain score of transcutaneous electrical nerve stimulation (TENS) mediation to patients During a prostate needle biopsy. Methods: TENS was administered to only the experimental group. The electric current was given in high frequency (40-100 pps) and low intensity ($2-50{\mu}s$) from the waiting room stage until the end of the procedure. The average time spent was 35 minutes. Following 10 minutes of retention in the rectum, there was a biopsy. In two groups, the pain score was assessed twice when vas pain penetrated into the rectum, during the needle biopsy. The Beta endorphin concentration level was assessed through blood gathering 2 times in the Nuclear Medicine Labs before and after the test. Results: There was not much difference in pain levels from both groups when a microscope probe penetrated into the rectum and in the time when tissues were collected. However, the average overall pain level was reduced during those two procedures. The plasma beta endorphin level was increased in the TENS medicated group compared with the unmedicated group after the procedures were completed. Conclusion: The research indicates that TENS was desirable to be considered as a non-invasive method for controlling pain.

경피신경전기자극 후 상부 승모근 활성도와 EMG gap의 변화 (Changes of Upper Trapezius Muscle Activity and EMG Gap After Transcutaneous Electrical Nerve Stimulation in Subjects With Myofascial Pain Syndrome)

  • The purpose of this study was to compare visual analogue scale (VAS), pain threshold (PT), $%RMS_{RVC}$, and EMG gaps before and after applying transcutaneous electrical nerve stimulation (TENS) on the upper trapezius muscle at the patients with myofascial pain syndrome (MPS). The subjects were 4 men and 10 women composed of both the inpatients and outpatients who were diagnosed as MPS at Wonju Medical Center. VAS and PT measurements were performed to assess the subjective pain level. The reference voluntary contraction (RVC) test was performed for 15 seconds for normalization on the bilateral trapezius muscle using surface electromyography (sEMG). After 3-minute resting time, the EMG signal was recorded while performing a typing activity for 2 minutes and then TENS was applicated with a comfortable intensity for 10 minutes. The EMG activity of the upper trapezius muscle was recorded during typing for 2 minutes. The results of study were as follows: 1) VAS score was significantly decreased on the more painful side after treatment, however, it was not significantly different on the less painful side. 2) PT was increased after treatment on both sides, however, it was not significantly different between before and after the TENS application. 3) The EMG activity during typing was significantly decreased after treatment, and 4) The EMG gaps were significantly increased after TENS treatment compared to before it. Consequently, the study showed that TENS was effective in decreasing VAS, $%RMS_{RVC}$, and in increasing EMG gaps. The EMG gap analysis could be a useful method to measure pain in patients with MPS in the upper trapezius.

침형 경피신경 전기자극과 은침 전기자극 치료가 압통 역치에 미치는 효과 (Effects of Acupuncture-Like Transcutaneous Electrical Nerve Stimulation and Silver Spike Point Therapy on Pressure Pain Sensitivity)

  • The purpose of this study was to determine a more effective method for shoulder pain reduction. Forty-five normal subjects were randomly assigned to one of 3 groups a control group, an acupuncture-like transcutaneous electrical nerve stimulation(ALTENS) group, and a silver spike point(SSP) group. Each person in the ALTENS and SSP groups was measured for pressure pain threshold before and after a 20 minute treatment. The control group was also checked for pressure pain threshold before and after a 20 minute period but no "treatment" was given. The major findings were as follows ; 1)The ALTENS and SSP groups showed significant differences before and after treatment but the control group showed no significant difference. 2)When the three groups were compared, the only significant difference was between the SSP group and the control group. As mentioned above, it may be concluded that both ALTENS and SSP therapy were effective in reducing shoulder pain when measured directly after treatment. However, SSP did not show any superior effect. Further study should be done to determine the effective the maintained pain reduction with post-treatment time lapse.

경피신경전기자극의 빈도-강도가 만성 요통 환자의 요통장애지수와 유연성에 미치는 영향 (Effect of Frequency and Intensity of Transcutaneous Electrical Nerve Stimulation on Patients with Chronic Low Back Pain)

  • 이 연구의 목적은 경피신경전기자극을 저빈도-저강도, 저빈도-고강도, 고빈도-저강도, 고빈도-고강도 네 개의 치료조건으로 나누어 만성요통환자에게 적용하였을 때 각기 다른 주파수 빈도와 강도가 만성요통환자의 미치는 영향을 알아보는 것이다. 연구대상은 평균연령 39.19(${\pm}12.96$)세, 발병기간 8.57(${\pm}3.07$)개월인 요통환자 32명이었다. 치료기간은 2주 횟수는 총 10회로 1일 1회 15분간 시행하였으며 치료부위는 통증이 가장 심한 요추주변 부위에 시행하였다. 치료 전 후의 변화를 평가하기 위해 요통정도(요통장애지수)와 요추부 유연성을 측정하였다. 각 군간 윌콕슨, 맨-휘트니 검정을 하였고, 통계적 유의성은 p< .05로 하였다. 결과는 다음과 같다. 네 군 모두 실험 후 요통장애지수와 요추부 유연성에 유의한 결과가 나왔으며, 저빈도군에서는 고강도가, 고빈도군에서는 고강도가 보다 효과적이었다. 고빈도-고강도가 요통장애지수와 요추부 유연성에 유의한 차이는 없었지만 가장 효과적이었다.

Balance trainer training with transcutaneous electrical nerve stimulation improves spasticity and balance in persons with chronic stroke

  • Objective: The purpose of this study was determine the effect of Balance Trainer training with Transcutaneous Electrical Nerve Stimulation (TENS) on spasticity and balance in persons with chronic stroke. Design: Randomized controlled trial. Methods: A total of 30 subjects with hemiparetic stroke were recruited and randomly divided into the Balance Trainer training with TENS group (n=15) and Balance Trainer training with placebo TENS group (n=15). The Balance Trainer training with TENS group practiced additional Balance Trainer training with TENS for 30 minutes a day, 5 days per a week during 4 weeks and the Balance Trainer training with placebo TENS group practiced additional Balance Trainer training with placebo TENS for the same period. Spasticity and balance were assessed by ability (static balance, dynamic balance) and were measured before and after the 4-week programs. Results: The result of spasticity and dynamic balance were improved significantly in both groups (p<0.05). The Balance Trainer training with TENS group showed significantly greater improvement in spasticity of the gastrocnemius & dynamic balance, compared to the Balance Trainer training with placebo TENS group (p<0.05). The Balance trainer training with TENS group showed a significant improvement in static balance, especially during the eye-closed condition (p<0.05). Conclusions: The Balance Trainer training with TENS was effective in improving spasticity and balance in subjects with chronic stroke. Based on these results, it is suggested that Balance Trainer training with TENS could clinically be used more actively in conjunction with conventional physical therapy.

경피신경전기자극과 호흡근 저항운동 동시 적용이 만성 뇌졸중 환자의 호흡근 근긴장도와 폐 기능에 미치는 영향 (The Effect of the Resistance Respiratory Muscle Exercise with Transcutaneous Electrical Nerve Stimulation on Respiratory Muscle Tone and Pulmonary Function of Chronic Stroke Patients)

  • PURPOSE: This study was conducted to investigate the effect of the resistance respiratory muscle exercise with transcutaneous electrical nerve stimulation (TENS) on the respiratory muscle tone and pulmonary function of stroke patients. METHODS: Twenty stroke patients were divided into the TENS group (n = 7), placebo TENS group (n = 7), and control group (n = 6), and each intervention was performed on the three groups 5 times a week for 4 weeks. The assessment was carried out by measuring changes in the muscle tone of the latissimus dorsi and abdominal external obliques, and pulmonary function. RESULTS: In this study, the TENS group and the placebo TENS group had significant increases in the paretic side latissimus dorsi muscle tone, forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and peak expiratory flow (PEF). There was no significant difference in changes in respiratory muscle tone between the three groups. The pulmonary function was significantly different between the three groups, and it was observed from the results of the post-hoc test that FVC showed a significant increase in the TENS group and the placebo TENS group compared to the control group. CONCLUSION: Through this study, it was found that the respiratory muscle resistance exercise was more effective as a method to increase respiratory muscle tone and pulmonary function in stroke patients than combined transcutaneous electrical nerve stimulation.

Evaluation of transcutaneous electrical nerve stimulation as an adjunct therapy in trigeminal neuralgia - a randomized double-blind placebo-controlled clinical study

  • Background: Trigeminal neuralgia (TN) is a severe form of pain that affects the daily activities of a patient. Transcutaneous electrical nerve stimulation (TENS) therapy is an emerging option for the treatment of acute and chronic pain. The aim of this study was to evaluate the effect of TENS therapy as an adjunct to drug therapy for the treatment of TN. Methods: A total of 52 patients diagnosed with TN according to the International Classification of Headache Disorders (version 3) were included. Each patient was randomized to either the TENS or placebo TENS groups. Intervention was given in continuous mode and 100-Hz frequency for 20 mins biweekly for 6 weeks. Parameters were measured at baseline, TENS completion and 3 months, 6 months, and 1 year of follow up. The parameters observed were mean carbamazepine dose, mean visual analog scale (VAS) score, mean present pain intensity (PPI) score, and functional outcome. Non-parametric analyses, one-way ANOVA and the Kruskal-Wallis test were applied for intragroup comparisons, while the Mann-Whitney U test and independent t-test were used for intergroup comparisons of variables. The chi-square test was applied to analyze categorical data. Results: Compared to the placebo TENS group, the mean dose of carbamazepine in the TENS group was significantly reduced at TENS completion, as well as at 6 months and 1 year follow up. Changes in mean VAS score, mean PPI score, and functional outcome did not show significant differences between the groups (P>0.05). Conclusion: TENS therapy does not lead to any changes in pain levels but it may reduce the mean dose of carbamazepine when used as an adjunct treatment in patients with TN.

경피적 전기자극이 항문수술 후 통증에 미치는 효과 (Effects of Transcutaneous Electrical Nerve Stimulation for Relief of Perianal Pain after Anal Surgery)

  • Background: Transcutaneous Electrical Nerve Stimulation (TENS) has been widely used for pain relief after surgery instead of opioid analgesics therapy. This study was designed to study the effectiveness of TENS on perianal pain after anal surgery. Methods: Forty-eight patients who underwent anal surgery were evaluated in this suudy. Caudal anesthesia with 1.5% lidocaine 30 ml mixture 1:200,000 epinephrine was performed. TENS was done at 100 Hz for 25 min. duration, postoperatively at 4 hrs and 24 hrs. Pain was also measured at the 4 hrs and 24 hrs postoperatively by visual analogue scale (VAS). VAS value were compared before and after TENS application. Results: Values of VAS after TENS were significantly lower compared to those before TENS. Conclusions: From these results, we concluded, TENS may be effective for postoperative perianal pain relief after anal surgery.

융복합기반 러시안전류와 경피신경전기자극이 앞십자인대재건술 환자의 넙다리네갈래근의 통증, 근력, 기능에 미치는 효과 (Effect of Convergence-Based Russian Current and Transcutaneous Electrical Nerve Stimulation at Quadriceps Muscles on Pain, Strength, and Performance in Persons with Anterior Cruciate Ligament Reconstruction)

  • 본 연구는 앞십자인대 재건술 환자를 대상으로 융복합기반 러시안전류와 경피신경전기자극을 적용하였을 때 통증, 근력 및 기능에 미치는 영향을 비교하고자 하였다. 본 연구는 앞십자인대 재건술 환자 40명(러시안전류 적용군 20명, 경피신경전기자극 적용군 20명)을 대상으로 실시하였다. 두 군은 1회 20분, 주5회 4주 동안 각각의 전기중재를 실시하였고, 냉치료와 기기를 사용한 연속수동관절가동운동을 무릎관절주변에 각각 20분간 실시하였다. 치료적 중재의 영향을 알아보기 위하여, 본 연구는 중재 전후에 시각적 상사척도, 무릎 굽힘 및 폄 근력, 무릎상해와 관절염 결과지수, 한국판 다리 기능척도를 측정하였다. 두 군 모두 시각적 상사척도, 무릎 굽힘 및 폄 근력, 무릎상해와 관절염 결과지수, 한국판 다리 기능척도 점수에서 치료적 중재 이후에 통계학적으로 유의한 개선을 보여주었다. 또한, 러시안전류 적용군은 경피신경전기자극 적용군보다 무릎 폄 근력과 한국판 다리 기능척도에서 중재 후 유의한 증가를 보였다. 본 연구의 결과를 바탕으로 앞십자인대 재건술 환자에게 있어 두 중재 방법 모두 효과적인 개선을 보였지만, 러시안전류가 경피신경전기자극보다 무릎근력과 다리기능을 개선하는데 더 효과적이라고 보고하며, 향후 임상에서 앞십자인대 재건술 환자의 재활에 러시안전류의 적용에 긍정적인 이점이 있다고 제언하는 바이다.
