Breast Reconstruction after Blunt Breast Trauma: Systematic Review and Case Report Using the Ribeiro Technique

  • Blunt breast trauma occurs in 2% of blunt chest injuries. This study aimed to evaluate the evidence on breast reconstruction after blunt trauma associated with the use of a seat belt. Also, we describe the first case of breast reconstruction using the Ribeiro technique. In November 2022, a systematic search of MEDLINE, EMBASE, and Google Scholar databases was conducted. The literature was screened independently by two reviewers, and the data was extracted. Our search terms included breast, mammoplasty, blunt injury, and seat belts. In addition, we present the case of a woman with a left breast deformity and her reconstruction using the inferior Ribeiro flap technique. Six articles were included. All included studies were published between 2010 and 2021. The studies recruited seven patients. According to the Teo and Song classification, seven class 2b cases were reported. In five cases a breast reduction was performed in the deformed breast with different types of pedicles (three superomedial flaps, one lower flap, one superior flap). Only one case presented complications. The case here presented was a type 2b breast deformity in which the lower Ribeiro pedicle was used successfully without complications during follow-up. Until now there has been no consensus on reconstructive treatment due to the rarity of this entity. However, we must consider surgical treatment individually for each patient. We believe that the Ribeiro technique is a feasible and safe alternative in the treatment of posttraumatic breast deformities, offering very good long-term results.

  • Skid mark and coefficient of friction are usually utilized to calculate the velocity and behavior of vehicles. For a critical case such as traffic accident reconstruction, however, the initial velocity of the car should be calculated precisely. In this study, in order to estimate the speed at the brake onset, rapid braking tests were executed on the proving ground. We compared with a skid length and wheel locking time of an expert driver and a general person. We verified that the skid mark of expert driver occurs longer than general person's. A new method is proposed to determine the speed of a vehicle at the brake onset of maximum braking, which could be applied to a reconstruction of vehicle with Non-ABS.

손상된 하지에 시행한 유리조직 이식술 (Free Flap Transplantation for the Injured Lower Extremities)

  • The ability for tissue transfer based on microvascular anastomosis has created a revolution in microsurgical free tissue transplantation. The advantages of cutaneous flaps are that they are soft, durable and provide a good cosmetic reconstruction and muscle flaps have a more vigorous blood supply and a longer, larger vascular pedicle than cutaneous flaps. From June 1992 through May 1997, 68 patients had received reconstructive microsurgery in the lower extremity at Department of Orthopedic Surgery, Chonbuk National University Hospital. The results were as follows. 1. The age distribution was from 15 years of age to 67 and male were 59 cases and female 9 cases. 2. The most common cause was traffic accident(54 cases, 79.4%) and followed chronic osteomyelitis(9 cases, 13.2%), industrial accident(3 cases, 4.4%), burn(1 case, 1.5%) and farm injury(1 case, 1.5%). 3. Latissimus dorsi myocutaneous flap were 25 cases(36.8%), rectus abdominis muscle flap 21 cases(30.9%), gracilis muscle flap 10 cases(14.7%), dorsalis pedis flap 9 cases(13.2%), groin 2(2.9%) and vascularized iliac osteocutaneous flap 1(1.5%). 4. 61 cases(89.7%) of 68 cases were survived and the exposed vital tissues and bones were covered and revealed good cosmetic results.

요추부 천공지 유리피판을 이용한 대전자부 결손의 재건 - 증례보고 - (Reconstruction of Greater Trochanteric defect using Lumbar Artery Perforator Free Flap - A Case Report -)

  • There could be several methods for trochanteric reconstruction including local flap, pedicled perforator flaps, free flap, etc. We performed greater trochanteric reconstruction with lumbar artery perforator free flap in some aberrant method. So we report this experience with review of literatures. A 42-year-old man visited our hospital with a large soft tissue defect in his left greater trochanteric area by traffic accident. The patient had wide skin and soft tissue defect combined with open femur fracture. During one month period of admission, he underwent femur open reduction and wound debridement four times. After that we planned thoracodorsal perforator free flap reconstruction. The flap was outlined as large as $20{\times}15\;cm$ and elevated in a suprafascial plane from the lateral border. During intramuscular perforator dissection, we found that two 1.5 mm diametered perforator vessels coursed inferomedially toward second lumbar region. Finally the flap became lumbar artery perforator flap based on second lumbar artery perforator as a main pedicle. After flap transfer, the perforator vessels were connected with inferior gluteal artery and vein microsurgically. The operation was successful without uneventful course. We found no significant postoperative complication and donor site morbidity during six months follow up periods. Lumbar artery perforator flap could be an alternative procedure for thoracodorsal perforator flap in some patients with anatomic variant features.

교통사고 재현 프로그램 PC-CRASH의 주요 충돌 인자 설정 (Establishment of Important Impact Parameters of Traffic Accident Reconstruction Program "PC-CRASH")

  • 현재 국내에서는 교통사고 재현 방법이 노면 흔적물에 의존하여 이루어지고 있고, 노면 흔적물이 없는 경우 교통사고 재현은 불가능하게 된다. 이러한 단점을 보완하기 위해 외국에서는 충돌모델을 이용한 교통사고 재현프로그램을 활용하고 있는 추세이다. 현재 우리나라에서 가장 많이 사용하는 교통사고 재현 프로그램 PC-CRASH를 활용하기 위해서는 각 사고마다 알맞은 충돌특성 인자 값을 사용자가 직접 입력하여 사용하여야 하나, 이를 활용할 수 있는 활용자료가 부족한 실정이다. 본 연구는 국내에서 실제 발생된 교통사고 사례에 대해 교통사고 재현 프로그램인 PC-CRASH와 충돌에 영향을 미치는 충돌 인자들로 교통사고를 재현하였고, 차량의 최종위치간 거리와 자세에 어떠한 영향을 미치는가를 알아보았다. 또한 실제 역 해석을 하기 위해 차량의 최종위치와 자세만으로 각각의 사례에 적절한 인자값을 추정할 수 있는 회귀식을 구성하였고, 통계적으로 신뢰성을 검증하였다. 사고 재현에 필요한 주요 충돌인자들의 초기치 설정시 추정식을 이용할 경우, 사고 재현 프로그램 활용 시, 시간 단축 효과를 프로그램 내부에 있는 유전자 알고리즘의 반복횟수로 추정식의 통계적 검증을 하였다.

  • Soft tissue defects in oral & maxillofacial region caused by tumor resection, trauma, congenital deformities have been treated in autogenous soft tissue flap, allogenic material, free dermal graft, fascia graft. Of these methods, autogenous dermis graft had initially been applied in hernia treatment at the beginning of nineteenth century and have been applied in soft tissue augmentation coverage of vital structure, dead space removal and reconstruction of fascia. A fat graft is used in reconstruction of orbit at the enucleation, restoration of facial contours, etc. In this case, patient with chin soft tissue defect in traffic accident was treated in autogenous dermis-fat graft from patient's abdominal and gluteal region. Chin defect was reconstructed favorably. There was no severe atrophy of grafted area 12 months postoperatively. We will report the result that is favorable esthetically with literature review.

미세수술을 이용한 수부 재건술 (The Reconstruction of Hand with Microsurgery)

  • The authors analyzed the clinical results of the reconstructive surgery for injuried hand with microsurgery in 33 patients, 35 cases at the department of orthopaedic surgery, school of medicine, Kyung Hee university from 1985 to 1992 and the results were as followings. 1. There were 31 men and 4 women who had a mean age of 23 years(range, 3 to 44 years) and the follow up evaluations averaged 19 months. 2. The causes of the injury were machinery injury in 25 cases, traffic accident in 2, frostbite in 4, burn in 3 and fall down in 1. 3. For the reconstructive procedure, scapular free flap was applied in 6 cases, radial forearm flap in 7, dorsalis pedis free flap in 4, neurovascular island flap in 6, gracilis free flap in 1, wrap around flap in 6, toe to thumb in 5. 4. 32 cases(91.4%)were successful in reconstructive surgery with microsurgery exept the failure of scapular free flap in 2 cases and dorsalis pedis free flap in 1. 5. The causes of failure in scapular free flap were infection in 1 case and thrombosis in 1. In dorsalis pedis free flap, the cause of failure was infection. In the analysis of above results, the reconstruction with microsurgery was effective procedure for reconstruction of injuried hand.

족부와 족관절 배부의 피복을 위한 비골 동맥 천공지를 기초로 한 역혈행성 지방근막 피판 (Adipofascial Flap Distally Based on the Perforating Branch of the Peroneal Artery for Coverage of Dorsum of the Foot and Ankle)

  • Soft tissue reconstruction of dorsum of the foot and ankle has long presented challenging problems for the reconstructive surgeon. Limitations of available local tissue, the need for specialized tissue, and donor site morbidity restrict the options. In an effort to solve these difficult problems, we have begun to use adipofascial flap based on the perforating branch of the peroneal artery. We present our early experience of 5 patients treated with this flap. Our patients ranged from 6 to 26 years in age and included 3 males and 2 females. The etiologies of the wounds were secondary to traffic accident, and crushing injury. The flaps had reverse flow in all patients. The flap and the adjoining raw area were covered with a full-thickness skin graft, while the donor site at the lateral aspect of the leg was closed primarily without grafting. The skin graft was taken from the inguinal area, which was closed primarily. Compared with other flap, this adipofascial flap is thinner, producing less bulkiness to the recipient site and minor aesthetic sequelae to the donor site. In our opinion, this flap is versatile, effective, and an addition to the armamentarium of the reconstructive surgeon for coverage of difficult wounds of the foot and ankle.

외측 상완 유리 판을 이용한 사지 재건술 (Reconstruction of the Extremities with Lateral Arm Free Flap)

  • Introduction: To cover the exposed tendons and bones in the foot and hand which need coverage and abundant vascular flow, lateral arm flaps were transferred. Lateral arm flap is a thin and innervated fasciocutaneous flap with a lower lateral cutaneous nerve and posterior radial collateral artery. Materials and methods: From October 1992 through September 2003, we have performed 5 lateral arm flaps for reconstruction of the exposed achilles tendons in 2 cases and the exposed forearm extensors, 2nd to 5th metacarpal bones and scaphoid each 1 case. The causes were traffic accident in 2 cases and machinary injury in 3 cases. Age range was between 31 to 74 (average 50) and all male except 1. Posterior lateral collateral artery and venae comitantes were anastomosed by end to end in 3 cases and vena comitante in 2 cases. Lower lateral cutaneous nerve was anastomosed with a branch of superficial radial nerve in 2 cases. Results: The results were evaluated by survival of the flap, sensory discrimination, cosmesis and comfort in the activities of the daily living. All flaps were survived. Sensory recovery was graded as deep cutaneous pain sensibility in 2 cases. Cosmesis was moderately satisfied and comfort was good except 1 as moderate. Postoperative defatting procedure was done in 1 case and skin abrasion was occurred in 1 case. Conclusion: Lateral arm flap was suitable for coverage of the exposed achilles tendons and exposed forearm extensors, metacarpals and scaphoid in the wrist.

For a critical case such as traffic accident reconstruction, however, the initial velocity of the car should be calculated precisely. In this study, the skid marks on dry asphalt pavement were measured, and the velocity at brake onset was precisely recovered. A passenger car with new tires and non-contact optical speedometer were set up for the tests. A new methodology to determine the more precise velocity for Non-ABS vehicle at braking onset were suggested.
