To evaluate the recovery rates to increase toxigenic C. difficile, the selective enrichment broth culture methods were compared with commonly used cytotoxin assays and toxigenic culture. First, the enrichment culture, using the selective medium broth for 2 to 5 days, was performed and then, toxigenic C. difficile was confirmed by C. difficile toxin gene-specific PCR after being cultured on C. difficile selective agar. The sensitivity of C. difficile from the enrichment culture (100%) was higher than that of C. difficile selective agar culture (93.8%), while positive predictive values (PPV) were low; 72.7% (16/22) and 88.2% (15/17). PPV of the enrichment culture are not high. Recently, combinations of C. difficile selective agar culture, C. difficile A & B assays, glutamate dehydrogenase, and nucleic acid amplification method are widely used. The enrichment culture was disadvantageous in PPV, turn-around time, and cost. So, what we performed is not considered as a common method of diagnosis of C. difficile-associated diarrhea.
The ascomycete fungus Mycosphaerella graminicola (synonym Zymoseptoria tritici) is an important pathogen of wheat causing economically significant losses. The primary nutritional mode of this fungus is thought to be hemibiotrophic. This pathogenic lifestyle is associated with an early biotrophic stage of nutrient uptake followed by a necrotrophic stage aided possibly by production of a toxin or reactive oxygen species (ROS). In many other fungi, the genes CREA and AREA are important during the biotrophic stage of infection, while the NOXa gene product is important during necrotrophic growth. To test the hypothesis that these genes are important for pathogenicity of M. graminicola, we employed an over-expression strategy for the selected target genes CREA, AREA, and NOXa, which might function as regulators of nutrient acquisition or ROS generation. Increased expressions of CREA, AREA, and NOXa in M. graminicola were confirmed via quantitative real-time PCR and strains were subsequently assayed for pathogenicity. Among them, the NOXa over-expression strain, NO2, resulted in significantly increased virulence. Moreover, instead of the usual filamentous growth, we observed a predominance of yeast-like growth of NO2 which was correlated with ROS production. Our data indicate that ROS generation via NOXa is important to pathogenicity as well as development in M. graminicola.

Clostridium perfringens (C. perfringens) may cause diarrhea and enterotoxemia in adult and young livestock, leading to problems in the production and management of farms. Four hundred fecal samples were collected from 25 goat farms located in Gyeongsangbuk-do Province in the Republic of Korea. Sixteen C. perfringens strains were isolates from fecal samples, and the isolates were identified as type A (n=11) and type D (n=5). Additionally, ${\alpha}$- and ${\varepsilon}$-toxin genes were detected in 16 and 5 strains by PCR, respectively, and the enterotoxin gene was presented in 2 strains. The antibiotic susceptibility test was performed using the disk diffusion method and E-test method. In the disk diffusion method, ampicillin (n=16) and chloramphenicol (n=15) were highly susceptible to 16 C. perfringens isolates. In the E-test method, ampicillin, amoxicillin, amoxicillin/clavulanic acid and meropenem were susceptible to more than 14 of 16 C. perfringens isolates. This study indicates that administration of antibiotics such as ampicillin, amoxicillin/clavulanic acid and meropenem can prevent and treat C. perfringens infections in goats.
Clostridium difficile present in feces of food animals may contaminate their meats and act as a potential source of C. difficile infection (CDI) to humans. C. difficile resistance to antibiotics, its production of toxins and spores play major roles in the pathogenesis of CDI. This is the first study to evaluate C. difficile prevalence in retail raw animal meats, its antibiotics susceptibilities and toxigenic activities in Al-Jouf, Saudi Arabia. Totally, 240 meat samples were tested. C. difficile was identified by standard microbiological and biochemical methods. Vitek-2 compact system confirmed C. difficile isolates were 15/240 (6.3%). Toxins A/B were not detected by Xpect C. difficile toxin A/B tests. Although all isolates were susceptible to vancomycin and metronidazole, variable degrees of reduced susceptibilities to moxifloxacin, clindamycin or tetracycline antibiotics were detected by Epsilon tests. C. difficile strains with reduced susceptibility to antibiotics should be investigated. Variability between the worldwide reported C. difficile contamination levels could be due to absence of a gold standard procedure for its isolation. Establishment of a unified testing algorithm for C. difficile detection in food products is definitely essential to evaluate the inter-regional variation in its prevalence on national and international levels. Proper use of antimicrobials during animal husbandry is crucial to control the selective drug pressure on C. difficile strains associated with food animals. Investigating the protective or pathogenic potential of non-toxigenic C. difficile strains and the possibility of gene transfer from certain toxigenic/ antibiotics-resistant to non-toxigenic/antibiotics-sensitive strains, respectively, should be worthy of attention.
Filamentous fungi that could be classified into Aspergillus flavus/oryzae were isolated from traditionally fermented meju commercially available in Korea. The samples were analyzed for aflatoxin B1 and ochratoxin A contamination by HPLC; however, no toxin was detected. In addition, fungal and bacterial metagenomic sequencing were performed to analyze the microbial distribution in the samples. The results revealed that the distribution and abundance of fungi and bacteria differed considerably depending on the production regions and fermentation conditions of the meju samples. Through morphological analysis, ITS region sequencing, and assessment of the aflatoxin-producing ability, a total of 32 A. flavus/oryzae strains were identified. PCR analysis of six regions with a high mutation frequency in the aflatoxin gene cluster (AGC) revealed a total of six types of AGC breaking point patterns. The A. flavus/oryzae strains did not exhibit the high amylase activity detected in the commercial yellow koji strain (starter mold). However, their peptidase and lipase activities were generally higher than that of the koji isolates. We verified the safety of the traditionally fermented meju samples by analyzing the AGC breaking point pattern and the enzyme activities of A. flavus/oryzae strains isolated from the samples. The isolated strains could possibly be used as starter molds for soybean fermentation.
This review aimed to update our knowledge of the classification, pathophysiology, prognosis, and treatment of trigeminal neuralgia (TN), with the intention of establishing better treatment protocols. The latest version of the International Classification of Headache Disorders uses an etiology-based approach to characterize TN patients, potentially contributing to the development of targeted treatment measures. Noticeable changes in the recent European Academy of Neurology guidelines for the management of TN include the use of magnetic resonance imaging for exclusion of secondary TN and differentiation of idiopathic and classical TN. Additionally, the use of botulinum toxin type A as an addon therapy for mid-term treatment of TN has also been included. Though there has been limited recent progress in the treatment of TN, previous studies emphasize the importance of customized, multidisciplinary management protocols that include drug therapy optimization; provision of continuous education and support; and timely referral of medically refractory patients for surgery in order to achieve favorable prognosis. Furthermore, slow but growing evidence on gene mutations will help elucidate the pathophysiology of TN and contribute to the development of targeted drugs that are effective and safe.

목적: 본 연구에서는 신생아실과 신생아 중환자실에서 발생한 methicillin-resistant Staphylococcus aureus (MRSA) 유행에서 환자와 보균자에서 분리된 MRSA의 분자역학적 연관성을 조사하여 유행의 감염원과 전파경로를 파악하고자 하였다. 방법: MRSA 유행기간인 2017년 8월부터 9월까지 피부감염 및 패혈증 환자들과 보균자로부터 분리된 MRSA 균주를 대상으로 유전형 및 병원성 인자를 분석하고 항생제 감수성 결과를 수집하였다. 결과: 연구기간 동안 신생아실(n=27)과 신생아 중환자실(n=14)에 총 41명의 신생아들이 입원하였다. 그 중, 7명(피부감염[n=6], 패혈증[n=1])에서 MRSA 감염이 확진되었고, 보균자 4명이 발견되었다. 신생아와 접촉이 있는 의료진 32명 중 3명이 MRSA를 비강에 보균하였다. 피부감염 유행 원인 균주는 Staphylococcal chromosomal cassette mec (SCCmec) type II, sequence type (ST) 89, spa type t375였고, 뮤피로신 저농도 내성을 포함하여 항생제 다제내성을 보였다. 패혈증을 일으킨 균주는 SCCmec type IVa, ST 72, 새로운 spa type인 t17879였다. 신생아 4명에게 집락된 MRSA 균주들은 다양하였으나 SCCmec type IVa, ST 72, spa type t664가 의료진과 신생아 2명에서 공통적으로 분리되었다. Panton-Valentine leukocidin (PVL) toxin 유전자가 신생아에게 집락된 모든 균주에서 발견되었다. 결론: 피부감염 유행을 일으킨 MRSA 균주는 항생제 다제내성을 보이는 균주였다. 신생아 MRSA 보균자에게서 분리된 균주는 모두 PVL 독소 유전자를 보유하였다. 유행기간 동안 다양한 MRSA 균주가 신생아들에게서 분리되기 때문에, 효과적인 감염 관리 및 추가 환자발생의 차단을 위하여 분자역학조사를 통하여 원인균을 확인하고 전파경로를 파악하는 것이 중요하다.
최근 1인 가구, 맞벌이 가정의 급격한 증가로 인한 인구·사회학적 변화의 영향으로 즉석 반찬류의 소비가 빠르게 증가하고 있다. 이들 식품은 별도의 가열 등의 처리없이 조리되거나 조리 후 상온에서 보관되는 경우가 많아 미생물 오염에 매우 취약하다. 이에 본 연구에서는 즉석 반찬류의 미생물 오염 위험도를 분석하여 반찬류의 위생상의 문제점을 파악하고, 보관온도에 따른 미생물 품질변화를 조사하였다. 2022년 경상남도 7개 지역의 대형할인점에서 구입한 반찬류 100건에 대해 조리방법별로 식중독 윈인균과 위생지표균 검사를 진행하였다. 식중독 원인균 검사에서 Bacillus cereus는 51건(51.0%, 51/100), Clostridium perfringens는 3건(3.0%, 3/100)이 분리되었지만, 식품공전의 공통기준 및 규격 범위를 초과하는 제품은 없었다. 총 51개의 B. cereus 분리균주에서 5종류의 장내독소와 1종류의 구토독소를 분석한 결과, 장내독소 entFM, nheA, hblC, cytK, bceT 유전자의 보유율은 100.0%, 94.1%, 58.8%, 56.9%, 41.2% 순으로 각각 확인되었고, 구토 독소인 CER 유전자는 2.0%만이 보유하고 있었다. 총 3개의 C. perfringens 분리균주에서 식중독을 일으키는 독소 유전자(cpe)는 검출되지 않았다. 위생지표균 검사에서 김치류, 생채류, 젓갈류, 절임류, 나물류, 조림류 순으로 일반세균과 대장균군이 높게 검출되었다. 반찬류의 보관온도(4℃와 20℃)에 따른 일반세균수의 변화는 20℃에 보관할 시 젓갈류를 제외한 대부분의 반찬류에서 증가하는 경향을 보였다. 반찬류는 구입 즉시 냉장보관할 것을 권장하며, 시중에 유통되는 반찬류 제품의 위험성과 안전성을 평가하고 관리방안 제시를 위해 지속적인 모니터링과 추가 연구가 필요한 것으로 사료된다.
목 적 : 포도알균 열상 피부 증후군(Staphylococcal Scalded Skin Syndrome)은 포도알균의 피부박탈성 독소에 의하여 전신성 수포와 낙설을 일으키는 포도알균의 피부 감염증의 한 형태이다. 환자들의 임상양상과 치료의 결과, 합병증 등을 알고 자 본 연구를 시행하였다. 방 법 : 2002년 2월부터 2005년 12월까지 특징적인 피부병변을 보여 포도알균 열상 피부 증후군으로 진단된 53명의 환자들을 대상으로 의무기록을 후향적으로 검토하였고 임상형을 전신형, 중간형, 부전형 등으로 분류하고 2003년 9월이후 부터 발생한 환자의 비강 내 검체를 이용하여 원인 균 및 독소를 규명하였다. 결 과 : 1) 연구기간 동안 4S로 진단되어 치료한 환자는 총 53명이었다. 발병연령은 생후 20일부터 7세까지 분포하였으며, 평균연령은 2.8세였다. 성별은 남자가 35명이었고 여자는 18명으로 남여 성비는 1.9:1이었다. 2) 임상적인 유형은 전신형이 6례(11%), 중간형이 29례(55%), 부전형이 18례(34%)로 중간형이 가장 많았다. 대부분의 환자에서 혈액 검사 상 백혈구 증다증이나 C반응단백의 증가는 없었다. 3) 2003년 9월 이후에 15명 환자의 비강에서 얻은 검체 중 14명의 환자에서 얻은 검체에서 포도알균을 분리하였고 피부박탈성 독소는 독소 B의 유전자를 확인하였고 Panton-Valentine leukocidin의 유전자는 음성으로 나왔다. 4) 입원하여 치료받은 환자의 경우 평균 입원 기간은 7.3일이었으며 사용한 항생제는 amoxacillin/clavulanate, ampicillin/sulbactam, cefuroxime, vancomycin 등이었다. 모든 환자에서 전신적인 낙설이 나타났으며 임상형이나 사용한 항생제 종류에 관계없이 모든 환자가 특별한 합병증이 없이 회복이 되었다. 결 론 : 창원과 인근 지역에서 단기간 내에 지역사회 획득 메치실린 내성 포도알균감염으로 인한 다수의 4S 환자가 연속적으로 발생하였으며 원인은 피부박탈성 독소 B이었다.

Staphylococcus pseudintermedius는 개에서 기회감염을 유발하는 병원체이며, 공중보건학적으로도 주요한 인수공통 병원체이다. 개에서 분리된 S. pseudintermedius 균주들은 주로 항생제 내성 및 개에서 피부 감염을 유발하는 주요 원인균으로 연구되어 왔지만, 가축에서 분리된 S. pseudintermedius 균주들의 항생제 내성 및 장내 독소 생성에 대한 정보는 매우 제한적이다. 본 연구에서는 개, 돼지, 육우에서 분리된 S. pseudintermedius 균주들에서 18가지의 장내 독소 (staphylococcal enterotoxin; SE) 유전자와 toxic shock syndrome toxin 유전자(tst-1)의 분포양상을 조사하였다. 또한, S. pseudintermedius 균주들의 항생제 내성 양상과 더불어 mecA 유전자 및 SCCmec type 또한 확인하였다. 육우에서 분리한 하나의 균주를 제외한 모든 개와 돼지 분리주 들이 4개 이상의 항생제에 내성을 보였으며, 개에서 분리된 6개의 균주 중 4개의 S. pseudintermedius 균주들이 메티실린 내성과 더불어 SCCmec V를 가진 것으로 확인 되었다. 총 11개의 SE 유전자들 (seb, sec, see, seg, sei, sej, sel, seo, sep, seq, seu) 및 tst-1가 개, 돼지 및 육우로부터 분리된 S. pseudintermedius 균주들에서 확인 되었으며, 대부분의 분리주들 (83%)에서 2개 이상의 SE 유전자들이 확인 되었고, 그 중 sel (42%) 및 sep (42%)가 가장 빈번하게 검출 되었다. 본 연구를 통하여 반려견에서 뿐만 아니라 주요 가축에서 존재하는 S. pseudintermedius 균주들에서 높은 항생제 내성 양상을 확인 하였으며, 항생제 내성과 더불어 여러 staphylococcal enterotoxin 및 tst-1 유전자들을 전파할 가능성을 확인 하였다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
