Comparing Inhaler Use Technique Based on Inhaler Type in Elderly Patients with Respiratory Disease

  • Background: The aim of this study was to investigate inhaler device handling in elderly patients. Inhaler devices with respect to misuse and error correction were also compared. Methods: Inhaler use technique was assessed using standardized checklists at the first visit and 3-month follow-up visit after retraining. The primary outcome was difference in the acceptable use ratio among inhaler devices. Secondary outcomes included differences in error correction, the most common step of misuse, and factors affecting the accuracy of inhaler use. Results: A total of 251 patients (mean age, 76.4 years) were included. The handling of 320 devices was assessed in the study. All patients had been trained before. However, only 24.7% of them used inhalers correctly. Proportions of acceptable use for Evohaler, Respimat, Turbuhaler, Ellipta, and Breezhaler/Handihaler were 38.7%, 50.0%, 61.4%, 60.8%, and 43.2%, respectively (p=0.026). At the second visit, the acceptable use ratio had increased. There were no significant differences among inhaler types (Evohaler, 63.9%; Respimat, 86.1%; Turbuhaler, 74.3%; Ellipta, 64.6%; and Breezhaler/Handihaler, 65.3% [p=0.129]). In multivariate analysis, body mass index, Turbuhaler, and Ellipta showed positive correlations with acceptable use of inhalers, whereas Chronic Obstructive Pulmonary Disease Assessment Test score showed a negative correlation. Conclusion: Although new inhalers have been developed, the accuracy of inhaler use remains low. Elderly patients showed more errors when using pressurized metered-dose inhalers than using dry powder inhalers and soft-mist inhalers. However, there were no significant differences in misuse among inhaler devices after individual training. Results of this study suggests that repeat training is more important than inhaler type.

The Relationship between Physical Activity Participation, Physical Fitness Level, and Academic Performance in ROTC

  • The purpose of this study was to investigate the relationship between physical activity participation habits, physical fitness level, and academic performance among the Reserve Military Training Corps (ROTC). Participants in the current study were 71 ROTCs from J University. Physical activity participation was investigated using the Global Physical Activity Questionnaire (GPAQ), and the level of physical fitness was measured by health-related physical fitness evaluation. For academic performance, university grade data (GPA) was approved by the head of the relevant school district, and the data were collected with the consent of the participants, and each data was analyzed. The correlation between physical activity participation habits, physical fitness level, and GPA was analyzed, and GPA by physical fitness level were compared. This study found that there was no statistically significant correlation between physical activity participation and academic performance. Otherwise, there was a statistically significant correlation between major subjects and total physical fitness score, cardiorespiratory fitness, muscular endurance, flexibility, and body composition. By the level of physical fitness, the ROTCs in the high-physical fitness group showed statistically significantly higher academic performance than the ROTCs in the low-physical fitness group. In addition, low level of physical fitness ROTC group was lower than the other. In conclusion, ROTCs with a high physical fitness level had a positive effect on improving learning ability compared to those who did not. By recommending an autonomous but systematic exercise training program, it is suggested to improve the physical strength of ROTCs and to cultivate excellent future military officers.

Association between Cognitive Decline and Ten Heavy Metals (인지기능 저하와 체내 중금속 10종 간 연관성 분석)

  • Background: Due to the rapid aging of the South Korean population, neurological diseases such as dementia are increasing. Many studies have reported that the incidence of dementia is associated with environmental factors along with age. Objectives: This study analyzed the association between cognitive function and ten heavy metals in the body: arsenic, aluminum, chromium, manganese, cobalt, nickel, iron, copper, zinc, and lead. Methods: From 2018 to 2019, a total of 120 participants who suffered from cognitive impairment were recruited for this study. Blood and urine samples were collected and analyzed for heavy metal concentrations using an inductively coupled plasma mass spectrometer. Demographic information was obtained through face-to-face questionnaires completed by a trained investigator. Cognitive function was evaluated with the Korean version of the Mini-Mental State Examination and the Korean version of the Boston Name Waiting Test. The associations between cognitive function scores and heavy metal concentrations were analyzed using multiple logistic regression analysis. Results: The average age of the 120 participants was 72.7 years, and 69.2% were female. The mean of the MMSE-K and K-BNT scores were 22.9 and 37.9, respectively. The geometric mean of aluminum (Al) was 8.42 ㎍/L. MMSE-K was associated with iron (Fe), but the significance was removed in the logistic regression based on 24 points. K-BNT was significantly associated with aluminum and the odds ratio for K-BNT above 38 decreased by 45% as the aluminum concentration increased. Conclusions: The association between aluminum and the K-BNT score indicated that aluminum is associated with language-related cognitive decline. Based on this result, further study will be conducted by considering co-exposure effects of heavy metals including aluminum.

Effects of different stocking density and various phytogenic feed additives dosage levels on growing-finishing pigs

  • This study was to investigate the effects of different phytogenic feed additives (PFA) dosage levels in growing- finishing pigs stressed by high stocking density. A total of 72 mix sexed 12 weeks growing pigs ([Landrace × Yorkshire] × Duroc) with initial body weight (BW) of 49.28 ± 4.58 kg were used for 8 weeks. There were 3 replicate pens in each treatment group, with 3 pigs per pen. The dietary treatment groups consisted of basal diets in animal welfare density (negative control [NC]), basal diet in high stocking density (positive control [PC]), PC + 0.04% essential oil (ES1), PC + 0.08% essential oil (ES2), PC + 0.10% bitter citrus extract & essential oil (CES1), PC + 0.20% bitter citrus extract & essential oil (CES2), PC + 0.05% grape pomace extract (GP1), PC + 0.10% grape pomace extract (GP2). The reduction of space allowance decreased (p < 0.05) average daily gain, feed efficiency, and digestibility of dry matter, crude protein, and gross energy. Also, the fecal score of PC groups increased (p < 0.05) compared with other groups. Basic behaviors (feed intake, standing, lying) were inactive (p < 0.05) and singularity behavior (biting) was increased (p < 0.10) under high stocking density. There was no difference in blood profile. However, the supplementation of PFA alleviated the negative effects such as reduced growth performance, nutrient digestibility, and some increasing stress indicators in th blood (cortisol) and animal behavior (biting). In conclusion, the negative effect of high stocking density was most effectively mitigated by the normal dosage of the mixture of bitter citrus extract and essential oil additive (CES1).

Impact of Alcohol Consumption on Quality of Life, Depressive Mood and Metabolic Syndrome in Obstructive Lung Disease Patients: Analysis of Data from Korean National Health and Nutrition Examination Survey from 2014 and 2016

  • Background: The objective of this study was to investigate whether alcohol consumption might affect the quality of life (QOL), depressive mood, and metabolic syndrome in patients with obstructive lung disease (OLD). Methods: Data were obtained from the Korean National Health and Nutrition Examination Survey from 2014 and 2016. OLD was defined as spirometry of forced expiratory volume in 1 second/forced vital capacity <0.7 in those aged more than 40 years. QOL was evaluated using the European Quality of Life Questionnaire-5D (EQ-5D) index. Patient Health Questionnaire-9 (PHQ-9) was used to assess the severity of depressive mood. Alcohol consumption was based on a history of alcohol ingestion during the previous month. Results: A total of 984 participants with OLD (695 males, 289 females, age 65.8±9.7 years) were enrolled. The EQ-5D index was significantly higher in alcohol drinkers (n=525) than in non-alcohol drinkers (n=459) (0.94±0.11 vs. 0.91±0.13, p=0.002). PHQ9 scores were considerably lower in alcohol drinkers than in non-alcohol drinkers (2.15±3.57 vs. 2.78±4.13, p=0.013). However, multiple logistic regression analysis showed that alcohol consumption was not associated with EQ-5D index or PHQ-9 score. Body mass index ≥25 kg/m2, triglyceride ≥150 mg/dL, high-density lipoprotein <40 mg/dL in men and <50 mg/dL in women, and blood pressure ≥130/85 mm Hg were significantly more common in alcohol drinkers than in non-alcohol drinkers (all p<0.05). Conclusion: Alcohol consumption did not change the QOL or depressive mood of OLD patients. However, metabolic syndrome-related factors were more common in alcohol drinkers than in non-alcohol drinkers.

Effects of energy levels on growth performance, carcass characteristics, and fatty acid composition of Holstein steers at different slaughter ages

  • We investigated the effect of energy levels on growth performance, carcass characteristics, and fatty acid composition of Holstein steers at different slaughter ages. Forty Holstein steers with an average body weight (BW) of 234.21 ± 7.42 kg and 7.78 ± 0.22 months were randomly allocated to two experimental groups; a 22-month-old experimental group and a 24-month-old experimental group. Total digestible nutrients (TDN) for each group were set to 70% (T1) and 72% (T2) during fattening phase and 74% (T3) and 76% (T4) for the finishing phase, respectively. No difference was observed in the final BW between the experimental groups during the fattening phase. However, in the finishing phase, the final BW of T4 (820.31 kg) group was significantly higher than that of T1 (745.57 kg; p < 0.05). The average daily gain (ADG) of T2 group in the finishing phase and overall period were 1.27 kg and 1.26 kg, respectively, which were significantly higher than those of T1 (1.11 kg and 1.12 kg; p < 0.05). The feed conversion ratio (FCR) and TDN conversion ratio of T2 group in the finishing phase also decreased by 10.23% and 7.73%, respectively, compared to those of T1. The cold carcass weight of T4 group was significantly higher (p < 0.05) than that of T1, whereas back fat thickness, longissimus area, and marbling score were not significantly different among groups. No differences were observed in physicochemical characteristics of the carcass including moisture, crude protein, and crude fat content among groups. However, the composition of fatty acids differed significantly between the groups. The content of C18:0 was significantly lower (p < 0.05) in T4 than in T1 group, and the content of C18:2 was higher in T4 than in T1 and T3 (p < 0.05). Therefore, feeding Holstein steers at a high-energy feeding level during the fattening and finishing phases improves ADG and reduces the slaughter age from 24 months to 22 months.

The Development of Evaluation Indicator for Eco-experience in Rural Village (농촌마을 생태체험 공간으로 활용하기 위한 연못형습지 평가지표 개발)

  • This study was conducted to develop the health evaluation indicator of pond wetland in order to enhance the quality of eco-experience in rural area. The methods for development of evaluation indicator were consisted of 3 stages; 1st, the precedent assessment protocol was applied to 10 pond wetlands in farm villages, and eco-experience expert survey was conducted to selection the evaluation item and criteria at 2nd and 3rd stages. In the results of applying the precedent assessment protocol, we found out two problems; 1) the evaluation result of value determination and conservation value were too simple, and 2) the score by evaluation items were nearly the same, because evaluation criteria in not precisely for application in rural area. These results were reflected to expert survey. According to the 1st survey results, they suggested that 4 items should be maintained, and 3 items should be deleted, and 2 items should be modified among a total of 9 times. Therefore, it was modified into the evaluation protocol having a total of 8 items. According to the 2nd survey results, the selected evaluation items were generally proper. With regard to items for assessing the health condition of pond wetland, we selected a total of 8 items; (1) Connectivity to forest, (2) Connectivity between water body and wetland connection, (3) Number of Vegetation, (4) Surround land use, (5) Interspersion of Vegetation, (6) Crossing Structure, (7) Wetland size, and (8) Outlet structure. In addition, it was suggested that accessibility, visibility, and trash were need for utilization of pond wetland as a place for eco-experience. It is expected that the selected evaluation indicator can help to utilize the pond wetland as an eco-experience space in rural area, and maintain the pond wetland as a space for conservation of biodiversity.

Feeding Effects of Whole Crop Rice based TMR on Growth Performance and Carcass Characteristics of Hanwoo Steers (사료용 벼 위주 TMR 급여가 거세 한우의 생장 능력 및 도체 특성에 미치는 영향)

  • This experiment was conducted to investigate the effect of whole crop rice (WCR) based TMR on growth performance and carcass characteristics of Hanwoo steers. WCR "Yeongwoo"was harvested at yellow ripen stage and ensiled for 60 days. The crude protein (CP), acid detergent fiber (ADF), neutral detergent fiber (NDF), in vitro dry matter digestibility (IVDMD) and total digestible nutrient (TDN) content was 8.4 %, 28.0 %, 53.8 %, 72.4 % and 66.8 %, respectively. For silage quality, pH was 4.37 and lactic and butyric acid content were 2.84 and 0.04 % in DM. Sixteen Hanwoo steers (8-mon-old) were allocated into either a control (commercial TMR) and WCR-TMR (WCR-based TMR) group. The TMR were fed according to the feeding stage phase: growing (Initiate~14 month), early fattening (15 month~21 month) and late fattening (22 month~30 month). The body weight of control group increased (P<0.05) until early fattening stage, but late growing stage of WCR-TMR group was higher than that of control (P<0.05). Average daily gain (ADG) was significantly greater (P<0.05) in WCR-TMR group (total 0.78 kg/head) compared to control (total 0.66 kg/head) except for late fattening stage. The marketing weight and carcass weight were higher in WCR-TMR group (726 vs 765 kg; 417.8 vs 450.4 kg). The back fat thickness (11.75 vs 13.00 mm), Longissimus dorsi area (88.00 vs $89.88cm^2$) and yield index (65.87 vs 64.30) were not different between the two groups (P>0.05) and also no difference in meat yield grade (A : B : C = 2 : 4 : 2). Marbling score (4.00 vs 4.13), meat color (4.75 vs 4.75), fat color (3.13 vs 2.88), texture (1.25 vs 1.50) and maturity (2.00 vs 2.00) were not significant difference between the two groups and meat quality grade ($1^{{+}{+}}:1^+:1:2:3=0:2:4:2:0$) was also not different. In conclusion, TMR feeding based on WCR silage showed superiority in carcass yield and ADG compared to control TMR. It is considered that the use of WCR for feed is a necessary option for the substitution of the imported forages and the government's policy for rice production adjustment.

DEVELOPMENT OF BIOCOMPATIBLE DRESSING MATERIAL MADE OF COLLAGEN AND AMNIOTIC MEMBRANE AND WOUND HEALING EXPERIMENT IN RAT (양막과 콜라겐을 이용한 생체 적합 드레싱 소재 개발 및 백서 창상치유 실험)

  • Purpose of study: Partial thickness skin graft is the golden standard regimen for full-thickness skin defect caused by burn or trauma. However, in case of extensive burns of more than 50% of total body surface area, the donor site is not sufficient to cover all defects. As a second choice, allograft, xenograft and synthetic materials have been used to treat skin defect. Among them the amniotic membrane(AM) was used as a biological dressing for centuries because of its potential for wound healing. In this study, quantification of EGF in AM and effect of AM-collagen complex on full thickness skin defects was examined. Materials & Methods: The concentration of EGF in fresh, deep frozen and freeze-dried AM was evaluated by ELISA. EGF-R immunostaining was performed in freeze-dried AM. SD rats weighing 250${\sim}$300g was used for wound healing experiment. Three full thickness skin defects(28mm diameter) were made on dorsal surface of SD rat. The control group was covered by Vaselin gauze and AM-collagen complex and $Terudermis^{(R)}$. was grafted in two other defects. Healing area, Cinamon's score were evaluated before biopsy. Grafted sites were retrieved at 3 days, 1 week, 2 weeks and 4 weeks after operation. H & E and Factor VIII immunohistochemical stain was performed to evaluate the microscopic adhesion and structural integrity and microvessel formation. Results: 1. EGF concentration of fresh, deep frozen and freeze-dried AM showed similar level and EGF-R was stained in epithelial layer of freeze-dried AM. 2. At 4 weeks after grafting, the healing area of AM-collagen and Terudermis group was 99.29${\pm}$0.71% and 99.19${\pm}$0.77 of original size. However, that of control group was 24.88${\pm}$2.90. 3. The Cinamon's score of AM-Collagen and $Terudermis^{(R)}$. group at 4 weeks was 15.6${\pm}$1.26 and 14.6${\pm}$3.13 and that of control group was 3.7${\pm}$0.95. Significant difference was observed among control and experimental groups(p<0.05). 4. Histologic examination revealed that AM protected leukocyte infiltration and epithelial migration was nearly completed at 4 weeks. $Terudermis^{(R)}$. group showed mild neutrophil infiltration until 2 weeks and completion of epithelization at 4 weeks. Control group showed massive leukocyte infiltration until 4 weeks. 5. Microvessels were increased sharply at 1 week and control group at 1 and 4 week showed significant differences with $Terudermis^{(R)}$. group of same interval(p<0.05) but no differences were found with AM group(p<0.05). Conclusion: EGF and EGF-R were well preserved in freeze-dried AM. AM attached to collagen acted as excellent biologic dressing which had similar effect with $Terudermis^{(R)}$. AM showed anti-inflammatory action and healing was completed at 4 weeks after full-thickness skin defect.

Effect of $\beta$-glucan Extracted from Youngji Mushroom on the Growth Performance of Weaning Pigs (영지버섯에서 추출한 $\beta$-glucan 이 자돈의 생산능력에 미치는 영향)

  • This experiment was selected a $\beta$-glucan producing mushroom strain and developed industrial media, and used to $\beta$-glucan as an alternative for antibiotics in weaned pigs. Yields of mycelial biomass and extracellular $\beta$-glucan from Youngji (Ganoderma lucidum) mushroom was 8.52g/L and 4.49g/L respectively. Also, we prepared optimum formula for mushroom cultivations. A total of 144 pigs ($8.6{\pm}0.9$ kg average body weight, weaned $20{\pm}3$ days of age) were allotted to 4 different treatment groups and replicated 4 times with 8 pigs per replicate in randomized complete block design. Treatments were T1) NC (negative control, basal diet), T2) PC (positive control, basal diet+0.255% antibiotics), T3) NC+0.2% $\beta$-glucan and T4) PC+0.2% $\beta$-glucan. The T2 and T4 treatments were significantly higher in feed efficiency by antibiotics group (p<0.05), however, there was no significant differences in terms of average daily gain (ADG) and average daily feed intake (ADFI) during phase I (0~14 days). In phase II (15~28 days), Pigs fed with antibiotics and $\beta$-glucan (T4) had greater ADG than other treatments (p<0.05), while no differences were observed in ADFI and feed efficiency. During the whole experiment period, the ADG of T4 treatment was higher than other treatments. Pigs fed with $\beta$-glucan (T3 and T4) had greater diarrhea score and moisture content than other treatments (p<0.05). Pigs fed with $\beta$-glucan (T3 and T4) had greater moisture content than other treatments (p<0.05). However, there was no significant differences in diarrhea score and mortality of weaned pigs. There was marginal reductions in feed cost measured feed cost per weight gain used in antibiotics and $\beta$-glucan added diet during phase I. In the second phase, the treatment supplemented with antibiotics had a significantly lower feed cost per weight gain compared to the other treatments. The results from these experiments suggests that $\beta$-glucan is likely able to improve the growth performance, and reduce feed cost although they do not have similar effects like antibiotics in weaning pigs.
