The Effectiveness of Stress Intervention Program for Hemodialysis Patients in Korea: A meta-analysis (국내 혈액투석환자의 스트레스 중재에 대한 효과 연구: 메타분석)

  • The purpose of this study was to meta-analyze the stress intervention program applied to patients with hemodialysis in Korea to confirm the effectiveness of the stress intervention program and to identify the characteristics and trends of the stress intervention method. A total of 10 studies were selected, and Quality Assessment tool was used MINORS and the data was analyzed using Comprehensive Meta Analysis Version 3.0 and Review manager version 5.3 The Quality Assessment score of studies was 21.2 points, and the effect size on the psychological stress was medium(g=-.72), and the effect size on cortisol of the physiological stress was also medium(g=-.52). Subgroup analysis showed that Music therapy and aromatherapy, individual providing, over 60 minutes per session and total of 10-20 times were more effectively reduced stress. Findings of this meta-analysis would be helpful for health professionals to provide more effective stress intervention program. Further randomized controlled trials of stress intervention program for patients with hemodialysis are more needed.

Structural Analysis of Deepwater Steel Catenary Riser using OrcaFlex (OrcaFlex를 이용한 심해 SCR 구조 해석)

  • The design challenges when attempting to obtain sufficient strength for a deepwater steel catenary riser (SCR) include high stress near the hang-off location, an elevated beam-column buckling load due to the effective compression in the touchdown zone (TDZ), and increased stress and low-cycle fatigue damage in the TDZ. Therefore, a systematic strength analysis is required for the proper design of an SCR. However, deepwater SCR analysis is a new research area. Thus, the objective of this study was to develop an overall analysis procedure for a deepwater SCR. The structural behavior of a deepwater SCR under various environmental loading conditions was investigated, and a sensitivity analysis was conducted with respect to various parameters such as the SCR weight, weight of the internal contents, hang-off angle (HOA), and vertical soil stiffness. Based on a deepwater SCR design example, it was found that the maximum stress of an SCR occurred at a hang-off location under parallel loading direction with respect to the riser plane, except for a wave dominant dynamic survival loading condition. Furthermore, the tensile stress governed the total stress of the SCRs, whereas the bending stress governed the total stress at the TDZ. The weight of the SCR and internal contents affected the maximum stress of the SCR more than the HOA and vertical soil stiffness, because the weight of the SCR, including the internal contents, was directly related to its tensile stress.

The Stress Concentration Caused by Pin-hole in Femur after Computer-navigated Total Knee Arthroplasty: A Finite Element Analysis (컴퓨터 네비게이션을 이용한 슬관절 전치환술에서 핀 홀에 의한 응력 집중: 유한요소해석)

  • Total knee arthroplasty(TKA) using computer-assisted navigation has been increased in order to improve the accuracy of femoral and tibial components implantation. Recently, a few clinical studies have reported on the femoral stress fracture after TKA using computer-assisted navigation. The purpose of this study is to investigate the stress concentration around the femoral pin-hole for different pin-hole diameter, the modes of pin penetration by finite element analysis to understand the effects of pin-hole parameters on femoral stress fracture risk. A three-dimensional finite element model of a male femur was reconstructed from 1 mm thick computed tomography(CT) images. The bone was rigidly fixed to a 25 mm above the distal end and 1500 N of axial compressive force and 12 Nm of axial torsion were applied at the femoral head. For all cases, transcortical pin penetration mode showed the highest stress fracture risk and unicortical pin penetration mode showed the lowest stress concentration. Pin-hole diameter increased the stress concentration, but pin number did not increase the stress dramatically. The results of this study provided a biomechanical guideline for pin-hole fracture risk of the computer navigated TKA.

  • Earthquake response analyses are conducted for the investigation of the ground shaking during the 1995 Hyogoken-nambu earthquake. Port Island a man made island with about 8{{{{ KAPPA m^2 }} area is chosen for this purpose Because earthquake measurement with vertical array was conducted there. Strain dependent characteristics of soil can be modeled well into Hardin-Drnevich Model. Four analyses are conducted : total stress analysis by equivalent linear method non-linear method. and two effective stress analyses. All analyses except equivalent linear analysis show fairy good agreement with observed record mainly because the non-linear behavior of Holocene clay layer has predominant effect on the behavior of fill, However detailed investigation show that effective stress analyses give much better prediction than total stress analyses.

Study on Fatigue Durability Analysis of Poclain Bucket (포크레인 버켓의 피로 내구성 해석에 대한 연구)

  • This study analyzes about poclain bucket through fatigue durability analysis. Maximum equivalent stress and total deformation are shown at the lower of bucket and edge part respectively. Among the cases of nonuniform fatigue loads, 'SAE bracket history' with the severest change of load becomes most unstable but 'Sample history' becomes most stable. In case of 'Sample history' with the average stress of -10000Pa to 5000MPa and the amplitude stress of 0 to 6000MPa, the possibility of maximum damage becomes 3%. This stress state can be shown with 5 or 6 times more than the damage possibility of 'SAE bracket history' or 'SAE transmission'. The structural result of this study can be effectively utilized with the design of poclain bucket by investigating prevention and durability against fatigue damage.

  • In clinical orthopaedics, bone resoption in the cortex is often seen post operatively on X-rays or bone densitometry after total hip replacement (THR) in the form of cortical osteoporosis or atropy. Stress shielding of bone occurs, when a load, normally carried by the bone alone, is shared with an implant as a result, the bone stresses are abnormal and with remodelling analysis this may cause extensive proximal bone resoption, possibly weakening the bone bed to the point of failure. The author made finite element models of the cemented and non-cemented type implanted femoral stem with bone resorption of the proximal medial femur and studied the feed back effect of the various degree of bone resoption to THR system by parametric analysis on the stress of the femoral stem and interface. The results of the present finite element analysis implied that the extent of proximal bone resorption has the effect of more increasing stress on the distal stem tip, cement mantle and interface in both type of femoral stem and this high distal stress possibly can cause the mechanical failure of loosening or failure after THR.

A study on the determinants of job stress responses of the staff nurses (일반 간호사의 직무 스트레스 반응에 대한 결정 요인)

  • Purpose : This paper was attempted to identify the job stress related factors among the staff nurses and to provide the basic data concerning development of stress management program focused on hospitals. Method : The subjects were 309 staff nurses at two general hospitals in Seoul. Data were collected with self-reported questionnaires and analyzed by SPSS-PC+10.0 for descriptive analysis, ANOVA, stepwise multiple regression, factor analysis. Results : The subjects exhibit significantly highest level of 'the participation in decision making factor'. The mean score of 'control coping strategies' was higher than 'avoid coping strategies'. The mean scores of social support and stress responses were high. The main factor that affected the stress responses was 'the job characteristic factor' and it was explained 23.0% out of the total variance of the stress responses. Also, it would be explained 42.6% out of the total variance of the stress responses with 'the control coping strategies, work overload factors, social support, and participation in decision making factors'. Conclusion: For developing the hospital- focused stress management program for staff nurses, 'the participation in decision making factors' and 'the job characteristics' should be considered. Also, the organizational efforts and supports should be required to support and use of 'control coping strategies' of nurses

Seismic Integrity Analysis of an Electric Distributing Board Using the Response Spectra Analysis Method (응답스펙트럼해석법을 이용한 배전반의 내진건전성 해석)

  • In this study, a response spectrum analysis of an electric distributing board (EDB) was conducted to investigate seismic integrity in the design stage. For the seismic analysis, the required response spectra of a safe shutdown earthquake with 2% damping (RRS/SSE-2%) specified in GR-63-CORE Zone 4 was used as the ground spectral acceleration input. A finite element method modal analysis of the EDB was also performed to examine the occurrence of resonance within the frequency range of the earthquake response spectrum. Furthermore, static stress caused by deadweight was analyzed. The resultant total maximum stress of the EDB structure was calculated by adding the maximum stresses from both seismic and static loads using the square root of the sum of the squares (SRSS) method. Finally, the structural safety of the EDB was investigated by comparing the resultant total maximum stress with the allowable stress.

Development of a "Grandmothering Stress Index" for Korean Day-care Grandmothers (손자녀 양육 스트레스 측정도구 개발 - 주간양육 할머니를 중심으로 -)

  • Purpose: The study was to develop an instrument to measure grandmothering stress for Korean day-care grandmothers and to test the validity and reliability of the instrument Method: The items of the instrument were based on a literature review and secondary data, After content validity tests and a pilot test, 20 items were developed. In order to test the reliability and validity of the scale, data were collected from 126 grandmothers. Result: After a factor analysis, five factors and 15 items were selected. These explained 67.2% of the total variance. The first domain was termed 'Health problems', and explained 18.1 % of the total variance, and the second domain of 'Possibility of role substitution' explained 13.8%. The third and the fourth domains were 'Relations with adult children' and 'Grandchildren's characteristics' explaining 12.5% and 12.1 %, respectively. The last domain explained 10.8% of the total variance with the theme 'Restriction of social life' After accomplishing the reliability analysis, Cronbach's alpha coefficient was determined to be .75. Conclusions : This initial step in the development of a Grandmothering Stress Index is valuable to reflect the subject of Grandmothering stress in senior citizens in Korea. Future study should to refine the constitution of the instrument.

  • Dynamic fracture is investigated in plate applied by impacting bar. Numerical simulations of the experiments are made by using a finite element method(FEM) code, LS-DYNA. The eroding surface-to-surface contact allows between impacting bar and impacted plate. The occurrence of hourglass deformations in an analysis can invalidate results and hourglass energy is minimized to obtain the good accuracy of result. Total, internal and kinetic energies, von Mises plastic stress and X,Y,Z velocities of impacting bar are analyzed in this study.
