  • The treatment for oral and maxillofacial carcinoma with chemotherapeutic agents is evaluated by many effective methods to reduce the tumor mass and cancer cell proliferation. However these chemotherapy have many serious side effects, such as bone marrow suppression, renal toxicity, G-I troubles. Therefore a possible approach to develop a clinically applicable chemotherapeutic agent is to screen anticancer activity of Taxol which is known to have very little side effect and have been used to breast cancer and ovarian carcinoma. Taxol is a new anti-microtubular anti-cancer agent extracted from the bark of the Pacific yew, Taxus brevifolia. Paclitaxel(Taxol) acts by promoting tubulin polymerization and over stabilizing microtubules agianst depolymerization. Despite the constant improvements of methods of the cancer treatment especially chemotherapy, the rate of cancer metastasis and recurrent are not decreased. Thus the investigation of new drug which have very little side effect and a possible clinically application continues to be a high priority. Considering that the Taxol have shown very effective chemotherapeutic agent with relatively low toxicity in many solid tumors, it deserves to evaluate its efficacy in oral squamous cell carcinoma. In this study, to investigate the in-vivo and in-vitro anti-cancer efficacy of Taxol in oral squamous cell carcinoma and lastly, the potency of Paclitaxel in the clinical application for oral cancer was evaluated. In vivo study, after HN22 cell line were xenografted in nude mice, the growth of tumor mass was observed, 3 mg/Kg taxol was injected intraperitoneally into nude mice containing tumor mass. The methods of these study were measurement of total volume of tumor mass, histopathologic study, immunohistochemical study, drug resistance assay, growth curve, MTT assay, flow cytometry, cDNA microarray in vivo and in vitro. The results were obtained as following. 1. The visual inspection of the experimental group showed that the volume of the tumor mass was slightly decreased but no significant difference with control group. 2. Ki-67 index was decreased at weeks 4 in experimental group. 3. Microscopic view of the xenografted tumor mass showed well differentiated squamous cell carcinoma and after Taxol injection, some necrotic tissue was seen weeks 4. 4. The growth curve of the tumor cells were decreased after 1day Taxol treatment. 5. According to the MTT assay, HN22 cell line showed relative drug resistancy above $5\;{\mu}g/ml$ concentrations of Taxol. 6. In drug resistance assay, the decrease of cell counts was seen relatively according to concentration. 7. In Flow cytometry, G2M phase cell arrests were seen in low concentration of the Taxol, while S phase cell arrests were seen in high concentration of the Taxol. 8. Using cDNA microarray technique, variable gene expression of ANGPTL4, TXNRD1, FAS, RRAGA, CTGF, CYCLINEA, P19, DUSP5, CEBPG, BTG1 were detacted in the oral squamous cell carcinoma cell after taxol treatment. In this study paclitaxel is effective against oral squamous cell carcinoma cell lines in vitro, but week effect was observed in vivo. So we need continuous study about anticancer effect of taxol in vivo in oral squamous cell carcinoma.

한국전쟁의 교훈과 대비 -병력수(兵力數) 및 부대수(部隊數)를 중심으로- (The lesson From Korean War)

  • Just before the Korean War, the total number of the North Korean troops was 198,380, while that of the ROK(Republic of Korea) army troops 105,752. That is, the total number of the ROK army troops at that time was 53.3% of the total number of the North Korean army. As of December 2008, the total number of the North Korean troops is estimated to be 1,190,000, while that of the ROK troops is 655,000, so the ROK army maintains 55.04% of the total number of the North Korean troops. If the ROK army continues to reduce its troops according to [Military Reform Plan 2020], the total number of its troops will be 517,000 m 2020. If North Korea maintains the current status(l,190,000 troops), the number of the ROK troops will be 43.4% of the North Korean army. In terms of units, just before the Korean War, the number of the ROK army divisions and regiments was 80% and 44.8% of North Korean army. As of December 2008, North Korea maintains 86 divisions and 69 regiments. Compared to the North Korean army, the ROK army maintains 46 Divisions (53.4% of North Korean army) and 15 regiments (21.3% of North Korean army). If the ROK army continue to reduce the military units according to [Military Reform Plan 2020], the number of ROK army divisions will be 28(13 Active Division, 4 Mobilization Divisions and 11 Local Reserve Divisions), while that of the North Korean army will be 86 in 2020. In that case, the number of divisions of the ROK army will be 32.5% of North Korean army. During the Korean war, North Korea suddenly invaded the Republic of Korea and occupied its capital 3 days after the war began. At that time, the ROK army maintained 80% of army divisions, compared to the North Korean army. The lesson to be learned from this is that, if the ROK army is forced to disperse its divisions because of the simultaneous invasion of North Korea and attack of guerrillas in home front areas, the Republic of Korea can be in a serious military danger, even though it maintains 80% of military divisions of North Korea. If the ROK army promotes the plans in [Military Reform Plan 2020], the number of military units of the ROK army will be 32.5% of that of the North Korean army. This ratio is 2.4 times lower than that of the time when the Korean war began, and in this case, 90% of total military power should be placed in the DMZ area. If 90% of military power is placed in the DMZ area, few troops will be left for the defense of home front. In addition, if the ROK army continues to reduce the troops, it can allow North Korea to have asymmetrical superiority in military force and it will eventually exert negative influence on the stability and peace of the Korean peninsular. On the other hand, it should be reminded that, during the Korean War, the Republic of Korea was attacked by North Korea, though it kept 53.3% of troops, compared to North Korea. It should also be reminded that, as of 2008, the ROK army is defending its territory with the troops 55.04% of North Korea. Moreover, the national defense is assisted by 25,120 troops of the US Forces in Korea. In case the total number of the ROK troops falls below 43.4% of the North Korean army, it may cause social unrest about the national security and may lead North Korea's misjudgement. Besides, according to Lanchester strategy, the party with weaker military power (60% compared to the party with stronger military power) has the 4.1% of winning possibility. Therefore, if we consider the fact that the total number of the ROK army troops is 55.04% of that of the North Korean army, the winning possibility of the ROK army is not higher than 4.1%. If the total number of ROK troops is reduced to 43.4% of that of North Korea, the winning possibility will be lower and the military operations will be in critically difficult situation. [Military Reform Plan 2020] rums at the reduction of troops and units of the ground forces under the policy of 'select few'. However, the problem is that the financial support to achieve this goal is not secured. Therefore, the promotion of [Military Reform Plan 2020] may cause the weakening of military defence power in 2020. Some advanced countries such as Japan, UK, Germany, and France have promoted the policy of 'select few'. However, what is to be noted is that the national security situation of those countries is much different from that of Korea. With the collapse of the Soviet Unions and European communist countries, the military threat of those European advanced countries has almost disappeared. In addition, the threats those advanced countries are facing are not wars in national level, but terrorism in international level. To cope with the threats like terrorism, large scaled army trops would not be necessary. So those advanced European countries can promote the policy of 'select few'. In line with this, those European countries put their focuses on the development of military sections that deal with non-military operations and protection from unspecified enemies. That is, those countries are promoting the policy of 'select few', because they found that the policy is suitable for their national security environment. Moreover, since they are pursuing common interest under the European Union(EU) and they can form an allied force under NATO, it is natural that they are pursing the 'select few' policy. At present, NATO maintains the larger number of troops(2,446,000) than Russia(l,027,000) to prepare for the potential threat of Russia. The situation of japan is also much different from that of Korea. As a country composed of islands, its prime military focus is put on the maritime defense. Accordingly, the development of ground force is given secondary focus. The japanese government promotes the policy to develop technology-concentrated small size navy and air-forces, instead of maintaining large-scaled ground force. In addition, because of the 'Peace Constitution' that was enacted just after the end of World War II, japan cannot maintain troops more than 240,000. With the limited number of troops (240,000), japan has no choice but to promote the policy of 'select few'. However, the situation of Korea is much different from the situations of those countries. The Republic of Korea is facing the threat of the North Korean Army that aims at keeping a large-scale military force. In addition, the countries surrounding Korea are also super powers containing strong military forces. Therefore, to cope with the actual threat of present and unspecified threat of future, the importance of maintaining a carefully calculated large-scale military force cannot be denied. Furthermore, when considering the fact that Korea is in a peninsular, the Republic of Korea must take it into consideration the tradition of continental countries' to maintain large-scale military powers. Since the Korean War, the ROK army has developed the technology-force combined military system, maintaining proper number of troops and units and pursuing 'select few' policy at the same time. This has been promoted with the consideration of military situation in the Koran peninsular and the cooperation of ROK-US combined forces. This kind of unique military system that cannot be found in other countries can be said to be an insightful one for the preparation for the actual threat of North Korea and the conflicts between continental countries and maritime countries. In addition, this kind of technology-force combined military system has enabled us to keep peace in Korea. Therefore, it would be desirable to maintain this technology-force combined military system until the reunification of the Korean peninsular. Furthermore, it is to be pointed out that blindly following the 'select few' policy of advanced countries is not a good option, because it is ignoring the military strategic situation of the Korean peninsular. If the Republic of Korea pursues the reduction of troops and units radically without consideration of the threat of North Korea and surrounding countries, it could be a significant strategic mistake. In addition, the ROK army should keep an eye on the fact the European advanced countries and Japan that are not facing direct military threats are spending more defense expenditures than Korea. If the ROK army reduces military power without proper alternatives, it would exert a negative effect on the stable economic development of Korea and peaceful reunification of the Korean peninsular. Therefore, the desirable option would be to focus on the development of quality of forces, maintaining proper size and number of troops and units under the technology-force combined military system. The tableau above shows that the advanced countries like the UK, Germany, Italy, and Austria spend more defense expenditure per person than the Republic of Korea, although they do not face actual military threats, and that they keep achieving better economic progress than the countries that spend less defense expenditure. Therefore, it would be necessary to adopt the merits of the defense systems of those advanced countries. As we have examined, it would be desirable to maintain the current size and number of troops and units, to promote 'select few' policy with increased defense expenditure, and to strengthen the technology-force combined military system. On the basis of firm national security, the Republic of Korea can develop efficient policies for reunification and prosperity, and jump into the status of advanced countries. Therefore, the plans to reduce troops and units in [Military Reform Plan 2020] should be reexamined. If it is difficult for the ROK army to maintain its size of 655,000 troops because of low birth rate, the plans to establish the prompt mobilization force or to adopt drafting system should be considered for the maintenance of proper number of troops and units. From now on, the Republic of Korean government should develop plans to keep peace as well as to prepare unexpected changes in the Korean peninsular. For the achievement of these missions, some options can be considered. The first one is to maintain the same size of military troops and units as North Korea. The second one is to maintain the same level of military power as North Korea in terms of military force index. The third one is to maintain the same level of military power as North Korea, with the combination of the prompt mobilization force and the troops in active service under the system of technology-force combined military system. At present, it would be not possible for the ROK army to maintain such a large-size military force as North Korea (1,190,000 troops and 86 units). So it would be rational to maintain almost the same level of military force as North Korea with the combination of the troops on the active list and the prompt mobilization forces. In other words, with the combination of the troops in active service (60%) and the prompt mobilization force (40%), the ROK army should develop the strategies to harmonize technology and forces. The Korean government should also be prepared for the strategic flexibility of USFK, the possibility of American policy change about the location of foreign army, radical unexpected changes in North Korea, the emergence of potential threat, surrounding countries' demand for Korean force for the maintenance of regional stability, and demand for international cooperation against terrorism. For this, it is necessary to develop new approaches toward the proper number and size of troops and units. For instance, to prepare for radical unexpected political or military changes in North Korea, the Republic of Korea should have plans to protect a large number of refugees, to control arms and people, to maintain social security, and to keep orders in North Korea. From the experiences of other countries, it is estimated that 115,000 to 230,000 troops, plus ten thousands of police are required to stabilize the North Korean society, in the case radical unexpected military or political change happens in North Korea. In addition, if the Republic of Korea should perform the release of hostages, control of mass destruction weapons, and suppress the internal wars in North Korea, it should send 460,000 troops to North Korea. Moreover, if the Republic of Korea wants to stop the attack of North Korea and flow of refugees in DMZ area, at least 600,000 troops would be required. In sum, even if the ROK army maintains 600,000 troops, it may need additional 460,000 troops to prepare for unexpected radical changes in North Korea. For this, it is necessary to establish the prompt mobilization force whose size and number are almost the same as the troops in active service. In case the ROK army keeps 650,000 troops, the proper number of the prompt mobilization force would be 460,000 to 500,000.

산화 질소 억제제가 문맥 고혈압 쥐의 혈역학 변화에 미치는 영향 (The Effects of Nitric Oxide Inhibitor on Hyperdynamic Circulation in Portal Hypertensive Rats)

  • 만성 문맥압 항진증에서 관찰되는 과혈류 순환과 말초혈관 이완에 대한 산화질소의 역할을 규명하기 위해 본 연구를 시행하였다. 실험동물은 수컷 흰쥐를 이용하여 문맥을 부분결찰하여 문맥 고혈압을 유발시킨 군과 겉보기 수술만을 시행한 대조군으로 구분하고 문맥 고혈압군은 문맥 부분 결찰후 부터 혈역학 측정 전까지 식수를 경구 투여한 식수 투여군과 산화질소 억제제인 $N^{\omega}$-Nitro-L-Arginine(1mg/kg/day)를 경구 투여한 NNA 투여군으로 나누었다. 혈역학적 측정은 수술 2주 후에 시행하였고 $^{51}Cr$$^{57}Co$-labeled microspheres를 이용하여 심박출량, 조직 혈류량, 문맥-전신 단락률, 문맥압, 말초혈관 저항 내장혈관 저항 등을 측정하였다. 평균 동맥압은 대조군 $129.3{\pm}9.6mmHg$, 식수 투여군은 $111.3{\pm}5.5mmHg$로 대조군에 비해 유의하게 감소되어 있었고, NNA 투여군은 $128.7{\pm}19.8mmHg$로 식수 투여군에 비해 증가되어 있었다. 심박출량은 대조군의 $105.2{\pm}6.5ml/min$에 비해 식수 투여군에서 $144.2{\pm}17.9ml/min$로 증가되었으며, NNA 투여군은 $89.9{\pm}14.4ml/min$로 식수 투여군에 비해 감소되어 있었다. 전말초 저항은 식수 투여군에서 $6.0{\pm}0.9dyne/sec/cm^5{\times}10^5$로 대조군의 $9.5{\pm}0.8dyne/sec/cm^5{\times}10^5$에 비해 감소되었고, NNA 투여군은 $11.2{\pm}2.1dyne/sec/cm^5{\times}10^5$로 식수 투여군에 비해 증가되어 있었다. 문맥으로 유입되는 혈류량은 대조군 $16.61{\pm}5.03ml/min$, 식수 투여군은 $29.66{\pm}4.27ml/min$로 식수 투여군에서 유의한 증가가 있었으며, NNA 투여군은 $11.43{\pm}2.24ml/min$로 식수 투여군에 비해 감소되어 있었다. 내장혈관 저항은 식수 투여군에서 $2.59{\pm}0.44dyne/sec/cm^5{\times}10^5$로 대조군의 $6.61{\pm}3.08$dyne/sec/$cm^5{\times}10^5$에 비해 감소되었으며, NNA 투여군은 $8.09{\pm}2.04$dyne/sec/$cm^5{\times}10^5$로 식수 투여군에 비해 증가되어 있었다. 문맥-전선 단락율은 대조군의 $1.35{\pm}0.42%$ 에 비해 식수 투여군은 $95.42{\pm}2.73%$로 증가되었으며, NNA 투여군은 $73.36{\pm}17.67%$로 식수 투여군보다 감소되어 있었다. 문맥압은 대조군 $7.94{\pm}1.29mmHg$, 식수 투여군 $17.16{\pm}3.17mmHg$, NNA 투여군은 $16.67{\pm}2.24mmHg$로 대조군에 비해 식수 투여군과 NNA 투여군에서 증가되었으나, 양군 사이에 유의한 차이는 없었다. 문맥 저항은 NNA 투여군에서 $12.23{\pm}3.93dyne/sec/cm^5{\times}10^5$로 식수 투여군의 $4.74{\pm}1.20yne/sec/cm^5{\times}10^5$에 비해 증가되었다. 결론적으로 문맥의 부분결찰로 유도된 만성 문맥압 항진 쥐에서 산화질소 합성 억제제인 NNA를 투여할 경우 문맥압의 변화 없이 동맥압의 증가, 심박출량의 감소, 내장 및 전신 혈관 저항의 증가 등이 유발되었다. 이상의 결과를 고려해 볼 때 만성 문맥압 항진증시 관찰되는 과혈류 순환의 형성에 산화질소가 중요한 역할을 하는 것으로 생각된다.

Effect of Trichostatin A on Anti HepG2 Liver Carcinoma Cells: Inhibition of HDAC Activity and Activation of Wnt/β-Catenin Signaling

  • Purpose: To investigate the effect of deacetylase inhibitory trichostatin A (TSA) on anti HepG2 liver carcinoma cells and explore the underlying mechanisms. Materials and Methods: HepG2 cells exposed to different concentrations of TSA for 24, 48, or 72h were examined for cell growth inhibition using CCK8, changes in cell cycle distribution with flow cytometry, cell apoptosis with annexin V-FTIC/PI double staining, and cell morphology changes under an inverted microscope. Expression of ${\beta}$-catenin, HDAC1, HDAC3, H3K9, CyclinD1 and Bax proteins was tested by Western blotting. Gene expression for ${\beta}$-catenin, HDAC1and HDAC3 was tested by q-PCR. ${\beta}$-catenin and H3K9 proteins were also tested by immunofluorescence. Activity of Renilla luciferase (pTCF/LEF-luc) was assessed using the Luciferase Reporter Assay system reagent. The activity of total HDACs was detected with a HDACs colorimetric kit. Results: Exposure to TSA caused significant dose-and time-dependent inhibition of HepG2 cell proliferation (p<0.05) and resulted in increased cell percentages in G0/G1 and G2/M phases and decrease in the S phase. The apoptotic index in the control group was $6.22{\pm}0.25%$, which increased to $7.17{\pm}0.20%$ and $18.1{\pm}0.42%$ in the treatment group. Exposure to 250 and 500nmol/L TSA also caused cell morphology changes with numerous floating cells. Expression of ${\beta}$-catenin, H3K9and Bax proteins was significantly increased, expression levels of CyclinD1, HDAC1, HDAC3 were decreased. Expression of ${\beta}$-catenin at the genetic level was significantly increased, with no significant difference in HDAC1and HDAC3 genes. In the cytoplasm, expression of ${\beta}$-catenin fluorescence protein was not obvious changed and in the nucleus, small amounts of green fluorescence were observed. H3K9 fluorescence protein were increased. Expression levels of the transcription factor TCF werealso increased in HepG2 cells following induction by TSA, whikle the activity of total HDACs was decreased. Conclusions: TSA inhibits HDAC activity, promotes histone acetylation, and activates Wnt/${\beta}$-catenin signaling to inhibit proliferation of HepG2 cell, arrest cell cycling and induce apoptosis.

학교급식에 Cook/Chill System 적용을 위한 품질보증연구(II) - 돼지불고기 - (A Quality Assurance Study for the Application of Cook/Chill System in School Foodservice Operation (II) - Pork Bulgogi (Broiled Sliced Pork with Sauces) -)

  • 본 연구는 학교급식에서 Cook/chill system으로 생산 가능한 음식으로 돼지 불고기를 선정하고 모의실험을 통해 급식생산체계를 반복 실시함으로서 식품 위해 분석 중요관리점(HACCP)을 규명하고, 저장기간중의 음식품질 평가를 통해 합리적인 저장기한을 설정하고자 수행되었으며, 그 연구결과는 다음과 같다. 돼지불고기는 냉동상태로 운반되지 않아 온도상승이 일어난 검수 단계를 제외하면 각 생산 단계별 기준이 준수된 양호한 상태 하에서 생산된 것으로 나타난다. 생산단계별 미생물 분석결과, 원재료($4.26{\pm}0.11\;Log\;CFU/g$)에서 중온균수가 조리하지 않은 식품 기준 이내이나 다소 높게 나타났고, 양념장($5.97{\pm}0.04\;Log\;CFU/g$)에서는 조리하지 않은 식품 기준에 근접한 수준이었으며, 양념으로 재우는 과정($5.56{\pm}0.21\;Log\;CFU/g$)도 위험 수준이었다. 가열 조리 후 최종 음식온도가 $8.25{\pm}3.54^{\circ}C$에 도달하였으나 중온균수($5.17{\pm}0.04\;Log\;CFU/g$)가 급식 단계 음식기준을 초과한 위험한 수준이었고, 기타 미생물은 검출되지 않았다. 급속 냉각과 저장 1일, 3일, 5일 동안도 중온균수가 급식단계 음식 기준에 근접한 위험한 상태였고 기타 미생물은 검출되지 않았다. 재가열 처리에 의해 저장 1일($4.62{\pm}0.22\;Log\;CFU/g$), 3일($4.55{\pm}0.20\;Log\;CFU/g$), 5일($4.25{\pm}0.16\;Log\;CFU/g$) 모두 중온균수는 감소하여 급식단계 음식기준에 충족한 상태가 되었다. 배분 3조건에서도 급식단계 음식 기준 이내에 들었다. 저장 5일간 이화학 분석 결과 pH, 산가, 휘발성 염기 질소 모두 저장 5일에 유의적으로 증가하였고, 관능평가에서는 모든 항목들이 유의적인 차이를 보이지 않았다. 티아민 정량 분속 결과 ,가열 전의 티아민 함량을 100%으로 했을 때, 가열 후에는 78.6%로 손실이 일어났으며, 냉각과 저장 1일, 3일은 티아민 손실을 거의 일으키지 않는 것으로 나타났다. 그러나 저장 5일에는 티아민이 현저히 저하되어 62.5% 보유에 그쳤다. 저장기간에 따른 미생물적, 이화학적, 관능적 품질을 분석한 결과와 티아민 함량의 변화를 고려하여 돼지불고기의 저장기한을 3일로 제안하며, 생산 단계별 온도-소요시간 측정 및 미생물 분석을 통해 규명된 중요관리점은 돼지고기와 양념장재료인 파, 마늘, 생강의 구입 및 검수, 가열조리, 냉각, 저장, 재가열과 배식단계였다.

  • 인공하천에서 탁수가 부착조류 군집에 미치는 영향을 평가한 연구는 실내에 인공수로 4개를 설치하고, 대조군 2개와 처리군 2개로 반복실험을 실시하였다. 사용된 기질은 1% agar을 바른 슬라이드 글라스($7.5{\times}2.5\;cm$)이었고, 지점당 40개씩 설치하였다. 조사는 2010년 3월 29일부터 4월 10일까지 총 13일간이었으며, 6일간은 부착조류가 부착할 수 있는 공백기를 주었고, 조사 7일부터 24시간 간격으로 약 50분(50 L)간 고농도의 탁수($2\;g\;L^{-1}$, 300 NTU)를 주입 시켰다. 분석항목은 수질요인(수온, DO, pH, 전기전도도, 탁도, SS, $NO_2$-N, $NO_3$-N, $NH_4$-N, $PO_4$-P, TN, TP)과 생물량(Chl-${\alpha}$, AFDM), 종조성올 분석하였다. 분석 결과, 처리군의 탁수 농도는 162.2~173.2 NTU로 나타났다. 탁수 발생에 따라 감소하는 경향을 보인 수질 항목은 수온, DO 및 TN으로 나타났고, 증가하는 경향을 보인 항목은 SS, $NO_2$-N, $NO_3$-N, $NH_4$-N 및 TP로 나타났다. 부착조류의 생물량(Chl-${\alpha}$, AFDM) 및 밀도는 탁수 주입 후 3 일간 크게 영향을 받지 않았으나 3일 이후부터는 뚜렷한 감소를 보였다. 본 연구의 결과는 단기간의 간헐적인 탁수라 할지라도 그 기간이 일주일 정도 지속된다면 부착 규조류 생물량을 크게 감소시킬 수 있음을 보여주었고, 그를 통한 하천 먹이사슬의 구조적, 기능적 교란의 잠재성을 시사한다.

면역기능증강성 동암 바이오스 신물질에 대한 3개월간의 마우스 투여후의 면역학적 및 혈액학적 변화 (Immunostimulntory Effects of Immu-Forte at 3 Months Post-Treatment in Mice)

  • 체내 면역 증강정을 관찰하기 위하여 체내 임파구의 활성 및 증가 여부를 3개월간 의뢰한 물질인 Immu-Forte EX, Immu-Forte 된장고추장, Immu-Forte A, Immu-Forte를 투여한 후, flowcyometry 및 sandwitch ELISA를 이용하여 측정하였다. 본 실험의 실험 기간 중에 동물의 질병이나 감염에 의한 면역학적인 변화가 있었는지를 확인하기 위하여 혈액학적인 검사를 실시하였으며, 혈액학적인 검사에서 Alkaline phosphatase, T protein, Albumin, Creatine, ALT, AST, Total bilirubin, Potasium을 검사해 본 결과 대조군과 비교 했을때 투여군 전군에서 정상적인 범위내 수치를 나타내는 것으로 나타났다. 이러한 결과로부터 본 실험에서 나온 결과는 질병이나 감염에 의한 것이 아닌 생체내에서의 면역성 증강에 의한 것으로 판단할수 있는 근거를 제공한다고 판단되어진다. 한편, 3개월간 장기 투여시 Immu-Forte EX, Immu-Forte 된장고추장, Immu-Forte A과 Immu-Forte 의 모든 물질은 전반적으로 면역 세포와 사이토카인의 수치를 유의적으로 증가시켰다 그러나, 3개월간 Immu-Forte EX를 투여 후 혈중콜레스테롤 수치를 검사해 본 결과 대조군$(118\pm24.4 mg/dL)$과 비교하였을때, 고용량$(126\pm7.7 mg/dL)$, 중간용량$(121\pm8.4 mg/dL)$, 저용량$(109\pm24.Smg/dL)$으로서 통계학적인 유의적 감소로는 보이지 않는다. 각각의 물질의 면역학적인 변화양상을 살펴보면 물질 동암 EX는 중간 농도군에서는 CD4 T 림파구, CD8 림파구, 대식세포, IL-12, IFN-r가 대조군에 비해서 유의적으로 증가했으며 떠농도에서는 전체 T 임파구, 전체 B 임파구, CD4 T 임파구, 대식세포, IL-2, IL4, IL-12가 대조군에 비해서 유의적으로 증가했다. 물질 Immu-Forte 된장고추장에서는 고농도에서는 CD4 T 임파구, IL-2, IL-4, IL-12가 유의적으로 증가했으며, 중간농도에서는 CD4 임파구, 대식세포, IL-2,IL-4,IL-12 가 유의적으로 증가했고, 저농도에서는 전체 T 임파구, CD4 T 임파구, IL-2, IL-4, IL-12가 유의적으로 증가했다. 물질 Immu-Forte A의 경우 고농도에서 대식세포,자연살해세포, IL-12가 중간농도에서는 전체 T임파구, CD4 T임파구, 대식세포, 자연살해세포, IL-2, IL-4, IL-12가 저농도에서는 자연살해세포가 유의적으로 증가했다. 물질 Immune-Forte f의 경우 고농도에서 전체 B 임파구, IL-4, IL-12가 중간농도에서 전체 T임파구, 전체 B임파구, CD4 T임파구, CD8 T 임파구, 대식세포, IL-2, IL-4, IL-12, IFN-r가 저농도에서는 자연살해세포, IL-12가 유의적으로 증가하였다. 이상과 같이 각각의 물질은 전반적으로 면역증강 효과가 있는 것으로 나타났으며 그 면역증강을 유도하는 세포나 사이토카인은 서로 정확히 일치하지 않는 것으로 보아 각각의 물질이 다른 기전을 통해서 면역증강을 유도하는 것으로 생각된다.

국민건강보험 발전방향 (Future Direction of National Health Insurance)

  • It has been forty years since the implementation of National Health Insurance (NHI) in South Korea. Following the 1977 legislature mandating medical insurance for employees and dependents in firms with more than 500 employees, South Korea expanded its health insurance to urban residents in 1989. Resultantly, total expenses of the National Health Insurance Service (NHIS) have greatly increased from 4.5 billion won in 1977 to 50.89 trillion won in 2016. With multiple insurers merging into the NHI system in 2000, a single-payer healthcare system emerged, along with separation policy of prescribing and dispensing. Following such reform, an emerging financial crisis required injections from the National Health Promotion Fund. Forty years following the introduction of the NHI system, both praise and criticism have been drawn. In just 12 years, the NHI achieved the fastest health population coverage in the world. Current medical expenditure is not high relative to the rest of the Organization for Economic Cooperation and Development. The quality of acute care in Korea is one of the best in the world. There is no sign of delayed diagnosis and/or treatment for most diseases. However, the NHI has been under-insured, requiring high-levels of out-of-pocket money from patients and often causing catastrophic medical expenses. Furthermore, the current environmental circumstances of the NHI are threatening its sustainability. Low birth rate decline, as well as slow economic growth, will make sustainment of the current healthcare system difficult in the near future. An aging population will increase the amount of medical expenditure required, especially with the baby-boomer generation of those born between 1955 and 1965. Meanwhile, there is always the problem of unification for the Korean Peninsula, and what role the health insurance system will have to play when it occurs. In the presidential election, health insurance is a main issue; however, there is greater focus on expansion and expenditure than revenue. Many aspects of Korea's NHI system (1977) were modeled after the German (1883) and Japanese (1922) systems. Such systems were created during an era where infections disease control was most urgent and thus, in the current non-communicable disease (NCD) era, must be redesigned. The Korean system, which is already forty years old, must be redesigned completely. Although health insurance benefit expansion is necessary, financial measures, as well as moral hazard control measures, must also be considered. Ultimately, there are three aspects that we must consider when attempting redesign of the system. First, the health security system must be reformed. NHI and Medical Aid must be amalgamated into one system for increased effectiveness and efficiency of the system. Within the single insurer system of the NHI must be an internal market for maximum efficiency. The NHIS must be separated into regions so that regional organizers have greater responsibility over their actions. Although insurance must continue to be imposed nationally, risk-adjustment must be distributed regionally and assessed by different regional systems. Second, as a solution for the decreasing flow of insurance revenue, low premium level must be increased to an appropriate level. Likewise, the national reserve fund (No. 36, National Health Insurance Act) must be enlarged for re-unification preparation. Third, there must be revolutionary reform of benefit package. The current system built a focus on communicable diseases which is inappropriate in this NCD era. Medical benefits must not be one-time events but provide chronic disease management. Chronic care models, accountable care organization, patient-centered medical homes, and other systems that introduce various benefit packages for beneficiaries must be implemented. The reimbursement system of medical costs should be introduced to various systems for different types of care, as is the case with part C (Medicare Advantage Program) of America's Medicare system that substitutes part A and part B. Pay for performance must be expanded so that there is not only improvement in quality of care but also medical costs. Moreover, beneficiaries of the NHI system must be aware of the amount of their expenditure through a deductible payment system so that spending can be profiled and monitored. The Moon Jae-in Government has announced its plans to expand the NHI system; however, it is important that a discussion forum is created so that more accurate analysis of the NHI, its environments, and current status of health care system, can take place for reforming NHI.

수돗물을 용수로 사용한 결구상추의 수경육묘시 배양액내 $\textrm{NH}_4\textrm{H}_2\textrm{PO}_4$ 에 관한 연구 (Study on $\textrm{NH}_4\textrm{H}_2\textrm{PO}_4$ in Nutrient Solution using Tap Water during Hydroponic raising of Crisp Lettuce)

  • 본 실험은 수돗물을 용수로 사용한 결구상추(Lactuca sativa var. capitata)의 수경육묘시 배양액의 pH에 대한 NH$_4$H$_2$PO$_4$의 효과를 조사하기 위하여 수행되었다. 배양액의 pH를 안정시키기 위하여 시작배양액과 추가배양액의 조성을 달리하여 공급하였다. 배양액의 pH는 추가배양액의 공급시 급격한 하락을 나타낸 후에 회귀하는 경향을 보였다. 배양액의 pH는 온도가 높아짐에 따라 증가하였고, EC는 반대로 감소하는 일중변화를 보였다. 1, 2차 실험에서 pH는 추가배양액 공급 이후에 NH$_4$H$_2$PO$_4$ 0.25me/$\ell$ 처리구에서 7정도를 유지하였으나 NH$_4$H$_2$PO$_4$ 3me/$\ell$과 6me/$\ell$ 처리구에서는 6.4-6.5정도를 유지하였다. 3차 실험에서 NH$_4$H$_2$PO$_4$ 0.25me/$\ell$ 처리구의 pH는 6.7에서 7.4 까지 서서히 증가하였으나, NH$_4$H$_2$PO$_4$ 3me/$\ell$과 6me/$\ell$ 처리구에서는 6-6.5에서 5-5.5로 감소하였다. 따라서 배양액의 pH를 안정시키기 위하여는 배양액에 존재하는 NH$_4$H$_2$PO$_4$의 전체 양을 1me/s$\ell$과 7me/6$\ell$사이로 설정하거나, 농도를 기준으로 0.25 me/$\ell$와 3 me/$\ell$ 사이로 하는 것이 적정하리라 사료된다.

소아 개심술에 있어서 변형초여과법의 효과 (Effects of Modified Ultrafiltration in Pediatric Open Heart Surgery)

  • 개심술시 사용되는 체외순환에 따른 혈액희석, 염증성반응 등으로 인하여 개심술후 혈관외용적 증가 및 조직내 수분증가로 인하며 심장 및 폐기능 저하는 잘 알려진 사실이며 특히 소아에서는 심한 경우에는 장 기부전이 초래 될 수 있다. 본 연구는 여러가지 장점을 갖고 있는 변형초여과법을 임상적용하여 개심술 직 후 심폐기능에 미치는 영향을 평가하고자 하였다. 연구 A): 1996년 4월부터 1996년 8월 까지 삼성의료원 흉부외과에서 변형초여과법을 실시한 총 71명의 환자중 전향적연구을 실시한 소아환자 26명을 대상으로 하여 변형초여과법전후의 혈역학적 수치를 비교하 였다. 변형초여과법 전후의 .적혈구 용적량은 28.3$\pm$3.6%에서 33.8$\pm$4.0%로 증가 (P<0.001), 중심정맥압은 7.8 k3.7mmHg에서 6.9$\pm$2.gmmHg로 감소하였으며(PfO.001), 수축기 혈압은 66.7$\pm$11.2mmH프에서 76.2 11.8 mmHg로 증가하였다( p < 0.02). 연구 B): 변형초여과법이 폐에 미치는영향을 평가하기 위하여 2세 미만의 심실중격결손 환자중 변형초여 과법군(12명)과 초여과법을 적용하지 않은 대조군(14명)으로 하여 수술직후 최대호기압, 폐포-모세혈관 산소 분압차, 기관지 삽관기\ulcorner 등을 비교하였다. 최대호기압은 각각 20.0$\pm$2.4cm1120,22.4$\pm$2.3cm1120로 변형초여 과법군이 의미 있게 낮았으며(p<0.03), 폐포-모세혈관 산소분압차 및 기관지 상관기간은 변형초여과법 적용 군에서 낮았으나, 통계적으로 유의한 차이는 없었다. 상기연구를 통하여 변형초여과법을 사용함으써 효과적으로 혈액을 농축시킬수 있으며, 더 낮은 중심정맥 암에서 더 높은 수축기 혈압 등의 혈역학적 이득이 있으며, 술후 폐포-모세혈관 산소분압차 및 최대호기압 의 감소 등, 폐기능 향상의 효과가 있음을 알 수 있었다. 본 연구팀은 향후 계속적으로 소아 개심술후 변 형초여과법을 적용하여 이에 대한 효과.및 기전에 대한 연구를 계속시행할 예정이다.
