Mandibular fractures have higher incidence rate compared to facial bone fractures because of relatively prominent form. Specially, mandibular angle fractures were known as common fracture site because of morphological anatomic structure. The mandibular third molar appears to be the most frequent impacted tooth. The mandibular third molar have influence on mandibular angle fractures according to it's eruption state. Also, the mandibular angle fracture including the third molar may influence on post operative infection whether the third molar is in impacted or extracted state when reduction of fracture site is operated. The presence or absence and degree of impaction of the mandibular third molar were assessed for each patient and were related to the occurrence of the mandibular angle fracture. The extraction or non extraction of the mandibular third molar were related to the occurrence of the post operative infection in the reduction of mandibular angle fractures. In the presence of mandibular third molar, the possibility of mandibular angle fractures were relatively common. When viewing the mandibular third molar at occlusal plane, the possibility of mandibular angle fractures were high in the class C group. The possibility of mandibular angle fractures were high in the mesial angulation and partial impaction. There was a statistically significant difference(P<0.05). In complete erupted mandibular third molar, the possibility of post operative infection occurs quite often, but there was no statistical significant difference(P>0.05). In the extracted or non extracted of mandibular third molar, the post operative infection showed no statistical significant difference(P>0.05). With the results mentioned above, mandibular third molar was significantly more susceptible to mandibular angle fracture. In the reduction of mandibular angle fracture, it was recommended that mandibular third molar should be extracted especially in case of pericoronitis, periodontitis and other infections.

Purpose: To describe the features of impacted upper canines and their relationship with adjacent structures through three-dimensional cone-beam computed tomography (CBCT) images. Materials and Methods: Using the CBCT scans of 79 upper impacted canines, we evaluated the following parameters: gender, unilateral/bilateral occurrence, location, presence and degree of root resorption of adjacent teeth (mild, moderate, or severe), root dilaceration, dental follicle width, and presence of other associated local conditions. Results: Most of the impacted canines were observed in females (56 cases), unilaterally (51 cases), and at a palatine location (53 cases). Root resorption in adjacent teeth and root dilaceration were observed in 55 and 47 impacted canines, respectively. In most of the cases, the width of the dental follicle of the canine was normal; it was abnormally wide in 20 cases. A statistically significant association was observed for all variables, except for root dilaceration (p=0.115) and the side of impaction (p=0.260). Conclusion: Root resorption of adjacent teeth was present in most cases of canine impaction, mostly affecting adjacent lateral incisors to a mild degree. A wide dental follicle of impacted canines was not associated with a higher incidence of external root resorption of adjacent teeth.
Purpose: This study was performed to evaluate the prevalence of all types and subtypes of dental anomalies among 7- to 35-year-old patients by using panoramic radiographs. Materials and Methods: This cross-sectional study was conducted on 1649 people in Hamadan City, in 2012-2013. The prevalence of four types and 12 subtypes of dental anomalies was evaluated by two observers separately by using panoramic radiography. Dental anomalies were divided into four types: (a) shape (including fusion, taurodontism, and dens invagination); (b) number (including hypodontia, oligodontia, and hyperdontia); (c) structure (including amelogenesis imperfecta, dentinogenesis imperfecta, and dentin dysplasia); and (d) position (including displacement, impaction, and dilacerations). Results: The reliability between the two observers was 79.56% according to the Kappa statistics. The prevalence of dental anomalies diagnosed by panoramic radiographs was 29%. Anomalies of position and number were the most common types of abnormalities, and anomalies of shape and structure were the least in both genders. Anomalies of impaction (44.76%), dilacerations (21.11%), hypodontia (15.88%), taurodontism (9.29%), and hyperdontia (6.76%) were the most common subtypes of dental anomalies. The anomalies of shape and number were more common in the age groups of 7-12 years and 13-15 years, respectively, while the anomalies of structure and position were more common among the other age groups. Conclusion: Anomalies of tooth position were the most common type of dental anomalies, and structure anomalies were the least in this Iranian population. The frequency and type of dental anomalies vary within and between populations, confirming the role of racial factors in the prevalence of dental anomalies.
Purpose: The purpose of this study was to determine the prevalence of missing and impacted third molars in people aged 25 years and above. Materials and Methods: The study sample of 3,799 patients was chosen randomly from patients who visited Pusan National University Dental Hospital and had panoramic radiographs taken. The data collected included presence and impaction state, angulation, and depth of impaction of third molars, and radiographically detected lesions of third molars and adjacent second molars. Results: A greater percentage of men than women retained at least one third molar. The incidence of third molars decreased with increasing age. The incidence of partially impacted third molars greatly declined after the age of 30. Vertically impacted maxillary third molars and horizontally impacted mandibular third molars were most frequent in all age groups. Among the maxillary third molars, those impacted below the cervical line of the second molar were most frequent in all age groups, and among the mandibular third molars, deeply impacted third molars were most frequent in those aged over 40. Dental caries was the most common radiographic lesion of the third molars. Mesioangularly impacted third molars showed radiographic lesions in 13 (9.5%) adjacent maxillary second molars and 117 (27.4%) mandibular second molars. Conclusion: The number of remaining third molars decreased and the percentage of Class C depth increased with age. Caries was the most frequent lesion in third molars. Partially impacted mesioangular third molars showed a high incidence of caries or periodontal bone loss of the adjacent second molar. Regular oral examination will be essential to keep asymptomatic third molars in good health.
Early identification of maxillary canine impaction is very important, because of treatment duration time. Analysis used by 3-dimensional computed tomography is more important than before in treatment planning. The aim of this study was to analysis of factors associated with the treatment during time of forced eruption of the impacted maxillary canine using 3-dimensional computed tomography. 17 patients were analysed in Daejeon dental hospital of Wonkwang university(16 patients with unilateral impactions and 1 patient with bilateral impaction). In total, 18 maxillary impacted cannines were analysed by 3-dimensional computed tomography. The results obtained were as follow; 1. There was positive correlation between the age of patient and treatment duration time. 2. There was positive correlation between the distance of occlusal plane to cusp of maxillary impacted canine and treatment duration time, and negative correlation between the length of maxillary impacted canine and treatment duration time. 3. There was positive correlation between the angulation of midsagittal plane to axis of maxillary impacted canine and treatment duration time, and negative correlation between occlusal plane to axis of maxillary impacted canine and treatment duration time. In conclusion, age, tooth length, distance from the canine cusp tip to the occlusal plane, angulation of tooth axis to occlusal plane and midsagittal plane allow that we can predict treatment duration time.
치아형성 과정 중 치배 주위의 물리적 작용이나 대사 환경의 변화로 치아 발생은 영향을 받을 수 있다. 유치열기에 가해진 외상은 계승 영구치의 발생에 장애를 초래시킬 수 있는 가장 대표적인 물리적 요인으로 이로 인한 합병증으로 계승 영구치의 법랑질 변색 및 저형성증이 가장 흔히 발생하고 드물게 매복, 이소 맹출, 치근 형성 정지 및 치근 만곡 등이 나타난다. 본 증례에서는 만 6세 5개월의 여자 어린이 환자가 안면부 외상을 주소로 내원하여 상악 좌우 유중절치의 정출성 탈구, 상악 우측 유측절치의 함입성 탈구, 상악 좌측 유측절치의 구개측 측방 탈구로 진단되었고, 상악 치은부의 열상과 상악 전치부의 치조골 골절이 관찰되었다. 내원 당일 치과 응급실에서 동요도가 심한 상악 좌우 유중절치의 발치 후 치은 봉합술을 시행하였다. 24개월 정기 검진 중 방사선 사진 상에서 상악 좌,우측 측절치의 백악 법랑 경계(Cemento enamel junction, CEJ) 근방에 치근 만곡이 관찰되었고 상악 중절치 치근의 치관부 1/3부위에서 치근 형성이 정지되고 있는 양상이 관찰되었다. 치조골 골절이 어린이에서 흔하게 나타나는 외상의 유형은 아니나 본 증례에서와 같이 치근 발육이상 등 여러 합병증을 일으킬 수 있으므로 외상을 입은 환아에서 치조골 골절 여부에 대한 면밀한 검사 및 주기적인 검진은 예후 예측 및 향후 치료에 있어서 중요할 것이다.
매복치의 치료는 그 발생원인, 치아의 발육단계, 매복된 위치 및 맹출 경로, 환자의 협조도 등을 고려하여 치료방법을 결정한다. 일반적으로 맹출에 영향을 줄 수 있는 유전적, 전신적 유발인자가 있다면 이의 치료와 더불어 맹출 유도를 위한 조기치료의 필요성이 있으며, 맹출에 장애를 주는 물리적 요소가 존재한다면 이를 우선적으로 제거해야 한다. 그러나 맹출과 관련된 어떠한 요인이나 맹출공간 부족 등이 발견되지 않았다면 유치 발거 이외에 치아를 외과적으로 노출시킨 후 교정장치를 부착하고 교정력을 이용하는 치아맹출 유도법, 그리고 매복치를 외과적 재위치 또는 이식하는 방법을 사용할 수 있다. 자가치아이식술을 시행할 경우 치아발육 상태를 고려한 적절한 시기의 선택, 적절한 치조와 형성 및 이식치아의 발거와 식립에 있어 외상의 최소화 등이 성공에 중요한 요소다. 본 증례에서는 상악우측중절치의 미맹출을 주소로 내원한 7세 여자 환자에서 CT검사결과 상악우측중절치의 치관부가 전비극에 근접하여 협측으로 위치되어 있으며 치관부에서 만곡이 관찰되었다. 치근은 Nolla's stage 7정도의 발육을 보였으며 이에 자가치아이식을 시행하였다. 주기적인 관찰결과 치근흡수 등의 합병증 없이 양호한 치유양상이 관찰되었다.
치아종은 치성 종양 중 가장 흔한 형태로 대체로 병소의 크기나 위치에 따라 증상이 없는 경우가 대부분이며 성장이 제한적이다. 보통 10대 이전에 주로 진단되며 종종 영구치의 매복이나 맹출 지연을 유발한다. 형태에 따라 복합 치아종과 복잡 치아종으로 구별된다. 복잡 치아종은 치성 조직이 조직화되지 않은 덩어리로 형태학적으로 정상 치아 형태를 닳지 않으며 전체 치아종의 약 25%, 악골내 발생하는 치성 종양의 약 22%를 차지하고 남성에서 하악 구치부에 호발한다. 치아종의 원인은 아직 밝혀져 있지 않으나, 아마도 해당 부위의 국소적인 외상, 감염, 유전적 원인이 관여 할 것이라 추측된다. 치아종의 치료는 대부분 보존적인 외과적 적출술이 추천되며 재발은 거의 없다. 본 증례는 상악 제 1대구치의 매복을 일으킨 복잡 치아종으로 진단된 두 어린이에게서 병소의 외과적 적출술 및 개창술을 시행하고 관찰한 결과 약 2년후 제 1 대구치가 자발적으로 맹출함을 보여 이에 보고하는 바이다.
목적: 이 연구의 목적은 개방된 인접면 접촉이 인접하는 자연치와 임플란트에 어떠한 영향을 주는지 알아보고자 하였다. 연구 재료 및 방법: 조선대학교 치과병원에서 2008부터 2018년 사이에 개방된 인접면 접촉으로 제작된 구치부 임플란트 수복물을 장착한 환자 중에서 유지관리기간이 최소 3년 이상이고 인접하는 자연치아가 건강하면서 대합치가 고정성 수복물인 임플란트를 20개 선정하여 실험군(Group A)으로 하였다. 동일한 선정기준 하에 같은 기간 통상적인 방법으로 제작된 구치부 임플란트 20개를 대조군(Group B)으로 하였다. 두 그룹사이에 임플란트와 접하는 자연치의 우식, 식편압입, 임플란트의 변연골 상실을 비교 평가하였다. 결과: Group A과 Group B 사이에 치아우식과 식편압입 및 변연골 흡수의 발생빈도는 통계학적으로 유의성이 없었다. Group A와 Group B의 평균 변연골 흡수양은 각기 0.80 ± 0.39 mm, 1.1 ± 0.43 mm였으며, 두 그룹사이에 통계학적으로 유의한 차이가 있었다. 결론: 이 실험의 결과내에서, 개방된 인접면 접촉을 가진 임플란트 보철은 인접하는 자연치와 임플란트에 어떠한 유해한 영향을 주지 않으므로 선택적인 상황에서 임상에 적용할 수 있을 것으로 사료된다.
