골 유착성 임프란트 보철수복시 자연지대치와의 고정유무에 따른 유한요소법적 응력분석 (FINITE ELEMENT ANALYSIS OF STRESSES INDUCED BY OSSEOINTEGRATED PROSTHESES WITH OR WITHOUT CONNECT10N BETWEEN NATURAL TOOTH AND OSSEOINTEGRATED ABUTMENTS)

  • The purpose of this study was to examine, by the method of finite element analysis, how implant geometry with or without connection between natural tooth and osseointegrated abutments affected the stress distribution in surrounding bone and osseointegrated prosthesis. The mandibular first and second molars were removed and the two osseointegrated implants were placed in the first and second molar sites. Stress analysis induced by prostheses with connection(Model A)or without connection(Model B) between natural tooth(second bicuspid) and two osseointegrated abutments(first molar and second molar) was performed under vertical point load(Load P1) or distributed point load(Load P2). The results were as follows; 1. Under vertical point load, mesial tilting was shown in both Model A and Model B and inferior displacement of Model A was greater than that of Model B in the second bicuspid. 2. Under vortical point load, the first and second molars showed mesial tilting in both Model A and Model B, and inferior displacement of them was similar in Model A and Model B and was less than that of the second bicuspid. 3. Under distributed point load, mesial displacement was shown in Model A and Model B and inferior displacement of Model A was less than that of Model B in the second bicuspid. 4. Under distributed point load, mesial tilting was shown and inferior displacement of Model A was similar to that of Model B in the first and second molars. 5. In Model A under vertical point load, high stress was concentrated in the corneal portion of first molar and distributed throughout the second molar and the second bicuspid, and the stress distribution of the second molar was greater than that of the second bicuspid. 6. In Model B under vertical point load, high stress was concentrated in the coronal and mesio-cervical portion of the first molar. 7. In Model A under distributed point load, high stress was concentrated in the mesio-cervical portion of the first molar and evenly distributed throughout the second molar and the second bicuspid. 8. In Model B under distributed point load, high stress was concentrated in the disto-cervical portion of the second bicuspid and evenly distributed throughout the first and second molars.

어린이에서 함치성 낭과 연관된 매복 소구치와 대구치의 치료 (IMPACTED PREMOLARS AND MOLARS ASSOCIATED WITH DENTIGEROUS CYSTS IN CHILDREN)

  • 치아 매복은 어린이 환자 진료 시에 자주 관찰되는 맹출 장애이다. 어린 환자에서 매복된 치아가 존재하는 경우, 함치성 낭과 연관이 있을 가능성이 높다. 함치성 낭은 제3대구치를 제외하고는 상악 견치, 하악 소구치 부위에서 가장 호발하며, 점진적인 증식으로 악골이 팽윤되어 안모를 변화시키며, 주위 악골의 파괴와 치근의 흡수를 야기하거나 침범된 치아의 변위를 유발할 수 있으므로, 조기 진단과 적절한 치료가 무엇보다도 중요하다. 함치성 낭과 연관된 치아가 과잉치나 지치라면, 치아의 발거를 포함한 완전한 낭종 적출술이 적절한 치료라 할 수 있지만, 그렇지 않은 경우에는 환자의 심리적, 정신적 외상을 예방하기 위해 원인 치아의 보존이 고려되어야 할 것이다. 이뿐만 아니라, 치아의 변위 정도, 골 파괴 정도, 치근의 성숙도, 주위 치아와의 관계, 환자의 교합과 구강 악안면 영역의 성장 양상 등도 같이 고려되어야 할 것으로 생각된다. 본 증례에서는 위와 같은 사항들을 고려하여, 함치성 낭과 연관된 매복 소구치와 대구치를 낭종 적출술 후 공간 유지, 외과적 수술과 교정적 견인, 외과적 발거 후 교정적 배열 등의 방법을 통해 양호한 치료 결과를 얻었기에 보고하는 바이다.

평기어의 동접촉 해석 (Dynamic Contact Analysis of Spur Gears)

  • A numerical method is presented for the dynamic analysis of spur gears rotating with very high angular speeds. For an efficient computation each gear is assumed to consist of a rotating rigid disk and an elastic tooth having mass, and finite element formulations are used for the equations of motion of the tooth. The geometric constraint is imposed between the rigid disk and the elastic tooth to fix them, and contact condition is imposed between the meshing teeth of the gears. At each iteration of each time step the Lagrange multiplier and contact force are revised by using the constraint error vector, and then the whole equations of motion are time integrated with the given Lagrange multiplier and contact force. For the accurate solution the velocity and acceleration constraints as well as the displacement constraint are satisfied by the monotone reductions of the constraint error vectors. Computing procedures associated with the iterative schemes are explained and numerical simulations are conducted with the spur gears.

고령 환자의 기존 의치를 치과기공소에서 수리한 증례 (Denture repair for elderly patients in dental laboratories: a case report)

  • The need for repairing removable dentures has grown as the population had aged. The direct methods allow existing dentures to be repaired without interrupting their use. However, if patient compliance is low, direct methods may be challenging. Moreover, attaching an artificial tooth to a metal base is a complicated procedure because it necessitates casting a retentive element and soldering it to a metal base. This clinical report describes how to add an artificial tooth to a metal base, reline denture bases, and reestablish occlusion on relined removable dentures using indirect methods. Existing removable dentures were successfully repaired and their service life was efficiently extended using the methods described.

복합레진 인레이 수복시 와동형태에 따른 치아파절에 관한 유한요소법적 연구 (A THREE DIMENSIONAL FINITE ELEMENT ANALYSIS WITH CAVITY DESIGN ON FRACTURE OF COMPOSITE RESIN INLAY RESTORED TOOTH)

  • Fracture of cusp, on posterior teeth, especially those carious or restored, is major cause of tooth loss. Inappropriate treatments, such as unnecessarily wide cavity preparations, increase the potential of further trauma and possible fracture of the remaining tooth structures. Fracture potential may be directly related to the stresses exerted upon the tooth during masticatory function. The purpose of this study is to evaluate the fracture resistance of tooth, restored with composite resin inlay. In this study, MOD inlay cavity prepared on maxillary first premolar and restored with composite resin inlay. Three dimensional finite element models with eight nodes isoparametric solid element, developed by serial grinding-photographing technique. These models have various occlusal isthmus and depth of cavity, 1/2, 1/3 and 1/4 of isthmus width and 0.7, 0.85 and 1.0 of depth of cavity. The magnitude of load was 474 N and 172 N as presented to maximal biting force and normal chewing force. These loads applied onto ridges of buccal and lingual cusp. These models analyzed with three dimensional finite element method. The results of this study were as follows : 1. There is no difference of displacement between width of occlusal isthmus and depth of cavity. 2. The stress concentrated at bucco-mesial comer, bucco-disal comer, pulpal line angle and the interface area between internal slopes of cusp and resin inlay. 3. The vector of stress direct to buccal and lingual side from center of cavity, to tooth surface going on to enamel. The magnitude of vector increase from occlusal surface to cervix. 4. The crack of tooth start interface area, between internal slop of buccal cusp and resin inlay. It progresses through buccopulpal line angle to cervix at buccomesial and buccodistal comer. 5. The influence with depth of cavity to fracture of tooth was more than width of isthmus. 6. It would be favorable to make the isthmus width narrower than a third of the intercuspal distance and depth of cavity is below 1 : 0.7.

생활치 표백술 후 수종의 자유 산소기 제거제 처리가 복합 레진-법랑질 전단 접착 강도 및 파절 양상에 미치는 영향 (THE EFFECT OF REMOVAL OF RESIDUAL PEROXIDE ON THE SHEAR BOND STRENGTH AND THE FRACTURE MODE OF COMPOSITE RESIN-ENAMEL AFTER TOOTH BLEACHING)

  • Tooth bleaching has been prevailing recently for its ability to recover the color and shape of natural teeth without reduction of tooth material. However, it has been reported that bleaching procedure adversely affects the adhesive bond strength of composite resin to tooth. At the same time the bond strength was reported to be regained by application of some chemical agents. The purpose of this in vitro study was to investigate the effect of the removal of residual peroxide on the composite- enamel adhesion and also evaluated fracture mode between resin and enamel after bleaching. Sixty extracted human anterior and premolars teeth were divided into 5 groups and bleached by combined technique using of office bleaching with 35 % hydrogen peroxide and matrix bleaching with 10% carbamide peroxide for 4 weeks. After bleaching, the labial surfaces of each tooth were treated with catalase, 70% ethyl alcohol, distilled water and filled with composite resin. Shear bond strength was tested and the fractured surfaces were also examined with SEM. Analysis revealed significantly higher bond strength values. (p<0.05) for catalase-treated specimens, but water-treated specimens showed reduction of bond strength, alcohol- treated specimens had medium value between the two groups(p<0.05). The fracture mode was shown that the catalase group and the alcohol group had cohesive failure but the water sprayed group had adhesive failure. It was concluded that the peroxide residues in tooth after bleaching seems to be removed by gradual diffusion and the free radical oxygen from peroxide prevents polymerization by combining catalyst in the resin monomer. Therefore it may be possible to eliminate the adverse effect on the adhesion of composite resin to enamel after bleaching by using water displacement solution or dentin bonding agent including it for effective removal of residual peroxide.

선생 유치의 외상 후 발생한 만곡된 하악 영구 중절치의 자발적 맹출 : 증례보고 (Spontaneous Eruption of a Dilacerated Mandibular Central Incisor after Trauma of a Primary Tooth : Two Case Reports)

  • 치아 만곡은 치아의 치관부 혹은 치근부에서 발생한 장축의 갑작스러운 굽힘을 의미한다. 이는 주로 선행 유치에 대한 기계적 외상이 원인이 되어 발생한다. 만곡된 치아는 종종 맹출 장애를 동반한다. 치아 만곡은 하악에서 드물게 발생하지만, 함입과 같은 심한 외상성 손상 이후에 충분히 발생할 수 있다. 본 증례는 두 명의 환아에서 확인된 선행 유치의 함입성 외상 이후 발생한 하악 영구 중절치에서의 만곡에 대한 증례를 보고하고 있으며, 두 증례 모두에서 만곡 치아의 자발적 맹출이 관찰되었다. 첫 번째 증례는 치관과 치근의 경계부 주위에서 측방치근만곡을 보였으며, 두 번째 증례는 치관만곡을 보였다. 본 증례를 통해서 유치의 외상 이후 하악 영구 중절치에서 치관부 만곡 혹은 치관과 치근 경계부에서의 측방만곡이 관찰된다면, 만곡치아의 자발적 맹출을 기대해 볼 수 있음을 확인할 수 있었다.

  • Ankylosed tooth is defined as 'the discontinuance of normal passive tooth eruption without any mechanical barrier'. Ankylosed tooth treatment is a challenge to dental clinicians. In treatment of maxillary molar ankylosis cases there are risks of oro-antral fistula, displacement of root fragments into the maxillary sinus, as well as the necessity for providing additional sinus bone augmentation for future implant placement. In this study, we suggested a new technique using a piezoelectric device and a lateral side approach to the maxillary sinus leading to the simultaneous removal of the ankylosed maxillary molar and sinus grafting for the purpose of implant site development.

치아 미백제가 치아에 미치는 영향과 부작용 최소화를 위한 처치에 관한 고찰 (Effect of bleaching on human teeth and reduced treatment on negative influence -Review)

  • The purpose of the review article was to summarize and discuss the available information concerning the effect of bleaching on human teeth and reduced treatment on negative influence. Tooth bleaching effect was differ from extent of concentration and application period of a tooth bleaching agent, certainly full knowledge prior treatment about adverse effect possible appearance and follow clinical treatment for the least reduce. It remains unclear in how far those observation may result in significant adverse effect under clinical conditions. Nevertheless, further investigation are necessary to elucidate these aspect more precisely. The findings of the study were as follows : 1. It is recommended to delay placement of restorations after termination of bleaching therapy for at least 1-3 weeks. 2. Reduced negative influence that is clinical feasibility of catalase in protecting bleached surface against Oxygen radical. 3. The residual peroxide in tooth after bleaching seems to be removed by gradual diffusion and it may be possible to eliminate the adverse effect on tooth by using water displacement solution, ethylalcohol and aceton including it for effective removal of free radical oxygen.
