금인레이 와동의 폭경이 응력분포와 변위에 미치는 영향에 관한 3차원 유한요소법적 연구 (A THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS ON STRESS AND DISPLACEMENT RELATED TO ISTHMUS WIDTH OF GOLD INLAY CAVITY)

  • The purpose of this study was to evaluate the fracture resistance of tooth restored with gold inlay. A profound understanding of the isthmus width factor, which is one of the several parameters of cavity designs, would facilitate the appropriate cavity preparation in a specific clinical situation. In this study, the cavities for gold inlay were prepared in maxillary left first premolar. A three-dimensional model was designed using I-DEAS program. The model was composed of 2515- nodes and 2172 isoparametric brick elements. In the model isthmus width was varied into 1/4, 1/3 and 1/2 of intercuspal width respectively, and numeric values of the material properties of enamel, dentin and gold was set. Three types of load : concentrated load, divided load and distributed load was 500N. The empty cavities in the model were also examined using divided load and distributed load. The three - dimensional Finite Element Method was used to analysis the displacement and stress distribution. The results were as follows : 1. All of the experimental models which were filled with gold inlay revealed similar direction of displacement to that of the natural tooth model under the same load type. But in the models with empty cavities, as the isthmus width increased, the degree of displacement increased in the case of divided load type. 2. All experimental models which were filled with gold inlay showed stress concentration at load points, but in the models with empty cavities at divided load type, as isthmus width increased, stress was concentrated at the comer of the pulpal floor. 3. In the models with empty cavities at divided load type, tooth fracture was expected regardless of isthmus width, but all experimental models which were filled with gold inlay after cavity preparation were not susceptible to fracture. 4. In all experimental models which were filled with gold inlay after cavity preparation, displacement patterns were similar under both concentrated and divided load types. In the models with empty cavities, a divided load resulted in a bucco-lingual cuspal displacemenat in both sides, but a distributed load resulted in a lingual displacement of the tooth.

In-vitro investigation of the mechanical friction properties of a computer-aided design and computer-aided manufacturing lingual bracket system under diverse tooth displacement condition

  • Objective: The purpose of this study was to compare the static (SFF) and kinetic frictional forces (KFF) of a computer-aided design and computer-aided manufacturing lingual bracket (CAD/CAM-LB) with those of conventional LB (Con-LB) and Con-LB with narrow bracket width (Con-LB-NBW) under 3 tooth displacement conditions. Methods: The samples were divided into 9 groups according to combinations of 3 LB types (CAD/CAM-LB [Incognito], Con-LB [7th Generation, 7G], and Con-LB-NBW [STb]) with 3 displacement conditions (no displacement [control], maxillary right lateral incisor with 1-mm palatal displacement [MXLI-PD], and maxillary right canine with 1-mm gingival displacement [MXC-GD]; n = 6/group). While drawing a 0.016-inch copper or super-elastic nickel-titanium archwire with 0.5 mm/min for 5 minutes in a chamber maintained at $36.5^{\circ}C$, SFF and KFF were measured. The Kruskal-Wallis method with Bonferroni correction was performed. Results: The Incognito group demonstrated the highest SFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD, Stb-MXC-GD] < [7G-MXC-GD, 7G-MXLI-PD, 7G-control] < [Incognito-MXLI-PD, Incognito-control, Incognito-MXC-GD]; p < 0.001). However, there were no significant differences in SFF among the 3 displacement conditions within each bracket group. Within each displacement condition, the Incognito group demonstrated the highest KFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD] < Stb-MXC-GD < 7G-MXLI-PD < [7G-control, 7G-MXC-GD] < [7G-MXC-GD, Incognito-MXLI-PD, Incognito-control] < [Incognito-control, Incognito-MXC-GD]; p < 0.001). MXC-GD exhibited higher KFFs than MXLI-PD in the same bracket group. Conclusions: The slot design and ligation method of the CAD/CAM-LB system should be modified to reduce SFF and KFF during the leveling/alignment stage.

  • The purpose of this study was to compare and evaluate the stress distribution and displacement developed in the abutment teeth and residual ridge area by madibular unilateral distal extension removable partial denture with 2 different retainer designs. The retainers on right and left canine and right 2nd molar were Alters clasp in one model and telescopic crown in the other model. The stress distribution of abutment teeth and residual ridge area on two model were compared and analyzed with 3-dimensional finite element method. 150N and 400N forces were applied vertically, 30 degree and horizontally on the central fossa area of left 1st molar of the removable partial denture, and then stress distribution patterns were analyzed and compared. The results were as follows 1. As the magnitude and angulation of applied force were increased, the magnitude of stress on the right and central residual ridge area and the right canine of the telescopic type increased and comparing to those of the Alters clasp type. 2. As the magnitude and angulation of applied force were increased, the mesial direction of displacement on the right residual ridge area and the right tooth of the telescopic type increased and the distal direction of displacement on left residual ridge area and the left canine increased comparing to those of Akers clasp type. 3. As the vertical force was applied, the distal direction of the displacement of the right tooth were greater and that of the left canine was smaller and the upward displacement of the right canine was greater in telescopic partial denture than those of Akers clasp type. 4. As the 30 degree force was applied, the mesial direction of the displacement of the right tooth were greater and the distal direction of the displacement of the left canine was smaller and the upward displacement of the right canine was greater in telescopic partial denture than those of Akers clasp type. In the horizontal force the results were same in right area tooth but the distal direction of displacement was greater in left canine. 5. In both removable partial dentures, as the magnitude and degree of force were increased, the stress and displacement were increased. The compressive force was dominative than the ten sile force. 6. In both removable partial dentures, the magnitude of stress was greater on mucosal tissue area than that of the alveolar bone area on distal extension residual ridge area but the result was reversed on anterior residual ridge area. The displacement was always greater on mucosal tissue area than that of alveolar bone area.

In-vitro assessment of the accuracy and reliability of mandibular dental model superimposition based on voxel-based cone-beam computed tomography registration

  • Objective: This study was performed to evaluate the accuracy and reliability of a newly designed method to achieve mandibular dental model superimposition, using voxel-based cone-beam computed tomography (CBCT) registration. Methods: Fourteen dry cadaveric mandibles and six teeth extracted from patients with severe periodontitis were used to establish 14 orthodontic tooth-movement models. The protocol consisted of two steps: in the first step, voxel-based CBCT mandible superimposition was performed; the reference comprised the external portion of the symphysis, extending to the first molar. The laser-scanned dental model image was then integrated with the CBCT image to achieve mandibular dental model superimposition. The entire process required approximately 10 minutes. Six landmarks were assigned to the teeth to measure tooth displacement, using tooth displacement on the superimposed laser-scanned mandibles as the reference standard. Accuracy was evaluated by comparing differences in tooth displacement based on the method and the reference standard. Two observers performed superimposition to evaluate reliability. Results: For three-dimensional tooth displacements, the differences between the method and the reference standard were not significant in the molar, premolar, or incisor groups (p > 0.05). The intraclass correlation coefficients for the inter- and intra-observer reliabilities of all measurements were > 0.92. Conclusions: Our method of mandibular dental model superimposition based on voxel registration is accurate, reliable, and can be performed within a reasonable period of time in vitro, demonstrating a potential for use in orthodontic patients.

국소의치 유지장치의 설계변화에 따른 지지조직의 3차원적 유한요소법 응력분석 (A 3-DIMENSIONAL FINITE ELEMENT STRESS ANALYSIS ON THE SUPPORTING TISSUES OF REMOVABLE PARTIAL DENTURES WITH VARIOUS RETAINER DESIGNS)

  • The purpose of this study was to evaluate the stress distribution developed in the supporting structures by mandibular distal extension removable partial dentures with 2 different direct retainer designs and with or without indirect retainer and abutment splinting. The examined direct retainers on the second bicuspid abutment tooth were Akers clasp and RPA clasp, the indirect retainer was located on the mesial fossa of the first bicuspid, and the first and second bicuspid were splinted in case of tooth splinting. Total 8 cases were compared and analyzed with 3-dimensional finite element method. 150N were applied vertically on the artificial teeth of the removable partial denture, and then stress distribution patterns were analyzed and compared. The results were as follows : 1. The forces transmitted to the abutment tooth were primarily from the occlusal rests. 2. The abutment tooth was displaced distally when the force was applied. The compressive stress was observed at the distal root surface of the abutment tooth and the tensile stress, at the mesial root surface. 3. The denture base was displaced posteriorly and inferiorly when the force was applied. At the more distal portion of the denture base, the greater displacement was observed.And the anterior portion of the major connector was displaced superiorly. 4. The occlusal rest placed on the distal part of the abutment tooth tended to tip the tooth more posteriorly than did one on the mesial part of that tooth. 5. Severe superior displacement was observed at the anterior portion of the major connector in case of removable partial dentures without indirect retainer. 6. In case of tooth-splinting, the stress was distributed through all the root surface of both abuments. In case of no tooth-splinting, the stress was concentrated on the distal root surface of the primary abutment.

  • The surgical removal of the wisdom teeth is obligate when forceps extraction fails or when the wisdom teeth are impacted. The surgical removal of impacted maxillary third molars is a commonly performed procedure usually associated with few complications & little morbidity. The most frequent complications are tooth root fracture, maxillary tuberosity fracture, tooth displacement into the maxillary sinus & oroantral fistula formation. A rarely reported complication is the displacement of a tooth into the infratemporal fossa. The method of prevention of this complication is by the placement of either a finger or periosteal elevator posterior to the tooth during extraction. To remove the displaced upper third molar is very difficult & has many complications, e.g., persistent bleeding & nerve damage. When the wisdom teeth is displaced, it is initially necessary to gain access to bone by developing a mucoperiosteal path of delivery is developed by additional bone removal or, preferably planned sectioning of the tooth. There are many approaching techniques to remove the displaced upper third molar. This following report describes the surgical technique of displaced upper third molar in the pterygopalatine fassa by the midpalatal &transpharyngeal approach.

유치의 치근단 병소에 의해 변위된 영구치의 자발적 맹출 (THE SPONTANEOUS ERUPTION OF DISPLACED PERMANENT TOOTH BY PERIAPICAL LESION OF PRIMARY TEETH)

  • 유치의 치수감염은 주로 치아우식증이나 외상에 의해 발생한다. 유치의 치수 감염을 적절히 치료하지 않으면, 계승영구치의 법랑질 형성 부전이나 매복, 치배의 위치 변화, 치근 만곡 등의 합병증을 초래한다. 유치의 치근단 병소에 의한 계승영구치는 여러 방향으로 변위가 가능하다. 변위된 계승영구치의 치료법으로는 유치 발거 후 주기적 관찰, 외과적인 노출, 교정적 견인, 치아이식, 영구치의 발거 등이 있다. 본 증례들에서는 초진시 임상적 및 방사선학적 검사 결과, 유전치의 치근단 병소에 의해 변위된 영구치 치배가 관찰되었다. 변위 정도와 치근 발육 정도, 맹출 방향 등을 고려하여 유치 발거 후 변위된 영구치의 자발적 맹출 유도를 기대하였다. 두 증례에서 유치의 발거 후 주기적 관찰만으로 변위된 영구치의 자발적 맹출의 결과를 얻을 수 있었다.

Validation of three-dimensional digital model superimpositions based on palatal structures in patients with maximum anterior tooth retraction following premolar extraction

  • Objective: This study aimed to evaluate the superimposition accuracy of digital modes for measuring tooth movement in patients requiring anterior retraction after premolar extraction based on the proposed reference regions. Methods: Forty patients treated with bilateral maxillary first premolar extraction were divided into two groups: moderate retraction (< 7.0 mm) and maximum retraction (≥ 7.0 mm). Central incisor displacement was measured using cephalometric superimpositions and three-dimensional (3D) digital superimpositions with the 3rd or 4th ruga as the reference point. The Wilcoxon signed-rank test and linear regression analyses were performed to test the significance of the differences and relationships between the two measurement techniques. Results: In the moderate retraction group, the central incisor anteroposterior displacement values did not differ significantly between 3D digital and cephalometric superimpositions. However, in the maximum-retraction group, significant differences were observed between the anteroposterior displacement evaluated by the 3rd ruga superimposition and cephalometric methods (p < 0.05). Conclusions: This study demonstrated that 3D digital superimpositions were clinically as reliable as cephalometric superimpositions in assessing tooth movements in patients requiring moderate retraction. However, the reference point should be carefully examined in patients who require maximum retraction.

고밀도 제로터 펌프용 신형 치형설계 및 특성해석 (New Tooth Type Design and Characteristic Analysis for High Density Gerotor Pump)

  • The gerotor type device is mainly used in low-speed, high-torque hydraulic motors, and is also applied as a small priming hydraulic pump. For this reason, many studies have been conducted to increase the efficiency of the gerotor pump. In this paper, we propose a new tooth profile design method different from the existing method. The new tooth design is made by modifying the tooth surface using the amplification function of the trajectories, created along the inner and outer rolling circles around the base circle. The shape of the mate rotor is then created using rotation simulation techniques. Such shapes are described as hypercloid. The designed hypercloid rotor is compared with the existing trochoid rotor, and the characteristics of the parameters and volumetric displacements are analyzed. Through this process, the optimum design with larger volumetric displacement than the existing rotor is achieved.

Achievement of Anterior Teeth Arrangement Using a Single Stage of Clear Aligner

  • Purpose: The aim of this study was to evaluate the degree of achievement of anterior tooth movement using a thermoplastic clear aligner by superimposing pretreatment, setup, and post-treatment models and to analyze the factors that influence it. Materials and Methods: Forty patients who underwent clear aligner treatment at the Department of Orthodontics were included in the study. The pretreatment, setup, and post-treatment models were superimposed, and the achievement rates were evaluated. We analyzed the factors that caused changes in achievement rate on each condition. Result: The average achievement rate of total tooth movement was 57.65%. Mesiodistal and anteroposterior displacements were achieved better than vertical displacement and rotation. In each displacement, posterior, mesial, and extrusive movements were better achieved than the rotation. Spacing was better corrected than crowding, especially in the mesiodistal direction. Most of the tooth movements were achieved better when using 3-step thicknesses of aligners were used. Conclusion: The treatment results of clear aligners may differ depending on the type of tooth movement, age and other treatment conditions. Clinicians should consider unfavorable conditions based on the results of this study.
