발치와의 혈소판 농축 혈장과 골 이식술 시행시 골 형성에 대한 임상 및 조직병리학적 연구 (Clinical and Histopathological Study Using Platelet Rich Plasma and Bone Grafts in Extraction Sockets)

  • The socket preservation procedure was a simple and effective technique, and has better prognosis for implantation. The socket preservation usually used barrier membrane in combination with/without alloplastic bone materials. A recently study had shown that a regenerative therapy to tooth extraction utilizing growth factors made better results. Platelet-rich plasma was clinically easy method that acquired the growth factors, and is known that accelerated new bone formation and mineralization of bone graft materials. The purpose of this study was to evaluate clinical and histopathologic results which occur following socket preservations using platelet-rich plasma and bovine bone powder. Twelve patients who required extraction of one or more teeth for implantation at the department of periodontics in Dankook University Dental Hospital were selected. Extraction sockets were treated by using platelet-rich plasma and bovine bone powder. 3 months later, we observed clinical and histopathological results as follows: 1. Internal vertical measurement was an average of 7.33mm preoperatively and statistically significantly decreased to an average of 1.42mm postoperatively(p<0.05). 2. External vertical measurement was an average of 3.33mm preoperatively and decreased to an average of 2.75mm postoperatively; therefore there was no significant difference. 3. Horizontal measurement was an average of 7.75mm preoperatively and statistically decreased to an average of 6.08mm postoperatively(p<0.05). 4. Osteocyte-like cells and new bone formation connected with bovine bone grafts were observed in histopathologic examination. This study implied that platelet-rich plasma and bovine bone powder grafts were effective treatment for socket preservation and regeneration of severe bony defect made by implantation failure.

Maxillary alveolar bone evaluation following dentoalveolar expansion with clear aligners in adults: A cone-beam computed tomography study

  • Objective: To investigate the effects of maxillary orthodontic expansion on the alveolar bone tissue in adult patients treated with aligners by using cone-beam computed tomography. Methods: Thirty patients (22 females and 8 males; mean age: 36.3 years) were treated with Invisalign® aligners. Cone-beam computed tomography and digital models were obtained before (T0) and after (T1) upper arch expansion. The bone thicknesses in the cervical, middle, and apical areas of the incisors, canines, premolar, and first molars were buccally and palatally measured, totaling 96 areas and 2,880 measurements. The buccolingual inclinations and transverse measurements of the teeth were obtained from digital models to correlate them with the bone changes. The statistical tests used were Student's t-test, analysis of variance, and Pearson's correlation tests (p < 0.05). Results: From the 96 areas evaluated, 84 revealed an increase or stability in the alveolar bone thickness and twelve displayed significant bone loss. Bone changes did not correlate with the tooth inclination and transverse measurements. Conclusions: Within the limitation of the present study, mild levels of upper arch expansion obtained with Invisalign® aligners in adult patients did not result in any clinically significant loss of alveolar bone thickness.

발치와의 결손부위에 $Regenafil^{TM}$의 임상적 적용에 관한 조직학적 연구 (Histologic evaluation of $Regenafil^{TM}$ on defects of extraction socket)

  • The purpose of this study is to evaluate histologic result of bone substituting material on defects followed tooth extraction. We compare the histologic findings control, DFDBA, Bio-Oss(R), and $Regenafil^{TM}$, Briefly, mandibular premolar teeth were extracted available for bone filling. All alveolar sites were checked after extraction and thoroughly debrided with a dental curet to remove the periodontal ligament. Extraction sites were prepared dehiscence on buccal side 7mm height from alveolar crest. The graft materials were filled into the extraction socket and dehiscenc defects. The animals were sacrificed 12 weeks after implantation. Both treated and control mandibular sites were histologically evaluated with light microscopy. Histologic observation at 12 weeks revealed that control and experimental sites were healed uneventfully and directly apposed to new bone without any adverse tissue reaction. DFDBA and Bio-Ossn(R)sites maintain width of alveolar crest but were not fully resorbed. $Regenafil^{TM}$ sites also maintain width and particles were resorbed more than other graft materials. From this results, it was suggested that $Regenafil^{TM}$ is promising boen substituting materials maintaining the width of alveolar crest and height follewed tooth extraction.

Secondary closure of an extraction socket using the double-membrane guided bone regeneration technique with immediate implant placement

  • Purpose: Immediate implantation presents challenges regarding site healing, osseointegration, and obtaining complete soft-tissue coverage of the extraction socket, especially in the posterior area. This last issue is addressed herein using the double-membrane (collagen membrane+high-density polytetrafluoroethylene [dPTFE] membrane) technique in two clinical cases of posterior immediate implant placement. Methods: An implant was placed immediately after atraumatically extracting the maxillary posterior tooth. The gap between the coronal portion of the fixture and the adjacent bony walls was filled with allograft material. In addition, a collagen membrane (lower) and dPTFE membrane (upper) were placed in a layer-by-layer manner to enable the closure of the extraction socket without a primary flap closure, thus facilitating the preservation of keratinized mucosa. The upper dPTFE membrane was left exposed for 4 weeks, after which the membrane was gently removed using forceps without flap elevation. Results: There was considerable plaque deposition on the outer surface of the dPTFE membrane but not on the inner surface. Moreover, scanning electron microscopy of the removed membrane revealed only a small amount of bacteria on the inner surface of the membrane. The peri-implant tissue was favorable both clinically and radiographically after a conventional dental-implant healing period. Conclusions: Secondary closure of the extraction socket and immediate guided bone regeneration using the double-membrane technique may produce a good clinical outcome after immediate placement of a dental implant in the posterior area.

  • Cemento-osseous dysplasia occurs in the tooth bearing areas of the jaws and is probably the most common fibro-osseous manifestation. They are usually classified into three main groups according to their extent and radiographic appearance: periapical (surrounds the periapical region of teeth and are bilateral), focal (single lesion) and florid (scleroticsymmetrical masses) cemental-osseous dysplasias. Florid cemento-osseous dysplasia clearly appears to be a form of bone and cemental dysplasia that is limited to the jaws. Patients do not have laboratory or radiologic evidence of bone disease in other parts of the skeleton. For asymptomatic patients, the best management consists of regular recall examinations with prophylaxis and the reinforcement of good home hygiene care to control periodontal disease and prevent tooth loss. The treatment of symptomatic patients is more difficult. At this stage, there is an inflammatory component caused by the disease and the process is basically a chronic osteomyelitis involving dysplastic bone and cementum. Antibiotics might be suggested, but are not always effective. Two cases of florid cemento-osseous dysplasia diagnosed in two Korean females are reported with a review of the relevant literature.

  • Inadequate keratinized mucosa around dental implants can lead to more plaque accumulation, tissue inflammation, marginal recession and attachment loss. We evaluated the effects of free gingival and extracellular matrix membrane grafts performed to increase the insufficient width of keratinized tissue around dental implants in the posterior mandible. A 47-year-old female patient presented with discomfort due to swelling of the lower right second premolar area. Due to severe destruction of alveolar bone, the tooth was extracted. After 3 months, a guided bone regeneration (GBR) procedure was performed and then a dental implant was placed 6 months later. During the second-stage implant surgery, free gingival grafting was performed to increase the width of the keratinized tissue. After 12 months, a clinical evaluation was performed. A 64-year-old female patient had a missing tooth area of bilateral lower molar region with narrow zone of keratinized gingiva and horizontal alveolar bone loss. Simultaneous implant placement and GBR were performed. Five months after the first-stage implant surgery, a gingival augmentation procedure was performed with an extracellular matrix membrane graft to improve the width of the keratinized tissue in the second-stage implant surgery. After 12 months, a clinical evaluation was performed. In these two clinical cases, 12 months of follow-up, revealed that the increased width of the keratinized tissue and the deepened oral vestibule was well maintained. A patient showed a good oral hygiene status. In conclusion, increased width of keratinized tissue around dental implants could improve oral hygiene and could have positive effects on the long-term stability and survival rate of dental implants. When planning a keratinized tissue augmentation procedure, clinicians should consider patient-reported outcomes.

  • 빠르며 정확하고 안전한 치아이동을 목표로 삼고 있는 교정치료에서 최근 새로이 도입된 피질골 절제술과 견인 골형성술을 응용한 치아이동에 대하여 알아보았다. 특히 견치나 대구치의 후방이동은 기존의 치아이동 양식으로는 조절이 어렵고. 장기적인 기간이 불가피한 치아이동이다. 피질골 절제술과 견인 골형성술을 동반하여 상당히 효과적인 원심이동을 기할 수 있었던 증례를 통하여 적용 술식, 견인 장치 등을 논하고 그 효과를 파악하였다. 이런 술식을 통해 빠른 치아 이동과 이에 따른 전반적인 치료기간의 감소가 가능하였으며, 무리한 치아이동에서 발생할 수 있는 고정원 소실이나 치근흡수, 치주조직의 파괴 같은 부작용도 줄일 수 있었다.

  • The present study was designed to elucidate the effects of the Co-60 γ irradiation and/or calcium-deficient diet on the periodontal tissue formation in rat pups. The pregnant three-week old Sprague-Dawley rats were used for the study. The experimental group was divided into two groups, irradiation/normal diet group (Group 2) and irradiation/calcium-deficient diet group (Group 3). The control group was non-irradiation/normal diet group (Group 1). The abdomen of the rats at the 19th day of pregnancy were irradiated with single absorbed dose of 350 cGy. The rat pups were sacrificed on the 14th day after delivery, and the maxillae including molar tooth germ were taken. The specimens including the 1st molar tooth germ were prepared to make tissue sections for light and transmission electron microscopy. Some of tissue sections for light microscopy were stained immunohistochemically with anti-fibronectin and anti-osteonectin antibodies. The results were as follows; 1. In the periodontal ligament forming area, the fibroblasts of Group Z showed irregular arrangement and low activity. The immunoreactivity between the fibroblasts and collagen fibers was decreased, compared with Group 1. The fibroblasts of Group 3 showed atrophic change and clumped nucleus. The collagen fibers showed cystic change and low immunoreactivity to the fibronectin. 2. In the cementum forming area, the cementoblasts of Group 2 showed decrease of number and atrophic change. The cementoblasts of Group 3 showed edematous change, atrophy of cytoplasm, and clumping of nucleus. 3. In the alveolar bone forming area, the bone of Group 2 was thin and various degree of immunoreactivity to the osteonectin. Group 3 showed edematous osteoblasts, fibrous degeneration of bone marrow, and weak immunoreactivity to the osteonectin.

Splint 및 비외과적 치주치료를 통하여 치주질환에 이환된 치아의 안정화 증례 보고 (Stability of periodontally compromised teeth after splint and non-surgical therapy: two cases followed-up for 1 to 3 years)

  • 본 증례는 치주적으로 이환된 하악 전치부를 치은연상 치석제거술과 치근활택술을 포함한 비외과적 치주치료, 교합 조정 및 치아 간 치아고정술을 통해 회복한 경우이다 임상적, 방사선학적 평가를 술 후 1 - 3년간 시행하였다. 두 증례 모두 임상적 계측치에서 향상된 결과를 보였고 방사선학적 평가에서 현저한 치조골 및 치조백선의 재생이 관찰되었다. 이번 증례는 발치가 고려되는 심도의 치주질환에 이환된 치아에서 비외과적 치주치료와 치아고정술을 통해 치아를 보존할 수 있는 가능성을 보여준 사례로 보고하는 바이다.

Postoperative malocclusion after maxillofacial fracture management: a retrospective case study

  • Purpose: Various complications occur when a maxillofacial fracture is malunionized or improperly resolved. Malocclusion is the most common complication, followed by facial deformity, temporomandibular joint disorder (TMD), and neurological symptoms. The purpose of this study was to evaluate the dental treatment of postoperative complications after maxillofacial fracture. Materials and methods: In this study, nine patients with a postoperative complication after maxillofacial fracture who had been performed the initial operation from other units and were referred to the authors' department had been included. Of the nine patients, six had mandibular fractures, one had maxillary fractures, one had maxillary and mandibular complex fractures, and one had multiple facial fractures. All the patients had tooth fractures, dislocations, displacements, and alveolar bone fractures at the time of trauma, but complications occurred because none of the patients underwent preoperative and postoperative dental treatment. Malocclusion and TMD are the most common complications, followed by dental problems (pulp necrosis, tooth extrusion, osteomyelitis, etc.) due to improper treatment of teeth and alveolar bone injuries. The patients were referred to the department of dentistry to undergo treatment for the complications. One of the nine patients underwent orthognathic surgery for a severe open bite. Another patient underwent bone reconstruction using an iliac bone graft and vestibuloplasty with extensive bone loss. The other patients, who complained of moderate occlusal abnormalities and TMDs such as mouth-opening limitation, underwent occlusal treatment by prosthodontic repair and temporomandibular joint treatment instead of surgery. Results: One patient who underwent orthognathic surgery had complete loss of open bite and TMD after surgery. One patient who underwent reconstruction using an iliac bone graft had a good healing process. Other patients were treated with splint, injection, and physical therapy for mouth-opening limitation and temporomandibular joint pain. After treatment, the TMDs were resolved, but the remaining occlusal abnormalities were resolved with prosthetic restoration. Conclusions: Considering the severity of malocclusion and TMJ symptom and the feasibillity of reoperation, nonsurgical methods such as orthodontic and prosthodontic treatments and splint therapy can be used to manage the dental and TMD complication after the trauma surgery. However, reoperation needs to be strongly considered for severe malocclusion and TMD problem.
