In esthetic treatments, it is important to consider the arrangement of teeth and the relation between the teeth and soft tissues. A precise diagnosis and establishing an appropriate treatment plan is essential in an esthetic treatment of anterior maxillary teeth. For a fixed prosthesis to meet esthetic expectations, It is crucial to achieve symmetry and adequate proportions of the gingival contour around the crowns. To achieve an esthetic improvement and creating a favorable environment for gingival healing of a physiologic gingival contour, gingivectomy, crown lengthening and forced eruption can be applied to the appropriate site. All ceramics or porcelain laminate veneer can be selected for esthetic improvement of teeth contour and shade. In this case report, malposition of the remaining teeth made it hard to get an esthetic appearance. Gingivectomy, crown lengthening and provisional restoration insertion were performed before the final prosthesis fabrication to reform the gingival form. This case presents satisfying result esthetically and functionally.

Overdenture is one of the methods of patients with severely worn dentition. Remaining natural teeth help preserve alveolar bone and proprioception and provide rigid support. Also, overdenture distributes the concentrated stress applied to the abutment tooth and denture supporting tissues. There is an advantage that it can provide long-term stability to denture. In this case, the patient with only a few teeth due to severe wear and caries was satisfied with good function and favorable esthetics by using overdenture.
Objectives : The purpose of this study was to investigate the relationship between the oral health status of elderly people and their oral health-related quality of life. Methods : The subjects were the elderly people over 65 years old in Busan. A total of 479 elderly people participated in the study from general hospital, two research institutes, eight dental clinics, six welfare institutions, sixteen senior centers, and three nursing homes. The oral health impact profile-14 (OHIP-14) was used in the routine dental checkups of the elderly people. Results : Those who had no oral symptoms had a better oral health-related quality of life. Among the subfactors of the OHIP-14, significant differences were shown in functional limitation(p=0.001), physical pain(p<0.001), emotional discomfort(p<0.001), physical disability(p=0.001), emotional disability (p=0.001), social disability(p=0.005), physical handicap(p=0.003) and total OHIP-14(p<0.001). Those who had 18 sound teeth(natural teeth) or more had a better oral health-related quality of life. Among the subfactors of the OHIP-14, significant differences were shown in functional limitation(p<0.001), physical pain(p=0.007), emotional discomfort(p=0.019), physical disability(p=0.018), Emotional disability(p=.032) and total OHIP-14 (p=0.006). Conclusions : The results revealed a close relationship between oral health status and oral health-related quality of life. The number of sound teeth(natural teeth) and frequency of toothbrushing had a more positive influence. Therefore oral health programs for the elderly people can preserve remaining teeth. Toothbrushing is the best way to improve the quality of life in the elderly people.
In the case of CAD/CAM ceramic inlay restorations, if isthmus width is widened too much, it may cause fracture of remaining tooth structure or loss of bonding at the luting interface because of excessive displacement of buccal or lingual cusps under occlusal loads. So to clarify the criterior of widening isthmus width, this study was designed to test the tensile bond strength and bond failure mode between dentin and ceramic cemented with luting composite resin cements. Cylindrical ceramic blocks(Vita Cerec Mark II, d=4mm) were bonded to buccal dentin of 40 freshly extracted third molars with 4 luting composite resin cements(group1 : Scotchbond Resin Cement/Scotchbond Multi-Purpose, group2 : Duolink Resin Cement/ All-Bond 2, group3: Bistite Resin Cement/Ceramics Primer, and group4:Superbond C&B). Tensile bond test was done under universal testing machine using bonding and measuring alignment blocks(${\phi}ilo$ & Urn, 1992). After immersion of fractured samples into 1 % methylene blue for 24 hours, failure mode was analysed under stereomicroscope and SEM. Results: The tensile bond strength of goup 1, 2 & 4 was $13.97{\pm}2.90$ MPa, $16.49{\pm}3.90$ MPa and $16.l7{\pm}4.32$ MPa, respectively. There was no statistical differences(p>0.05). But, group 3 showed significantly lower bond stregnth($5.98{\pm}1.l7$ MPa, p<0.05). In almost all samples, adhesive fractures between dentin and resin cements were observed. But, in group 1, 2 & 4, as bond strength increased, cohesive fracture within resin cement was observed simultaneously. And, in group 3, as bond strength decreased, cohesive fracture between hybrid layer and composite resin cement was also observed. Cohesive fracture within dentin and porcelain adhesive fracture were not observed. In conclusion, although adhesive cements were used in CAD/CAM -fabricated ceramic inlay restorations, the conservative priciples of cavity preparation must be obligated.
Objectives: Some antioxidants are believed to restore dentin bond strength after dental bleaching. This study was done to evaluate the influence of antioxidants on the bond strength of bleached bovine dentin. Materials and Methods: Thirty incisors were randomly assigned to 10 groups (two unbleached control and eight bleached groups:immediate bonding IB, 4 wk delayed bonding DB, 10% sodium ascorbate treated SA, 10% ${\alpha}$-tocopherol treated TP groups). Teeth in half of groups were subjected to thermal stress, whereas the remaining groups were not. Resin-dentin rods with a cross-sectional area of $2.25mm^2$ were obtained and microtensile bond strength was determined at a crosshead speed of 1 mm/min. Fifteen specimens were prepared for SEM to compare the surface characteristics of each group. The change in dentin bond strength from thermal stress and antioxidant treatment was evaluated using two-way analysis of variance (ANOVA) and Sheffe's post hoc test at a significance level of 95%. Results: The control group exhibited the highest bond strength values, whereas IB group showed the lowest value before and after thermocycling. The DB group recovered its bond strength similar to that of the control group. The SA and TP groups exhibited similar bond strength values with those of the control and DB groups before thermocycling. However, The TP group did not maintain bond strength with thermal stress, whereas the SA group did. Conclusions: Applying a 10% sodium ascorbate solution rather than 10% ${\alpha}$-tocopherol solution for 60 sec is recommended to maintain dentin bond strength when restoring non-vitally bleached teeth.

Purpose: The aim of this randomized clinical trial was to assess whether chemical cleansing using a sulfonic/sulfuric acid gel solution (HBX) as an adjunct to scaling and root planing (SRP) resulted in a decrease in residual plaque and calculus in deep periodontal pockets compared to SRP alone. Methods: Fifty-six patients with 56 hopeless posterior teeth, scheduled for extraction due to severe periodontitis, were enrolled in this study. Each tooth was randomly assigned to 1 of the 2 experimental procedures. The test teeth were subjected to the irrigation of the subgingival area with HBX for 2 minutes, followed by SRP with hand and ultrasonic instruments for 14 minutes, and then extracted. The control teeth received only mechanical instrumentation before extraction. Residual biofilm was evaluated on photographs and measured as total area and percentage of root surface covered by remaining plaque (RP) or calculus (RC) after treatment. Results: The initial pocket depth (PD) and total subgingival root surface area were similar between the 2 treatment groups. After treatment, the total subgingival root area covered by RP and RC was statistically significantly larger (P<0.001) in the control group than in the test group. The test teeth showed a lower percentage of RP, but a higher percentage of RC than the control teeth (both P<0.001). Complete calculus removal was achieved in 42% of the control teeth surfaces and in 25% of the test teeth surfaces for a PD of 4 mm. Conclusions: The additional chemical cleansing with HBX resulted in a statistically significant improvement in bacterial plaque removal during SRP of deep pockets, but it was not effective in reducing calculus deposits.
Purpose: To assess the effects of object position in the field of view (FOV) and application of a metal artifact reduction (MAR) algorithm on the diagnostic accuracy of cone-beam computed tomography (CBCT) for the detection of vertical root fractures(VRFs). Materials and Methods: Sixty human single-canal premolars received root canal treatment. VRFs were induced in 30 endodontically treated teeth. The teeth were then divided into 4 groups, with 2 groups receiving metal posts and the remaining 2 only having an empty post space. The roots from different groups were mounted in a phantom made of cow rib bone, and CBCT scans were obtained for the 4 different groups. Three observers evaluated the images independently. Results: The highest frequency of correct diagnoses of VRFs was obtained with the object positioned centrally in the FOV, using the MAR algorithm. Peripheral positioning of the object without the MAR algorithm yielded the highest sensitivity for the first observer (66.7%). For the second and third observers, a central position improved sensitivity, with or without the MAR algorithm. In the presence of metal posts, central positioning of the object in the FOV significantly increased the diagnostic sensitivity and accuracy compared to peripheral positioning. Conclusion: Diagnostic accuracy was higher with central positioning than with peripheral positioning, irrespective of whether the MAR algorithm was applied. However, the effect of the MAR algorithm was more significant with central positioning than with peripheral positioning of the object in the FOV. The clinical experience and expertise of the observers may serve as a confounder in this respect.
Objectives: To compare the formation of dentinal defects using stainless-steel hand K-files (HFs), rotary files, reciprocating files, and Self-Adjusting File (SAF), when used for oval root canals. Materials and Methods: One hundred and forty extracted human mandibular premolar with single root and oval canal were selected for this study. Oval canals were confirmed by exposing to mesio-distal and bucco-lingual radiographs. Teeth with open apices or anatomic irregularities were excluded. All selected teeth were de-coronated perpendicular to the long axis of the tooth, leaving roots segments approximately of 16 mm in length. Twenty teeth were left unprepared (control), and the remaining 120 teeth were divided into 6 groups (n = 20) and instrumented using HF (size 40/0.02), Revo-S (RS; size 40/0.06), ProTaper NEXT (PTN; size 40/0.06), WaveOne (WO; size 40/0.09), RECIPROC (RC; size 40/0.06), and the SAF (2 mm). Roots were then sectioned 3, 6, and 9 mm from the apex, and observed under stereomicroscope, for presence of dentinal defects. "No defect" was defined as root dentin that presented with no visible microcracks or fractures. "Defect" was defined by microcracks or fractures in the root dentin. Results: The control, HF, and SAF did not exhibit any dentinal defects. In roots instrumented by RS, PTN, WO, and RC files exhibited microcracks (incomplete or complete) in 40%, 30%, 55%, and 50%, respectively. Conclusions: The motor-driven root canal instrumentation with rotary and reciprocating files may create microcracks in radicular dentine, whereas the stainless-steel hand file instrumentation, and the SAF produce minimal or less cracks.
The purpose of this study was to identify the risk of dysphagia among patients that visited prosthodontics department, and evaluate the difference in risks arising from oral conditions and disease in order to preliminarily intervene the various influencing factors of dysphagia. A questionnaire was given to patients that were aged 65 years or older who visited the prosthodontics department between September to December 2017. The data was collected and analyzed using the t-test, $x^2-test$ and logistic regression analysis. The mean age of the patients was 75 years. Out of 300 patients, 206 patients (68.7%) had a risk of dysphagia. There were statistically significant differences between the -non-risk and risk groups, which included the number of natural teeth, total number of teeth including prosthesis, denture use, denture discomfort, number of tooth brushing, oral dryness, digestive system diseases, and musculoskeletal diseases. Among these, oral dryness was a risk factor while the total number of teeth, including natural teeth and prostheses, served as a protective factor. More than half of the elderly patients were at risk of dysphagia. Oral dryness is influenced by many factors and it should continuously be managed. Patients should fully recover their masticatory function by preserving the remaining teeth and compensating for the missing teeth. A dental practitioner should be fully aware of the risk of dysphagia in elderly patients and be able to intervene and offer proper patient health care in advance through treatment guidelines and education.

Preservation of the remaining periodontal ligament cells on an avulsed tooth is very important to the successful outcome of replantation. HBSS is recommended as the most suitable storage medium for the avulsed tooth that cannot be replanted immediately. But their availability near the site of an accident is doubtful. The purpose of this in vitro study was to compare periodontal ligament cells stored in different storage media obtained easily on the spot. Human periodontal ligament cells were collected from the premolar teeth extracted for orthodontic treatment. Cells were cultured in ${\alpha}-MEM$ culture medium containing 20% FBS, at $37^{\circ}C$ 100% humidity, in a 5% $CO_2$ incubator. Cells were cultured in 96 well culture plate, $5{\times}10^4$ cells per well with ${\alpha}-MEM$ and incubated for 24 hours. After discarding the medium, those cells were cultured in ${\alpha}-MEM$ contained with 10% FBS, pasteurized milk, sterilized saline, unstimulated saliva and bench-dried state at $25^{\circ}C$ room temperature for 30, 60, 90, 120, 180 minutes respectively. And then each group was measured using MTT assay. The results were as follows. 1. Between the group of each time, there was statistically significant difference. Periodontal ligament cells viability was highest in pasteurized milk and was reduced stepwisely in sterilized saline, unstimulated saliva and bench-dried state(p<0.05). 2. between the time of each group, there was statistically significant difference(p<0.05) but was no statistically significant difference at 90-120 minutes in pasteurized milk and at 60-90 minutes and 120-180 minutes in sterilized saline(p>0.05). In conclusion, HBSS as storage medium of an avulsed tooth is not practical on the spot. Insteadily pasteurized milk can be recommended to maintain the periodontal ligament cells viability.
