Composite resin have very important roles as a esthetic fillng material. Today we are confusing by a numerous number of the polymer options in the market. Are there any complications such as tooth sensitivity? Do they have stabilization in the long term aspect? These are the reasons why many dentists don't use composite resin in my country. But the problems can be overcome by choosing the best suited materials and meticulous clinical procedures. So that we are able to expand our clinical boundary. I would like to introduce the clinical techniques of Dr.Gordon Christensen in Utah, and overall clinical cases under the base of CRA newsletter.
Purpose : This study sought to evaluate the dimensional stability of the SITT (Silicone Index Tooth Tray) impression system and to determine whether providing space for wash impression material in SITT is a necessary step in obtaining accurate prostheses. Materials and methods : After mounting metal dies with shoulder and chamfer margins arbitrarily, SITT was fabricated using Blu-mousse$^{(R)}$. To test the dimensional stability of the SITT system for margin design, the shoulder margin and chamfer margin were evaluated. Furthermore, to test the effect of space for wash impression material, 0.5mm space in SITT and zero space in SITT were statistically compared. Results : 1. There was no significant difference between the group with shoulder margin and that with chamfer margin. 2. There was no significant difference between the group with 0.5mm space and that with zero space for wash impression material. Conclusions : Considering the limitations of this study, the dimensional stability of the SITT system did not interfere with the margin design. Space for the wash impression material was also unnecessary.
In general, metallic bio-materials is more widely used in solid tissue like bone or tooth than flexible tissue such as skin or muscle. Especially, Cobalt Chrome Molybdenum(Co-Cr-Mo), which is used in tooth surgery, has a great corrosion resistance. Because this bio-material is non-toxic in human body, and has a bio-compatibility that the vital reaction is not occurred with tissue in body. However the chemical reaction is occurred by fatal matter that deteriorate the property of material surface in conventional polishing, and it can affect to fatal disease in human body or decrease the material properties such as hardness, yield strength or bio-compatibility. This surface in poor condition can cause development of corrosion or bacteria. In this study, MR fluid polishing is used to minimize the scratch, pit or surface flaws generated in conventional polishing. Surface roughness is measured according to the polishing condition to obtain fine surface condition.
The present report presents a case of dens invaginatus (DI) in a patient with 4 maxillary incisors. A 24-year-old female complained of swelling of the maxillary left anterior region and discoloration of the maxillary left anterior tooth. The maxillary left lateral incisor (tooth #22) showed pulp necrosis and a chronic apical abscess, and a periapical X-ray demonstrated DI on bilateral maxillary central and lateral incisors. All teeth responded to a vitality test, except tooth #22. The anatomic form of tooth #22 was similar to that of tooth #12, and both teeth had lingual pits. In addition, panoramic and periapical X-rays demonstrated root canal calcification, such as pulp stones, in the maxillary canines, first and second premolars, and the mandibular incisors, canines, and first premolars bilaterally. The patient underwent root canal treatment of tooth #22 and non-vital tooth bleaching. After a temporary filling material was removed, the invaginated mass was removed using ultrasonic tips under an operating microscope. The working length was established, and the root canal was enlarged up to #50 apical size and obturated with gutta-percha and AH 26 sealer using the continuous wave of condensation technique. Finally, non-vital bleaching was performed, and the access cavity was filled with composite resin.

The purpose of this study is to evaluate whether the ashed tooth powder is utilized as an alternative material of the implant. For this purpose the author performed the experimental study to investigate the tissue response of sintered toothash and its histocompatibility. Bony defects to expose the body of marrow, $1{\times}1cm$ in size, were created in the right and left mandibular body of mature dog, and then the ashed tooth powders were filled in right side and the blood clot was filled in the left side as an control. The dogs were sacrificed at 4th, 7th, and 16th week after implantation and histologic examination was performed. The results of this study were obtained as follows : 1. Any inflammatory response was not noted after implanting of the ashed tooth powder during the whole experimental period. 2. At 4th week, ashed tooth powders were surrounded by mature connective tissue. And we could observe hydroxyapatite crystal structure within the ashed tooth powder. 3. At 7th week, we could observe that macrophage phagocyted the small granules of ashed tooth powders. 4. At 16th week, the union with host bone by growth of new trabeculae was observed. And there were remnants of ashed tooth power within some of new trabeculae.
Objectives : The purpose of this study was to evaluate the effects of tooth bleaching agent contained 35% hydrogen peroxide on the color, microhardness and surface roughness of tooth-colored restorative materials. Methods : Four types of tooth-colored restorative materials, including a composite resin(Filtek Z350 ; Z350), a flowable composite resin(Filtek P60 : P60), a compomer(Dyract$^{(R)}$ AP ; DY), and a glass-ionomer cement(KetacTM Molar Easymix ; KM) were used in the study. The specimens($8mm{\times}5mm$) were made by using a customized acrylic mold. Each material was divided into two groups equally(n=40) : experimental group(35% HP) and control group(distilled water). 35% HP group was treated 30 mim/5 days for 15 days. Each 30 minute treatment session consisted of two 15 minute cycles of gel application with 20 second light exposure. The authors measured the color, microhardness, and roughness of the specimens before and after bleaching. The data were analyzed with ANOVA and T-test. Results : 35% HP group showed an apparent color change(${\Delta}E^*$) than control group. In particular, DY and KM showed a noticeable color change and statistically significant differences(p<0.05). 35% HP group showed a reduction in microhardness. Z350 and P60 does not have a statistically significant difference(p>0.05), DY and KM showed a statistically significant difference(p<0.05). Percentage microhardness loss(PML) of control group was 0.6 to 5.5% in the group, 35% HP group was 6.6 to 34.6%. Roughness was increased in 35% HP group after bleaching. Especially DY and KM were significantly increased(p<0.05). Conclusions : Bleaching agents may affect the surface of existing restorations; therefore, they should not be used indiscriminately when tooth-colored restorations are present.
The Purpose of this study was to investigate material differences in stress transmission among various artificial teeth and denture base materials. For this study, a two-dimensional finite element model and a two-dimensional photoelastic model of a mandible with complete denture were made. A resin tooth and a porcelain tooth were used as artificial teeth, and a resin base, a metal lined base, and a soft-liner lined base were used as denture bases. An occlusal load was applied and principal stresses generated in the supporting tissues were compared. To test the impact stress transmission, strain gauge attached to the denture base specimens made of the different materials were made in thick and thin groups. Voltage outputs from hitting the specimen with a steel ball were compared. The results were as follows : 1. In FEM, increasing the mucosal thickness reduced the maximum principal stresses in the supporting tissues, but altering the tooth materials and the base materials induced no difference in the stresses. 2. In photoelastic model study, no difference in fringe order among the specimens were observed, but the thick mucosa group and the soft-liner lined group revealed a more uniform distribution of the load. 3. In strain measuring, the impact force transmission was highest in the soft-liner lined group, and was the lowest in the metal lined group(p<0.01). 4. In the thin group using the resin base, the porcelain tooth showed higher impact stress transmission than the resin tooth(p<0.01), but no difference was observed between them in the thick group. In the soft-liner lined group, the porcelain tooth showed higher impact stress transmission than the resin tooth(p<0.01), but no difference was observed between them in the metal lined group. 5. The thick group showed lower impact stress transmission than the thin group(p<0.01).
Tooth bleaching has been prevailing recently for its ability to recover the color and shape of natural teeth without reduction of tooth material. However, it has been reported that bleaching procedure adversely affects the adhesive bond strength of composite resin to tooth. At the same time the bond strength was reported to be regained by application of some chemical agents. The purpose of this in vitro study was to investigate the effect of the removal of residual peroxide on the composite- enamel adhesion and also evaluated fracture mode between resin and enamel after bleaching. Sixty extracted human anterior and premolars teeth were divided into 5 groups and bleached by combined technique using of office bleaching with 35 % hydrogen peroxide and matrix bleaching with 10% carbamide peroxide for 4 weeks. After bleaching, the labial surfaces of each tooth were treated with catalase, 70% ethyl alcohol, distilled water and filled with composite resin. Shear bond strength was tested and the fractured surfaces were also examined with SEM. Analysis revealed significantly higher bond strength values. (p<0.05) for catalase-treated specimens, but water-treated specimens showed reduction of bond strength, alcohol- treated specimens had medium value between the two groups(p<0.05). The fracture mode was shown that the catalase group and the alcohol group had cohesive failure but the water sprayed group had adhesive failure. It was concluded that the peroxide residues in tooth after bleaching seems to be removed by gradual diffusion and the free radical oxygen from peroxide prevents polymerization by combining catalyst in the resin monomer. Therefore it may be possible to eliminate the adverse effect on the adhesion of composite resin to enamel after bleaching by using water displacement solution or dentin bonding agent including it for effective removal of residual peroxide.

PURPOSE. While dental implants have displayed high success rates, poor mechanical fixation is a common complication, and their biomechanical response to occlusal loading remains poorly understood. This study aimed to develop and validate a computational model of a natural first premolar and a dental implant with matching crown morphology, and quantify their mechanical response to loading at the occlusal surface. MATERIALS AND METHODS. A finite-element model of the stomatognathic system comprising the mandible, first premolar and periodontal ligament (PDL) was developed based on a natural human tooth, and a model of a dental implant of identical occlusal geometry was also created. Occlusal loading was simulated using point forces applied at seven landmarks on each crown. Model predictions were validated using strain gauge measurements acquired during loading of matched physical models of the tooth and implant assemblies. RESULTS. For the natural tooth, the maximum vonMises stress (6.4 MPa) and maximal principal strains at the mandible ($1.8m{\varepsilon}$, $-1.7m{\varepsilon}$) were lower than those observed at the prosthetic tooth (12.5 MPa, $3.2m{\varepsilon}$, and $-4.4m{\varepsilon}$, respectively). As occlusal load was applied more bucally relative to the tooth central axis, stress and strain magnitudes increased. CONCLUSION. Occlusal loading of the natural tooth results in lower stress-strain magnitudes in the underlying alveolar bone than those associated with a dental implant of matched occlusal anatomy. The PDL may function to mitigate axial and bending stress intensities resulting from off-centered occlusal loads. The findings may be useful in dental implant design, restoration material selection, and surgical planning.
