Predictive Factors of Ectopic Eruption of the Maxillary First Permanent Molar (상악 제1대구치 이소 맹출의 예측 인자)

  • In order to provide a diagnostic basis for predicting the possibility of the self-correction of ectopic first permanent molars, differences among normal eruption, reversible and irreversible ectopic eruption of maxillary first permanent molars were retrospectively analyzed. The angles of the long axes and the occlusal lines between the maxillary first permanent molar and the adjacent tooth were measured by panoramic radiographs. The occlusal relationship of second primary molars was also investigated. There is a statistically significant difference between the ectopic eruption group and normal group (p < 0.05), but not between the reversible and irreversible ectopic eruption groups (p > 0.05). The angles between the second primary molar and the first permanent molar, the second primary molar and the second permanent molar in ectopic groups showed a smaller degree than those of the control group. Mesial step was found more frequently in the ectopic eruption group than the normal group. In conclusion, the angulation of the first permanent molar and tooth germ of the maxillary second permanent molar showed close relation with ectopic eruption of the maxillary first permanent molar and ectopic first permanent molar is likely to occur in class III patients with maxillary deficiency.

Successful Conservative Surgical Treatment of Ameloblastic Fibroma in the Posterior Maxilla : A Case Report (상악 구치부에 발생한 법랑모세포섬유종의 성공적인 보존적 수술 : 증례 보고)

  • Ameloblastic fibroma (AF) is a rare odontogenic ectomesenchymal tumor that is frequently seen in the first two decades of life, and occurs in the mandible. The most proper management of AF has been a recent topic of debate because of its recurrence and malignant transformation. This report describes AF in a 4-year-old male, which was a unilocular radiolucency on the maxillary right primary molar area with a scalloped border and corticated margin. The tumor was treated conservatively with enucleation and curettage, and the decision was made to preserve the right primary second molar. A biopsy confirmed it as AF. During the 43 months of follow-up, the patient had no evidence of recurrence or malignant transformation. Moreover, the radiographic examination revealed the generation of tooth germ to be a permanent second premolar. This report shows a case of AF in the posterior maxilla of a 4-year-old boy and discusses the conservative therapeutic approach to this tumor. Therefore, the age of the patients should be an important consideration when choosing conservative or radical surgery in a young AF patient.

Pyogenic Granuloma with Severe Mandibular Bone Destruction (심한 하악골 파괴를 동반한 화농성 육아종)

  • Pyogenic granuloma is a overzealous proliferation of a vascular type connective tissue as a result of some minor trauma and is a well circumscribed elevated, pedunculated or sessile benign inflammatory lesion of skin and mucous membrane. The clinical features of pyogenic granuloma are indicative but not specific and nearly all cases of pyogenic granulomas are superficial in nature, and there is little if any mention in the literature of these lesions producing alveolar bone even jaw bone loss. This case is somewhat unique in that the lesion was an obvious histologic pyogenic granuloma; however, it appeared to invade the mandibular bone which resulted in the loss of the adjacent teeth. A 12-year-old boy came to Seoul National University Dental Hospital with chief complaints of left facial swelling. The features obtained were as follows; Plain radiograms showed a large well-circumscribed radiolucent lesion on left mandibular ramus area, which made severe expansion of lingual cortex and displacement of lower left 3rd molar tooth germ. Computed tomograms showed large soft tissue mass involving left masticator space with destruction of left mandibular ramus. Histologically, sections revealed loose edematous stroma with intense infiltration of inflammatory cells and proliferation of vascular channels. Also, there were focal areas of extensive capillary proliferation, bone destruction and peripheral new bone formation.

A Histopathological Study of Pulp Tissue Reactions to Glutaraldehyde and Formocresol in Puppy's Primary Teeth (Glutaraldehyde 및 Formocresol이 유견유치 치수조직에 미치는 영향에 관한 병리조직학적 연구)

  • This study was undertaken to evaluate the pulpal responses to the pulp-capping materials such as glutaraldehyde and formocresol in pulpotomy technique, especially in the primary dentition. Mandibular primary canines and molars of 5 dogs (aged about 8-9 weeks)were selected for this study. The intervals of observation for histologic study of pulpotomized primary teeth with 2% glutaraldehyde, formocresol and calcium hydroxide in the usual manner ranged from 2 hours, 1 week, 2 weeks, 3 weeks and 5 weeks after experiments respectively. Each specimens were fixed with 10% formalin and decalcified in 5% nitric acid. All slides were stained with Hematorylin-Eosin and examined histopathologically. The results were as follows; 1. In calcium hydroxide groups, formation of dentin bridge was initiated in 1 week after experiments and completed in 5 weeks after experiments. 2. Formation of dentin bridge was not seen, whereas necrosis of pulp tissue was noted, in formocresol and glutaraldehyde groups. 3. Duration of tissue reactions and tissue changes were similar, in formocresol and glutaraldehyde groups. 4. In formocresol and glutaraldehyde groups, amputation surfaces of the pulp were covered with blood clots, beneath which coagulation necrois was noted, but inflammatory cells were not prominent, in 2 hours and 1 week after experiments. But coagulation necrosis was proceeded to the apical portion, accompanied by infiltration of inflammatory cells, since 2 weeks after experiments. And suppuration or gangrene of the pulp tissue were noted in 3 weeks and 5 weeks groups. 5. Suppuration or gangrene of pulp seemed to provoke the resorption of dentin wall, and inflammatory changes and resorption of roots were noted in the periodontal membrane near the periapical region. 6. As compared with calcium hydroxide groups, resorption of the root was pronounced in form or cresol and glutaraldehyde groups. Effects of medicaments to the succedaneous tooth germ were not seen.

Eruption Guidance of Distally Displaced Mandibular Second Premolar by the Hemisection of Primary Second Molar: Two Case Reports (선행 유치의 편측치아절제술에 의한 원심으로 변위된 하악 제2소구치의 맹출 유도)

  • Distal displacement of the tooth germ of the mandibular second premolar (MnP2) leads to its impaction and obturation of the eruption path of the mandibular first molar delaying its eruption. The present case report describes the treatment of 2 cases of eruption guidance for distally displaced developing MnP2 that caused delayed eruption of the mandibular first molar. Intentional extraction of primary predecessor results in the mesial shift of the displaced MnP2. However, unfavorable distal ectopic eruption of the mandibular first premolar after the premature loss of primary second molar has been previously reported. Hemisection and sequential extraction of the mandibular primary second molar were performed to mesially shift the distally displaced MnP2, while preventing unfavorable distal ectopic eruption of the mandibular first premolar.

ROOT RESORPTION OF PRIMARY TEETH WITHOUT PERMANENT SUCCESSORS (계승영구치가 선천적 결손된 유치의 치근 흡수)

  • Root resorption of primary teeth usually occurs as the succeeding permanent teeth erupt, which induces differentiation of the hemopoietic cells into osteoclasts. Their root resorption pattern reflects the eruption path of the succeeding permanent teeth, and eventually the primary teeth shed as their succeeding permanent teeth erupt. Even when a permanent tooth germ is congenitally missing, root resorption of the corresponding primary tooth may still occur due to various factors, such as inflammation, traumatic occlusal force, and weakness of periodontium etc. Such congenital missing of permanent teeth is a commonly observed phenomenon in human be ing, and it often accompanies delayed retention of primary teeth. The etiologic factors for congenital missing in elude not only systemic diseases, but also local factors and human evolution process. In the radiographs of the cases in this report, the primary teeth without succeeding permanent teeth show pathologic root resorption. Root resorption progressed about 1/2~3/4 of the roots, and the surfaces of the resorption area were irregular. Considering high susceptibility of the periodontal ligament of primary teeth to root resorption, pathologic root resorption of primary teeth with delayed retention can be explained by the increased masticatory muscle force and abnormal occlusion developed during the mixed dentition. When the primary teeth without succeeding permanent teeth are lost, decision for space maintenance is required and long-term treatment plan for further prosthetic or orthodontic treatment should be establsihed.

  • Marsupialization and decompression constitute a well-established procedure for treating cavitary bone lesions of the jaw. The technique can be a primary treatment option, especially for pediatric patients with large cysts or lesions involving vital anatomical structures, such as a developing tooth germ. In this procedure, a decompression stent, such as a customized acrylic obturator or space-maintaining appliance, silicone tube, or nasal cannula, is inserted to maintain the patency of the cyst. However, this may cause clinical problems, such as irritation or trauma to the adjacent tissues, as well as discomfort to the patient, or failure of the stent due to cyst shrinkage. It can also be a reason for patient noncompliance. In the cases described here, a minimally invasive marsupialization technique using a metal tube made from a 16-gauge needle was used for odontogenic cysts in pediatric patients associated with unerupted teeth. Through this method, the lesions were removed, with patient cooperation, and the cyst-associated teeth erupted spontaneously.

PERIPHERAL ODONTOMA IN A CHILD: CASE REPORT (소아에서 발생한 변연성 치아종의 증례보고)

  • The odontoma is a hamartoma of odontogenic origin and consists of enamel, dentin, cementum and pulp tissue. Two types of odonotma are histologically recognized: complex and compound. Odontoma may be the cause of noneruptoin or impaction of teeth, formation of cyst and resorption of adjacent bone. The recommended treatment for an odontoma is conservative surgical excision. No propensity for recurrence has been noted. Peripheral odontoma is relatively rare lesion and shows the histological characteristics of an intraosseous odontoma. This report described a 3-year-old Korean girl with a firm gingival mass in the posterior mandibular area, which had been gradually enlarging over 18months. Radiographic examination showed a radiopaque mass but no evidence of underlying intraosseous lesion. Excisional biopsy was performed for the mass removal and histopathological examination. The result of histopathological study was identified as hamartous supernumerary tooth germ. Based on the clinical presentation, radiographic and histopathologic findings, we concluded that the final diagnosis was peripheral odontoma. For three years after surgery, there is no clinical sign of recurrence.

IMMOBILIZATION OF LOWER MANDIBULAR ALVEOLAR BONE FRACTURE USING RESIN-WIRE OPEN CAP SPLINT (하악 유전치부 치조골 골절시 resin-wire open cap splint를 이용한 고정)

  • Traumatic injuries to the primary dentition are commonly encountered problems in dental practice. It is found that 30% of the children had injuries to the primary dentition and 22% to the permanent dentition. The greatest incidence of trauma to the primary dentition occurs at the ages of 2 and 3 as children start to learn motor coordination. Because teeth and alveolar bone are traumatized simultaneously, alveolar bone fractures are likely to occur when multiple teeth are involved in injuries. Dental splints are indicated for the management of maxillofacial fractures. They enable anatomic reduction of fractured segments and help immobilization and maintenance of the fragments after reduction. They also act as a stabilizer during rehabilitation. Various types of dental splints are available. In this case, routine resin-wire splint technique could not be applied because of the child's uncooperative behavior. Oral sedation was not indicated because N.P.O. had not been preceded. Therefore, we decided to use open-cap acrylic splint instead. Stabilization using open cap acrylic splint requires minimum chair time with reduced discomfort to both patient and dentist. It is an effective means of splint for uncooperative children and especially useful when other means of fixation have been failed. Because trauma on the primary dentition can affect the underlying permanent tooth germ, it is important to monitor eruption process of the permanent dentition.
