상악 우측 유전치부에 발생한 삼중치 : 증례 보고 (Triplication of upper right primary anterior teeth : a case report)

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

발생중인 생쥐 치아 및 치주조직에서 치주인대-특이 단백질; PDLs22의 발현 (Expression of PDL-specific protein;PDLs22 on the developing mouse tooth and periodontium)

  • The periodontal ligament(PDL) is a unique tissue that is crucial for tooth function. However, little is known of the molecular mechanisms controlling PDL function. PDL-specific protein;PDLs22 had been previously identified as a novel protein isolated from cultured human PDL fibroblasts using subtraction hybridization between human gingival fibroblasts and PDL fibroblasts. The aim of this study was to examine the expression pattern and tissue localization of PDLs22 protein in embryonic and various postnatal stages of developing mouse using immunohistochemical staining. Embryos (E18) and postnatal (P1, P4, P5, P15, P18) were decapitated and the heads were fixed overnight in a freshly prepared solution of 4% paraformaldehyde. Some specimens were decalcified for $2{\sim}4$ weeks in a solution containing 10% of the disodium salt of ethylenediamine-tetraacetic acid (EDTA). Next, tissues were dehydrated, embedded in paraffin and sectioned serially at $6{\mu}m$ in thickness. Polyclonal antiserum raised against PDLs22 peptides, ISNKYLVKRQSRD, were made. The localization of PDLs22 in tissues was detected by polyclonal antibody against PDLs22 by means of immunohistochemical staining. The results were as follows; 1. Expression of PDLs22 protein was not detected in the tooth germ of bud and cap stage. 2. At the late bell stage and root formation stage, strong expression of PDLs22 protein was observed in developing tooth follicle, osteoblast-like cells, and subodontoblastic cells in the tooth pulp, but not in gingival fibroblasts, ameloblasts and odontoblasts of tooth germ 3. In erupted tooth, PDLs22 protein was intensely expressed in PDL and osteoblast-like cells of alveolar bone, but not in gingival fibroblasts, mature osteocytes and adjacent salivary glands. 4. In the developing alveolar bone and mid-palatal suture, expression of PDLs22 protein was seen in undifferentiated mesenchymal cells and osteoblast-like cells of developing mid-palatal suture, but not in mature osteocytes and chondrocytes. These results suggest that PDLs22 protein may play an important role in the differentiation of undifferentiated mesenchymal cells in the bone marrow and PDL cells, which can differentiate into multiple cell types including osteoblasts, cementoblasts, and PDL fibroblasts. However, more researches should be performed to gain a better understanding of the exact function of PDLs22 protein which related to the PDL cell differentiation.

방사선조사와 저칼슘식이가 백서구치의 상아질과 백악질형성에 미치는 영향에 관한 실험적 연구 (THE EFFECTS OF IRRADIATION AND CALCIUM-DEFICIENT DIET ON DENTIN AND CEMENTUM FORMATION OF RAT MOLAR)

  • The present study was designed to elucidate the effects of the Co-60 γ irradiation and/or calcium­deficient diet on the dentin and cementum formation of rat molar. The pregnant three-week old Sprague­Dawley rats were used for the study. The experimental group was divided into two groups, irradiation/normal diet group and irradiation/calcium-deficient diet group. The control group was non­irradiation/normal diet group. The abdomen of the rats at the 19th day of pregnancy were irradiated with single absorbed dose of 350cGy. The rat pups were sacrificed on the 14th day after delivery and the maxillae including molar tooth germ were taken. The specimens including the 1st molar tooth germ were prepared to make tissue sections for light and transmission electron microscopy. Some of tissue sections for light microscopy were stained immunohistochemically with anti-fibronectin antibody. The results were as follows; 1. The Hertwig's epithelial root sheath cells, which are related to the differentiation of the tooth-forming cells, showed irregular cellular arrangement, decrease of intercellular junctional complex, and decreased immunoreactivity to the fibronectin after irradiation. These were more severe in the irradiation/calcium-deficient diet group. 2. The cementoblasts at the cementum-forming area showed chromatin clumpings after irradiation. The immu noreactivity to the fibronectin was weaken after irradiation, especially irradiation/calcium-deficient diet group. 3. The odontoblasts at the dentin-forming area showed increase of lysosomes in the cytoplasm and destruction of intercellular junctional complex. The irradiation/calcium-deficient diet group showed decrease of number and density of the electron dense particles and a large number of vacuoles scattered in the dentin matrix. The immunoreactivity was weaken.

  • Tooth impaction represents the stop of eruption by clinical and radiographical disturbance in eruption path or the dislocation of tooth germ. The most common factor in tooth eruption disorders are spacial deficiency with other causes reported to be odontogenic tumors, periodontal ligament injury, etc. Impaction of the mandibular second molar is relatively rare and reported in about 3 out of 1,000 people. Because the second molars tend to erupt in a mesial direction, this situation can lead to serious problems if untreated, including dental caries, periodontal disease and root resorption of the first molar. Treatment of this problem includes, surgical repositioning and orthodontic forced eruption. Because each procedure have the definite advantages and disadvantages, and influenced by circumferential environment, these have limits for successfu1 recovery as independent treatment. In a case at St. Mary's Hospital, we performed successful correction of a horizontal impacted mandibular second molar using a miniplate skeletal anchorage system. We introduce this treatment as a valid method for an impacted second molar and consider a oromaxillofacial surgeon's role in tooth movement treatment.

상악 유전치의 치근단 병소로 인한 만곡 영구전치 (DILACERATION CAUSED BY PERIAPICAL LESIONS OF THE DECIDUOUS INCISOR : CASE REPORT)

  • 치아만곡은 조직-형태분화기에 나타나는 치아형태의 이상으로 석회화된 부위와 미석회화된 부위간의 장애로 인해서 치관 및 치근 모두에서 발생하는 기형이며, 상악 절치에서 호발하며 매복 가능성이 높다고 보고되었다. 만곡의 원인으로는 유치의 외상 치배의 이소성 발육, 낭종이 있으며 그외에 otodental syndrome, Hurler syndrome, 쇄골두개이골증에서 보고되었다. 본 증례는 치아우식증으로 인해서 발생된 치근단병소가 치아만곡을 유발할 수 있는 원인이 될 수 있다고 생각하였기에 이에 보고하는 바이다.

유치열에 나타난 삼중치에서 치수 치료 : 증례 보고 (Pulp Treatment of Triple Tooth in Primary Dentition: Two Case Reports)

  • 유치열에서 드물게 관찰되는 삼중치는 세 치아의 비정상적인 융합으로 나타난다. 몇몇 문헌들에서 이중치에 대해 보고하고 있지만, 삼중치에 대한 보고는 매우 드물다. 이러한 다중치는 유치열에서 더 흔하게 관찰되며, 몇 가지 임상적 문제점을 나타낸다. 심미적인 문제 뿐만 아니라 치아 우식증 및 파절에 취약함, 영구 치배의 결손, 교정적, 치주적 문제를 야기할 수 있으며, 복잡한 치아 구조로 인해 치수 치료를 어렵게 한다. 첫 번째 증례는 만 1세 7개월 남아의 상악 유전치부에 나타난 삼중치로, 깊은 우식으로 치수 치료가 필요할 것으로 예상되었고, Mineral Trioxide Aggregate (MTA)를 이용하여 부분 치수 절단술을 시행하였다. 두 번째 증례는 만 1세 9개월 남아의 하악 유전치부에 나타난 삼중치로 파절을 주소로 내원하였다. 파절편 조각을 제거한 뒤, 잔존 치아에서 치수 절제술을 시행하였다. 각각 24개월, 6개월간의 정기 검진시까지 특별한 합병증은 나타나지 않았으며, 수복물 또한 비교적 양호하게 유지되었다.

Osteocalcin Expression and Mineralization in Developing Tooth of Xenopus laevis

  • Osteocalcin (OC) is the most abundant noncollagenous protein of extracellular matrix in the bone. In an OC deficient mouse, bone formation rates are increased in cancellous and cortical bones. OC is known as a negative regulator of mineral apposition. OC is also expressed in the tooth of the rat, bovine, and human. However, little is known about OC during tooth development in Xenopus. The purpose of this study is to compare the expression of OC with mineralization in the developing tooth of Xenopus, by using von Kossa staining and in situ hybridization. At stage 56, the developmental stage of tooth germ corresponds to the cap stage, and an acellular zone was apparent between the dental papilla and the enamel organ. From stage 57, calcium deposition was revealed by von Kossa staining prior to OC expression, and the differentiated odontoblasts forming predentin were located at adjoining predentin. At stage 58, OC transcripts were detected in the differentiated odontoblasts. At stage 66, OC mRNA was expressed in the odontoblasts, which was aligned in a single layer at the periphery of the pulp. These findings suggest that OC may play a role in mineralization and odontogenesis of tooth development in Xenopus.

  • This is a case of the patient, a 15 -year-old girl, presented a marked necrotic osteomyelitis due to misusage of the arsenic compound for devitaliaztion of the lower right 2nd molar in the right posterior part of the mandible. She was obtained good satisfactory results by sequestrectomy of necrotic bone and by removing of lower right 2nd molar and unerupted 3rd molar tooth germ.

Apoptosis during Rat Tooth Development

  • Teeth develop via a reciprocal induction between the ectomesenchyme originating from the neural crest and the ectodermal epithelium. During complete formation of the tooth morphology and structure, many cells proliferate, differentiate, and can be replaced with other structures. Apoptosis is a type of genetically-controlled cell death and a biological process arising at the cellular level during development. To determine if apoptosis is an effective mechanism for eliminating cells during tooth development, this process was examined in the rat mandible including the developing molar teeth using the transferase-mediated dUTP-biotin nick labeling (TUNEL) method. The tooth germ of the mandibular first molar in the postnatal rat showed a variety of morphological appearances from the bell stage to the crown stage. Strong TUNEL-positive reactivity was observed in the ameloblasts and cells of the stellate reticulum. Odontoblasts near the prospective cusp area also showed a TUNEL positive reaction and several cells in the dental papilla, which are the forming pulp, were also stained intensively in this assay. Our results thus show that apoptosis may take place not only in epithelial-derived dental organs but also in the mesenchyme-derived dental papilla. Hence, apoptosis may be an essential biological process in tooth development.
