Occlusion of the Internal Carotid Artery due to Intracranial Fungal Infection

  • In recent years the immunocompromised population has increased rapidly to include people with acquired immune deficiency syndrome (AIDS), drug abusers, and transplant patients. Accordingly, the incidence of intracranial fungal infection has increased. Our institution experienced 2 cases of internal carotid artery (ICA) occlusion due to invasion of the cavernous sinus by an intracranial fungal infection. The first case was a 60-year-old man who presented with headache, eye pain, conjunctival injection, right-sided diplopia, and blurred vision. Infected tissues within the frontal and ethmoid sinuses were removed via bifrontal craniotomy and endoscopic sinus surgery through the Caldwell Luc approach. The second case was a 63-year-old woman who developed right-sided facial pain after a tooth extraction. The infection was not controlled despite continuous use of antifungal agents, resulting in death from sepsis. We believe that when intracranial fungal infection is suspected in a patient with orbital symptoms and a focal neurologic deficit, immediate angiographic investigation of possible ICA occlusion is warranted. Aggressive treatment with antifungal agents is the only way to improve prognosis.

  • The age structure has been experiencing substantial change due to the decreased birth rate as well as the increased life expectancy. Gorge Magnus, an English economist, casts warnings of population ageing which has the potential of huge socioeconomic impact human society has never experienced before. The prediction that proportion of elderly people in need of oral health care will increase substantially is a new challenge to dentists in the future. The old paradigm that the aged person is just the person who was born earlier and needs the same conventional oral health care should be shifted to the new one. Elderly people tend to express their political interest related with health care system by actively participating in the national elections. The need to sustain economic status for the extended life span makes them seek eagerly esthetic health care to maintain sound social function. Most of them are under multiple chronic diseases and take related medicines. In addition, many studies report about mental change as well as physical change among the aged people. Since the prevalence of dental diseases among the aged is higher than other chronic devastating diseases, the aged seeking oral health care will increase. The aged who has different physical and psychological status as well as chronic disease and related medicine will show unexpected response to the conventional oral health care. In addition, the impact of tooth loss is substantial physically, mentally and emotionally. Dentist should prepare different approaches for the elderly dental patient.

Severe periodontitis with tooth loss as a modifiable risk factor for the development of Alzheimer, vascular, and mixed dementia: National Health Insurance Service-National Health Screening Retrospective Cohort 2002-2015

  • Purpose: The purpose of this study was to evaluate severe periodontitis with tooth loss as a modifiable risk factor for Alzheimer dementia (AD), vascular dementia (VaD), and mixed dementia (MD) using the National Health Insurance Service-National Health Screening Retrospective Cohort database with long-term follow-up over 14 years. Methods: Multivariate Cox hazards regression analysis was applied to a longitudinal retrospective database, which was updated in 2018, to evaluate the association between severe periodontitis with few remaining teeth and dementia after adjusting for potential risk factors, including sociodemographic factors and comorbid diseases. Results: Among 514,866 individuals in South Korea, 237,940 (46.2%) participants satisfying the inclusion criteria were selected. A total of 10,115 age- and sex-matched participants with severe periodontitis and 10,115 periodontally healthy participants were randomly selected and evenly assigned. The results showed that the risks of AD (hazard ratio [HR], 1.08), VaD (HR, 1.24), and MD (HR, 1.16) were significantly higher in patients with severe periodontitis with 1-9 remaining teeth after adjustment for sociodemographic factors, anthropomorphic measurements, lifestyle factors, and comorbidities. Conclusions: Severe periodontitis with few remaining teeth (1-9) may be considered a modifiable risk factor for the development of AD, VaD, and MD in Korean adults.

성인의 치주질환에 관한 지식, 인식과 신념에 미치는 영향 (Influential Factors for the Knowledge and Awareness of Adults on Periodontal Diseases and Their Belief)

  • 본 연구는 학교사회구성원들의 치주질환인식도, 지식, 신념 및 태도와 치주건강을 분석하여 구강보건교육과 계속적인 관리를 위한 방법을 연구하는 기초 자료를 마련하고자 구강보건실을 내방하는 환자들을 대상으로 치주질환에 대한 생각과 치주건강도를 조사하여 관련 영향요인을 파악하였다. 치주질환의 기술 역학적 특성의 인지도는 2.77점, 초기의 치은염 증상에 대한 지식 정도에 관한 문항의 평균은 2.97점, 치주질환 발생요인에 대한 인지도에 관한 문항의 평균은 2.90점, 치주질환의 예방 및 치료에 대한 인지도에 관한 문항의 평균은 2.95점, 치주질환에 대한 신념 및 태도에 관한 문항의 평균은 3.04로 높고 올바른 인지도, 지식도, 신념 및 태도를 가진 것으로 나타났다. 치주질환 관련 문항 중 비교적 낮은 점수의 문항은 노화에 의한 치아상실에 대한 인식과 음식물과 치주질환 예방과 발생과의 관계 인지, 칫솔질 시간과 잇몸건강과의 관계, 올바른 칫솔질 방법과 관리법 등이었다. 초기의 치은염 증상에 대한 지식 정도에 영향을 미치는 요인은 연령과 음주횟수, 치주낭 깊이, 성별이었으며, 치주질환 발생요인에 대한 인지도에 영향을 미치는 요인은 성별, 나이, 3분 칫솔질이, 치주질환의 예방 및 치료에 대한 인지도에 영향을 미치는 요인은 음주횟수가 유의한 영향관계에 있는 것으로 나타나 건강을 위한 생활행태 개선이 필요한 것으로 파악되었다.

구설순치(口舌脣齒)질환에 대한 한국 침구서적의 치료법 비교 연구 (A Literature Study on the Korean Acupuncture for Oral, Glottal, Labial and Dental diseases)

  • Background : The varieties of manifestations referred to oral, glottal, labial and dental diseases has been long complicated physicians in the efficient diagnosis and treatment of these diseases. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of them Objectives : Establish a distinctive and efficient acupuncture method for the treatment of oral, glottal, labial and dental diseases based in literature research Method : We reviewed four Korean medical literature, "Guide to Swollen Sore Treatment", "Treasured Mirror of Eastern Medicine", "Experiential Prescriptions of Acupuncture and Moxibustion", and "Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", and analyzed the therapeutic characteristics in the treatment of oral, glottal, labial and dental diseases Result : 1. According to "Guide to Swollen Sore Treatment", we could noted frequent application of blood letting methods in the affected area, then rinse ones mouth with salty water and further application of taro plaster. Also found acupuncture methods utilizing heated mole cricket or silkworm in the sublingual region. Regarding herbal method, Realgar was rubbed in the affected area. Mainly used acupuncture points in the treatment of oral, glottal, labial and dental diseases were GV20, GB20, LU5, and auricular anterior hairline 2. In "Treasured Mirror of Eastern Medicine", blood letting method in the sublingual region and burning needle searing method were the most frequently applied in the treatment of oral, glottal, labial and dental diseases. Moxibustion was also applied in the treatment of labial and dental diseases. Particularly, said to apply 3 moxa cones in ear zones of both sides, when no medicine is effective. This demonstrates how emphasized the importance of moxibustion in this kind of disease. Mainly used acupuncture points were GV16, CV24, LI4, EX-HN12, and EX-HN13 3. In "Experiential Prescriptions of Acupuncture and Moxibustion", no other methods beside wrist. Superstitious methods like applying moxibustion on the tooth picture drawn on the roofing tile need further confirmation. Mainly used acupuncture points were LI4, ST36, and HT7. 4. "Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", identified the cause of diseases according to visceral pattern identification. Stomach and Spleen in charge of vocal, lingual, and labial disease, Kidney for dental disease, and further scrutinizing identification according to Liver, Heart, Stomach, Lung, and Kidney manifestations. Used supplementation and draining needling methods of self meridians and other correlating meridians. Conclusions : After previous study on stroke and eye diseases, we could also find various efficient methods according to oral, glottal, labial and dental diseases, through literature research of korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

Wnt/β-Catenin 신호조절에 의한 백악질 형성의 이해 (Understanding of Cementum Formation by the Wnt/β-Catenin Signaling)

  • Periodontal disease is one of the major dental diseases. Currently, various methods are used for healing and successful regeneration of periodontal tissue damaged by periodontal disease. The periodontal ligament and alveolar bone have received considerable interest for use in periodontal tissue regeneration and induction. However, as the functions of the factors required for tooth attachment and key regulatory factors for periodontal tissue regeneration in the cementum have recently been identified, interest in cementum formation and regeneration has increased. Dental cementum forms in the late phase of tooth development because of the reciprocal regulatory interaction between cervical loop epithelial cells and surrounding mesenchymal cells, which is regulated by various gene signaling networks. Many attempts have been made to understand the regulatory factors and cellular and molecular mechanisms associated with new cementum formation. In this paper, we reviewed the study outcomes to date on the regulatory factors that induce cementum formation and regeneration, focusing on understanding the roles and functions of Wnt signaling in the regulation of cementum formation. In addition, we aimed to obtain information on the useful reciprocal regulatory factors that mediate cementum formation and regeneration through a series of molecular mechanisms.

류마티스 관절염 진단자의 치석제거 경험과 관련요인 (Scaling experience and related factors in people diagnosed with rheumatoid arthritis)

  • Objectives: The purpose of this study was to examine the scaling experience and related factors of people diagnosed with rheumatoid arthritis. Methods: The subjects were 30,680 people diagnosed with rheumatoid arthritis from the 2014 community health survey who were $${\geq_-}19$$ years old. Data were analyzed by chi-square test and multiple logistic regression analysis. Results: Those having regular scaling included well educated people, drinkers, more than three times tooth brushing practice, and regular dental checkup. The respondents were in the higher quartile of income And the respondents who were in the higher quartile of income were less likely to have scaling experience (p<0.05). Conclusions: The scaling experience was closely related with the age, educational level, residential area, income, drinking, tooth brushing practice, and regular dental checkup. It is necessary to investigate the influencing factors of rheumatoid arthritis and periodontal diseases in the further studies.

Flavonoids as a Possible Preventive of Dental Plaque

  • To test flavonoids for antibacterial activity against oral micraorganisms, flavonoids, quercetrin and naringenin, were incorporated into two pharmaceutical preparations in the form of tooth paste. Samplees of dental plaque, the msot accused dental deposit which initiates the gingival and periodental diseases, were collected from the teeth surface of ten dental students at one week interval before and after using placebo, followed by two formulae of tooth paste containing 0.1% of quercetrin and naringenin (formulas I and II, respectively). The amount of dental plaque was assessed by the quigley and Hens index. Then plaque samples were subjected to bacteriological examination of Gram stain and plate counts of microorganisms. The results revealed that most of Gram negative cocci and bacilli were highly affected by the two formulae : the number of actinomycetes were decreased after using formula I and disappeared completely by the sue of formula II, while the number of Gram positive streptococci was highly decreased after the treatment with the two formulae. These results indicate a possible use of flavonoids to inhibit dental plaque formation.

교정 치료 중인 환자의 치주수술 (Mucogingival surgery for patients under orthodontic treatment)

  • Gingival recession is one of the common mucogingival problems during the orthodontic treatment. The causes of the gingival recession are similar to gingival recession in patients with periodontal diseases. Accumulation of bacterial deposits around the natural teeth induces the gingival inflammation and gingival recession occurs in the teeth with the lack of the supporting bone. However, malpositioned teeth which are labially positioned teeth or rotated teeth are more risky for gingival recession. Once root is exposed to oral cavity due to gingival recession, the orthodontic tooth movement is compromised and esthetic problems appeared. In addition, excessive gingival recession over the mucogingival junction jeopardizes the oral hygiene control, which has a risk of further gingival recession and bone loss around the tooth. To cover exposed root or to prevent further gingival recession, mucogingival surgery with gingival graft is recommended for the patients under orthodontic treatment. This case report aimed to present the mucogingival treatments of gingival recession observed during orthodontic treatment. Case I had had initial slight gingival recession before the orthodontic treatment. However, during the retraction phases, the gingival recession progressed and the periodontal treatment was referred. In case II, miller Class III gingival recession was occurred after correction of rotation. Both cases were treated by coronally advanced flap with free gingival grafts and recovered to the level of adjacent teeth despite of complete root coverage was not achieved in Case II. After periodontal treatment, orthodontic treatment was successfully completed. In conclusion, mucogingival surgery during the orthodontic treatment is recommended for the successful orthodontic treatment as well as periodontal health.

Recognition and management of palatogingival groove for tooth survival: a literature review

  • Palatogingival groove (PGG) is an anomaly in the maxillary anterior teeth, often accompanied by the area of bony destruction adjacent to the teeth with no carious or traumatic history. The hidden trap in the tooth can harbor plaque and bacteria, resulting in periodontal destruction with or without pulpal pathologic change. Related diseases can involve periodontal destruction, combined endodontic-periodontal lesions, or separate endodontic and periodontal lesions. Disease severity and prognosis related to PGG depend on several factors, including location, range, depth, and type of the groove. Several materials have been used and recommended for cases of extensive periodontal destruction from PGG to remove and block the inflammatory source and recover the health of surrounding periodontal tissues. Even in cases of severe periodontal destruction, several studies have reported favorable treatment outcomes with proper management. With new options in diagnosis and treatment, clinicians need a detailed understanding of the characteristics, treatment, and prognosis of PGG to successfully manage the condition.
