만성 소화불량증 환자에서 설 색상과 심박변이도의 경향성 파악 (Trends in Tongue Color and Heart Rate Variability in Chronic Dyspepsia Patients)

  • Objectives From observing the tongue of a patient, one can assess the health status; this method has been frequently used in traditional Korean Medicine (KM) clinics. In particular, KM posits that the color of the tongue is highly related to digestive functions. In this study, the color of tongue and heart rate variability (HRV) were compared between chronic dyspepsia (CD) patients and healthy subjects. Methods Healthy subjects and CD patients with functional dyspepsia (FD), gastroesophageal reflux disease (GERD), or chronic gastritis (CG) were enrolled for the study. Profile view images of the tongue were acquired by using a computerized tongue image acquisition system (CTIS). The color of the tongue body was extracted from the non-coated region on the tongue images. Results Color differences in CIE L*a*b* color space between the three sub-types of CD patients and healthy subjects were analyzed by using multiple linear regression analysis with age and sex as the factors. The variable b* was significantly lower in GERD patients than in the controls (p=0.017). Variable a* was significantly lower in CG than in the controls (p=0.03). No significant difference was seen between FD and controls. In GERD, the tongue body seems to be intense red in color; in CG, pale red. Frequency domain analysis showed that HF was significantly lower in GERD patients than in the controls (p=0.041). Conclusions The color of the tongue body and HF of HRV can be used for diagnosing digestive functions in health care.

Implementation of Intelligent Electronic Acupuncture Needles Based on Bluetooth

  • In this paper, we present electronic acupuncture needles we have developed using intelligence technology based on Bluetooth in order to allow anyone to simply receive customized remote diagnosis and treatment by clicking on the menu of the smartphone regardless of time and place. In order to determine the health condition and disease of patients, we have developed a software and a hardware of electronic acupuncture needles, operating on Bluetooth which transmits biometric data to oriental medical doctors using the functions of automatically determining pulse diagnosis, tongue diagnosis, and oxygen saturation; the functions are most commonly used in herbal treatment. In addition, using fuzzy logic and reasoning based on smartphones, we present in this paper an algorithm and the results of completion of hardware implementation for electronic acupuncture needles, appropriate for the body conditions of patients; the algorithm and the hardware implementation are for a treatment time duration by electronic acupuncture needles, an automatic determinations of pulse diagnosis, tongue diagnosis, and oxygen saturation, a function implementation for automatic display of acupuncture point, and a strength adjustment of electronic acupuncture needles. As a result of our simulation, we have shown that the treatment of patients, performed using an Electronic Acupuncture Needles based on intelligence, is more efficient compared to the treatment that was performed before.

Strain elastography of tongue carcinoma using intraoral ultrasonography: A preliminary study to characterize normal tissues and lesions

  • Purpose: The aim of this study was to evaluate the quantitative strain elastography of tongue carcinoma using intraoral ultrasonography. Materials and Methods: Two patients with squamous cell carcinoma (SCC) who underwent quantitative strain elastography for the diagnosis of tongue lesions using intraoral ultrasonography were included in this prospective study. Strain elastography was performed using a linear 14 MHz transducer (Aplio 300; Canon Medical Systems, Otawara, Japan). Manual light compression and decompression of the tongue by the transducer was performed to achieve optimal and consistent color coding. The variation in tissue strain over time caused by the compression exerted using the probe was displayed as a strain graph. The integrated strain elastography software allowed the operator to place circular regions of interest (ROIs) of various diameters within the elastography window, and automatically displayed quantitative strain (%) for each ROI. Quantitative indices of the strain (%) were measured for normal tissues and lesions in the tongue. Results: The average strain of normal tissue and tongue SCC in a 50-year-old man was 1.468% and 0.000%, respectively. The average strain of normal tissue and tongue SCC in a 59-year-old man was 1.007% and 0.000%, respectively. Conclusion: We investigated the quantitative strain elastography of tongue carcinoma using intraoral ultrasonography. Strain elastography using intraoral ultrasonography is a promising technique for characterizing and differentiating normal tissues and SCC in the tongue.

혀에서 발생한 일차성 결핵 (Primary Tuberculosis of the Oral Tongue: A Case Report and Literature Review)

  • Tuberculosis is primarily a pulmonary disease and extrapulmonary involvement of the oral cavity is an infrequent occurrence accounting for 0.2 - 1.5% of all extrapulmonary sites. The authors report a case of primary tuberculosis of the oral tongue in a 60-years-old male patient who visited the clinic for ongoing tongue pain that he experienced for several months. The lesion was initially indistinguishable from the malignancy, but differential diagnosis was performed on the biopsy with Ziehl-Neelseen stain. After treatment with anti-tuberculosis, the oral lesion healed completely without surgery.

혀에 발생한 신경초종 1예 및 고찰 (Schwannoma of Tongue : A Case Report and Review)

  • Schwannoma is a relatively uncommon, slow-growing benign tumor that is derived apparently from the Schwann cell. Tongue originated schwannomas are exceedingly rare within 1% and therefore often are not immediately included in the differential diagnosis and oral cavity tumors. In the current study, authors report a case of a schwannoma located at the tongue of a 49 years old patient treated by complete surgical excision. The patient is under clinical control, with no signs of recurrence even after 6 months.

한의 진단 추론과 진단 학습 방법 (Reasoning and Learning Methods for Diagnosis in Oriental Medicine)

  • We in this paper propose the method for diagnosis patients through the reasoning based on the diagnosis ontology in oriental medicine. In prior studies, it is simply diagnosed with the information of main symptoms, optional symptoms, and tongue / pulse. In addition, ontology itself has subjective opinions of oriental medical doctors for patients in form of axioms. There is a problem in latter case that it is difficult for other oriental medical doctors to change knowledge within the ontology. In order to solve these problems, we have constructed the diagnosis ontology and the reasoning algorithm as followings: First, in order to raise the diagnosis accuracy, we constructed the diagnosis ontology with pattern identifications, main symptoms, optional symptoms, and tongue / pulse. We also utilize the diagnosis points described in the pathology textbook, which has been studied in all of domestic oriental medical colleges. This information is represented as OWL instances in ontology, not OWL axioms so that it can be easily updated. Second, we suggest the algorithms for diagnosis reasoning and learning method based on the ontology. We have implemented the reasoning and learning system according to the diagnosis algorithm. In future study, we will construct the diagnosis ontology with all of pattern identifications and symptoms within the pathology textbook.

기능성 소화불량 환자 97명의 변증유형별 특성 (Pattern Identification of 97 Functional Dyspepsia Patients and the Characteristics of Each Pattern Type)

  • Objective: This study was designed to identify and explore the pathological patterns of functional dyspepsia (FD) patients. We also evaluated the usefulness of the Pattern Identification Questionnaire by comparing it with other assessment tools for FD. Methods: We recruited 97 FD patients based on the Rome III criteria for FD diagnosis. The pathological patterns of the subjects were determined by the Pattern Identification Questionnaire. Their dyspepsia-related symptoms were assessed using the Gastrointestinal Symptom Questionnaire (GIS) and the Pyeongwi-san (Pingwei-san) Patternization Questionnaire. Depressive symptoms were evaluated with the Beck Depression Inventory (BDI) and quality of life with the Functional Dyspepsia-Related Quality of Life (FD-QoL) Questionnaire. Tongue coating was measured by the Digital Tongue Diagnosis System (DTDS). Results: The male to female ratio was 1:1.1, and the forties and fifties age groups were largest in number. The spleen deficiency and phlegm-dampness pattern was the most common pattern found among the FD patients. No significant differences in the GIS, BDI, FD-QoL, and DTDS scores were found among the five pattern types. All pattern types showed significant correlation with GIS, Pyeongwi-san Patternization Questionnaire, and FD-QoL scores. Conclusions: Pattern Identification Questionnaire can not only identify the pathological pattern types of FD patients but also evaluate the severity of their symptoms. Compared to conventional assessment tools for FD, it could enable a more dynamic evaluation of FD patients reflecting the severity of dyspeptic symptoms and the quality of life. Further studies on the Pattern Identification of FD patients are anticipated in order to improve the diagnosis and therapy for Korean FD patients.

혀의 점막하에 발생한 신경초종 환자의 치험례 (Submucosal Schwannoma of Tongue: A Case Report and Brief Review of Literature)

  • Schwannoma is a benign, slow-growing, tumor of the peripheral nerves without specific symptoms, so that early diagnosis may be difficult. Though approximately 25~40% of all schwannomas occur extracranially in the head and neck region, only 1% of schwannomas are reported in the oral cavity. An 18-years-old female patient visited our clinic with a mass on the middle-right-dorsal surface of the tongue slowly growing for 1.5 years. The patient underwent the surgical removal of the neoplasia under general anesthesia. The mass was well capsulated and a cleavage plane was easily found. There was no recidivation during the course of a one-year follow-up. The treatment for schwannoma is surgical excision of the lesion and recurrence after excision of schwannoma is rare. The final diagnosis is made after a histological examination. Differential diagnoses must be made in relation to malignant tumors and in relation to numerous benign neoformations based on epithelial and connective tissues.

거대설의 산전 초음파 진단에 대한 고찰 (Prenatal Ultrasonographic Diagnosis of Fetal Macroglossia)

  • Macroglossia is a relatively uncommon condition that occurs in pediatric patients for several reasons and contributes to variety of functional problems. Most of macroglossia arises from tissue overgrowth and tongue muscle hypertrophy. There are no definite guideline in prenatal management or diagnosis in this conditions. However, macroglossia is often associated with syndrome or congenital disease, prenatal diagnosis is important in early detection. There are difficulty in measurement of tongue size, and standardization. Macroglossia can be risky in some aspects, such as airway obstruction. In this review, the author suggest prenatal ultrasonographic findings of macroglossia, investigate differential diagnosis of conditions associated with macroglossia, and management in clinical situation. Macroglossia, when present, can cause a number of functional and aesthetic problems for individuals. Treatment of this problem is challenging and controversial.

Comparison of the Tongue-Palate Pressure Patterns According to the Tongue Pressure in Community-Dwelling Older Adults

  • Background: Oral frailty has garnered considerable interest following its identification as a risk factor for physical frailty. The Korean oral frailty diagnosis criteria have emphasized the need for extensive research on oral frailty diagnostic items and interventions. Our study performed an in-depth analysis of the tongue-palate pressure patterns in healthy community-dwelling older adults. Methods: Of the 217 older adults aged ≥60 years who visited a senior center in Wonju, 205 participants who completed tongue pressure measurement were included in the final analysis. Pressure changes over time were recorded by instructing the participants to press their tongue against the hard palate with for 7 seconds per cycle. The participants were divided into the normal and abnormal tongue pressure (NTP and ATP, respectively) groups based on whether they achieved the target tongue pressure at least once; tongue pressure patterns were compared between the groups. Furthermore, the average time taken to achieve the standard tongue pressure value was calculated for the participants in the NTP group and used to evaluate the decrease in tongue pressure in the ATP group. Results: Among the 205 participants, 40.5% had ATP. The tongue pressure graph revealed a gentle and consistent incline that was maintained even after achieving standard tongue pressure in the NTP group. The graph was more extreme in the ATP group, and the changes in the pressure type varied across individuals; the tongue pressure was only 48.4%, 40.7%, 31.9%, and 22.6% of the NTP in the participants in their 60s, 70s, 80s, and ≥90s, respectively (p<0.05). Conclusion: Tongue pressure weakness was observed in 40.5% of the healthy community-dwelling older adults. Furthermore, ATP graphs were observed in the patients with tongue pressure weakness. Thus, activities improving the oral function in community-dwelling older adults and systematic oral rehabilitation programs should be devised to promote normal swallowing.
