만성 경막하 혈종의 성장에 대한 뇌 CT 소견 및 치료 방침 (Evolution of Chronic Subdural Hematoma based on Brain CT findings and Appropriate Treatment Methods)

  • Purpose: The objectives of this study are to classify chronic subdural hematomas based on brain computerized tomographic scan (CT scan) findings and to determine the mechanism of evolution and treatment methods. Methods: One hundred thirty-nine patients who were diagnosed with a chronic subdural hematoma and who available for follow up assessment 6 months post-surgery were analyzed retrospectively. The presence of trauma and past medical history were reviewed and evaluation criteria based on brain CT scan findings were examined. Results: Initial brain CT scans revealed a chronic subdural hematoma in 106 patients, a subdural hygroma in 24 patients, and an acute subdural hematoma in 9 patients. In all cases where the initial acute subdural hematoma had progressed to a chronic subdural hematoma, final was a hypo-density chronic subdural hematoma. In case where the initial subdural hygroma had progressed to a chronic subdural hematoma, the most cases of hematoma were hyper-density and mixed-density chronic subdural hematoma. In total, 173 surgeries were performed, and they consisted of 97 one burr-hole drainages, 70 two burr-hole drainages and 6 craniotomies. Conclusion: This study demonstrates that rebleeding and osmotic effects are mechanisms for enlarging of a chronic subdural hematoma. In most cases, one burr-hole drainage is a sufficient for treatment. However, in cases of mixed or acute-on-chronic subdural hematomas, other appropriate treatment strategies are required.

Coronary Artery Lumen Segmentation Using Location-Adaptive Threshold in Coronary Computed Tomographic Angiography: A Proof-of-Concept

  • Objective: To compare the lumen parameters measured by the location-adaptive threshold method (LATM), in which the inter- and intra-scan attenuation variabilities of coronary computed tomographic angiography (CCTA) were corrected, and the scan-adaptive threshold method (SATM), in which only the inter-scan variability was corrected, with the reference standard measurement by intravascular ultrasonography (IVUS). Materials and Methods: The Hounsfield unit (HU) values of whole voxels and the centerline in each of the cross-sections of the 22 target coronary artery segments were obtained from 15 patients between March 2009 and June 2010, in addition to the corresponding voxel size. Lumen volume was calculated mathematically as the voxel volume multiplied by the number of voxels with HU within a given range, defined as the lumen for each method, and compared with the IVUS-derived reference standard. Subgroup analysis of the lumen area was performed to investigate the effect of lumen size on the studied methods. Bland-Altman plots were used to evaluate the agreement between the measurements. Results: Lumen volumes measured by SATM was significantly smaller than that measured by IVUS (mean difference, 14.6 mm3; 95% confidence interval [CI], 4.9-24.3 mm3); the lumen volumes measured by LATM and IVUS were not significantly different (mean difference, -0.7 mm3; 95% CI, -9.1-7.7 mm3). The lumen area measured by SATM was significantly smaller than that measured by LATM in the smaller lumen area group (mean of difference, 1.07 mm2; 95% CI, 0.89-1.25 mm2) but not in the larger lumen area group (mean of difference, -0.07 mm2; 95% CI, -0.22-0.08 mm2). In the smaller lumen group, the mean difference was lower in the Bland-Altman plot of IVUS and LATM (0.46 mm2; 95% CI, 0.27-0.65 mm2) than in that of IVUS and SATM (1.53 mm2; 95% CI, 1.27-1.79 mm2). Conclusion: SATM underestimated the lumen parameters for computed lumen segmentation in CCTA, and this may be overcome by using LATM.

Computed Tomographic Angiography (CTA)의 검사 시 조영제 집적 정점시간에 영향을 미치는 특성 인자를 분석 (Analysis of Factors Influencing the Integrated Bolus Peak Timing in Contrast-Enhanced Brain Computed Tomographic Angiography)

  • Computed tomographic angiography (CTA)의 (조영제 집적 정점시간에 영향을 미치는 특성 인자를 분석하여: analyzing factors influencing on the integrated bolus peak timing in contrast-enhanced) 개인별 정점 시간을 계량적(Systems analysis)으로 산정해 검사에 직접 적용함으로써 예비검사(monitoring scan)에 의한 불필요한 방사선 피폭을 예방하는데 목적이 있다. CTA의 예비검사(monitoring scan)을 통해 얻은 조영제 집적 정점시간과 개인별 측정치들 간의 상관관계분석을 통해 영향인자를 파악한 다음, 다중선형회귀분석에 의한 회귀식으로 적정시간을 산출하였다. 결과로는 CTA의 평균 방사선 노출량은 $716.53mGy{\cdot}cm$였고, 예비검사(monitoring scan) 15.52 mGy (2 ~ 34 mGy)로 나타났다. 장기별 변환요소(conversion factor)를 적용한 결과, 전체 선량은 평균 1.5 mSv였고, 예비검사 선량은 0.23 mSv로 나타났다. 특성인자의 측정치와 조영제 정점시간의 상관관계분석 결과, 남성은 심박동수에서, 여성은 심박동수와 최저혈압, 혈당에서 음의 상관관계를 보였으며, 통계적으로 매우 유의하였다(p<.01). 회귀분석결과, 남성은 심박동수가 한 단계 증가할 때마다 -0.160배로, 여성은 최저혈압과 심박동수, 혈당에 따라 각각 -0.004, -0.174, -0.006배로 유의하게 감소하였다. 실측한 조영제 정점시간과 회귀식에 의해 산출된 정점시간의 일치도 검사에서 남여 대상자 모두에서 일치도가 매우 높았다. 본 연구에서는 검사 전에 환자 개인별 조영제 집적 정점시간을 산정하여 적용하면 예비검사를 생략함으로써 불필요한 방사선피폭을 예방할 수 있을 것으로 사료된다.

신세포암에서 $^{18}F-FDG-PET/CT$ ($^{18}F-FDG-PET/CT$ in Renal Cell Carcinoma)

  • Renal cell carcinoma is the most common histological type of renal malignancy, predominant in men and the primary treatment modality of this tumor is surgery. The role of diagnostic imaging in the management of this tumor is the evaluation of extent of disease as well as the detection and characterization of renal mass. US has long been a routine screening tool for kidney but tomographic imaging modalities such as CT and MRI begin to be also commonly used these days. On the other hand, the sensitivity of $^{18}F-FDG-PET$ in detection of renal mass is relatively low because of inherent limitation caused by FDG excretion pathway despite avid uptake of FDG to tumor cell per se. Many studies revealed FDG PET scan could play an important role in detection of metastatic lesions although the sensitivity for the detection of primary lesion is not so high. Furthermore, development of PET/CT scanner will make it possible to expand the indication of FDG PET scan in this malignancy.

종격동에 발생한 성장 기형종 증후군(Growing Teratoma Syndrome)의 치험 -1예 보고- (Treatment of Mediastinal Growing Teratoma Syndrome - A case report -)

  • 특이 병력 없던 15세 남자가 내원 한 달 전부터 시작된 우측 흉통과 함께 기침을 호소하여 본원 응급실로 내원하였다. 흉부 전산화 단층촬영에서 $10\times15\times16cm$크기의 전 종격동 종양으로 상대 정맥과 심장을 압박하면서 침범하고 있는 소견을 보였다. 수술 절제가 불가능한 것으로 판단하여 항암 및 방사선 치료를 시행하였다. 이후 종양 표지자는 거의 정상화되었으나 오히려 종양의 크기가 증가하는 소견을 보여, 종격동의 성장 기형종 증후군(Mediastinal Growing Teratoma Syndrome)으로 판단되어 수술을 시도하였다. 종양의 수술적 제거 후 환자는 24개월째 잔존 종양 및 재발 없이 외래 관찰 중이다.

Anterior Cervical Discectomy and Fusion Using a Stand-Alone Polyetheretherketone Cage Packed with Local Autobone : Assessment of Bone Fusion and Subsidence

  • Objective : It remains debatable whether cervical spine fusion cages should be filled with any kind of bone or bone substitute. Cortical and subcortical bone from the anterior and posterior osteophytes of the segment could be used to fill the cage. The purposes of the present study are to evaluate the clinical outcomes and radiological outcomes including bone fusion and subsidence that occurred after anterior cervical discectomy and fusion using a stand-alone cage packed with local autobone graft. Methods : Thirty-one patients who underwent anterior cervical fusion using a stand-alone polyetheretherketone (PEEK) cage packed with local autobone graft from July 2009 to december 2011 were enrolled in this study. Bone fusion was assessed by cervical plain radiographs and computed tomographic scan. Nonunion was evaluated according to the absence of bony bridge on computed tomographic scan. Subsidence was defined as a ${\geq}2$ mm decrease of the interbody height at the final follow-up compared to that measured at the immediate postoperative period. Results : Subsidence was observed in 7 patients (22.6%). Of 7 patients with subsidence greater 2 mm, nonunion was developed in 3. Three patients with subsidence greater 2 mm were related with endplate damage during intraoperative endplate preparation. Solid bone fusion was achieved in 28 out of 31 patients (90.3%). Conclusion : With proper patient selection and careful endplate preparation, anterior cervical discectomy and fusion (ACDF) using a stand-alone PEEK cage packed with local autobone graft could be a good alternative to the standard ACDF techniques with plating.

허혈성 대퇴골두 괴사 유발견에서 전산화 단층 영상 평가 (Computed Tomographic Assessment of Experimentally Induced Legg-Calve-Prethes Disease in Dogs)

  • This purpose of this study was to describe and compare the radiographic and computed tomographic(CT) appearance of the experimentally induced Legg-Calve-Perthes Disease(LCPD) in dogs. LCPD was experimentally induced at the left femoral head in 7 litters of a dog. The lesions were evaluated with radiography, CT, and contrast CT every week from day 7 to day 70 after operation. Histopathologic examination was performed on 92th day after operation. Every images were evaluated according to the staging system of the osteonecrosis published by Association Research of Circulation Osseous in 1997 and the scoring(0-4). On CT scan, diffuse porotic lesion and focal sclerotic lesion were detected on day 14 and on day 21 after operation, respectively. Subchondral fracture, articular collapse and crescent sign with decreased attenuation were shown on day 28 after operation for the first time and no change of the sign were detected from operation day 9 to day 70 after operation. Focal porotic lesion and irregular radiopacity of femoral head were detected on day $41{\pm}7.48$ and on day $51{\pm}5.29$ after operation respectively on radiographs. In scoring evaluation, 2.00 was scored on day 14 after operation on CT scan, on day 56 after operation on radiographs, respectively This study has shown that CT is more suitable for early diagnosis of LCPB and has superior sensitivity than radiography. Also, CT has been expected to be important for staging and treatment of LCPD.

Establishment of Injection Protocol of Contrast Material in Pulmonary Angiography using Test Bolus Method and 16-Detector-Row Computed Tomography in Normal Beagle Dogs

  • The aim of this study was to establish an injection protocol of a test bolus and a main bolus of contrast material for computed tomographic pulmonary angiography (CTPA) for visualizing optimal pulmonary arteries in normal beagle dogs. CTPA using a test bolus method from either protocol A or B were performed in each of four normal beagle dogs. In protocol A, CTPA was conducted with a scan duration for around 8 s, setting the contrast enhancement peak of the pulmonary trunk in the middle of the scan duration. The arrival time to the contrast enhancement peak was predicted from a previous dynamic scan using a test bolus (150 mg iodine/kg) injected with the same injection duration using for a main bolus (450 mg iodine/kg). In protocol B, CTPA was started at the predicted appearance time of contrast material in the pulmonary trunk based on a previous dynamic scan using a test bolus injected with the same injection rate as a main bolus. CTPA using protocol A showed the optimal opacification of the pulmonary artery with pulmonary venous contamination. Proper CTPA images in the absence of venous contamination were obtained in protocol B. CTPA with a scan duration for 8 s should be started at the appearance time of contrast enhancement in the pulmonary trunk, which can be identified exactly when a test bolus is injected at the same injection rate used for the main bolus.

Spontaneous Dissecting Aneurysm of the Anterior Cerebral Artery

  • Spontaneous dissection of the anterior cerebral artery is an unusual cause of subarachnoid hemorrhage. We present a case of a dissecting aneurysm of the anterior cerebral artery presenting with subarachnoid hemorrhage. A 51-year-old woman presented to our hospital with severe headache. Neurological examination demonstrated neck stiffness, decreased visual acuity of the left eye, and left ankle weakness. Computed tomographic scans showed subarachnoid hemorrhage. The initial cerebral angiogram demonstrated a slightly narrowed caliber and mild poststenotic dilation of the right A1 segment. A second cerebral angiogram 14 days later revealed no change in the focal narrowing of the proximal A1 segment but marked progression of the dilatation of the distal A1 segment. Right pterional craniotomy was performed. A sausage-like dilation of the right A1 segment was found with no definite mural hematoma. This abnormal right A1 segment was wrapped with a Sundt clip. A postoperative computed tomographic scan revealed Infarction of the right head of the caudate nucleus and the anterior limb of the right internal capsule. If a dissecting aneurysm is suspected, serial angiographic studies should be performed because of the possibility of dynamic changes over a short period.
