An Analysis on the Measurement and Factors of the Foot for Korean Female especially Focusing on the Patients of the Hallux Valgus (한국 성인여성 중 무지외반증으로 인한 발 변형환자의 수술 전·후 발부위 계측치 및 요인분석)

  • In order to study the foot deformity hallux valgus, the rate of which is currently increasing, 235 patients who were diagnosed with hallux valgus and treated in an orthopedic hospital in Seoul had their feet calibrated before and after surgery using 3d radio-scans. Data from 209 cases was analyzed and scored numerically. We scored 10 items for length, 4 for width, 3 for angle, and 2 for height, for a total of 20 items. Each individual's feet showed great variation in most of the items and significant differences after surgery, especially in regards to length and height, which increased after surgery. Angle, width, and length of the distal parts of the toes decreased after surgery. Based on the results of our analysis, we conclude that surgery brings about significant changes in structure and measurement of feet. This research confirmed that there is significant variation in foot form and individual differences based on lesion size and location and, thus, it is difficult for hallux valgus patients to find ready-made shoes that fit them properly. Foot changes before and after operation for hallux valgus were analyzed and the results quantified; our results should be considered during product design by shoe companies whose target is adult women.

  • The integration of CAD/CAM/CAE in product development is the key to realize concurrent engineering. Generally, different systems are employed in product development departments. These different systems create a lot of trout)toes such as difficulty in communication, misunderstanding and so on. A new approach to integrate CAD/CAM/CAE in one system based on CATIA thor the end-to-end process in cylinder head development is presented. Hulti Model Technology (MMT) is used to create consistent and associated CAD models for the end-to-end process in cylinder head development. The concept and method to create and organize multi models are discussed. A typical four-layer structure of HHT for mechanical products is defined. The multi level structure of the cylinder head models based on MMT is provided. The CAD models of cylinder head created based on MMT can be used as the consistent model. All models in the downstream of cylinder head development such as structure analysis, CFD, sand core design , casting simulation and so oil are associated with the CAD models. Practice shows the approach in this paper enables the development process to be carried concurrently and can obviously shorten time to the market, reduce product cost and improve product quality.

  • Even though many researchers studied the foot shape and dimension, those applications lacked. The purpose of this study was to develop insole pattern of elderly women according to footprint. Discrepancy in the classification criteria among of foot parameters complicates attempts for elderly women classification of foot sole. To develop a footprint-based classification technique for the classification of foot sole types by allowing simultaneous use of several parameters. Foot sole data from static standing footprints were recorded from 48 elderly women. The factors of footprint shape were determined. Cluster analysis was applied to obtain individual foot sole classifications. The classification model of foot insole is proposed for a classification of footprint in elderly women. An application of ANOVA, Duncan's analysis, frequency analysis, factor analysis, and cluster analysis have been made to footprint data. In order to make clear foot sole characteristics, the factors of footprint shape have been discussed. The results are as follows. The factors of footprint shape have been classified into four types: foot length, sole slope, outside sole slope, and foot width. The types of foot sole shape have been classified into four types: longed, shortened, outside sloped, and toes sloped.

  • Basal cell nevus syndrome is inherited as an autosomal dominant trait with variable expressivity. This syndrome comprises a number of abnormalities such as multiple nevoid basal cell carcinomas of the skin, skeletal abnormalities as bifid rib and fusion of vertebrae, central nervous system abnormalities as mental retardation, eye abnormalities, and multiple odontogenic kerato cysts. In 1960, Gorlin and Goltz first described the features of this disease as constituting a true syndrome; since then, it has been realized that it is much more complex and encompassing than initially thought. This patient has many symtoms of basal cell nevus syndrome. - we has known multiple jaw cysts through panorama and facial computed tomography. He has hyperchromatism on basal cell through skin biopsy. In ophthalmologic consult, he has blindness on right. On his past medical history, amputation was done on his toes for polyductalism. - So we report with literature reviews

A Case of Fetal Valproate Syndrome Associated with Both Elbow Joint Contractures (팔꿈치관절의 구축을 동반한 Fetal Valproate 증후군 1례)

  • Fetal valproate syndrome has been described as a distinctive pattern of minor anomalies of the face and digits. This pattern has not been delineated completely but appears to involve brachycephaly with a high forehead, shallow orbit, ocular hypertelorism, small nose, small mouth, low set posteriorly rotated ears, long overlapping fingers and toes, and hyperconvex fingernail. Cleft palate and congenital heart disease have occasionally been described in babies exposed to valproate during embryogenesis. We report a neonate born from an epileptic mother receiving sodium valproate during pregnancy. This neonate presented with characteristic facial abnormalities, both elbow contractures, and overlapping of right first and second toe, forth and fifth toe, and left first and second toe. This case raises the possibility that these abnormal appearances might be caused by intrauterine valproate exposure.

  • Inclusion body fibromatosis is a rare benign soft tissue neoplasm typically involving fingers and toes of children in mostly less than one year old. Histologic findings include spindle-shaped fibroblasts surrounded by dense stroma and small perinuclear eosinophilic inclusions in the cytoplasm. Although the tumor typically undergoes spontaneous regression, surgery is considered when functional impairment or deformity develops with the lesion. Unfortunately, recurrence rate was reported to be as high as 60 % following tumor excision. Authors would like to present our case where the tumor occurred in relatively older child and kissing lesion was found a few months after the surgery.

The Relationships of Treatment Side Effects, Family Support, and Quality of Life in Patient with Cancer (암환자의 치료 부작용, 가족지지, 삶의 질과의 관계 연구)

  • Purpose: The study was done to identify the relationship of treatment side effects, family support, and quality of life in patients with cancer, and factors influencing quality of life. Method: A convenience sample of 106 patients who were receiving cancer treatment at W hospital were interviewed using the Side Effects scale by Hur, Family Support scale by Kang, and QOL scale by Ro. Result: Results indicate that women experienced more severe side effects than men. There was a negative relationship between side effects and quality of life, and a positive relationship between family support and quality of life. The most bothersome side effects were changes in taste and appetite, followed by general weakness and fatigue. Side effects such as loss of hair, nausea, dizziness, numbness, pins and needles in fingers and toes, and dry mouth were also experienced. General weakness and family support were analysed as to whether they were factors influencing quality of life. Conclusion: The results revealed that relieving general weakness should be given high priority in nursing interventions for patients undergoing cancer treatment. In addition, nursing programs should be developed that can reinforce family support.

Subcutaneous Sarcoidosis Occurring in Both Chin and Toe

  • Sarcoidosis is a systemic inflammatory disease characterized by non-caseating granulomas of unknown origin. Of the fewer than 6% of sarcoidosis cases that occur in subcutaneous tissue, most occur on the face or forearm, but rarely in the toe. A 33-year-old man was admitted to our institute with a 2-cm mass on his chin and a 0.5-cm mass on his right fourth toe. Based on preoperative ultrasonography, epidermal cysts were suspected, and histopathological tests were performed after removing the masses. Histopathologically, the chin and toe tissue samples showed chronic granulomatous inflammation, without necrosis, indicative of sarcoidosis. Chest computed tomography revealed a large number of small nodules in both the interlobar fissures and the peribronchial area, and a large number of small lymph nodes in both the hilar and interlobar node areas. On the basis of the histopathologic and imaging findings, the patient was diagnosed with sarcoidosis. This very rare case of sarcoidosis, occurring in both chin and toe, suggests that thorough evaluation is needed to find other mass when we find sarcoidosis in facial area.

Research on Winter Working Environment and Working Clothes at a Construction Site (겨울철 건설현장의 작업 및 의복 환경 실태 조사)

  • The purpose of this study is to research the actual condition of working environment, working clothes and personal protective equipments on workers at a construction site in winter. The one to one interview by questionnaire was performed with 33 males and 2 females workers of a construction site in Daegu at $1.3^{\circ}C$ air temperature, 28% R.H., 4.7m/sec air velocity. The questionnaire consisted of 21 questions including 4 personal characteristics questions, 12 working environment/performance questions and 5 working clothes/personal protective equipments questions. It was modified from the original questionnaire developed by Finnish Institute Occupational Health. We found that physical stress of workers increased and the ability of performance decreased. The workers felt the coldest at hands/fingers, feet/toes and the face part of cheek, nose and ears. They rarely wore thermal clothing for winter and answered that the thermal gloves for winter did not provided enough warmth. Even if the workers recognized the necessity of personal protective equipments for their safety, they did not wear them for efficiency of working performance. These results will be useful elementary materials for development of working clothes and personal protective equipments from the viewpoint of improving both working performance and comfort.

A Study on Actual Footwear-wearing Conditions of College Students in Kwangju & Jeonam (광주, 전남지역 대학생의 신발착용 실태에 관한 연구)

  • The dual purposes of this study were to investigate footwear worn by college students who have become spoilt for choice in apparel & shoes, and point out the problems of their shoe-wearing habbits. The survey was administered in October, 2010 to 600 college residents in Kwangju-Jeonnam area and the results are as follows. Firstly, the most frequently worn footware by males were sneakers, followed by dress shoes and oxford shoes. Females followed a similar trend with sneakers being the most frequently worn, followed by high heeled shoes, flat shoes, wedge high heeled shoes and chucker boots. Secondly, both male and female students thought that the most comfortable and favorable shoes for them were sneakers and the most uncomfortable ones were high-heeled shoes and sandals. They thought chucker boots were both fashionable and comfortable. Thirdly, the degree of unsatisfaction for shoes was relatively high and the influencing factors for unsatisfaction were uncomfortable soles, aches around the toes and a frontward leaning posture of the body, etc. Finally, 87% of the female subjects who had experience wearing high-heeled(over 7cm height) shoes showed a significant increase in foot disorders than those who had never worn high-heeled shoes. According to the results, it is necessary to educate proper selection and wearing of footwear in order to prevent foot disorders and maintain overall health of the body.
