원심모형실험을 통한 전석층이 존재하는 제방에서의 파이핑 현상 모사 (Reproduction of Piping Failure Due to the Permeable Layer Using Centrifuge Test)

  • 본 논문에서는 제방 제체 하부지반에 투수계수 차이가 많이 나는 투수층(전석층)이 존재하는 김포제방에서 발생한 파이핑 현상을 모형실험 중 하나인 원심모형시험을 이용하여 모사하였다. 또한 원심모형실험과 동일 조건을 대상으로 수치해석을 수행하여 원심모형실험 결과와 비교하였다. 우선, 김포 제방을 바탕으로 원심모형실험을 수행할 단면을 결정하고, 간극수압계를 매설한 모형 제방에서 원심모형실험을 수행하였다. 실험을 통해 대부분의 물이 전석층으로 흐르는 것을 확인 하였으며 그로 인해 파이핑 현상이 발생하였다. 비디오 카메라 영상을 통해 기존의 연구 결과와 같이 전석층과 제내지 측 지표면과 거리가 가장 짧은 부분에서의 파이핑 현상을 확인하였다. 간극수압계를 통해 획득된 수압 분포 특성도 전석층으로의 물의 흐름으로 인해 파이핑 현상이 발생한 것임을 확인시켜주었다. 수치해석 결과 또한 원심모형실험 결과와 동일한 위치에서 파이핑 현상이 발생하는 것을 확인하였다. 원심모형시험을 통해 제방에서의 파이핑 현상을 모사하였으며 추후 제방 연구에서 원심모형시험을 이용하여 다양한 실험 및 현상을 평가, 예측 할 수 있을 것이라 판단된다.

사구종양의 치료 (Treatment of Glomus Tumor)

  • 본 대학 부속 성모병원에서 경험한 사구종양 11례를 후향적으로 분석하였다. 전례가 여자로, 특징적인 동통 및 압통, 그리고 냉각 과민성의 3 주증상을 보였다. 손톱밑 사구종양이 8례, 발톱밑 사구종양이 1례, 그리고 원위 수지 수질(pulp)내 사구종양이 2례이었다. 10례에서 합병증없이 만족스러운 결과를 얻었으며, 손톱바닥을 종괴와 함께 절제한 1례는 손톱기형으로 경과추시중이다. 사구종양은 완전한 절제시 국소재발이나 전이를 하지 않는 양성종양이나, 손톱 바닥의 손상 시 손톱의 변형을 초래할 수 있으므로 완벽한 절제 및 손톱바닥의 세심한 재 봉합이 이루어져야하겠다.

Patterns of morphological variation in the Schlegel's Japanese gecko (Gekko japonicus) across populations in China, Japan, and Korea

  • Background: Studies of morphological variation within and among populations provide an opportunity to understand local adaptation and potential patterns of gene flow. To study the evolutionary divergence patterns of Schlegel's Japanese gecko (Gekko japonicus) across its distribution, we analyzed data for 15 morphological characters of 324 individuals across 11 populations (2 in China, 4 in Japan, and 5 in Korea). Results: Among-population morphological variation was smaller than within-population variation, which was primarily explained by variation in axilla-groin length, number of infralabials, number of scansors on toe IV, and head-related variables such as head height and width. The population discrimination power was 32.4% and in cluster analysis, populations from the three countries tended to intermix in two major groups. Conclusion: Our results indicate that morphological differentiation among the studied populations is scarce, suggesting short history for some populations after their establishment, frequent migration of individuals among the populations, and/or local morphological differentiation in similar urban habitats. Nevertheless, we detected interesting phenetic patterns that may predict consistent linkage of particular populations that are independent of national borders. Additional sampling across the range and inclusion of genetic data could give further clue for the historical relationship among Chinese, Japanese, and Korean populations of G. japonicus.

반월형 절골술을 이용한 무지 외반증의 치료 (Proximal Dome Osteotomy for Hallux Valgus)

  • Purpose: The purpose of this study was to evaluate the radiographic results and complications after the proximal dome osteotomy for hallux valgus. Material and Methods: 127 cases of clinically moderate to severe hallux valgus from October 1994 to September 1997 were included in this study. All had been surgically corrected with proximal dome osteotomy, bunionectomy, and distal soft tissue release. We compared the hallux valgus angle(HVA) and intermetatarsal angle(IMA) at preoperative, postoperative 3 weeks, postoperative 6 weeks, and postoperative 3 months. Also we reviewed the postoperative com plications. Result: The HVA averaged $34.1^{\circ}$ at preoperative, $4.3^{\circ}$ at 3 weeks after operation, $8.1^{\circ}$ at 6 weeks after operation, and $10.2^{\circ}$ at 3 months after operation. The lMA averaged $14.6^{\circ}$ at preoperative, $5.1^{\circ}$ at 3 weeks after operation, $5.6^{\circ}$ at 6 weeks after operation, and $7.3^{\circ}$ at 3 months after operation. We experienced 7 cases of malunion, 5 cases of limitation of motin at the first metatarso-phalangeal joint, 3 cases of hallux varus deformity, 2 cases of delayed union. Conclusion: Proximal dome osteotomy for moderate to severe hallux valgus deformity was considered as one of the effective treatment methods. And we try to avoid limitation of motion at the first metatarso-phalangeal joint after operation.

건강보험심사평가원 자료를 바탕으로 한 당뇨발과 절단에 관한 보고 (A Report on Diabetic Foot and Amputation from the Korean Health Insurance Review & Assessment Service Data)

  • Purpose: This study reports on limb amputations in diabetic patients according to gender, age, and region based on the data from the Korean Health Insurance Review & Assessment Service. Materials and Methods: The number of amputations was compared by region, age, gender, and year, as well as by femoral region, lower leg, foot, and toe in diabetic patients who received limb amputation. This analysis was performed based on the data from the Korean Health Insurance Review & Assessment Service, between January 2009 and December 2014. Results: The total number of amputations between the study period was 9,155. The number of patients who were treated at hospitals for diabetes in 2009 was 1.9 million, among which, 1,214 patients underwent amputation. In 2014, the incidence of diabetes was 1,747 in 2.58 million individuals. With this rising incidence of diabetes, the amputation of limbs due to diabetes is increasing every year. In particular, the following regions were amputated more often: femoral region, 2.3%; lower legs, 19.6%; feet, 18.1%; and toes 60.0%. Regarding gender differences, males showed a higher amputation rate than females for all body parts. With respect to region, Seoul was the highest with 30.2%, followed by Gyeonggi with 19.9%, and Busan with 8.8%. According to age, older age showed greater diabetic amputation rate. Conclusion: In accordance with the rising incidence of diabetes, the diabetic amputation is also increasing. Here, we showed that toes were amputated with the highest percentage and males had greater amputation rate than females for all body parts. Moreover, amputation rate was highest in older diabetic patients, especially for those in their seventies. Additionally, Seoul was the region with highest amputation rate.

기능적 전기자극 치료기를 이용한 중풍구두가 편마비 환자의 보행에 미치는 영향 (The Effects Where the Stroke Shoes Which Use Functional Electric Stimulation Goes Mad to Walking of the Hemiplegia)

  • Purpose: An objective analysis and observations were to be done on hemiplegia patients that are wearing a walking support device, Stroke shoes. Their improvements in walking pace, the reduction of distance between the two knee joint, the increase of curve angle of the knee joint and their steps and the reduction of ankle joint upon swing phase were analyzed using a 20 walking analyzer. Methods: An examination was carried out to see the patients' communication skill and independent walking and then let them walk with the Stroke shoes on to get results before and after wearing it. Simi Reality Motion Systems GmbH (Germany, 2007) was used to analyze the results regarding knee joint and ankle joint angle changes of sagitta plane and coronal plane, stepping distances, distances between the knees and walking pace. Results: 1. The articulation angle of ankle joint during swing phase decreased and knee joint has shown a statistically significant increase in such value(p<0.05). 2. Only knee joint showed a significant increase in articulation angle during heel strike(p<0.05). 3. Knee joint showed a significant increase in articulation angle during toe off(p<0.05). 4. The distance between the two knees as well as their foot steps significantly decreased compared with when Stroke shoes were not worn(p<0.05). 5. Stroke shoes with FES have shown positive effects on the patients in improving their walking styles overall. (p<0.05). Conclusion: There was an improvement in rotation walking pattern by a reduction in the distance between the knees after wearing Stroke shoes with FES. Plantar flexion reduced that occurred in ankle joint during walking and flexion angle increased in knee joint, both of which improved foot drop which was a major problem in hemiplegia patients. Also it is believed that the device will have some positive influences on knee joint stiffening paralysis to aid in improving inefficient walking phases.

  • Docetaxel은 비소세포암에서 널리 사용되는 taxoid 계열의 항암제로, 조갑변화의 부작용은 한국에서 드물게 알려져 있다. 저자들은 비소세포암 4기인 62세 남환이 5차례 docetaxel 및 carboplatin 항암치료시행 7일 후 발생한 조갑하 농양 및 조갑 박리의 진단과 치료 1예를 경험하여 문헌 고찰과 함께 보고하는 바이다.

장애물 보행시 젊은 사람과 노인들의 보행 양식에 대한 이중과업 수행의 효과 (The Effects of Dual-Task on Stepping Over Obstacles From a Position of Quiet Stance in Younger and Older Adults: A Pilot Study)

  • 본 연구의 목적은 이중과업 방법론(dual task methology)을 사용해서 젊은 사람과 노인을 대상으로 독립된 두 사건(two separate concurrent events)을 동시에 수행하는데 요구되는 주의력에 대한 분석과 노인에서의 특징적 차이를 찾는 것이다. 본 실험은 대상자가 힘판(force plate) 위에서 장애물(10cm) 보행시에 경피자극(cutaneous stimulation)에 대하여 마이크로 스위치(micro-switch)를 사용하여 반응하면서 시행되었다. 힘판과 시간(temporal events) 그리고 반응시간(reaction time)에 관한 자료들은 1000 Hz의 주파수로 수집되었다. 반응시간은 대상자들이 서 있는 상태(baseline) 장애물 보행시(dual task)에서 수집되었다. 반응시간은 아중과업 조건에서 대상자 모두에게서 긴 것으로 나타났으며 특히 노인에서 정상 성인보다 반응시간이 긴 것으로 나타났다. 이중과업 조건 하에서 노인 대상자가 정상 성인에 비해 발가락이 장애물에 닫지 않고 통과할 수 있는 공간, 즉 토우 클리어런스( toe-clearance)와 슬관전 굴곡(knee flexion) 각도가 훨씬 큰 것으로 나타났다. 이중과업 조건하에서 모든 대상자가 족관절 배측굴곡(ankle dorsiflexion) 각도를 감소시키는 것으로 나타났다. 노인들은 젊은 대상자들보다 훨씬 더 긴(124 ms) 유각시간을 보여 주었으며 정상 장애물 보행시 유각 시간은 이중과업보다 50 ms 긴 것으로 나타났다. 이러한 보행 특성의 차이는 노인대상자들이 젊은 대상자들보다 장애물 보행시에 이중과업의 영향을 더 받았기 때문인 것으로 판단된다. 이중 과업 시행시 이러한 토우 클리어런스의 감소와 장애물 통과시에 보행 속도의 증가는 아마도 낙상의 가능성을 증가시키는 요인이 될 수 있는 것으로 보여진다. 본 연구의 결과는 다중과업(multitasks)을 필요로 하는 보행 훈련 프로그램(gait training program)의 개발과 시행에 있어서 기초적인 자료를 제공할 수 있는 것으로 보여진다.

만성 요통환자와 정상인의 균형반응 비교 (Comparison of the Balance Relations Between Healthy Subjects and Patients With Chronic Low Back Pain)

  • The purpose of this study was to identify the differences of the static and the dynamic balance reactions in the flexion syndrome (FS) and the extension syndrome (ES) group of the patients with chronic lowback pain (LBP) and healthy subjects. Twenty subjects were included in each group. By using EquiTest 5.02, the static balance was measured by the equilibrium score and the strategy score of sensory organization test (SOT), while the dynamic balance was measured by the latency of motor control test (MCT) and the sway energy of adaptation test (ADT). Oswestry Disability Index (OSI) was used to measure level of the disability in patients with chronic LBP. The equilibrium scores, the strategies of SOT, and thelatencies of MCT of three groups were compared with one way ANOVA, while the sway energy of ADT was compared with repeated measures one way ANOVA. The results of this study showed that the equilibrium scores and the strategy scores of SOT were significantly lower in patients with chronic LBP than in healthy subjects. The equilibrium scores and the strategy scores of SOT were significantly differences between the FS and ES groups in condition 5 (support surface was sway-referenced and visual information waseliminated by eye closure), and 6 (support surface was sway-referenced and visual information was altered by sway-referencing). The FS group showed delayed average reaction time at large posterior translation, however, the ES group showed delayed average reaction time at large anterior translation, Even though the sway energy of the patients with chronic LBP were greater than that of healthy subjects during the toe down (plantar flexion rotation), the values between the FS and ES groups didn't show any significant difference. The disability level showed highly correlation with the equilibrium score of the condition 5. As the results, the FS and ES groups divided by the their symptoms and signs in patients with chronic LBP showed different balance reaction. Therefore, more accurate evaluation and balance treatments are needed to focus on their symptoms and signs in patients with chronic LBP.

Chitinase 생성을 위한 배did 조건 최적화 (Optimization of Culture Conditions for toe Production of Chitinase)

  • Serratia marcescens KY와 Serratia marcescens ATCC 27117 두 균주 모두 기본 배지에 $K_2$HPO$_4$ 농도를 0.2 g/L 농도 첨가할 경우 최대 chitinase 생성을 나타냈다. 그러나 세포 성장에 따른 균체량은 기본 배지에 $K_2$PHO$_4$ 농도 0.2 g/L 이상에서는 균체량은 약간 감소한다. 1.2% colloidal chitin과 0.2 g/L의 $K_2$PHO$_4$가 포함된 기본 배지에 MgSO$_4$를 0.2-0.25 g/L를 첨가하였을 때 최대의 chitinase 생성을 보이고, 두 균주 모두 K, P, Mg 및 기타 mineral을 영양 요구 인자로서 필요한다. Colloidal chitin을 1.2% 함유하고 있는 기본 배지에 각종 탄소원의 종류에 따른 세포 성장과 chitinase 생성은 colloidal chitin만을 첨가하였을 때가 상대적으로 가장 우수하고, 탄소원을 첨가할 경우 Serratia marcescens는 모든 탄소원에서 chitinase 생성이 억제되었다. 또한 질소원에 따른 세포 성장과 chitinase 생성은 Serratia marcescens KY와 Serratia marcescens ATCC 27117 모두 tryptone이 가장 우수하였고, 2.0 g/L의 질소원 농도까지는 질소원 농도가 증가함에 따라 chitinase 생성은 증가하다가 2.0 g/L 이상의 농도에서는 질소원 농도가 증가함에 따라 chitinase 생성은 감소하였다. 이들 질소원 중 chitinase 생성은 trypotone>yeast extract > beef extract > asparagine 순서로 chitinaserk 생성되므로, Serratia marcescens는 chitinase 생성에 있어 vitamin B군과 같은 질소원을 growth factor로 요구한다.
