Background In this study, we characterize the morbidity at the donor-site of partial second toe pulp free flaps in terms of wound management as well as long-term outcomes. Methods A single-institutional retrospective review was performed for patients who had undergone partial second toe pulp free flap transfer to the fingertip. Patient charts were reviewed for infection, skin necrosis, wound dehiscence, and hematoma for the donor site. Additionally, a questionnaire survey was given to patients who had a follow-up of longer than 1 year to characterize long-term postoperative pain and appearance. Results The review identified a total of 246 cases. Early wound complications were significant for wound dehiscence (n=8) and hematoma (n=5) for a wound complication rate of 5.3%. The questionnaire was distributed to 109 patients, and 54 patients completed the survey. Out of these 54 patients, 15 patients continued to have donor-site pain (28%) at a mean follow-up period of 32.4 months. However, the pain intensity was relatively low in the range between 2 to 5, on a 0-10 scale. None of these patients felt this donor-site pain interfered significantly with daily activity, nor did any patient require pain medications of any type. Donor-site appearance was satisfactory to most patients. Conclusions The partial second toe pulp flap was associated with low rates of wound complications and favorable long-term outcomes. Given the functional and aesthetic gain in the recipient finger, donor-site morbidities appear acceptable in this patient population. This study can be helpful in counseling patients regarding donor-site morbidity during the informed consent process.
This paper presents the results of the experimental and analytical investigation for the fatigue strength of welded details frequently used in steel bridges, especially for the details with relatively lower fatigue strength. Considering the measured stress fatigue cracking initiated at toe of the transverse fillet weld joining the gusset plates to the web because of the stress concentration that developed as a result of the geometric conditions and the greater probability of microscopic discontinuities at the fillet weld toe A comparison was made of the stress calculated by considering geometric aspect of bead and measured at same position. They indicate that the geometric conditions of the weld toe result in similar stress concentration on both FEM models and test results. The test results were compared with the fatigue criteria of AASHTO, JSSC specifications. Specimens of 80 and 150mm gusset plate configuration tested either respectively equaled or exceeded the fatigue resistance provided by category D and E of the AASHTO specification. It also satisfied the category F and G of JSSC. Both WG1 and WG3 specimen tend to provide S-N curves with a store near -0.3 less than AASHTO and JSSC.
The purposes of this study were to investigate the relationship between anthropometric data of foot and other body sizes. to categorize the foot shape of elementary school boys and to find out determinant factors related the foot that enable us to deduce the foot shape and size for the design of more comfortable shoes. Subjects of this study were 249 elementary school boys of age ranged from 6 to 11 residing Seoul and lncheon area. Anthropometric sizes were measured with the direct measurement method using Martin scales and the indirect measurement method using digital photos. Pearson's correlation, factor analysis. cluster analysis. analysis of variance, post-hoc test, and cross tabs were performed for statistical analysis of the data by SPSS program. From the investigation on the relationship between foot-related items and body items, most items of foot measure were significantly related to body size items. However, angle of the foot did not related to other body sizes although other height items and mass items of the foot did have relationships with other body sizes. Results of ANOVA indicated there were significant differences in foot-related items except for items of foot angle and all body anthropometric items by subjects' age. This implicates big toe angle, little toe angle and foot ratio factors are required in sizing shoes besides foot length. On the basis of cluster analysis using factor scores. three different foot shapes were categorized. Type 1 was large and wide foot, Type 2 was small and narrow foot with large toe angle. and Type 3 was medium foot with no deformity on big toe. These three groups show significant differences in almost all measurement items. However, Rorher index and foot angle didn't show any significant differences among groups. This implicates the foot shape can be a determinant of shoe size.
Traumatic loss of the fingers present significant functional disability and the index finger is an important component of hand function. Since 1900, many attempts and efforts have been done in reconstruction of amputated fingers with toes. Authors clinically analyzed 8 cases of toe-to-finger transfer in traumatic amputation of the index finger to be followed for more than one year at Korea University Hospital from August 1982 to December 1991. The results were as follows: 1. The most common cause of injury was mechanical accident in 7 of 8 cases. 2. Average interval between injury and operation was 8 year 3 months. 3. Average operation time was 5 hours 58 minutes and average ischemic time of transferred toe was 1 hour 52 minutes. 4. Skin flaps have survived in 7 of 8 cases, and the functional results in 6 of 8 cases were over than fair.
Purpose: This study was conducted in order to investigate the kinematic gait parameter of lower extremities with different gait conditions (level walking, stair, ramp) in hemiplegic patients. Methods: Ten hemiplegic patients participated in this study and kinematic data were measured using a 3D motion analysis system (LUKOtronic AS202, Lutz-kovacs-Electronics, Innsbruk, Austria). Statistical analysis was performed using one-way repeated measure of ANOVA in order to determine the difference of lower extremity angle at each gait phase with different gait conditions. Results: Affected degree of ankle joint in the heel strike phase showed significant difference between level walking and climbing stairs, and toe off phase showed significant difference between level walking and climbing stairs, ramps, and climbing stairs. Affected degree of knee joint showed no significant difference in all attempts. Affected degree of hip joint in the toe off phase showed significant difference between level walking, ramps and stairs, and climbing ramps. Swing phase showed significant difference between sides for level walking and stairs, climbing ramps. Affected ankle joint of heel strike and toe off, and affected hip joint of toe off and the maximum angle of swing phase in the angle was increased. Unaffected side of the ankle joint, knee joint, and hip joint showed a significant increase in walking phase. Conclusion: These findings indicate that compared with level walking, different results were obtained for joint angle of lower extremity when climbing stairs and ramps. In hemiplegia patient's climbing ramps, stairs, more movement was observed not only for the non-affected side but also the ankle joint of the affected side and hip joint. According to these findings of hemiplegic patients when climbing stairs or ramps, more joint motion was observed not only on the unaffected side but also on the affected side compared with flat walking.
The use of welding process has been increased for manufacture of machine, bridges, ships, gas facilities and so on together with development of welding technique. Accordingly, it has been needed to develop the welding methods considering higher productivity and safety design for manufacture of their welding structures. In this study, it was studied basically on characteristics of fatigue strength and fatigue life in full penetrated cruciform fillet weld zone in relation to material thickness, welding passes, loading direction and notch radius of toe zone. Most of fatigue failure occurred in toe zone of cruciform fillet weld joint. Fatigue strength and fatigue life are under the influence of stress concentration due to notch radius and flank angle of toe zone. The metal of toe zone annealed and diffused by multi-layer welding and acicular ferrite structure formed by the result improved fatigue strength and fatigue life.
The purpose of this study was to analyzed body movement of the right and left directions relative to stance width. Seven college students(male, height $174.9{\pm}4.8cm$, weight $70.9{\pm}6.6kg$) participated in this study. For the purpose of this study, nine high-speed cameras and two force plates was utilized. Stance width was limited into small stance width(mean 42.9cm) and large stance width(mean 65cm). The results showed that (1) the small stance width took shorter time than the large stance during landing of the right feet to toe-off; (2) the small stance width was took longer movement of the right feet after split step comparing to the large stance; (3)there was no significant difference in the speed of center of gravity between toe-off and maximum speed; (4) the small stance width was greater angles to hip joint and knee joint during toe-off of the right feet comparing to the large stance; (5) the was no significant difference in values for force plates between the small stance and the large stance width during toe-off of the right feet.
Background: Hallux valgus (HV) is a foot deformity developed by mediolateral deviation of the first metatarsophalangeal joint. Although various foot-toe orthoses were used to correct the HV angle, verification of the effects of kinetics variables such as ground reaction force (GRF) through three-dimensional (3D) gait analysis according to the various type of orthoses for HV is insufficient. Objects: This study aimed to investigate the effect of soft and hard types of foot and toe orthoses to correct HV deformity on the GRF in individuals with HV using 3D motion analysis system during walking. Methods: Twenty-six subjects participated in the experiment. Participants had HV angle of more than 15° in both feet. Two force platforms were used to obtain 3D GRF data for both feet and a 3D motion capture system with six infrared cameras was used to measure exact stance phase point such as heel strike or toe off period. Total walk trials of each participant were 8 to 10, the walkway length was 6 m. Two-way repeated measures ANOVA was used to determine the effects of each orthosis condition on the various GRF values. Results: The late anteroposterior maximal force and a first vertical peak force of the GRF showed that the hard type orthosis condition significantly increased GRF compared to the other orthosis conditions (p < 0.05). Conclusion: There were significant effects in GRF values when wearing the hard type foot orthosis. However, the hard type foot orthosis was uncomfortable to wear during walking. Therefore, it is necessary to develop a new foot-toe orthosis that can compensate for these disadvantages.
This study was designed to figure out the changes in elderly women's foot size and shape by aging, to propose size specification for elderly women's shoes, and to produce regression equations using representative measurements items to estimate other measurements usually hard to get. Subjects were 118 women of 30-59 years and the 227 elderly women over 60 years. Martin's anthropometry was done on the right foot of each subject for 25 items. And 11 indirect measurement items were measured on both foot printing sole outline and picture in profile taken by digital camera. For statistical analysis on the anthropometric measurements by SPSS program, analysis of variance, post-hoc test(SNK-test), crosstabulation, multiple correlation analysis, regression analysis were performed. The results of the study are as follows. Firstly, it was found that the foot figures of elderly women over 60 years were smaller in girth and width than those of below 60 years. In addition, it was revealed that a big toe and a little toe of elderly women showed a tendency concentrating to the central axis of feet. The foot index of elderly was smaller in width and girth. Secondly, foot size distribution table of elderly group showed wider size ranges and covered smaller sizes than the below the age of 60, meaning wide variation in foot size of elderly women. Thirdly, the multiple correlation analysis showed high correlation of foot length/girth to other measurements, suggesting these two items could be used as representative items for elderly women's shoe size specification as other age groups. Regression equations were produced using foot length/girth to estimate other measurements, suggesting such items could be estimated effectively and utilized in on/off-line shoe manufacturing shop as heel to big toe length, heel to little toe length, exterior malleouls width, instep girth, ankle girth, etc. These results imply prudent features of elderly women's foot as diversity of foot shape and wide size specification range should be applied for ergonomic shoe design for them.
The purpose of this study was to identify the influence of wedged insole and foot progression angle (FPG) on lateral thrust of knee in healthy subjects. Fifteen healthy male subjects were recruited from Suncheon First College, in Suncheon. The subjects randomly walked at the comfortable velocity under five conditions: bare footed, medio-lateral $10^{\circ}$ wedged insoles, toe-in and toe-out gait. The lateral thrust was measured by a accelerometer with telemeter during walking. Data was collected while each subject walked for about 10 gait cycle on a flat, level walkway at their normal speed. The middle three gait cycle were used for averaged peak value of lateral acceleration. The three averaged peak value of lateral acceleration were collected under each condition at heel strike. The results showed that averaged peak value of lateral acceleration increased significantly in medial wedged insole and toe-in gait and decreased significantly in lateral wedged insole and toe-out gait as compared with bare footed (p<.05). These results suggest that wedged insole as well as walking strategy, such as foot progression angle, may prevent progression of degenerative knee osteoarthritis.
