Comparison of removal torque of saline-soaking RBM implants and RBM implants in rabbit tibias (토끼의 경골에서 RBM 표면처리 임플란트와 RBM 표면처리 후 Saline에 적신 임플란트의 제거회전력 및 표면분석 비교)

  • Purpose: The purpose of this study is to investigate the effect of the titanium implant soaked in saline after RBM surface treatment on the initial osseointegration by comparing the removal torque and the surface analysis compared to the titanium implant with only RBM surface treatment. Materials and methods: The control group was RBM surface treated implants (RBM), and the test group was implants soaked in saline for 2 weeks after RBM surface treatment (RBM+Sal). The control and test group implants were placed in the left and right tibiae of 10 rabbits, respectively, and at the same time, the insertion torque (ITQ) was measured. After 10 days, the removal torque (RTQ) was measured by exposing the implant site. FE-SEM, EDS, Surface roughness and Raman spectroscopy were performed for the surface analysis of the new implant specimens used in the experiments. Results: There was no significant difference in insertion torque and removal torque between RBM surface treated titanium implants and saline-soaked titanium implants after RBM surface treatment. Conclusion: Saline soaking after RBM surface treatment of titanium implants did not positively affect the initial osseointegration as compared to titanium implants with only RBM surface treatment.

Bio-hybrid dental implants prepared using stem cells with β-TCP-coated titanium and zirconia

  • Purpose: This study investigated periodontal ligament (PDL) restoration in osseointegrated implants using stem cells. Methods: Commercial pure titanium and zirconium oxide (zirconia) were coated with beta-tricalcium phosphate (β-TCP) using a long-pulse Nd:YAG laser (1,064 nm). Isolated bone marrow mesenchymal cells (BMMSCs) from rabbit tibia and femur, isolated PDL stem cells (PDLSCs) from the lower right incisor, and co-cultured BMMSCs and PDLSCs were tested for periostin markers using an immunofluorescent assay. Implants with 3D-engineered tissue were implanted into the lower right central incisors after extraction from rabbits. Forty implants (Ti or zirconia) were subdivided according to the duration of implantation (healing period: 45 or 90 days). Each subgroup (20 implants) was subdivided into 4 groups (without cells, PDLSC sheets, BMMSC sheets, and co-culture cell sheets). All groups underwent histological testing involving haematoxylin and eosin staining and immunohistochemistry, stereoscopic analysis to measure the PDL width, and field emission scanning electron microscopy (FESEM). The natural lower central incisors were used as controls. Results: The BMMSCs co-cultured with PDLSCs generated a well-formed PDL tissue that exhibited positive periostin expression. Histological analysis showed that the implantation of coated (Ti and zirconia) dental implants without a cell sheet resulted in a well-osseointegrated implant at both healing intervals, which was confirmed with FESEM analysis and negative periostin expression. The mesenchymal tissue structured from PDLSCs only or co-cultured (BMMSCs and PDLSCs) could form a natural periodontal tissue with no significant difference between Ti and zirconia implants, consequently forming a biohybrid dental implant. Green fluorescence for periostin was clearly detected around the biohybrid implants after 45 and 90 days. FESEM showed the invasion of PDL-like fibres perpendicular to the cementum of the bio-hybrid implants. Conclusions: β-TCP-coated (Ti and zirconia) implants generated periodontal tissue and formed biohybrid implants when mesenchymal-tissue-layered cell sheets were isolated from PDLSCs alone or co-cultured BMMSCs and PDLSCs.

Quantitative investigations of titanium alloy implants (타이태늄 합금의 생체적합도에 관한 연구)

  • Screw shaped implants of Titanium-13Zirconium-6Niobium(newly developed), Titanium-6Zirconium-6Sn-6Niobium(newly developed) and Titanium-6Aluminum-4Vanadium were machined with square top and inserted in rabbit bone for 3 months. Biomechanical tests(removal torque) showed Titanium-13Zirconium-6Niobium and Titanium-6Zirconium-6Sn-6Niobium to be more stable in the bone bed than those of Titanium-6Aluminum-4Vanadium. Titanium-13Zirconium-6Niobium implants demonstrated a mean removal torque of 31.59Ncm while Titanium-6Aluminum-4Vanadium demonstrated a mean removal torque of 25.27Ncm and Titanium-6Zirconium-6Sn-6Niobium revealed a mean removal torque of 37.44Ncm and were statistically significance in Wilcoxon Signed Rank test(P<0.05). Histomorphometrical comparisons were performed on $10\;{\mu}m$ thick undecalcified ground sections in the light microscope and Titanium-13Zirconium-6Niobium showed more mean bone-tometal contact ratio than to other twotitanium alloys but had no statistically significant differences were found among the three materials(P>0.01).

Scanning Electron Microscopic Study of the Effects of Citric Acid on the Change of Implant Surface According to Application Time (구연산의 적용시간에 따른 임플란트 표면변화에 대한 주사전자현미경적 연구)

  • The present study was performed to evaluate the effect of citric acid on the change of implant surface microstructure according to application time. Implants with pure titanium machined surface, titanium plasma-sprayed surface, and sand-blasted, large grit, acid etched surface were utilized. Implant surface was rubbed with pH 1 citric acid for $\frac{1}{2}$ min., 1 min., 1 $\frac{1}{2}$ min., 2 min., and 3min, respeaively in the test group and implant surface was not treated in the control group. Then, the specimens were processed for scanning electron microscopic observation. The following results were obtained. 1. Both test and control group showed a few shallow grooves and ridges in pure titanium machined surface implants. There were not significant differences between two groups. 2. In titanium plasma-sprayed surfaces, round or amorphous particles were deposited irregularly. The irregularity of titanium plasma-sprayed surfaces conditioned with pH 1 citric acid was lessened and the cracks were increased relative to the application time of pH 1 citric acid. 3. Sand-blasted, large grit, acid etched surfaces showed the macro/micro double roughness. The application of pH 1 citric acid didn't change the characteristics of the sand-blasted, large grit, acid etched surfaces. In conclusion, the application of pH 1 citric acid to titanium plasma-sprayed surface is improper. And pure titanium machined surface implants and sand-blasted, large grit, acid etched surface implants can he treated with pH 1 citric acid for peri-implantitis treatment if the detoxification of these surfaces could be evaluated.

INFLUENCE OF A FUNCTIONAL LOADING TIME ON BONE FORMATION AROUND OSSEOINTEGRATED TITANIUM IMPLANTS IN ADULT DOGS (성견에서 골유착성 타이타늄 임프란트의 기능적 노출 시기가 주위의 골형성에 미치는 영향)

  • The purpose of this study was to investigate the influence of early functional load around osseointegrated titanium implants. 24 titanium plasma spray coated implants (ITI HS-type) were placed into the previously extracted site in the mandible of six adult dogs. The implants were divided into three groups : the control group was the implants without abutment during the experimental period; the experimental group I was loaded by connecting the contoured abutment after 6 weeks of healing; the experimental group II was loaded after 12 weeks of healing: and the mandibular second premolar and surrounding tissues were selected for natural tooth group to compare the implanted group. All dogs were injected intravenously tetracycline, alizarin red S, and calcein for bone labeling. After the experimental period of 18 weeks, the dogs were sacrificed and longitudinal sections of the bone-implant interface were cut and observed using light microscope, scanning electron microscope, and fluorescence microscope. The results of the study were as follows: 1. Light and scanning electron microscopically, all implant surfaces were well contact with bone tissue at the cortical layer, but some areas of cancellous bone were not contact directly. 2. Fluorescence microscopically, number and size of the new secondary osteons around the implant were increased than those of the natural tooth. 3. Fluorescence microscopically, linear and concentrical fluorescence was observed at or near the surface of all implants, and the bone formation and remodeling of the implants loaded after 6 week of healing were great, and unloaded implants were worst. 4. Fluorescence microscopically, endosteal bone formation was greater than periosteal bone formation at or near the implants. 5. Fluorescence microscopically, number and size of linear and concentric fluorescence was increased at the lingual side than the buccal side of the loaded implants. The result of the study indicate the possibility of the early load to the implant via a prosthesis.

A STUDY ON THE BONE FORMATION OF OPEN TYPE AND CLOSED TYPE IMPLANTS (개방형과 폐쇄형 임플랜트 매식후 주위골 형성에 관한 실험적 연구)

  • A two-stage procedure is ideal for getting a successful osseointegration. But if a one-stage procedure can achieve a similar osseointegration, the one-stage procedure has several advantages. The purpose of this study was to observe the initial bone formation and bone remodeling of open type (nonsubmerged) and closed type (submerged) titanium implants. Eight ITI hollow-screws and eight Branemark fixtures were divided into two groups (submerged and nonsubmerged) and were installed on the lower jaws of four mongrel dogs. The animals were sacrificed three months later and bone sections with implants were processed for light microscopic and fluorescent microscopic observation. The results were as follows : 1 There was no significant difference in bone-to-implant contact between submerged and nonsubmerged implants. 2. Smooth surface titanium implants showed more bone-to-implant contact than that of titanium plasma coated implants histologically. 3. Under fluorescent microscopy, the active bone remodeling and new bone formation were observed in the interface zone. 4. Under fluorescent microscopy, submerged and nonsubmerged implants had no difference in bone remodeling pattern, and intramembranous bone formation was more prominent. 5. The connective tissue fibers orienting perpendicularly toward implant surface were oberved in the neck of implants.

EFFECT OF CASTING PROCEDURE ON SCREW LOOSENING OF UCLA ABUTMENT IN TWO IMPLANT-ABUTMENT CONNECTION SYSTEMS

  • STATEMENT OF PROBLEM: The cast abutment has advantages of overcoming angulation problem and esthetic problem. However, when a gold-machined UCLA abutment undergoes casting, the abutment surfaces in contact with the implant may change. PURPOSE: The purpose of this study was to compare the detorque values of prefabricated machined abutments with gold-premachined cast-on UCLA abutments before and after casting in two types of internal implant-abutment connection systems: (1) internal hexagonal joint, (2) internal octagonal joint. Furthermore, the detorque values of two implant-abutment connection systems were compared. MATERIALS AND METHODS: Twenty internal hexagonal implants with an 11-degree taper and twenty internal octagonal implants with an 8-degree taper were acquired. Ten prefabricated titanium abutments and ten gold-premachined UCLA abutments were used for each systems. Each abutment was torqued to 30 N㎝ according to the manufacturer's instructions and detorque value was recorded. The detorque values were measured once more, after casting with gold alloy for UCLA abutment, and preparation for titanium abutments. Group means were calculated and compared using independent t-test and paired t-test (${\alpha}$=0.05). RESULTS: The results were as follows: 1. The detorque values between titanium abutments and UCLA-type abutments showed significant differences in internal octagonal implants (P<0.05), not in internal hexagonal implants (P>0.05). 2. In comparison of internal hexagonal and octagonal implants, the detorque values of titanium abutments had significant differences between two connection systems on the initial analysis (P<0.05), not on the second analysis (P>0.05) and the detorque values of UCLA-type abutments were not significantly different between two connection systems (P>0.05). 3. The detorque values of titanium abutments and UCLA-type abutments decreased significantly on the second analysis than the initial analysis in internal hexagonal implants (P<0.05), not in internal octagonal implants (P>0.05). CONCLUSION: Casting procedures of UCLA-type abutments had no significant effect on screw loosening in internal implant-abutment connection systems, and UCLA-type abutments showed higher detorque values than titanium abutments in internal octagonal implants.

The XPS and SEM Evaluation of Various Technique for Cleansing and Decontamination of The Rough Surface Titanium Implants (수종의 방법으로 임프란트 표면 처치후 표면의 형태 및 성분 변화 분석에 관한 연구)

  • Osseointegrated titanium implants have become an integral therapy for the replacement of teeth lost. For dental implant materials, titanium, hydroxyapatite and alumina oxide have been used, which of them, titanium implants are in wide use today. Titanium is known for its high corrosion resistance and biocompatability, because of the high stability of oxide layer mainly consists of $TiO_2$. With the development of peri-implantitis, the implant surface is changed in surface topography and element composition. None of the treatments for cleaning and detoxification of implant surface is efficient to remove surface contamination from contaminated titanium implants to such extent that the original surface elemental composition. In this sights, the purpose of this study was to evaluate rough surface titanium implants by means of scanning electron microscopy(SEM) and X-ray photoelectron spectroscopy(XPS) with respect to surface appearance and surface elemental composition. Moreover, it was also the aim to get the base for treatments of peri-implantitis. For the SEM and XPS study, rough surface titanium models were fabricated for control group. Six experimental groups were evaluated: 1) long-time room exposure, 2 ) air-powder abrasive cleaning for 1min, 3) burnishing in citric acid(pH1) for 1min, 4) burnishing in citric acid for 3min, 5) burnishing in tetracycline for 1min, 6) burnishing in tetracycline for 3min. All experimental treatments were followed by 1min of rinsing with distilled water. The results were as follows: 1. SEM observations of all experimental groups showed that any changes in surface topography were not detected when compared with control group. (750 X magnification) 2. XPS analysis showed that in all experimental groups, titanium and oxygen were increased and carbon was decreased, when compared with control group. 3. XPS analysis showed that the level of titanium, oxygen and carbon in the experimental group 3(citric acid treatment for 1min, followed by 1min of distilled water irrigation) reached to the level of control group. 4. XPS analysis showed that significant differences were not detected between the experimental group 1 and the other experimental groups except of experimental group 3. The Ti. level of experimental group 2, airpowder abrasive treatment for lmin followed by 1min of saline irrigation, was lower than the Ti. level of tetracycline treated groups, experimental group 5 and 6. From the result of this study, it may be concluded that the 1min of citric acid treatment followed by same time of rinsing with distilled water gave the best results from elemental points of view, and can be used safely to treat peri-implantitis.

  • The purpose of this study was to observe the effects of early functional on bone formation around titanium plsama sprayed IMZ implants. 15 IMZ implants were inserted on the mandibular premolar areaa in 5 mondgrel dogs. The inplants were divided into 4 groups according to it’s loading schedule : Temporary abutments were connected for functional loading 6weeks, 9weeks, and 12weeks after implants installation in group I, II, and III, respectively. Abutments were not connected in the control group. After polyfluorochrome sequential labeling, all animals were sacrificed and bone sections includig implants and the 1st. molar were removed and processed for histologic evaluation. From light and fluorescence microscopic evaluation as well as histomorphometric and EPMA analysis, following results were obtained : 1. Light microscopically, the osteoid tissues were observed much in the compact bone of unloaded group, and cancellous bone formation observed more in early loaded group. 2. Calcification and remodeling of bone took place faster in loaded group, especially in 6 weeks group. 3. Moderate to severe marginal bone resorption was observed in 6 weeks group, mainly on buccal crest. 4. There was no difference in the percentages of bone-to-metal contact among the experimental groups. 5. In quantitative analysis using EPMA, there was no difference in the content of calcium and phosphorus among the experimental groups, regardless of distance.

A HISTOLOGIC COMPARATIVE STUDY OF LOADED AND UNLOADED TITANIUM IMPLANTS (LOADED IMPLANT와 UNLOADED IMPLANT의 조직학적 비교 연구)

  • In order to see the possible effect of the functional load-bearing after osseointegration of the titanium root form implant in dog a histologic study was conducted. One side of lower jaw was surgically prepared edentulousness and titanium implants were inserted. Some implants were functionally loaded through fixed detachable prosthesis and some are isolated and unloaded. The dog was sacrificed four months later and bone sections with implants were processed for histologic evaluation and the results were as follows ; (1) The bone to implant interface after four months of load bearing presented no mobility and no marginal bone loss radiographically and histologically. (2) The interface zone between compact bone and implant revealed a direct bone to implant contact and in some areas marrow tissue contacts were examined at the light microscopic level. (3) At the ultrastructural level the interface of surrounding compact bone matrix and implant, three types of superficial layers were found ; one with moderate electron dense amorphous granular substance layer, other with high electron dense fine granular substance layer, and another type of amorphous granular substance covered with high electron dense line of minute granules. (4) The osteoblasts in the marrow tissue neighboring implants and osteocytes in compact bone showed typical normal characteristics and in the marrow tissues some of lymphocytes and mast cells were observed. (5) The abscence of abnormal tissue reactions at a cellular level indicates a high degree of biocompatibility for the experimental titanium implant and basically no difference was found between functionally loaded and unloaded implants.
