The study for Compliance Mismatch in the End-to-End Anastomosis of Coronary Artery and PTFE (관상동맥과 PTFE의 End-To-End 문합에서 컴플라이언스 부적합에 관한 연구)

  • Finite element analysis of end-to-end artery/PTFE anastomosis recently have been researched. But, these studies were carried out without the compensation for the error of finite element analysis and assumed the artery and PTFE as the simple cylindrical shape in spite of being the fatty tissue which covers the heart. Therefore, we performed the convergency study with respect to increasing the element numbers and considered the fatty tissue as the elastic foundation in the finite element analysis. The results are as fallow : 1. An anastomosis with the thinner thickness and larger diameter PTFE than artery could reduce the compliance disagreement. 2. A fatty tissue was affected to reduce the compliance mismatch in the vicinity of anastomosis of different material. Therefore a hypercompliant zone become narrorw and a compliance discrepancy decrease between the artery and the PTFE about 70%. And radial displacement with respect to longitudinal direction of an artery and the PTFE anastomosis was similar to a sectional compliance.

  • Recently, active research has been going on to measure the elastic modulus of human soft tissue with medical ultrasound imaging systems for the purpose of diagnosing cancers or tumors which have been difficult to detect with conventional B-mode imaging techniques. In this paper, a real-time ultrasonic elasticity imaging system is implemented in software on a Pentium processor-based ultrasonic diagnostic imaging system. Soft tissue is subjected to external vibration, and the resulting tissue displacements change the phase of received echoes, which is in turn used to estimate tissue elasticity. It was confirmed from experiment with a phantom that the implemented elasticity imaging system could differentiate between soft and hard regions, where the latter is twice harder than the former, while operating at an adequate frame rate of 20 frames/s.

  • Soft tissue collapse around prepared teeth and pontic is inevitable after removal of the provisional restoration during the impression taking procedures. When inserting gingival retraction cord, soft tissue is displaced to an undesired contour. Viscosity of impression material also causes gingival displacement. Therefore, the consideration to transfer the prosthetically contoured soft tissue to master cast is required, especially in the esthetic area. In this report, the methods to maintain the soft tissue contour and transfer to the mastercast will be introduced. Harmonious contour of the soft tissue can be achieved with provisional restoration and be transferred to the master cast with two different techniques mentioned in this case report.

HISTOLOGIC CHANGE OF THE POSTERIOR ATTACHMENT IN ANTERIOR DISC DISPLACEMENT OF THE TEMPOROMANDIBULAR JOINT-A NEW MODEL OF INTERNAL DERANGEMENT IN RABBITS (실험가토의 악관절원판 변위시 후방부착조직의 변화)

  • This paper describes a new method to create an animal model for TMJ internal derangement in the New Zealand white rabbits and the light and electron microscopical changes of posterior attachment of them. Twenty six rabbits(2.5-3.0kg), four normal and twenty two experimental, were used. The right disc of experimental animal was displaced anteriorly without sectioning the posterior attachment and tied to the zygomatic arch with nylon not to be reduced to the original position. The left TMJ was sham-operated to be compared with its right experimental one. Normal animals were sacrificed one day and eight weeks after experiment. Experimental animals were sacrificed one day, ten days, three weeks, five weeks and eight weeks after surgery respectively. They were fixed intravenously with $2\%$ glutaldehyde under general anesthesia and the samples of them were processed for light and electron microscopic examination. The purpose of this experiment is to make a suitable animal model of disc displacement without reduction for studying and understanding the cellular and morphologic events in posterior attachment of TMJ including early changes which were difficult to be observed in human TMJs. The results of this investigation suggest the following conclusions : 1. Authors induced anterior disc displacement surgically in rabbits with new method to examine histologic changes of posterior attachment. Tissue reactions of this model seem to be similar to those observed in human disc displacement. We think this animal model for anterior disc displacement may be used to explore and evaluate objectively the effects of many treatment modalities in disc displacements. 2. The animal disease model showed inflammation at early stage(one and ten days). At this stage there were mild-to-severe mononuclear inflammatory cell infiltration, numerous newly formed vessels, vessel dilatation and engormement and many fibroblasts. 3. At middle stage(three weeks), fibrosis occurred, where fibroblasts decreased in number, but their cytoplasm was profuse indicating high activity. Collagen fibers increased in number and the tissue looked more dense. 4. At late stage(five weeks and eight weeks) showed degenerative changes including perforation of posterior attachment, disintegration of collagen fiber bundles, degeneration of fibroblasts, metastatic ossification, and dystrophic calcification.

CHANGE OF LATERAL SOFT TISSUE PROFILE AFTER SURGICAL CORRECTION OF MANDIBULAR PROGNATHISM (하악전돌증의 악교정수술후 연조직 변화에 관한 연구)

  • The purpose of this paper is to investigate changes in soft tissue in 22 patients treated by vertical ramus osteotomy and sagittal split ramus osteotomy for the correction of mandibular prognathism. 22 individuals, 12 males and 10 females, were selected from the patients with mandibular prognathism at the Department of Oral and Maxillofacial Surgery, Colledge of Dentistry, Kyung Hee University. Patient were analyzed with cephalogram taken 1 week before and at least 6 weeks after surgery under the same condition. Measurements were made constructed hard tissue and sop tissue points located on each before-and-after film tracing. Comparision were made of these figures to estimate the amount that the soft tissue followed the hard tissue structures in each surgical procedure : ratio of sop and hard tissue changes were formulated. The results were as follows. 1. The horizontal changes of Pogs and Bs as a ratio of the horizontal changes of Pog and B point were 1.02 and 1.16 respectively. 2. One millimeter of posterior changes at Pog resulted in 0.86mm of posterior change at Li and 0.09mm of posterior change at Ls. The greatest amount of sop tissue change occurred at Pogs, with substantially less posterior displacement at Bs, even less at Li and at least at Ls. 3. The ratio of LI to Li was 1:0.81 and the ratio of LI to Ls was not significant.(1 : 0.17) 4. The ULA(Cm-Sn-Ls) and the relative lower lip projection (LLP) was incnease4 but the relative upper lip projection (ULP) was slightly decreased 5. The angular change of the upper lip inclined angle (Ls-Sn/ANS-PNS) and lower lip inclined angle(Li-Pogs/Me-Go) expressed as a ratio of the posterior change of Pog were 0.57 and 0.20 respectively. 6. The ratio of the lower anterior facial height change of the soft tissue(Sn-Mes) to the hard tissue(ANS-Gn) were 0.78 and and the ratio of vertical height changes of the hard tissue and sop tissue to the posterior change of the Pog were 0.18 and 0.19 respectively. 7. The sop tissue angular change of facial convexity(G-Sn-Pogs) expressed as a ratio of the angular change of the hard tissue angle of facial convexity(N-A-Pog) was 1.24.

Hard and soft tissue profile changes following anterior subapical osteotomy in bimaxillary dentoalveolar protrusion patients (양악성 치조 전돌 환자의 근첨하 분절 골절단술 후 경조직 및 연조직 측모 변화)

  • The purpose of this study is to evaluate hard and soft tissue changes following the subapical osteotomy in bimaxillary dentoalveolar protrusion patients requiring maximal retraction adult female patients was selected. Surgical procedures were performed by the same surgeon, anterior subapical osteotomy techniques were employed on the maxilla and cephalometric radiograms were traced and superimposed using the best-fit method and two reference The results were as follows 1. The bodily movement of the maxillary anterior segment was achieved in a posterior moved posteriorly with a slight correction of the lower incisors. 2. The horizontal soft tissue measurements changed significantly after treatment, but Nt and Sn vertical soft tissue measurements indicated that Ls moved inferiorly and Li superiorly. 3. The correlation between hard and soft tissue changes indicated that ${\Delta}HId/{\Delta}HLi,\;{\Delta}HId/{\Delta}LL-Eline,\;{\Delta}Hpt.B/{\Delta}HILS,\;and\;{\Delta}UI-FH/{\Delta}NL$ were significant. 4. More lower lip relative to upper lip retraction was demonstrated in relation to Rickett's E-line. The ratio between upper lip displacement was $50\%$, and between the lower incisor and lower lip displacement was $60\%$. We conclude from the results that the anterior subapical osteotomy is an efficient treatment severe dentoalveolar protrusion and desire rapid results.

Evaluation of Biomechanical Properties of Fractured Adjacent Soft Tissue Due to Fracture Site Spacing During Closed Reduction After Forearm Fracture: Finite Element Analysis (전완 골절 후 도수 정복 시 골절 부위 간격에 따른 골절 인접 연부 조직의 생체역학적 특성 평가: 유한요소해석)

  • The purpose of this study is to evaluate the biomechanical properties of fractured adjacent soft tissue during closed reduction after forearm fracture using the finite element method. To accomplish this, a finite element (FE) model of the forearm including soft tissue was constructed, and the material properties reported in previous studies were implemented. Based on this, nine finite element models with different fracture types and fracture positions, which are the main parameters, were subjected to finite element analysis under the same load and boundary conditions. The load condition simulated the traction of increasing the fracture site spacing from 0.4 mm to 1.6 mm at intervals of 0.4 mm at the distal end of the radioulnar bone. Through the finite element analysis, the fracture type, fracture location, and displacement were compared and analyzed for the fracture site spacing of the fractured portion and the maximum equivalent stress of the soft tissues adjacent to the fracture(interosseous membrane, muscle, fat, and skin). The results of this study are as follows. The effect of the major parameters on the fracture site spacing of the fractured part is negligible. Also, from the displacement of 1.2 mm, the maximum equivalent stress of the interosseous membrane and muscle adjacent to the fractured bone exceeds the ultimate tensile strength of the material. In addition, it was confirmed that the maximum equivalent stresses of soft tissues(fat, skin) were different in size but similar in trend. As a result, this study was able to numerically confirm the damage to the adjacent soft tissue due to the fracture site spacing during closed reduction of forearm fracture.

A Clinical Experience of Direct Extension to Frontal Sinus of Orbital Dermoid Cyst (전두동을 침습한 안와부 피부모양기형낭의 치험례)

  • The dermoid cyst is the one of common space occupying orbital lesion. This lesion is regarded as a non-invasive tumor, but infrequently causes destruction of adjacent bony structure and displacement of adjacent tissue. We experienced a characteristic ovoid orbital dermoid cyst that occupied in the frontal sinus and causes displacement of the eyeball with well-defined lining. This 55-year-old male presented a mass in left orbit, which rapidly increased in size for past 12 months, and patient could not open left eyelid. We removed this tumor totally then reconstructed the orbital roof and frontal sinus with an iliac bone graft and polyethylene sheet(Medpor Newnan, USA). This patient was followed up for 12 months and patient obtained satisfactory result without any complication suck as recurrence or infection.

  • Since cardiac pacemaker was first totally implanted by Chardack, Gage and Greatbatch [1966], the electrical circuity of the cardiac pacemaker has been improved, modified, and refined. The problem of transvenous electrodes, however, is still remained; this may be due to electrode displacement, exit and/or entrance block, lead fracture and insulation defects. In permanent cardiac pacing, Irreversible loss of function of the transvenous electrode catheter eventually requires insertion of new lead. Authors now report one case that disclosed easy displacement of electrode tip in early phase of implantation and then two years and five months later, malfunctioning electrode could not be withdrawn from the cardiovascular system because it has become firmly enclosed by fibrous tissue along its course from the vein tract to the right ventricle. Under such circumstances, the electrode catheter tip was left in tricuspid annulus after being sutured at its entrance and burying the loop of lead in generator pocket. New other one electrode was then reimplantation through left external jugular vein.

Change of Glycosaminoglycan Distribution and Collagen Fibers Arrangement on Temporomandibular Joint Following Anterior Disc Displacement of the Rabbits

  • Purpose: This study was to determine the effects of surgical induction of anterior disc displacement (ADD) on the distribution of glycosaminoglycan (GAG) and collagen fiber arrangement in the rabbit temporomandibular joint (TMJ) tissues including articular cartilage of condyle, disc, retrodiscal tissue, and articular eminence. Methods: We used van Gieson staining and Alcian blue critical electrolyte concentration (CEC) method to observe change of collagen fibers on disc and to measure GAG up to 10 weeks in TMJ tissues after surgical induction of ADD on 25 rabbits. Results: CEC measurements for GAG showed 0.3 M, 0.4 M, 0.6 M, and 0.8 M at 1 week, 2 weeks, 3, 4, and 8 weeks, 10 weeks, respectively. This result indicated that GAGs shifted to highly sulphated ones as time passed. Disruption of collagen fiber arrangement in the disk occurred at 10 days and aggravated at 3 weeks. Conclusion: Our study showed degenerative osteoarthritis changes in rabbit TMJ following surgical induction of ADD up to 10-week period.
